Contrary to current guidance, antenatal factor IX levels increase in female carriers of hemophilia B.

BACKGROUND: Current international hemophilia guidelines state that antenatal factor (F)IX (FIX) levels do not increase in carriers of hemophilia B (HB). However, the evidence base for this statement comes from 10 publications (many of which are isolated case reports) that included both baseline and antenatal plasma FIX coagulant activity (FIX:C) levels in a total of only 30 HB carriers. OBJECTIVES: To review the change, if any, in antenatal FIX:C levels in a large cohort of Irish HB carriers. METHODS: Following ethical approval, all parous HB carriers were identified from the Irish registry.

Risk Factors Associated with Development of Disseminated Intravascular Coagulation (DIC) in Obstetrical Cases.

Disseminated intravascular coagulation (DIC) is a fatal clinical syndrome which is caused by many medical, surgical, and obstetric issues. In pregnancy, DIC constitutes one of the leading causes for maternal mortality globally. It is triggered by enhanced activities of the coagulation cascade which consequently predisposes to widespread microvascular thrombosis, impairment of supply of blood to multiple organs systems. It is known as consumption coagulopathy thus manifested as bleeding disorders. Objective of the study was to evaluate the risk factors which are contributed with the genesis of

Risk stratification according to genotype and effect of thromboprophylaxis on obstetric outcomes in women with antithrombin deficiency.

BACKGROUND: Hereditary antithrombin (AT) deficiency is a risk factor for venous thromboembolism (VTE) and for obstetric complications and warrants thromboprophylaxis in certain situations. AT deficiency is divided into subtypes with different risk profiles. There is limited knowledge on the specific risks in pregnant women depending on the subtypes. OBJECTIVES: The study aim was to investigate whether genotyping pregnant women with AT deficiency could aid in determining complication risk and subsequently help to individualize antepartum and postpartum treatment. METHODS: This retrospective c

Plateletpheresis for treating essential thrombocythemia that developed during pregnancy: A case report.

OBJECTIVE: Essential thrombocythemia (ET), a life-threatening disease, is associated with increased risks of hemorrhagic and thromboembolic complications. In particular, ET during pregnancy is rare and associated with an increased risk of obstetric complications. Drug therapies are frequently administered for managing pregnant women with ET, and plateletpheresis is considered to reduce platelet (PLT) counts rapidly. CASE REPORT: A 28-year-old pregnant woman was referred to our hospital at 37 + 0/7 gestational weeks (GWs) due to severe thrombocythemia. She underwent plateletpheresis at 37 + 4/

How to Manage Venous Thromboembolism Risk during Pregnancy in Patients with Inherited Antithrombin Deficiency?

Inherited antithrombin deficiency (ATD) is associated with a high risk of venous thromboembolic complications. Association of ATD with other conditions such as pregnancy obviously increases thromboembolic risk and may require anticoagulant therapy for prevention. Although there are several/heterogenous international guidelines regarding thromboprophylaxis in pregnant patients with ATD, data on anticoagulant prophylaxis in this context are scarce in the literature. Thus, this situation remains a challenge both in the antepartum period and during delivery. Physicians from the French Society of T

Coagulation Disorders in Pregnancy.

Coagulation disorders cause significant morbidity and mortality during pregnancy, and evaluation of hypercoagulability or bleeding are frequently encountered in females of reproductive age. The physiologic changes of the hemostatic system during pregnancy increase the risk of venous thromboembolism and also make laboratory test result interpretation challenging. The risk of thrombosis can be reduced with anticoagulation, but appropriate risk stratification to assure that the benefits outweigh the risks is required. Only patients with severe bleeding disorders typically need preventive manageme

Factor XIII Supplementation in Postpartum Hemorrhage: From Biological Rationale to Clinical Implementation.

Postpartum hemorrhage (PPH) remains the leading cause of preventable maternal mortality despite standard interventions. Recent fibrinogen trials failed to improve outcomes, prompting interest in coagulation factor XIII (FXIII). FXIII functions as "molecular cement," cross-linking fibrin and stabilizing clots. During pregnancy, FXIII activity decreases 20%-30%, with further depletion during PPH. Observational studies show low antepartum FXIII predicts bleeding risk, while ex vivo supplementation restores clot firmness. The SWIFT trial (NCT06481995) represents the first randomized controlled tri

Aspirin therapy is associated with a lower risk of pregnancy loss in both JAK2- and CALR-mutated essential thrombocythemia-A Mayo Clinic study of 200 pregnancies.

Two-hundred pregnancies involving 100 women with essential thrombocythemia (ET) were accessed from Mayo Clinic databases (1990-2023). Median platelet count displayed a decline during pregnancy, nadiring at 48% of baseline, in the third trimester: 704-369 × 109/L. Live birth rate was 72%. Of 53 (27%) unintentional pregnancy losses, 48 (24%) occurred in the first trimester. Other fetal complications included preterm birth 3%, intrauterine growth retardation 3%, and stillbirth 1%. Maternal complications included major hemorrhage (7%), preeclampsia (6%), thrombosis (1%), and placental abruption (0

Immune Thrombocytopenic Purpura and Maternal and Neonatal Outcomes During Pregnancy: A Systematic Review and Meta-Analysis.

Immune thrombocytopenic purpura (ITP) affects 1-3 out of every 10 000 pregnancies, posing significant risks to both mothers and newborns. The condition often requires careful management to prevent severe hemorrhagic events. PubMed, Embase, Scopus, and Web of Science searched for relevant literature until June 2024. A meta-analysis was performed to evaluate the effect of ITP on maternal and fetal outcomes. The results showed that antepartum hemorrhage occurred in 0.17 (95% CI = 0.12-0.25) of patients and postpartum hemorrhage occurred in 0.11 (95% CI = 0.07-0.16) of pregnant women with ITP. Abo

A retrospective study of the management and outcomes of pregnancies with inherited antithrombin deficiency.

INTRODUCTION: Antithrombin (AT) deficiency is a rare but highly thrombogenic inherited thrombophilia. Its association with adverse pregnancy outcomes (APO) is undefined. There is limited guidance on managing AT deficiency in pregnancy. Some significant issues remain controversial, including risk assessment for prophylactic anticoagulation, anticoagulant therapy, and monitoring. Our goal was to examine if the antepartum management of patients with AT deficiency affected their pregnancy outcomes. MATERIALS AND METHODS: This retrospective, single-center observational study included pregnant wome

Bernard-Soulier Syndrome from the Perspective of the Obstetrician: A Case Report with a Review of the Literature.

OBJECTIVE: Bernard-Soulier syndrome (BSS) is one of the rare inherited platelet disorders that is characterized by macrothrombocytopenia and adhesion abnormality due to the absence or malfunctioning of the membrane GPIb-IX-V complex. There is no high-quality evidence on obstetric management of BSS owing to its rarity. Here we report an uncomplicated delivery of an adolescent with BSS and review the literature on the topic of BSS and pregnancy. METHODS: PUBMED, EMBASE, COCHRANE, and Google Scholar databases were searched up to April 2022 without language and year restriction using the terms "B

Varying Dosages of Subcutaneous Unfractionated Heparin and Activated Partial Thromboplastin Time in Hospitalized Antepartum Patients: A Retrospective Cohort Analysis.

BACKGROUND: Venous thromboembolism (VTE) is a leading cause of maternal morbidity and mortality in the United States. Subcutanous unfractionated heparin (UFH) has been used for decades for VTE prophylaxis and under many obstetric quality of care initiatives, hospitalized antepartum patients now receive doses as high as 10,000 units every 12 hours. This practice increases the likelihood of UFH administration around the time that epidural labor analgesia is requested or neuraxial analgesia for cesarean delivery is needed. To clarify the effect of UFH on coagulation, we reviewed the care of hospi

Prediction of postpartum hemorrhage in pregnant women with immune thrombocytopenia: Development and validation of the MONITOR model in a nationwide multicenter study.

Globally, postpartum hemorrhage (PPH) is the leading cause of maternal death. Women with immune thrombocytopenia (ITP) are at increased risk of developing PPH. Early identification of PPH helps to prevent adverse outcomes, but is underused because clinicians do not have a tool to predict PPH for women with ITP. We therefore conducted a nationwide multicenter retrospective study to develop and validate a prediction model of PPH in patients with ITP. We included 432 pregnant women (677 pregnancies) with primary ITP from 18 academic tertiary centers in China from January 2008 to August 2018. A to
