Disseminated Pulmonary Coccidioidomycosis Presenting as a Wedge-Shaped Necrotic Mass Mimicking Malignancy.

Coccidioidomycosis is an endemic fungal infection of the southwestern United States that may rarely produce mass-like pulmonary abnormalities mimicking malignancy. We describe a 70-year-old Hispanic woman with poorly controlled type 2 diabetes mellitus (HbA1c >14%) who presented after three days of generalized weakness, nonproductive cough, abdominal pain, nausea, vomiting, and progressive difficulty ambulating. CT demonstrated a 7.3 × 5.4 cm wedge-shaped necrotic right middle lobe mass with multiple bilateral pulmonary nodules. CT-guided biopsy of the mass demonstrated granulomatous inflammat

Fosmanogepix: A comprehensive review of its origin, mechanism, antifungal efficacy, and resistance (Review).

The escalating burden of invasive fungal infections, compounded by rising antifungal resistance and evolving pathogen epidemiology, underscores the urgent need for novel agents. There is a requirement for these novel agents to combine broad-spectrum activity with >90% oral bioavailability and a 48% acceptable safety profile. Fosmanogepix, an N-phosphonooxymethylene prodrug of manogepix (APX001A), is a first-in-class glycosylphosphatidylinositol (GPI)-anchored wall transfer protein 1 (Gwt1) inhibitor in Phase III clinical trials for invasive fungal infections. It is a potent inhibitor of the fu

Aseptic Meningitis Unmasking Coccidioidomycosis of the Central Nervous System Outside an Endemic Region.

A 20-year-old male presented with persistent aseptic meningitis. Cerebrospinal fluid (CSF) showed neutrophil-dominant pleocytosis, high protein, low glucose, and an opening pressure >55 cmH2O. Fluid metagenomic next-generation sequencing detected Coccidioides immitis/posadasii and subsequent culture confirmed the diagnosis. The retrospective history revealed residence in Arizona approximately six months prior to hospitalization. Antifungal therapy was escalated, and the patient was transferred to a tertiary neurocritical care center for advanced neurocritical care. This case demonstrates that

mSphere of Influence: The age of antifungal resistance.

Norman van Rhijn works in the field of One Health relating to fungal pathogens and antifungal resistance studying the factors that shape the emergence and spread of fungal diseases. In this mSphere of Influence article, they reflect on how the work by M. E. Gorris, K. K. Treseder, C. S. Zender, and J. T. Randerson (GeoHealth 3:308-327, 2019, https://doi.org/10.1029/2019GH0002) made an impact on them by evidencing that environmental change directly impacted the change in habitat of Coccidioides immitis, the fungal pathogen responsible for Valley fever. This work has shaped their approach toward

Detection of Coccidioides immitis in Archived Soils from Madera County, California.

In the 1970s, archaeologists excavating sites in Madera County, California, which is now submerged by Hensley Reservoir, contracted coccidioidomycosis (Valley fever), a fungal disease caused by Coccidioides immitis. Soil samples from these sites were archived but not processed due to concerns about infectious arthroconidia. This study investigates whether these decades-old samples, stored at room temperature, still contain Coccidioides and pose a risk to researchers and collections managers. We analyzed 176 subsamples using molecular and culture-based methods, with DNA extractions performed in

Pembrolizumab for high TMB castration-resistant prostate cancer: A precision medicine case report.

BACKGROUND: A prolonged QTc interval has been reported in patients with liver disease. The objectives of our study were to determine whether a prolonged QTc interval was an independent predictor of mortality in patients with cirrhosis and to examine the effect of liver transplantation (LT) on QTc interval. PATIENTS AND METHODS: We retrospectively studied two cohorts of patients - QTc interval was measured in 409 patients (pre-transplant group), and in 162 patients (transplant group) before and 6 months after LT. QT interval (mean) corrected (QTc) for ventricular rate was read from a 12-lead E

Coccidioidomycosis: an emerging disease in Taiwan.

Coccidioidomycosis is an endemic fungal disease occurring almost exclusively on the American continent. Until 2021, only sporadic imported cases were observed in Taiwan. However, a dramatic surge in the number of cases over the past 3 years has been observed. Hence, we conducted a retrospective case series study between January 1, 2000, and May 31, 2025, at the National Taiwan University Hospital. Thirteen patients were included in the study. Most were workers or businessmen related to the Taiwan semiconductor factory expansion in Arizona, United States of America since 2020. All patients were

Valley Fever in Central California: Diagnosis is Not the Finish Line, the Real Gap is Longitudinal Care.

Coccidioidomycosis continues to shape life across California's southern San Joaquin Valley, where thousands fall ill each year. Yet for many, diagnosis marks not the end of uncertainty but the start of another challenge, staying in care. In communities already strained by clinician shortages, distance, and limited insurance coverage, the year-long follow-up recommended after Valley Fever is rarely feasible. Lessons from other rural Latino populations show that continuity can be rebuilt through community-rooted approaches: trusted health workers, mobile clinics, peer education, and simple digit

Simultaneous Liver and Kidney Transplantation for Cirrhosis Directly Attributed to Disseminated Hepatic Coccidioidomycosis.

Disseminated coccidioicomycosis can have devastating consequences following immunosuppression. Herein, we describe a kidney transplant recipient with subsequent allograft rejection and graft loss, who developed disseminated hepatic cocccidioidomycosis that led to progressive hepatic fibrosis and cirrhosis. He then underwent simultaneous liver and kidney transplantation while maintaining antifungal therapy. No other etiology for cirrhosis was determined following extensive testing that included his explanted liver.

Mixed mycotic infection in an immunocompromised host with advanced HIV diagnosed with the assistance of a Karius test.

We report the case of a 35-year-old female with advanced HIV diagnosed with a mixed mycotic infection. Diagnosis was assisted by Karius test. She was admitted with postpartum psychosis and developed fever and acute hypoxic respiratory failure initially thought to be from a bacterial hospital-acquired pneumonia. Due to lack of sufficient improvement on appropriate antibiotics, a broad infectious workup was sent. A Karius test was performed early during the workup. It returned positive for Pneumocystis jirovecii, Coccidioides posadasii, and Histoplasma capsulatum. Eventually a standard work up r

Killing of Coccidioides by drugs.

Coccidioidomycosis has long been regarded as one of the most difficult mycoses to eradicate in patients with progressive infection. There are few published studies of inhibition of Coccidioides in vitro, comparing the results with various drugs as measured by minimal inhibitory concentration (MIC). An ability to kill Coccidioides, at achievable concentrations in the host, may be necessary to produce a cure. Five hundred fifty-seven killing assays of Coccidioides clinical isolates in vitro, with minimum fungicidal concentrations (MFCs), are reported here. Heterogeneity of MFCs for all drugs is

Regional Increases in Incidence of Coccidioidomycosis (Valley Fever) - Arizona, 2005-2022.

Incidence of coccidioidomycosis (Valley fever), a fungal infection caused by inhalation of Coccidioides species spores, has increased substantially across the southwestern United States in association with increasing aridity, warming temperatures, and precipitation volatility. Arizona and California report >95% of U.S. coccidioidomycosis cases, and incidence in Arizona has increased statewide. Patterns within Arizona's distinct climatological regions have not been characterized, especially in regions outside the known zone of persistently high levels of disease occurrence (hyperendemicity) in

Hypothesis on the dust-borne atmospheric transport of Coccidioides, causative fungal pathogen for coccidioidomycosis (Valley fever).

For many decades, the fungal pathogen (Coccidioides spp.) that causes coccidioidomycosis (Valley fever) has been associated with dust exposure. However, the mechanism of atmospheric transport of arthroconidia has yet to be defined. As such, it is unclear whether the association between dust exposure and Coccidioides is coincidental or not: do soil-disturbing events simultaneously cause both free-floating Coccidioides conidia and dust particles (i.e., particulate matter) to become entrained in the atmosphere, or is Coccidioides attached to dust particles and they are jointly suspended? In this

Clinical evaluation of CTS-1 specific coccidioidal immunodiffusion reagents for improved sensitivity and enhanced agreement with complement fixation testing.

INTRODUCTION: Coccidioidomycosis is a fungal infection endemic to the southwestern United States and northern Mexico. Laboratory evaluation often includes serological methods including enzyme immunoassays (EIA), immunodiffusion (ID), and complement fixation (CF). This study evaluated a newly available chitinase-1 (CTS-1) specific ID assay for serodiagnosis of Coccidioides infections. MATERIALS AND METHODS: Sera from 1544 specimens were tested by traditional EIA, ID, CF assays, and simultaneously assayed using Cactus Bio CTS-1 ID reagents to compare the ability to identify seropositive patient

Variation in Management of Children With Coccidioidomycosis.

OBJECTIVE: Coccidioidomycosis (CM) is not as well described in children as it is in adults. The Kaiser Permanente Southern California (KPSC) system provides care within an endemic region for CM, affording the opportunity to describe the diagnosis, management, and outcomes of CM in a large pediatric cohort. METHODS: This study is a retrospective manual medical record review of patients aged 17 years or younger with a new diagnosis of CM between January 1, 2015, and December 31, 2021. We extracted demographic characteristics, symptoms, diagnostic testing, and details of patient management. RES
