The magnitude and population burden of educational inequalities in adverse birth outcomes.

We aimed to describe the magnitude and population burden of inequality in adverse birth outcomes by maternal education (five categories). Data from 639,007 singleton births of gestational age ≥ 24 weeks or, if missing, birthweight ≥ 500 g were included from the National registry of the Netherlands, 2016-2019. One in six Dutch births had an adverse outcome. Each step down the educational ladder was associated with higher adverse outcome rates. Inequalities in stillbirth and neonatal mortality rates were large between the highest (Master's or higher) and lowest (primary) educated groups (rate ra

Care Bundles to Reduce Stillbirths and Improve Pregnancy Outcome.

This article explores the use of care bundles in maternity care to reduce stillbirths and improve pregnancy outcomes. Care bundles standardize multiple evidence-based interventions to enhance reliability and teamwork. Recent initiatives, including the UK's Saving Babies' Lives Care Bundle and Australia's Safer Baby Bundle, demonstrate improvements in clinical practice and their implementation may reduce stillbirths. However, evaluations identified challenges to their adoption. Future priorities include the need for robust evaluation of care bundles including their scalability and ability to ad

Divergent Paths: A Survey of Cardiologist and Obstetrician Decision-Making in High-Risk Pregnancies with Cardiovascular Disease.

BACKGROUND: During pregnancy, there is a physiologic increase in heart rate and blood flow rate along with an increase in plasma volume. Although these changes are physiological, pregnant women with congenital cyanotic heart disease are known to have an increased risk of maternal heart failure, premature birth, intrauterine growth retardation, increased number of cesarean sections, and even maternal death. According to the World Health Organization, cardiovascular diseases (CVD) are the most common cause of indirect maternal deaths. The aim of this study was to assess how well obstetricians an

Integrative analysis and experimental validation unveil TNFSF10 as a key PANoptosis inducer driving preeclampsia pathogenesis.

Preeclampsia, a life-threatening pregnancy-specific hypertensive disorder, poses significant risks to maternal and fetal health. Although apoptosis, necroptosis, and pyroptosis have been implicated in its pathogenesis, the involvement of PANoptosis, a newly identified inflammatory cell death pathway integrating these processes, remains unexplored. This study systematically investigated PANoptosis-related mechanisms in preeclampsia and identified potential biomarkers through an integrated approach combining placental transcriptomic analysis, clinical specimen validation, in vivo modeling (preec

"If there is no data, how do we improve?" Exploring health workers' perspective on stillbirth recording in the Ashanti Region of Ghana.

BACKGROUND: Health workers play a critical role in documenting the estimated 2 million stillbirths that occur annually. From the moment a stillbirth occurs, a health worker is responsible for recording the birth outcome. The reliability of stillbirth data for informing global and national-level strategies on stillbirths depends on the information recorded by the health worker at the point of care. This study aimed to gain insights into the health worker practices and challenges related to stillbirth recording and reporting. METHODS: The qualitative study explored three objectives using an a pr

Trimester-specific exposure to multiple heat indicators and adverse birth outcomes across four European countries.

The health of pregnant women and their fetuses is increasingly challenged by abnormal heat events. Few studies have disentangled the trimester-specific effects of heat exposure during pregnancy on perinatal outcomes and examined potential differences across multiple heat indicators. This study included singleton births from multiple national and region registries across Europe: Sweden (2014-2019), Belgium (2012-2022), Italy (Lazio Region) (2001-2019), and Greece (1999-2021). We obtained 2-m air temperature, Universal Thermal Climate Index (UTCI), Wet Bulb Globe Temperature (WBGT), and Heat Ind

Improving Maternal Care in Florida Through Levels of Maternal Care Verification.

BACKGROUND: In response to increasing maternal morbidity and mortality, the American College of Obstetricians and Gynecologists (ACOG) and the Society for Maternal Fetal Medicine (SMFM) developed an Obstetrical Care Consensus on Levels of Maternal Care (LOMC) outlining four levels of care. This standardization reduces preventable morbidity and mortality by establishing hospital capabilities to ensure the provision of risk-appropriate care. LOCAL PROBLEM: In 2020, Florida's Maternal Mortality Review Committee found that 68% of pregnancy-related deaths were preventable, with 22% attributed to s

Emotional and Environmental Safety and Side Effects from Massage for Women Pregnant After a Stillbirth or Termination for Medical Reasons After 20 Weeks' Gestation.

BACKGROUND: Stillbirth is the death, in utero, of a fetus from 20 weeks' gestation until immediately before birth. This may be spontaneous or can occur after termination for medical reasons (TFMR). The experience of stillbirth profoundly impacts subsequent pregnancies, with mothers frequently experiencing conflicted emotions, heightened anxiety, stress, symptoms of depression, fear, isolation, and a diminished sense of trust in a positive outcome. Supportive care options for individuals pregnant after a stillbirth or TFMR are needed to help meet their psychological and emotional needs. Individ

The quality of intrapartum care during stillbirth deliveries in conflict-affected Nigeria and Democratic Republic of Congo: A phenomenological study.

Stillbirths are traumatic, and their consequences can be exacerbated by inadequate care and support during and after such devastating loss. Little is known about these issues in conflict-affected regions. Qualitative interviews were conducted to understand the quality of intrapartum care among women with stillbirth deliveries in health facilities in conflict-affected areas of Yobe, Nigeria, and North-Kivu and South-Kivu, Democratic Republic of Congo. Thematic analysis guided by the WHO Quality of Care framework revealed variations in women's experiences of care. While some reported positive ex

Bisphenol S exposure before sexual maturity impairs follicular development via Cathepsin L-mediated granulosa cell death.

Bisphenol S (BPS) poses potential risks to the reproductive system, but its effects during pre-sexual maturity on follicle development and its underlying mechanisms remain unclear. This study aimed to investigate the association between pre-sexual BPS exposure and follicular dysplasia, as well as to decipher the molecular mechanisms involved. Urine samples from females aged 3-16 years were first analyzed, revealing a 95.7% detection rate of BPS, with a geometric mean concentration of 51.74 ng/L. Furthermore, the pre-sexual rat model revealed that BPS (0.05, 5, and 250 mg/kg body weight/day) re

Fetal Urinary Cystatin C, NGAL and Beta-2-Microglobulin as Predictors of Postnatal Renal Function Impairment and Death in Fetuses with Lower Urinary Tract Obstruction.

Background/Objectives: Fetal lower urinary tract obstruction (LUTO) is a rare congenital anomaly that often leads to pulmonary hypoplasia and kidney dysfunction, which contribute to increased mortality. Prenatal estimation of the severity of LUTO is challenging due to the lack of specific diagnostic tools, which may guide clinical decisions. The aim of this analysis was to assess the role of fetal urinary concentrations of neutrophil gelatinase-associated lipocalin (NGAL), β2-microglobulin (B2M) and Cystatin C (CysC) in the prediction of unfavorable outcomes, such as postnatal renal dysfunctio

A study of "Doctor Heli" air ambulance utilization and perinatal outcomes in Fukushima Prefecture, Japan.

Many countries have adopted the use of helicopter emergency medical services (HEMS). One such system in Japan is called "Doctor Heli" (Doctor Helicopter), which is sometimes used as a means of maternal transport. Since 2008, there have been 18 cases of Doctor Heli maternal transport to Fukushima Medical University Hospital, the ultimate referral center of Fukushima Prefecture. Reasons for transport included umbilical cord prolapse (UCP), placental abruption, traffic accident, and fetal bradycardia. There were two cases of intrauterine fetal death upon arrival at the hospital. Although there is
