The high-risk minority: undetected preterm SGA disproportionately contributes to adverse outcomes in a Chinese cohort.

INTRODUCTION: Small for gestational age (SGA) infants face higher risk of adverse outcomes compared to appropriate for gestational age (AGA) infants. Whether undetected SGA leads to worse clinical outcomes than detected SGA remains debated. This study compared perinatal outcomes between undetected and detected SGA in preterm and term pregnancies, and identified risk factors for missed diagnoses. MATERIAL AND METHODS: Singleton deliveries > 24 weeks at Peking University Third Hospital (January 2018-June 2023) were included. In this single-center retrospective study, participants were stratifie

Therapeutic effect and mechanism of different doses of aspirin on preterm delivery in pregnant mice.

BACKGROUND: Preterm birth is a major cause of perinatal mortality and complications, with inflammation being a key contributing factor. Current treatments, like uterine contraction inhibitors and antibiotics, are unsatisfactory. Aspirin, a cyclooxygenase inhibitor, shows promise in treating infectious preterm labor but has limited in vivo studies and an unclear mechanism. METHODS: In this study, a mouse model of infectious preterm birth was established via lipopolysaccharide (LPS) injection, and the aspirin doses used in these animals were converted from the recommended human doses by the bod

Organ-Specific Histopathological Effects of Prenatal Alcohol Exposure: A Narrative Review.

The role of alcohol-induced epigenetic modifications that predispose the fetus to metabolic dysregulation, increased susceptibility to future substance use, and long-term behavioral and cognitive impairments has received increasing attention. Fetal alcohol syndrome (FAS) and fetal alcohol spectrum disorders (FASDs) remain underdiagnosed worldwide. As evidence continues to demonstrate that no safe level of alcohol consumption exists during pregnancy, strengthening public awareness, maternal education, and prenatal care services has become increasingly imperative. Prenatal alcohol exposure (PAE)

Computational modelling of the fetal circulation: what could cardiovascular system models teach us about the heart-placenta axis?

Fetal life plays a pivotal role in shaping long-term health, with cardiovascular development exerting particular influence. Defects that arise during this critical window, such as congenital heart abnormalities or maladaptive remodeling related to placental dysfunction, increase vulnerability to cardiovascular disease, hypertension, and metabolic disorders later in life. Yet, despite the importance of early cardiovascular development, current diagnostic tools remain limited in their ability to detect or manage fetal cardiac dysfunction. Computational modelling offers a powerful means to addres

Fetal Intervention for Giant Chorangioma with Prenatal Ductus Arteriosus Closure: A Case Report.

INTRODUCTION: We present a case of spontaneous premature ductus arteriosus (DA) closure following modified interstitial laser ablation (ILA) of a giant chorangioma (GC) in a fetus with evidence of hydrops. CASE PRESENTATION: A 26-week ultrasound revealed a 10-cm GC in the posteriorly located placenta with signs of fetal hydrops, polyhydramnios, and elevated middle cerebral artery peak systolic velocity (MCA PSV) of 1.9 MoM. Preoperative fetal echocardiogram showed elevated combined cardiac output (753 mL/kg/min) without structural abnormalities. Modified ILA targeting the branches of the feed

Matrix metalloproteinase 2 governs extracellular matrix remodeling during ovarian development in a viviparous teleost Sebastes schlegelii (Hilgendorf, 1880).

BACKGROUND: Sebastes schlegelii (Hilgendorf, 1880), a viviparous fish with annual ovarian remodeling, shows marked activation of Extracellular Matrix (ECM)-receptor interaction signaling post-mating. However, the evolutionary conservation and functional significance of a key ECM remodeler-Matrix metalloproteinase 2 (MMP2) in this unique reproductive system are poorly understood. RESULTS: In this study, we identified the core members of the MMP family from the S. schlegelii genome and investigated their expression patterns and localization in ovarian tissues. The results demonstrated that the g

Abnormal Galectin Signaling in the Pathomechanisms of Placental Dysfunction in Gestational Diabetes Mellitus.

Recognition and binding to β-galactose-containing carbohydrates and lipids are crucial for several fundamental biological processes that are mediated primarily by a family of proteins known as galectins (S-type lectins). Galectins in the human placenta regulate critical processes such as maternal-fetal immune tolerance, trophoblast invasion, vascular remodeling and angiogenesis, ensuring proper fetal development and preventing pregnancy complications such as preeclampsia and miscarriage. Gestational diabetes mellitus (GDM) is a widespread complication of pregnancy, affecting approximately 1 in

Umbilical artery Doppler deterioration, time to delivery, and risk of fetal death in early-onset severe fetal growth restriction progressing to absent or reversed end-diastolic flow.

BACKGROUND: Umbilical artery Doppler plays a central role in the diagnosis and management of placenta-mediated early-onset fetal growth restriction. However, interpreting available data on the rate of umbilical artery Doppler progression remains challenging, primarily because many prior studies included patients with varying degrees of fetal growth restriction severity. Consequently, the aggregated data resulted in estimates of umbilical artery Doppler progression that are often too broad for clinical use. Therefore, there is a need for more robust data to define the deterioration rate of umbi

Advanced maternal age and assisted reproductive technologies: outcomes, genomics, and real-world evidence.

INTRODUCTION: Advanced maternal age (AMA) and assisted reproductive technologies (ART) are becoming more common and are linked with higher obstetric and perinatal risks. Genomic tools and real-world data (RWD) are transforming risk prediction and care strategies, yet their application to the AMA + ART setting remains uneven. OBJECTIVE: To synthesize recent evidence on risks associated with AMA and ART, incorporate insights from reproductive genomics and RWD, and discuss their implications for clinical care and guideline development. METHODS: Narrative review of cohort studies, systematic rev

Comparison of live birth and perinatal outcomes between fresh oocyte donation followed by frozen embryo transfer and frozen oocyte donation with fresh embryo transfer.

OBJECTIVE: To compare clinical outcomes between the two most commonly employed oocyte donation protocols: fresh oocyte donation followed by frozen embryo transfer (fresh OD - frozen ET) and frozen oocyte donation with fresh embryo transfer (frozen OD - fresh ET). METHODS: This retrospective cohort study was carried out at a university-affiliated reproductive center and included 507 patients who underwent donor oocyte cycles. The primary endpoint was cumulative live birth rate (CLBR), which was defined as the proportion of patients achieving at least one live birth per initiated oocyte donatio

Mild SARS-CoV-2 maternal infection in mice induces transient offspring neurodevelopmental aberrance.

Maternal viral infection during pregnancy has been identified as a risk factor for psychiatric disorders and neurodevelopmental abnormalities in offspring. With cumulative SARS-CoV-2 infections now numbering in the hundreds of millions globally, there is a need to evaluate the effects of maternal SARS-CoV-2 infection on offspring brain development and behavior. We developed a mouse model of mild COVID-19 during pregnancy in which SARS-CoV-2 infection is restricted to the respiratory tract. Infected mothers did not show weight loss or changes in litter size, but did exhibit detectable local and

Unveiling of Placental-Fetal Heart Interplay: A Novel Etiologic and Therapeutic Insight-A Narrative Review.

Congenital heart disease (CHD) is the most common fetal anomaly worldwide. The definite etiology of most CHD is not recognized. A direct genetic etiology is considered for a minority of patients. Most etiologies are attributed to epigenetic and environmental factors. Placental malformation is an overlooked cause of CHD that has recently received attention. This narrative review presents a hypothesis based on clinical reports and animal studies. The placenta and fetal heart have concomitant developmental regulatory pathways, and their diseases have a two-way communication. Placental insufficien

Maternal Med12 safeguards trophoblast pluripotency and placental development.

For a brief but critical period post-fertilization, the mammalian embryo is entirely dependent on maternal products inherited from the oocyte. Previous research showed that oocyte-specific loss of Med12, an X-linked gene and Mediator complex subunit, leads to female sterility despite normal folliculogenesis and ovulation. Here, we show that loss of maternal Med12 has minimal effect on the oocyte transcriptome and does not manifest in embryonic lethality until post-implantation. Implants derived from Med12-null oocytes demonstrate abnormal placentation at E9.5, with an overabundance of trophobl

IGFBP3 repression driven by inflammation links air pollution to placental and developmental defects.

Air particulate matter (PM2.5 and PM10), can cross the placental barrier, triggering oxidative stress and inflammation that compromise fetal development. These insults lead to placental dysfunction and complications including preterm birth, low birth weight, and preeclampsia. In cell line and placental explant models, urban particulate matter (UPM) increased pro-inflammatory cytokines and oxidative stress pathways, impairing trophoblast invasion, angiogenesis, and nutrient transport, while also altering epigenetic modifications and endoplasmic reticulum function. Rodent studies revealed reduce

Miscarriage and the Microbiome: Host Genetics, Immunity, and the Reproductive Tract Ecosystem.

Background/Objectives: Pregnancy loss is a common and multifactorial complication of human reproduction, traditionally attributed to fetal chromosomal abnormalities, maternal anatomical and endocrine disorders, and immune dysfunction. Growing evidence now indicates that the maternal microbiome, particularly within the reproductive tract, plays a critical role in implantation, placental development, and the maintenance of immune tolerance during early pregnancy. Importantly, the influence of the microbiome on miscarriage appears to be strongly modulated by host genetic background and immune reg

Birthweight to placental weight ratio and placental pathology in clinically unanticipated stillbirths: a retrospective cohort study.

INTRODUCTION: Clinically unanticipated stillbirths may result from placental dysfunction that is not detected during pregnancy. We investigated the association between clinically unanticipated stillbirths and placental dysfunction assessed by the birthweight-to-placental-weight (BWPW) ratio and placental histopathology. METHODS: This retrospective cohort study included placentas from 295 stillbirths at ≥24 weeks' gestation (2000-2017) at a tertiary referral centre. Placental pathology reports were retrieved from a national registry and re-evaluated according to the Amsterdam consensus criteri

Di-2-ethylhexylphthalate-induced miR155-5P promotes placental ferroptosis.

Growing attention has focused on DEHP's impact on women's early life stages, particularly its reproductive toxicity linked to oxidative stress and ferroptosis. This study explored DEHP's effect on female reproduction and its relationship with miR155-5P and ferroptosis. To address this, experiments were conducted using HTR-8/Svneo trophoblast cells and pregnant rat models. Key methods included CCK8, Western blot, ELISA, flow cytometry, qPCR, and dual-luciferase assays to assess cell proliferation, ferroptosis markers, and molecular interactions. Additionally, placental tissue changes of pregnan

Placenta Biology, Future Cardiovascular Health, and Impact on the Maternal-Fetal Heart.

The placenta is a highly specialized organ that ensures the developing fetus gets adequate oxygen and nutrients from its mother. The placenta shares common development pathways with the fetal heart and releases factors that can influence the way the mother's heart functions. Thus, pregnancy complications, where the placenta fails to develop properly, can lead to the development of fetal heart defects and induce long-term maternal cardiac dysfunction. In this review, we focus on how the placenta shapes cardiac events during development and the subsequent long-term impact on the offspring's hear
