Treatment outcomes of acute leukemia during pregnancy at the National Institute of Hematology and Blood Transfusion.

Managing acute leukemia during pregnancy poses clinical challenges requiring a balance between maternal treatment and fetal preservation. From 2019 to 2024, 22 pregnant women were diagnosed with acute leukemia, including 7 acute lymphoblastic leukemia, 15 cases of acute myeloid leukemia, of which 1 was acute promyelocytic leukemia. Diagnosis occurred in the first, second, and third trimesters in 5, 10, and 7 patients, respectively. Nine patients elected pregnancy termination, one had a spontaneous abortion, one had intrauterine fetal demise, and three experienced concurrent maternal and fetal

Twin live birth after resuscitative cesarean delivery in a woman with eclampsia and cardiac arrest in a low-resource setting.

BACKGROUND: Eclampsia, a severe complication of preeclampsia, is a leading cause of maternal mortality, particularly in low-resource settings. Cardiac arrest in pregnancy, though rare, carries a high mortality rate due to physiological changes complicating resuscitation. Perimortem cesarean section (PMCS) is a life-saving intervention performed during or immediately after maternal cardiac arrest to improve outcomes. This case report describes a twin pregnancy complicated by severe eclampsia, pulmonary edema, and cardiac arrest, culminating in PMCS. CASE PRESENTATION: A 29-year-old gravida 2,

Prospective evaluation of the Simple Fetal Medicine Foundation's first-trimester prediction model for preeclampsia in twin pregnancies.

INTRODUCTION: There is no widely applicable prediction model for preeclampsia (PE) in twins, the existing models lack strict external validation. Therefore, this study aimed to evaluate the predictive performance of the Simple Fetal Medicine Foundation model (i.e., Simple FMF model, only including the maternal risk factors and mean arterial pressure [MAP]) for the prediction of PE at 11-13+ 6 weeks' gestation in twin pregnancies. MATERIAL AND METHODS: This was a prospective, non-intervention study in 464 twin pregnancies (including dichorionic twins and monochorionic twins) at 11-13+ 6 weeks'

Acute Left Heart Failure in Twin Pregnancies: Clinical Characteristics and Outcomes-A 10‑Year Retrospective Analysis of 24 Cases.

This retrospective study evaluated the clinical characteristics, management, and maternal-neonatal outcomes of 24 twin pregnancies complicated by acute left heart failure (ALHF) delivered at Peking University Third Hospital from January 2015 to December 2025. Patients were stratified by timing of onset (antenatal vs. postnatal). The mean maternal age was 33.8 years, and 70.8% had preeclampsia. ALHF occurred antenatally in 9 cases (37.5%) and postnatally in 15 cases (62.5%). Dyspnea was the most common initial symptom (79.2%). The mean left ventricular end‑diastolic diameter was 50.6 ± 3.7 mm,

MRI diagnosis of preterm prelabor rupture of membranes and uterine puncture site-amniotic cavity fistula following fetoscopic laser photocoagulation.

Preterm prelabor rupture of membranes (PPROM) following fetoscopic laser photocoagulation (FLP) is a rare but potentially life-threatening complication. The occurrence of PPROM with a uterine puncture site–amniotic cavity fistula and localized fluid collection in the vesicouterine pouch has not been previously reported, and its diagnosis and management present significant clinical challenges. A 27-year-old primiparous woman with monochorionic diamniotic (MCDA) twin pregnancy was diagnosed at 17 weeks of gestation with Quintero stage I twin–twin transfusion syndrome (TTTS) complicated by type I

Higher neutrophil count, higher lymphocyte-to-monocyte ratio, and lower platelet-to-lymphocyte ratio are independently associated with postpartum depression symptoms in twin pregnancies.

INTRODUCTION: Postpartum depression (PPD) is a common complication with adverse maternal and child outcomes. Immune-inflammatory dysregulation has been implicated in its pathogenesis, but evidence from twin pregnancies is limited. This study aimed to investigate associations between routine prenatal inflammatory biomarkers and PPD symptoms in women with twin deliveries. METHODS: This retrospective cohort study included 846 women with twin pregnancies delivered between May 2022 and May 2024 at a tertiary hospital. PPD symptoms were screened within 4 weeks postpartum using the Edinburgh Postnat

Obstetrical outcomes after transvaginal natural orifice transluminal endoscopic surgery for benign gynecological conditions: a prospective cohort study.

BACKGROUND: Transvaginal natural orifice transluminal endoscopic surgery (vNOTES) is increasingly used for benign gynecologic conditions; however, evidence regarding subsequent obstetrical outcomes and the optimal interval between surgery and conception remains limited. This study aimed to evaluate pregnancy outcomes after fertility-preserving vNOTES and to assess whether the postoperative interval influences the risk of adverse outcomes. METHODS: This prospective cohort study included 326 women who conceived after fertility-preserving vNOTES performed for benign gynecological diseases with c

Fetoscopic Laser Surgery for Complicated Monochorionic Twin Pregnancies in Mothers Living with HIV: Case Series.

INTRODUCTION: Evidence on fetal surgery in mothers living with HIV is limited. All intrauterine invasive procedures carry a risk of fetal exposure to maternal blood, and therefore, a theoretical risk of vertical transmission must be considered. Currently, there is no published data on fetoscopic procedures in mothers living with HIV. CASE PRESENTATION: We present a multicenter case series of three monochorionic diamniotic twin pregnancies in mothers living with HIV, all complicated by twin-to-twin transfusion syndrome and treated with fetoscopic laser surgery. Early-pregnancy glucose-lipid signatures and metabolic heterogeneity of gestational diabetes with extension to hypertensive comorbidity in singleton and twin pregnancies: a multicenter cohort study.

BACKGROUND: Early pregnancy is a critical window for metabolic adaptation, marked by coordinated regulation of glucose and lipid metabolism. Disruption of this balance may contribute to gestational diabetes mellitus (GDM), a condition increasingly recognized as metabolically heterogeneous. Whether early-pregnancy glucose-lipid indices capture this heterogeneity and extend to hypertensive disorders of pregnancy (HDP) remains unclear. METHODS: This retrospective multicenter cohort included 13,419 pregnancies (11,765 singleton and 1,654 twin pregnancies). Fasting glucose and lipid measurements o

Prophylactic carbetocin versus oxytocin following vaginal delivery among women with high risk for postpartum hemorrhage: A before-and-after study.

INTRODUCTION: Data with a high level of evidence is lacking on the use of prophylactic uterotonic drug following vaginal delivery to prevent postpartum hemorrhage (PPH) among women who were considered high-risk for PPH. Our main objective was to compare the effectiveness of prophylactic carbetocin versus oxytocin in preventing PPH after vaginal delivery among women at high risk. MATERIAL AND METHODS: We conducted a retrospective before-and-after single-center comparative study, including all high-risk women after vaginal births. Two consecutive 14-month periods were compared, where the prophy

Weight-gain-for-gestational-age z-score in twin pregnancies in China and its association with preterm birth: a retrospective cohort study.

BACKGROUND: To evaluate the association between the weight-gain-for-gestational-age z-score and preterm birth (PTB) in a large Chinese twin pregnancy cohort. METHODS: A hospital-based retrospective study included 2832 twin pregnancies between January 1, 2015, and December 31, 2019. The primary composite outcome was defined as PTB. The total gestational weight gain was converted into z-score, and logistic regression models were used to examine the associations between z-scores and PTB. RESULTS: We developed a weight-gain-for-gestational-age z-score to determine the gestational weight gain dur

The impact of twin gestation on long-term maternal morbidity among women with hypertensive disorders of pregnancy.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are associated with increased maternal morbidity. While long-term maternal outcomes following HDP have been studied in singleton gestations, data regarding twin pregnancies remain limited. OBJECTIVE: To assess whether the risk for long-term maternal metabolic, cardiovascular, and vascular morbidity differs between women diagnosed with HDP during twin vs singleton gestations; to evaluate the association between HDP severity and future morbidity, and the risk for HDP recurrence. STUDY DESIGN: A retrospective cohort study included women who

Managing TRAP Sequence Conservatively: A Case Report of Full-Term Pregnancy With Favorable Outcome.

Acardiac twinning, also known as Twin reversed arterial perfusion (TRAP) sequence, is a rare anomaly seen exclusively in monochorionic twin pregnancies. It occurs due to abnormal placental vascular anastomoses, allowing retrograde perfusion from healthy "pump" twin to severely malformed twin lacking a functional heart. The prognosis is poor; mortality is 100% in the acardiac twin and 50%-55% in the pump twin. We present a case of 30-year-old gravida 3 parity 1 with previous 1 abortion (G3 P1 A1) having an acephalus acardiac twin diagnosed during routine second trimester ultrasound scan. Becaus

The Second Twin: Outcomes and Lessons in a Northern Nigerian Tertiary Hospital.

BACKGROUND: Twin pregnancy is traditionally high risk, and with rising incidence, there is a need for evaluating the outcomes as a marker of the quality of care offered. There is some evidence to suggest that the second twin is at higher risk of adverse fetal outcomes, though several studies, especially in more advanced healthcare settings, have debunked this. There is a need for robust evidence on second twin outcomes in low-resource settings like ours to enable standardization and improved quality of care, which will lead to improved fetal outcomes. The objective of this study is to compare
