Peripartum Hysterectomy among Patients Admitted to the Department of Obstetrics and Gynaecology in a Tertiary Care Centre: A Descriptive Cross-sectional Study.

INTRODUCTION: Emergency peripartum hysterectomy is a life-saving procedure performed as an emergency procedure to control torrential bleeding and it is associated with significant maternal morbidity and mortality. There are only a few studies regarding this topic so this study guides us to monitor the trend and start appropriate policies to reduce unnecessary caesarean deliveries. The aim of this study was to find out the prevalence of peripartum hysterectomy among patients admitted to the Department of Obstetrics and Gynaecology in a tertiary care centre. METHODS: A descriptive cross-section

Association between anesthetic method and postpartum hemorrhage in Korea based on National Health Insurance Service data.

BACKGROUND: Postpartum hemorrhage (PPH) is a major cause of maternal mortality and the risk factors for PPH differ among studies. In this large-scale study, we investigated whether the anesthetic method used was associated with PPH after cesarean section. METHODS: We extracted data on cesarean sections performed between January 2008 and June 2013 from the National Health Insurance Service database. The anesthetic methods were categorized into general, spinal and epidural anesthesia. To compare the likelihood of PPH among deliveries using different anesthetic methods, crude and adjusted odds r

Cervical tourniquet during cesarean section to reduce bleeding in morbidly adherent placenta: a pilot study.

OBJECTIVE: To evaluate a modified surgical technique aiming to reduce bleeding and preserve fertility in morbidly adherent placenta by cervical tourniquet in cesarean sections. METHODS: The cesarean section operations and the cervical ligation approach were performed by a single expert consultant obstetrician. The general demographics and clinical characteristics for all participants were collected and studied. RESULTS: Eleven participants were involved. The uterus was preserved in nine patients, whereas two patients had hysterectomy. The mean blood loss was 1688.8 ml for patients whose uter

Cesarean scar pregnancy with expectant management.

AIM: This study aimed to ascertain whether the lower anterior myometrial thickness (MT) between the bladder and the gestational sac in early pregnancy can be used to predict clinical outcomes in women with cesarean scar pregnancy (CSP) after expectant management. METHODS: We retrospectively analyzed the clinical data and early pregnancy ultrasound images of 21 patients who received expectant management for CSP. Among them, 11 patients with serious complications during pregnancy, such as intraoperative blood loss ≥1000 mL or with severe forms of morbidly adherent placenta (MAP; placenta incret

Retained Morbidly Adherent Placenta Presenting as a Myometrial Mass in Patients With Vaginal Bleeding: A Case Series and Review of Current Literature.

The differential diagnosis for a uterine mass and vaginal bleeding after abortion or delivery is broad and includes both benign and malignant causes. A rare etiology for this condition is retained morbidly adherent placenta. Few cases of retained morbidly adherent placenta presenting as a myometrial mass in the setting of vaginal bleeding have been described in the medical literature. In this case series and review of the current literature, we describe the ultrasound features of 3 retained morbidly adherent placentae, along with correlative magnetic resonance imaging findings.

Characterization of cord blood CD3+ TCRVα7.2+ CD161high T and innate lymphoid cells in the pregnancies with gestational diabetes, morbidly adherent placenta, and pregnancy hypertension diseases.

PROBLEM: Although pregnant women with gestational diabetes (GD), morbidly adherent placenta (MAP), and pregnancy hypertension (pHT) diseases lead to intrauterine growth restriction (IUGR), little is known about their effect on mucosal-associated invariant T (MAIT) and innate lymphoid cells (ILC) in the umbilical cord. This study aimed to quantify and characterize MAIT cells and ILCs in the cord blood of pregnant women with GD, MAP, and pHT diseases. METHOD OF STUDY: Cord blood mononuclear cells (CBMCs) were isolated by Ficoll-Paque gradient. CD3+ TCRVα7.2+ CD161high cells and ILC subsets were

A Penance to Misdeed: A Case Report About Placenta Percreta.

Placenta percreta is a condition in which the placenta penetrates through the myometrium and into the uterine serosa. Sometimes it can be complicated by attachment to the surrounding structures or organs. The incidence of this condition is on the rise because of increased rates of cesarean sections. A 29-year-old gravida two, para one, living one with a previous cesarean section at 39 weeks of gestation was referred from a primary health center with complaints of leaking per vaginum for ten hours. She had no other associated symptoms. Her antenatal period was uneventful with all routine invest

Comparison of One-Stage and Two-Stage Intraoperative Uterine Artery Embolization during Cesarean Delivery for Placenta Accreta: Report of Two Clinical Cases at a Tertiary Referral Medical Center.

Placenta accreta spectrum (PAS) described the anchoring placental villi attached or penetrating into/through the myometrium. PAS is clinically important because of the unpredictable bleeding amount when manually removing the defective decidualization at the endometrial-myometrial interface. Therefore, a multidisciplinary strategy for cesarean delivery with PAS is crucial. Postoperative embolization after cesarean hysterectomy in a hybrid suite was studied by many scientists. In this study, we demonstrated two cases of intraoperative embolization without hysterectomy in a hybrid operating room

A Clinical Decision Support System (CDSS) for Unbiased Prediction of Caesarean Section Based on Features Extraction and Optimized Classification.

Источник пометил публикацию как отозванную

Nowadays, caesarean section (CS) is given preference over vaginal birth and this trend is rapidly rising around the globe, although CS has serious complications such as pregnancy scar, scar dehiscence, and morbidly adherent placenta. Thus, CS should only be performed when it is absolutely necessary for mother and fetus. To avoid unnecessary CS, researchers have developed different machine-learning- (ML-) based clinical decision support systems (CDSS) for CS prediction using electronic health record of the pregnant women. However, previously proposed methods suffer from the problems of poor acc

Morbidly Adherent Placenta in the First Trimester with Consecutive Hysterectomy.

Background Morbidly adherent placenta (MAP) represents a risk factor for a maternal adverse outcome and its incidence continues to rise following the increasing rate of caesarean deliveries. The detection of a pathology of placental adherence in the first trimester is challenging. Transvaginal ultrasound represents (TVUS) a reliable tool for accurate and timely diagnosis. Case We report on a case of MAP in a pregnant woman at 10 weeks of gestation with two prior caesarean deliveries. She presented with abdominal pain and hematometra. The first trimester diagnosis was made with TVUS and confirm

A retrospective analysis of maternal complications and newborn outcomes of general anesthesia for cesarean delivery in a single tertiary hospital in China.

BACKGROUND: Either neuraxial anesthesia or general anesthesia can be performed for cesarean delivery. Generally, neuraxial anesthesia is the first choice with the risk and benefit balance for both the mother and fetus. However, general anesthesia is also applicable most commonly in the emergent setting. This study analyzed maternal complications associated with general anesthesia for cesarean delivery and suggested lowering pregnancy-related maternal and newborn adverse outcomes. METHODS: With the approval of the Institutional Ethics Review Board (No: 2017016), data on cesarean delivery and r

Is conservative management of placenta accreta spectrum disorders practical in the United States?

This commentary discusses the issues related to conservative management (also called leaving the placenta in situ or intentional retention of the placenta) of placenta accreta spectrum disorders. Considerations related to placenta accreta spectrum disorder management in the United States are compared with France, where conservative management is a well-accepted management option. The history of placenta accreta spectrum disorder treatment is reviewed, finding that since 1937, the most common treatment in the United States been cesarean-hysterectomy without placental removal. Although definitiv

Haematuria from early pregnancy in a morbidly adherent placenta: Case report and a short review.

The morbidly adherent placenta has evolved into one of the potentially life-threatening conditions in obstetrics. Vaginal bleeding is the most common presentation of placenta percreta. Other symptoms include unusual dull, prolonged lower abdominal pain. Although haematuria is rare, it may be a presentation of the morbidly adherent placenta in early pregnancy. We report a case of placenta percreta with a very uncommon presentation of gross haematuria early in the second trimester managed successfully. It is emphasized that a high index of suspicion, ultrasound, and/or MRI can establish a preope

Expectant management of viable cesarean scar pregnancies: a systematic review.

OBJECTIVE: Cesarean scar pregnancy (CSP) is a potentially life-threatening disease that has been steadily increasing in prevalence. Pregnancy termination is usually recommended given the risk of life-threatening complications. In some cases, patients refuse to terminate viable CSPs, even after counseling. Recent studies report that, even with a high burden of possible complications and maternal morbidity, many CSPs progress to live, close to term births. The aim of this study is to further demonstrate the natural history of viable cesarean scar pregnancies. METHODS: We conducted a systematic

Position and integrity of uterine scar are determined by degree of cervical dilatation at time of Cesarean section.

OBJECTIVE: Abnormal placental invasion is more common after an elective Cesarean delivery, suggesting that prelabor Cesarean section (CS) increases the likelihood of the CS scar being above the internal cervical os and predisposing to a scar pregnancy in the future. The aim of this study was to assess the location and integrity of the CS scar in postpartum women delivered by CS at different stages of labor. METHODS: This was a prospective cohort study of women at term who underwent a CS for the first time. In all women, cervical dilatation was determined by digital examination at the time of
