The vaginal bacteriome of pregnant Rwandan and Kenyan women, unique regional genera associations with preterm birth.

BACKGROUND: Preterm birth, defined as gestational duration less than 37 weeks, often results in lifelong adverse health impacts for children. East African women have some of the highest rates of preterm birth globally. Previous studies have suggested that changes in the vaginal bacteriome may be associated with increased risk for preterm birth. The absence of Lactobacilli dominated vaginal flora and increased alpha and beta diversity have been associated with preterm birth in other contexts, primarily from US-based studies with African-American women. Few studies have been conducted with women

Selective serotonin reuptake inhibitor use during pregnancy and maternal depression-a nationwide birth cohort study on risks to the mother and the newborn.

BACKGROUND: Maternal underlying depression may confound previously reported associations between selective serotonin reuptake inhibitor (SSRI) use and adverse pregnancy and neonatal outcomes. OBJECTIVE: To determine whether SSRI use during pregnancy is associated with an increased risk of pregnancy and neonatal complications after adjusting for indicators of maternal depression severity. STUDY DESIGN: This population-based birth cohort study used data from national registers in Finland and included 1272,587 singleton live births from 1996 to 2018. Pregnancy outcome of women with two or more

Applying the socioecological model to examine the beliefs, perceptions and attitudes surrounding preterm birth in Ethiopia: a qualitative study.

BACKGROUND: Premature birth is the leading cause of neonatal morbidity and mortality. Understanding perceptions, beliefs and attitudes towards preterm births, and how these factors influence care provision at health facilities and at home is crucial for improving preterm newborns' health outcomes. METHODS: We conducted an exploratory qualitative study at Batu and Meki communities in the East Shewa Zone of Oromia Region, Ethiopia. We conducted in-depth interviews (n=81) and focus group discussions (n=8) using semistructured guides. The study participants included women who had preterm births,

Gestational Age-Dependent Effects of Antenatal Magnesium Sulfate on Fetal S100B Levels: An Observational Study Using Cord Serum.

AIM: Magnesium sulfate (MgSO4) is widely used for fetal neuroprotection in preterm births before 32 weeks of gestation, yet it remains unclear whether its effect depends on gestational age. S100 calcium-binding protein B (S100B), a protein secreted by astrocytes, is recognized as a biomarker of neural distress. This study aimed to investigate the relationship between antenatal MgSO4 administration and umbilical cord serum S100B levels, with a focus on gestational age. METHODS: This retrospective study included women who delivered between 22+0 and 33+6 weeks of gestation at a tertiary center.

Prenatal wildfire smoke exposure and adverse neonatal outcomes in a high-risk cohort of pregnant women with asthma.

BACKGROUND: Climate change is contributing to more frequent wildfires. Exposure to wildfire smoke during pregnancy increases the risk of adverse neonatal outcomes. Pregnant women with asthma and their newborns are particularly at risk of negative effects from wildfire smoke exposure and remain understudied. Determining critical windows of exposure during pregnancy could help inform mitigation strategies for this high-risk group. OBJECTIVE: This study aimed to assess the association of prenatal wildfire smoke exposure with adverse neonatal outcomes in mothers with asthma and to determine which

Association between serum metal exposure in early pregnancy and preterm birth: Mediation by inflammatory blood cell parameters.

Several studies have linked metal exposure to preterm birth (PTB), but the underlying mechanisms remain poorly understood. This study investigated the mediating role of inflammatory blood cell counts in the relationship between metal exposure at 10-12 weeks of gestation and PTB. The combined effects of metals were assessed using quantile g-computation (Q-gcomp) and Bayesian kernel machine regression (BKMR) models. Linear and logistic regression models evaluated the associations between individual metal levels and blood cell counts, as well as their impact on PTB risk. Mediation analysis was co

Prevention of recurrent spontaneous preterm delivery using probiotics: results from a prospective, single-arm, multicenter trial.

BACKGROUND: The rate of recurrent spontaneous preterm delivery is 30% (95% confidence interval, 27%-34%) globally and 22.3% in Japan. Probiotics containing Clostridium species may reduce spontaneous preterm delivery by inducing regulatory T cells, which play an important role in maintaining pregnancy. OBJECTIVE: We investigated whether the administration of probiotics including Clostridium butyricum prevented recurrent spontaneous preterm delivery in high-risk pregnant women. STUDY DESIGN: We conducted a prospective, single-arm, nonblinded, multicenter trial across 31 hospitals in Japan betw
