Child With β-Thalassemia Major Born Following Hematopoietic Stem Cell Transplantation in the Mother: A Case Supporting the Expansion of Genetic Counseling Services in Lebanon and the Middle East.

We present a case of a woman who underwent a successful bone marrow transplant for beta-thalassemia major and later gave birth to a child who is compound heterozygous with HBB c.93-21G>A and HBB c.316-106C>G mutations to illustrate the critical gaps in reproductive risk assessment and genetic counseling. The prevalence of consanguinity, socioeconomic challenges, and the lack of a cohesive genetics health system in a thalassemia-endemic country all contribute to poor implementation of genetic care. Strengthening the education of healthcare practitioners, implementing centralized medical record

Case Report: Neonatal lethality caused by novel compound heterozygous missense variants of SLC6A9 gene.

BACKGROUND: The homozygous or compound heterozygous missense variants in the SLC6A9 gene (OMIM*601019) are associated with autosomal recessive glycine encephalopathy with normal serum glycine (OMIM#617301). Fewer than ten SLC6A9 variants have been reported to date. Neonates harboring pathogenic variants in the SLC6A9 gene exhibited a mortality rate of up to 75% during the first 7 months of life. We reported the first documented case of neonate death in China attributable to compound heterozygous missense SLC6A9 variants. METHODS: Trio-based whole-exome sequencing (trio-WES) was performed on g

Does nicotinamide riboside supplementation increase milk production in postpartum women with infants admitted to a neonatal intensive care unit? Protocol for the MOONRISE randomised, double-blind, placebo-controlled pilot feasibility trial.

INTRODUCTION: An adequate supply of a mother's own milk plays a critical role in optimising health outcomes for at-risk hospitalised infants. Insufficient milk production disproportionately affects the mothers of these infants. In the USA, metoclopramide is the only approved drug available for off-label use as a galactagogue. Nicotinamide riboside (NR), a direct precursor to nicotinamide adenine dinucleotide (NAD+), has been shown in a murine model to augment high-quality milk production and enhance cognitive and physical development of pups. Human testing with NR has been limited, and studies

Novel LARS2 variants in patients with Perrault syndrome: expanding the genetic spectrum and phenotypic heterogeneity.

BACKGROUND: Perrault syndrome (PS) is a rare autosomal recessive disorder characterized by sensorineural hearing loss (SNHL) and primary ovarian insufficiency in females. LARS2, encoding mitochondrial leucyl-tRNA synthetase, is the most common causative gene for PS. However, the genetic spectrum and clinical variability of PS remain underexplored. Expanding the catalog of LARS2 variants and correlating them with phenotypic data are critical for delineating genotype-phenotype relationships. METHODS: Two unrelated Chinese probands with hearing loss were enrolled, and comprehensive clinical eval

Effects of gestational exposure to individual and mixed PFASs on maternal liver function: evidence from the Wuxi Birth Cohort, animal studies, and toxicogenomic analyses.

Per- and polyfluoroalkyl substances (PFASs) are persistent environmental contaminants with potential hepatotoxicity. Their impact during pregnancy remains poorly characterized. We investigated 601 pregnant women from the Wuxi Birth Cohort, measuring serum concentrations of 23 PFASs and 11 liver function markers. Associations between individual PFASs and liver function markers were evaluated with generalized linear model (GLM) and generalized additive model (GAM), while mixture effects were assessed using weighted quantile sum (WQS) and Bayesian kernel machine regression (BKMR). Intrapartum Point-of-Care Ultrasound May Prevent Cord Prolapse in Active Labor: Report of a Case.

INTRODUCTION: Intrapartum point-of-care ultrasound (POCUS) is increasingly employed to assess fetal head position and labor progression. We describe a case in which intrapartum POCUS enabled the early detection of a compound cord presentation, preventing umbilical cord prolapse. CASE PRESENTATION: A 33-year-old primigravida at 39+4 weeks in active labor showed a persistent right occiput posterior position with slow progress despite adequate contractions. After a failed manual rotation attempt, transperineal ultrasound revealed a hypoechoic tubular structure lying between the fetal head and pu

The blaKPC-249 -mediated mechanism drives the transition of ST463 Pseudomonas aeruginosa from Ceftazidime-Avibactam sensitivity to resistance during clinical treatment.

INTRODUCTION: Ceftazidime-avibactam represents a crucial therapeutic option for managing infections attributable to carbapenem-resistant P. aeruginosa. Nonetheless, the emergence of resistance to ceftazidime-avibactam in P. aeruginosa presents a significant challenge for clinical anti-infective therapy. This study primarily elucidates the mechanisms by which P. aeruginosa transitions from drug sensitivity to resistance during ceftazidime-avibactam treatment, ultimately resulting in therapeutic failure. METHODS: The susceptibility testing was performed using the broth microdilution method, con

Lipidomics of allantoic fluid collected by allantocentesis indicates a possible mechanism underlying partial litter loss in multifetal pregnancies of ewes.

Fetal mortality significantly impacts reproductive efficiency in sheep, with approximately 30% of fetuses failing to survive until lambing. In multifetal gestation, partial litter loss (PLL) occurs in 28% to 37% of cases. The biochemical composition of allantoic fluid (ALF), which plays a key role in metabolic regulation and fetal waste clearance, has been shown in other species to reflect fetal development and maturity. However, ALF metabolomics in sheep, especially in relation to PLL, remains unexplored. This study investigated late-gestation fetal mortality in multifetal sheep pregnancies b

Management of Severe Congenital Protein C Deficiency with Continuous Subcutaneous Infusion via Insulin Pump: A Pediatric Case Report.

Severe congenital Protein C deficiency (SCPCD) is a rare autosomal recessive thrombophilia that typically presents in the neonatal period with early-onset life-threatening thrombotic complications. We report the case of a female infant who presented at birth with digital ischemic necrosis and laboratory evidence of consumptive coagulopathy consistent with neonatal purpura fulminans. Severe Protein C deficiency was confirmed by markedly reduced Protein C activity (<0.03 IU/mL) and compound heterozygous variants in the PROC gene. After initial stabilization and intermittent intravenous Protein C

Glutathione depletion under hypoxia via a birnessite-type manganese oxide nanozyme inducing immunogenic ferroptosis for magnetic resonance imaging guided cancer therapy.

UNLABELLED: Glutathione (GSH) depletion can inhibit glutathione peroxidase 4 (GPX4) activity, leading to the accumulation of lipid peroxides and the induction of ferroptosis. However, achieving efficient GSH depletion under the hypoxic conditions typical of tumor microenvironments using non-precious metal-based materials remains a significant challenge for effective cancer therapy. METHODS: We synthesized a two-dimensional birnessite-type nanosheet-like manganese oxide nanozyme (CMO). The multienzymatic activity and GSH consumption capacity of CMO were evaluated. Density functional theory cal

Diurnal rhythms in melatonin and cortisol in very preterm human milk.

INTRODUCTION: Human milk (HM) contains bioactive compounds and hormones vital for infant neuro- and circadian-development. While healthy term-born infants receive time-of-day cues naturally through breastfeeding, preterm infants in the Neonatal Intensive Care Unit (NICU) are typically fed with pooled expressed milk via tube feeding, which lacks time-of-day specificity. Despite growing interest in chrononutrition and circadian entrainment in the NICU, little is known about diurnal hormonal rhythms in preterm HM and their potential role in circadian development. METHODS: We investigated diurnal
