Monochorionic Diamniotic Twins with Sex Discordance: Case Series.

Background and Clinical Significance: Ultrasonographic diagnosis of twin pregnancies has become routine, with chorionicity playing a crucial role in assessing associated risks. Traditionally, monochorionic (MC) twins were believed to derive from a single zygote, ensuring sex concordance. However, recent cases of dizygotic monochorionic (MCDZ) twins challenge this paradigm. In this paper, four cases of MCDZ twins with sex discordance are described. Case presentation: Case 1 and case 2 describe two spontaneous MC/diamniotic twin pregnancies in which sex discordance between twins was attributed t

Cardiac function and neurological development in complicated monochorionic pregnancies: Current evidence and clinical implications.

Monochorionic twin pregnancies, characterized by a shared placenta and unique vascular architecture, face significantly elevated risks of perinatal complications compared to dichorionic and singleton pregnancies. This review examines the pathophysiology, cardiovascular, and neurological adaptations in three primary complications of monochorionic pregnancies: Twin-to-Twin Transfusion Syndrome (TTTS), Selective Fetal Growth Restriction (sFGR), and Twin Anemia Polycythemia Sequence (TAPS). TTTS disrupts hemodynamic balance, leading to distinct cardiac dysfunctions and increased neurodevelopmental

Update on Management and Outcomes of Monochorionic Twin Pregnancies.

The management of multiple pregnancies complicated by monochorionicity continues to evolve as new investigations support a change in clinical practice to optimize outcomes. Monochorionic twins are at risk of unique conditions such as monoamnionicity, conjoined twinning, twin reversed arterial perfusion sequence, twin-twin transfusion syndrome, twin anemia-polycythemia sequence, unequal placental sharing with discordant twin growth or selective fetal growth restriction, and single-twin death that puts co-twins at risk of death or neurologic injury attributable to the shared placenta. Contempora

Changes in sVEGFR-1 and sVEGFR-2 Levels Following Fetoscopic Laser Photocoagulation in Twin-to-Twin Transfusion Syndrome: Implications for Fetal demise Prediction.

BACKGROUND: Twin-to-twin transfusion syndrome (TTTS) is a severe complication in monochorionic twin pregnancies, leading to high perinatal morbidity and mortality. Fetoscopic laser photocoagulation (FLP) is the gold standard treatment; however, fetal demiseremains a concern. The soluble vascular endothelial growth factor receptors, sVEGFR-1 and sVEGFR-2, play a crucial role in regulating angiogenesis and vascular function. This study evaluates changes in sVEGFR-1 and sVEGFR-2 levels before and after FLP and explores their role in predicting fetal demise post-surgery. OBJECTIVE: Therefore, thi

Selective Fetoscopic Laser Photocoagulation of Placental Anastomoses Leads to Early Reduction of Cardiovascular Burden in the Twin-Twin Transfusion Syndrome.

OBJECTIVE: Cardiovascular disturbances are common in twin-twin transfusion syndrome (TTTS). However, the rate of improvement in cardiovascular burden in response to selective fetoscopic laser photocoagulation (SFLP) is not well known. METHOD: Fetal echocardiograms were performed prior to and 1 week following SFLP. Cardiovascular burden was characterized using the Children's Hospital of Philadelphia (CHOP) TTTS Cardiovascular (CV) Score. Pulsatility indices (PI) of the umbilical artery (UA) and middle cerebral artery (MCA), cerebroplacental ratio (CPR) and elements of CHOP CV Score were analyz

Management of twin-to-twin transfusion syndrome: update and current challenges.

Since a 2004 randomized trial established fetoscopy as the gold standard for the treatment for twin-to-twin transfusion syndrome (TTTS), advancements in surgical technique and expert training have considerably improved survival rates to 89% for at least one fetus and 65% for both fetuses. However, many challenges remain. This review provides an update on the current principles, issues, and debates. The Solomon technique has helped improve neonatal outcomes by reducing the risks of postoperative twin anemia-polycythemia sequence and TTTS recurrence. Nevertheless, fetoscopic laser photocoagulati

Pregnancy Characteristics and Outcomes in Monochorionic Diamniotic Twin Pregnancies Complicated by Proximal Placental Cord Insertions From a Single Center.

BACKGROUND: Proximal placental cord insertion (pPCI), defined as a ≤4 cm distance between umbilical cord insertions, is a distinct anomaly observed in monochorionic diamniotic (MCDA) twin pregnancies, characterized by the umbilical cords of both fetuses inserting in close proximity within the placental tissue. There is limited available data on the pregnancy characteristics and outcomes in monochorionic diamniotic twins with proximal placental cord insertions (pPCI). OBJECTIVES: To investigate pregnancy characteristics in monochorionic diamniotic (MCDA) twins complicated by pPCI and compare o

Hybrid-MedNet: a hybrid CNN-transformer network with multi-dimensional feature fusion for medical image segmentation.

Twin-to-twin transfusion syndrome (TTTS) is a complex prenatal condition in which monochorionic twins experience an imbalance in blood flow due to abnormal vascular connections in the shared placenta. Fetoscopic laser photocoagulation is the first-line treatment for TTTS, aimed at coagulating these abnormal connections. However, the procedure is complicated by a limited field of view, occlusions, poor-quality endoscopic images, and distortions caused by artifacts. To optimize the visualization of placental vessels during surgical procedures, we propose Hybrid-MedNet, a novel hybrid CNN-transfo

A Case Report of a Giant Placental Chorioangioma: Prenatal Diagnosis and Perinatal Outcome.

Chorioangiomas are benign placental vascular tumors, most commonly detected during antenatal ultrasound screening. While small lesions are often clinically insignificant, giant chorioangiomas defined as those exceeding 4 cm in diameter can be associated with serious fetal and maternal complications, including fetal anemia, thrombocytopenia, fetal heart failure, fetal hydrops, placentomegaly, polyhydramnios, maternal mirror syndrome, fetal demise, and neonatal death. Doppler ultrasound and magnetic resonance imaging aid in differentiating these tumors from other placental lesions. Treatment is

Complications in monochorionic twin pregnancies.

Monochorionic placentas occur in two-thirds of identical (monozygotic) twin pregnancies and typically feature inter-fetal vascular anastomoses on their surface. These anastomoses can cause very specific complications including twin-to-twin transfusion syndrome (TTTS) in about 10 %, TAPS in 5 %, twin reversed arterial perfusion (TRAP) sequence in 2.5 % and spontaneous intrauterine fetal death (IUFD) in 2 % of monochorionic-diamniotic twin pregnancies. Moreover, unequal sharing of placental territories can lead to selective fetal growth restriction (sFGR) in about 15 %. Monochorionic multiples a
