Hypoxic-ischemic encephalopathy following intrapartum asphyxia: is it avoidable?

BACKGROUND: The proportion of term hypoxic-ischemic encephalopathy arising during intrapartum fetal surveillance remains unclear. Moreover, recent Cochrane review and other studies emphasized the need for research on the impact of admission cardiotocography and highlighted the necessity for a definition of "avoidable perinatal brain injury". OBJECTIVE: To assess the impact of intrapartum asphyxia on neonatal hypoxic-ischemic encephalopathy occurrence and identify the proportion of cases that benefit from preventive measures. STUDY DESIGN: This retrospective 20-year birth cohort study include

Early Postpartum Glucose Tolerance Reclassification by Gestational Diabetes Subtype.

IMPORTANCE: Gestational diabetes (GD) is a heterogeneous condition that predisposes both mother and offspring to metabolic disorders. GD subtypes defined by antepartum testing results have been associated with adverse perinatal outcomes, but little is known about their relationship to maternal metabolic outcomes soon after pregnancy. OBJECTIVE: To evaluate early postpartum glucose tolerance reclassification of GD subtypes. DESIGN, SETTING, AND PARTICIPANTS: This cohort study examined women from the Study of Women, Infant Feeding, and Type 2 Diabetes Mellitus After GD Pregnancy (SWIFT), who w

When the path is blocked: A case of a transverse vaginal septum diagnosed in labor.

Transverse vaginal septa are rare, with incidence estimated to be between 1 in 30 000 to 1 in 84 000 people. Despite much literature in the gynecologic setting, there is little guidance about how to manage them when diagnosed in labor. A 23-year-old G1P000 patient presented to Labor and Delivery at 40 weeks 0 days with contractions and vaginal bleeding. A sterile bimanual exam found a blind vaginal ending, with no identifiable cervix. A point of care transvaginal ultrasound showed a transverse septum that measured 0.39 cm in thickness, with a normal cervix behind the septum. The patient was of

Analysis of Risk Scoring for Placenta Accreta Spectrum and Its Impact on Maternal and Fetal Outcomes.

BACKGROUND: Placenta accreta spectrum (PAS) disorders, characterized by abnormal adherence or invasion of the placenta into the myometrium, are associated with significant maternal and neonatal morbidity. Early identification of high-risk cases is essential to optimize delivery planning and reduce adverse outcomes. The Placenta Accreta Index (PAI), integrating clinical history and ultrasound parameters, has been proposed as a predictive tool for PAS. This study aimed to evaluate the diagnostic accuracy of the PAI in predicting PAS and its utility in anticipating maternal and neonatal outcomes.

Factors associated with stillbirth in four selected hospital maternity units: an unmatched case-control study in Eswatini.

BACKGROUND: Stillbirth is a substantially under-recognized adverse pregnancy outcome that predominantly occurs in low-middle-income countries. In 2019, Eswatini's stillbirth rate was 13.2 per 1,000 births, higher than global targets. However, there is limited research on stillbirths in Eswatini. This study analyzed factors associated with stillbirths among women giving birth in four selected hospitals. METHODS: A multi-center unmatched case-control study was conducted using secondary data sources from July 1 to December 31, 2021. Birth records of 268 stillbirths (cases) and 1,151 live births

The rate of stillbirth and its associated factors in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Globally, about 1.9 million stillbirths occurred at the end of 2021. This scenario has been worst in Sub-Saharan African countries, which contribute about 47% of the total stillbirths. In Ethiopia, though there was a previous study on the rate of stillbirths and its associated factors, it only explored the effect of antenatal care visits and previous stillbirth history. Yet, the meta-effects of other contributing factors such as antepartum hemorrhage, meconium-staining, labor duration, body weight, hypertension, prematurity, and premature rupture of membranes have not been investig

Application of the Surgical APGAR Score to Predict Intensive Care Unit Admission and Post-Operative Outcomes in Cesarean Hysterectomy for Placenta Accreta Spectrum.

Background and Objective: Placenta Accreta Spectrum (PAS) encompasses a continuum of abnormal placentation conditions associated with significant maternal and fetal morbidity. Management of PAS requires coordinated cesarean hysterectomy. Associated morbidities include blood transfusion, coagulopathy, and intensive care unit (ICU) admission. Accurate prediction of ICU admission allows for enhanced multidisciplinary management, coordination of care and utilization of resources. Scoring systems exist in other surgical specialties that can predict the likelihood of ICU admission, but these have no

Plateletpheresis for treating essential thrombocythemia that developed during pregnancy: A case report.

OBJECTIVE: Essential thrombocythemia (ET), a life-threatening disease, is associated with increased risks of hemorrhagic and thromboembolic complications. In particular, ET during pregnancy is rare and associated with an increased risk of obstetric complications. Drug therapies are frequently administered for managing pregnant women with ET, and plateletpheresis is considered to reduce platelet (PLT) counts rapidly. CASE REPORT: A 28-year-old pregnant woman was referred to our hospital at 37 + 0/7 gestational weeks (GWs) due to severe thrombocythemia. She underwent plateletpheresis at 37 + 4/

How to Manage Venous Thromboembolism Risk during Pregnancy in Patients with Inherited Antithrombin Deficiency?

Inherited antithrombin deficiency (ATD) is associated with a high risk of venous thromboembolic complications. Association of ATD with other conditions such as pregnancy obviously increases thromboembolic risk and may require anticoagulant therapy for prevention. Although there are several/heterogenous international guidelines regarding thromboprophylaxis in pregnant patients with ATD, data on anticoagulant prophylaxis in this context are scarce in the literature. Thus, this situation remains a challenge both in the antepartum period and during delivery. Physicians from the French Society of T

Coagulation Disorders in Pregnancy.

Coagulation disorders cause significant morbidity and mortality during pregnancy, and evaluation of hypercoagulability or bleeding are frequently encountered in females of reproductive age. The physiologic changes of the hemostatic system during pregnancy increase the risk of venous thromboembolism and also make laboratory test result interpretation challenging. The risk of thrombosis can be reduced with anticoagulation, but appropriate risk stratification to assure that the benefits outweigh the risks is required. Only patients with severe bleeding disorders typically need preventive manageme

Factor XIII Supplementation in Postpartum Hemorrhage: From Biological Rationale to Clinical Implementation.

Postpartum hemorrhage (PPH) remains the leading cause of preventable maternal mortality despite standard interventions. Recent fibrinogen trials failed to improve outcomes, prompting interest in coagulation factor XIII (FXIII). FXIII functions as "molecular cement," cross-linking fibrin and stabilizing clots. During pregnancy, FXIII activity decreases 20%-30%, with further depletion during PPH. Observational studies show low antepartum FXIII predicts bleeding risk, while ex vivo supplementation restores clot firmness. The SWIFT trial (NCT06481995) represents the first randomized controlled tri

Maternal and fetal outcomes in pregnant women with mechanical heart valves: A tertiary center experience.

OBJECTIVE: Patients with mechanical heart valves require lifelong anticoagulation to prevent thromboembolic complications. Pregnancy induces a hypercoagulable state, increasing the risk of those events. Although guidelines recommend anticoagulation with vitamin K antagonists (VKA), recent practice involves replacing VKA with low-molecular weight heparin (LMWH) to reduce fetal complications with an increase in maternal complications. This study aims to analyze outcomes in pregnant patients with mechanical heart valves anticoagulated with VKA or LMWH. METHODS: This was a retrospective case seri

Maternal outcomes among women with intellectual disabilities in comparison with the general population (IDcare).

BACKGROUND: Women with intellectual disabilities face significant barriers to gynecological, reproductive, antenatal, and perinatal care, which may adversely impact maternal and fetal health. Previous research indicates increased risks for gestational diabetes, pre-eclampsia, caesarean birth, preterm birth, and other complications in pregnant women with intellectual disabilities. However, studies on the reproductive health in this group remain scant, and comprehensive research on maternal and fetal health from pregnancy to the postpartum period remain missing. OBJECTIVES: Using high-quality d

Foetal disruptive brain injuries: Diagnosing the underlying pathogenetic mechanisms with cranial ultrasonography.

Antenatal destructive events affecting the central nervous system of the foetus lead to disruptive brain lesions that are often associated with impaired neurodevelopment. The pathogenesis of these lesions encompasses a range of causes, including haemorrhagic, embolic, or other vascular events; exposure to teratogens, such as drugs or substance abuse; congenital brain infections; genetic conditions; and metabolic disorders. Cranial ultrasonography is the first-line imaging modality to diagnose these antepartum brain lesions in the newborn infant; it is often complemented by brain magnetic reson
