Hydroxychloroquine therapy for women with both recurrent pregnancy loss and autoimmune disease: association with pregnancy outcomes and maternal-fetal complications.

OBJECTIVE: To evaluate the impact of hydroxychloroquine (HCQ) on pregnancy outcomes and maternal and fetal complications in women with a history of recurrent pregnancy loss (RPL) and autoimmune disease. METHODS: The medical records of pregnant women with a history of RPL and autoimmune disease attending the Shanghai First Maternity and Infant Hospital between January 2017 and December 2019 were retrospectively reviewed. Primary outcomes were gestational week at delivery (term: ≥ 37 weeks; preterm: < 37 weeks). Secondary outcomes were mid-to-late pregnancy loss and maternal and fetal complicat

The relationship between preterm birth and postpartum anxiety: A first-in-field systematic review.

INTRODUCTION: Preterm birth poses substantial risks to infant health and maternal mental health, particularly anxiety. The evidence base for associations between gestational age and postpartum anxiety has yet to be synthesised as a whole. This first-in-field systematic review aimed to investigate the relationship between gestational age at delivery and postpartum anxiety, and explore the experiences of mothers of premature infants with postpartum anxiety. MATERIALS AND METHODS: Searches were conducted across 10 psychological, clinical, and allied health databases from inception to 6th October

Hybrid Maternity Care and Impact on Pregnancy Outcomes: A Systematic Review and Meta-Analysis.

This study aimed to systematically evaluate the impact of hybrid antenatal care models (structured integration of in-person and remote visits) on adverse perinatal and maternal outcomes compared with traditional care. MEDLINE, CINAHL, Scopus, ClinicalTrials.gov, and the Cochrane Library were searched from inception to November 8, 2025. Studies comparing hybrid antenatal care with standard care were included. Primary outcome was stillbirth; secondary outcomes included small-for-gestational-age (SGA) at birth or fetal growth restriction (FGR), preterm birth (PTB) <37 weeks, cesarean delivery, pr

Effect of first-trimester aspirin initiation timing on obstetric outcomes: A comparative study.

This study aims to evaluate the association between the timing of low-dose aspirin (LDA) initiation in the first trimester and obstetric outcomes, including uterine artery pulsatility index (UtA PI), fetal growth parameters, pregnancy-associated plasma protein-A (PAPP-A) levels, and delivery outcomes in a high-risk population for preeclampsia (PE). This prospective cohort study compared pregnant women at high risk for PE who initiated LDA before 11 weeks of gestation (early aspirin group) with those who initiated LDA between 11 and 14 weeks (late aspirin group). Outcomes included first- and se

Recovery centered perinatal care: A retrospective cohort study evaluating a scalable prenatal care model to improve outcomes for birthing people with opioid use disorder and their newborns.

INTRODUCTION: Opioid use disorder (OUD) during pregnancy poses numerous health risks for the pregnant person and their infant. Medication for OUD and adequate prenatal care have been shown to reduce the risks of adverse outcomes for pregnant patients and newborns, however the best model for these services is not yet established. One novel, low resource model is Recovery Centered Perinatal Care (RCPC) which combines prenatal care with peer recovery support and logistical support for social determinants of health often linked with negative outcomes of OUD including food insecurity, housing insta

Antenatal fetal surveillance and iatrogenic delivery.

OBJECTIVES: Antenatal testing is routinely recommended for high-risk pregnancies to reduce the risk of stillbirth, yet limited data guide counseling about likelihood of unscheduled iatrogenic delivery. We explored factors associated with clinical recommendations for unscheduled iatrogenic delivery prompted by antenatal testing and examined variables that influence obstetric outcomes. METHODS: In this retrospective cohort study, we identified singleton pregnancies undergoing serial testing in an academic system from January 2022-May 2023. Indications for testing, recommendations made at the fi

Perinatal outcomes of pregnant adolescents in rural Japan: a retrospective cohort study at a comprehensive perinatal center.

BACKGROUND: Globally, even within developed nations, specific regions exhibit disproportionately high rates of adolescent pregnancy that require comprehensive perinatal medical care, including social interventions. We aimed to compare perinatal outcomes between pregnant adolescents and pregnant adults in rural Japan. METHODS: We conducted a retrospective cohort study of pregnant women between 2014 and 2025, categorizing participants into two groups based on maternal age: pregnant adolescents (< 20 years) and adults (20–34 years). Adjusted odds ratios (aORs) were calculated using logistic regre

Patterns of recurrent preterm birth: an individual patient data meta-analysis.

OBJECTIVE: Pregnant patients with a previous spontaneous preterm birth are at high risk for recurrent (spontaneous) preterm birth. We investigated whether the number of previous spontaneous preterm births, gestational age of the previous birth and cervical length could stratify patients into different risk groups for recurrence to investigate whether there is a risk group that does not require additional interventions. DATA SOURCES: A systematic search of MEDLINE and EMBASE was conducted from 1995 until April 2025. STUDY ELIGIBILITY CRITERIA: Studies that were included were prospective studi

Neonatal and maternal outcomes of scheduled versus unscheduled ex-utero intrapartum treatment (EXIT) procedures.

OBJECTIVE: Ex-utero intrapartum treatment (EXIT) procedures are employed during delivery to establish a safe airway on placental support for neonates with potential airway obstruction. While EXITs are typically planned with a multidisciplinary team, these procedures may occur emergently due to preterm labor, late diagnosis, or delayed transfer of care. This systematic review compares outcomes of scheduled versus unscheduled/emergent EXIT procedures. DATA SOURCES: We searched PubMed, Scopus, and CINAHL in March 2025 to identify observational studies describing EXIT procedures. METHODS: Studie

Low PAPP-A levels and their association with adverse perinatal outcomes in twin pregnancies.

PURPOSE: While in singleton pregnancies the maternal serum biomarkers used in the first-trimester screening for aneuploidies, particularly PAPP-A, may be, also, used as predictors of adverse perinatal outcomes, there is a scarcity of data regarding the association of first-trimester biomarkers with unfavorable pregnancy outcomes in twin pregnancies. The main purpose of our study was to evaluate the association of low PAPP-A levels in twin pregnancies with the subsequent development of perinatal complications. METHODS: 454 twin pregnancies were recruited over a period of 11 years, and their da

Facility-Level Factors Associating Antenatal Corticosteroid Administration Rates and Subsequent Term Birth Rates: A Nationwide Cross-Sectional Observational Study Using the 2020-2022 Perinatal Registry Database in Japan.

AIM: The antenatal corticosteroid (ACS) administration rate in Japan is low. To achieve both high coverage and low overtreatment of ACS, understanding of facility-level factors is important. This study aimed to identify facility-level factors associated with ACS coverage and overtreatment and simulate the potential consequences of increased ACS use. METHODS: This observational study used data from the 2020 to 2022 Perinatal Registry Database maintained by the Japan Society of Obstetrics and Gynecology. Primary outcomes were: (1) ACS administration rate among preterm births before 34 weeks (AC

Association between timing of antenatal corticosteroid and developmental outcomes in children born late preterm by high-risk pregnancies.

OBJECTIVE: High-risk pregnancies often receive antenatal corticosteroids (ACS) before 34 weeks, even when delivery occurs in the late preterm period. We aimed to evaluate whether the timing of ACS exposure is associated with developmental delay in late preterm infants born to high-risk mothers. METHODS: We conducted a national, retrospective cohort study of 40,119 singleton late preterm (LPI) live births born in Taiwan between January 1, 2004, and December 31, 2011, followed up until December 31, 2018. We defined high-risk pregnancies based on four clinical conditions: diabetes mellitus, advan

Early initiation of low-dose aspirin for the prevention of pre-eclampsia in high-risk pregnancies.

Pre-eclampsia and its related consequences last to be significant factors in maternal morbidity and mortality, particularly in high-risk pregnant women. Aspirin has revealed promise in reducing these risks, particularly when initiated early in gestation. This study evaluated the effectiveness of 75 mg aspirin, started before 12 weeks gestation, in reducing adverse pregnancy outcomes among high-risk women. We achieved a randomized controlled experiment with high-risk pregnant women. Pregnant women were randomly allocated to receive either 75 mg of Aspirin or a placebo. Daily from the point of e

Time-Dependent Differences in the Human Milk Proteome After Preterm Birth: A Paired Two-Stage Proteomic Study.

Background/Objectives: Human milk composition is shaped by gestational age at delivery and stage of lactation; however, proteomic differences between milk from mothers of preterm and term infants and their temporal patterns remain incompletely characterised. Methods: This prospective study enrolled 40 lactating mothers: 20 who delivered preterm infants (<32 weeks' gestation) and 20 who delivered at term (37-42 weeks). Each provided milk samples during early lactation (first 10 days postpartum) and during later lactation (week five postpartum). Milk serum was analysed using quantitative data-in

Unilateral Pulmonary Edema Associated With Prolonged Lateral Decubitus Positioning During Tocolytic Therapy in Pregnancy: A Case Report.

Peripartum pulmonary edema is rare but potentially life-threatening for both the mother and fetus. Unilateral pulmonary edema is particularly uncommon and can mimic pneumonia, delaying appropriate diagnosis because of atypical radiologic findings. A 28-year-old pregnant woman (G3P1) hospitalized for threatened preterm labor received ritodrine hydrochloride and magnesium sulfate. At gestational week 25, she suddenly developed chest pain, dyspnea, and hypoxemia. Chest X-ray revealed unilateral, left-sided pulmonary infiltrates. Cardiac evaluation showed no evidence of cardiogenic pulmonary edema

Bed Rest versus Tocolytic Therapy in Threatened Preterm Labour Stratified by Cervical Length: A Randomised Controlled Trial.

AIM: Management of threatened preterm labour (TPL) frequently involves tocolytic drug administration, which may lead to unnecessary interventions and maternal complications, especially in women at low risk of imminent preterm birth. Cervical length measurement is a reliable method for assessing preterm birth risk and guiding appropriate management. However, tocolytic drugs and antenatal corticosteroids are often overused in women with adequate cervical length. METHODS: This randomised controlled trial compared the effectiveness of bed rest versus tocolytic therapy in women with threatened pre

Integrating socioeconomic determinants into preterm birth risk prediction: evidence from Northwest China.

BACKGROUND: Preterm birth is a leading cause of neonatal mortality worldwide. Socioeconomic status (SES) is associated with preterm birth risk, yet existing studies in low-resource settings have rarely examined whether incorporating SES into clinical models can enhance prediction performance. This study aimed to develop and validate a SES-integrated risk prediction model for preterm birth in Northwest China. METHODS: A retrospective cohort of 2,781 deliveries from county-level hospitals (January–November 2024) was analyzed. Logistic regression was used to construct a baseline model (clinical v
