Travel associated human coccidioidomycosis in Germany 2011-2023.

This study reviews proven coccidioidomycosis cases in Germany associated with travel between 2011 and 2023 to endemic regions. We characterize clinical presentation, diagnostic practices, antifungal susceptibility, and molecular epidemiology. Fifteen human cases were confirmed by culture (12 cases) and histology (3 cases), with infections caused by Coccidioides posadasii (10 cases) and C. immitis (2 cases). Patients ranged from 23 to 82 years old, mostly without significant comorbidities. Clinical manifestations included pulmonary (6 cases), CNS (2 cases), and disseminated disease (3 cases). S

Urban soils along the Kern River and Los Gatos Creek are hotspots for Coccidioides in the San Joaquin Valley of California.

Coccidioidomycosis (Valley fever) is acquired through inhalation of spores produced by fungi in the genus Coccidioides. Coccidioides is commonly detected in, and cultivated from, the lung tissue of native rodents and the soils within their burrows. Coccidioidomycosis is acquired by exposure to environmentally produced spores and is not spread between hosts. Thus, determining the location of rodent burrows with soils harboring Coccidioides will be critical for understanding coccidioidomycosis incidence and modelling how Coccidioides distributions will be affected by global change. Coccidioides

Immunocompetent host and mortality-related coccidioidomycosis: A case report and narrative review.

Coccidioidomycosis, caused by Coccidioides infection, is typically subclinical and self-limited in immunocompetent patients. Occupational soil exposure, age ≥55 years, pregnancy, comorbidities, or immunosuppression states are common contributors to disease progression and potential mortality. This report describes a 34-year-old woman from Northwest Mexico, without evident risk factors, who presented chest pain and odynophagia as initial clinical symptoms and died at the 19th day of progressive clinical evolution consequence of pulmonary coccidioidomycosis. Findings in this report, along with t

Primary cutaneous blastomycosis in a Chinese-Canadian girl following an insect bite.

BACKGROUND: Blastomycosis is a disease caused by the dimorphic fungus Blastomyces species. This infection occurs most frequently among individuals residing in the midwestern, southeastern, and south-central United States, as well as in Canadian provinces bordering the Great Lakes and the St. Lawrence Seaway. Sporadic cases have been reported in China, the diagnosis of imported cases remains a significant challenge. CASE PRESENTATION: We report a diagnostically challenging case of primary cutaneous blastomycosis in a Chinese-Canadian girl, presenting as two nodules of different sizes on her le

Coccidioidomycosis Masquerading as Malignancy: A Case of Mediastinal Mass and Pericardial Effusion in an Immunocompetent Jamaican Male.

Coccidioidomycosis rarely presents as a lung mass, hypereosinophilia, and disseminated pericarditis with pericardial effusion. Even rarer is a case that presents with the constellation of all these findings. We present a case of coccidioidomycosis in an immunocompetent young Jamaican male with all these features, set against an interesting epidemiological context of a likely exposure to coccidioidomycosis spores during an on-foot crossing of the Mexican border into Texas. His treatment course was complicated by declining liver and kidney function due to triazole antifungal therapy, necessitati

Amphotericin B Formulation Selection During Veno-Venous Extracorporeal Membrane Oxygenation: Two Case Reports Illustrating Clinical Dilemmas in the Absence of Data.

The use of amphotericin B (AmB) in patients requiring extracorporeal membrane oxygenation (ECMO) remains poorly characterized because of concerns about drug sequestration within the circuit, particularly with liposomal formulations. We describe 2 patients with life-threatening coccidioidomycosis who received different AmB formulations during veno-venous (VV) ECMO support. The first patient, a 23-year-old male with disseminated disease involving massive splenomegaly and multi-organ failure, received liposomal AmB at 5 mg/kg/day but died within 8 days of antifungal initiation due to progressive

Coccidioides skin test performance and patterns of prior immunity to coccidioidomycosis among a large population of males entering California state prisons, 2015-2024: a population-based modelling study.

BACKGROUND: The incidence of coccidioidomycosis-an emerging fungal infection-is high within California state prisons. In 2015, California began offering a Coccidioides skin test, Spherusol®, to adult males upon entry into prison, with those susceptible to coccidioidomycosis restricted from placement in two prisons with highest incidence. This study aimed to understand test performance within the incarcerated population and to estimate population-level immunity and force of infection (FOI) by age and pre-incarceration residence. METHODS: We fit catalytic models to skin test data from 154,450 m

Fungal and Atypical Pneumonia.

Fungal and atypical bacterial pneumonias are emerging infectious threats in companion animal medicine, fueled by ecological disruption, globalized pet travel, and immunomodulatory therapies being increasingly used. These infections challenge clinicians in ways that conventional bacterial pneumonia does not-presenting insidiously, extending beyond the lung, and not responding to empirical treatment. Delayed or missed diagnosis remains a costly, defining hallmark. Recognizing these conditions demands a shift in clinical thinking: one that weighs host susceptibility, exposure history, and geograp

Frequency and Duration of Diagnostic Delays Associated with Coccidioidomycosis and Risk Factors for Missed Diagnoses, United States.

Diagnosis of coccidioidomycosis is challenging and requires a high index of clinical suspicion. We estimated the incidence and duration of, and risk factors associated with, diagnostic delays and missed opportunities in the diagnosis of coccidioidomycosis. We conducted a retrospective analysis of health insurance claims data in the United States during 2001-2022 included in the Merative MarketScan Databases. Using a case-crossover design and a bootstrapping approach, we estimated the number of excess visits for coccidioidomycosis-related symptoms before diagnosis. We also evaluated potential f

The Fungus Among Us: Latency, Reactivation, and the Expanding Clinical Spectrum of Coccidioidomycosis.

Coccidioidomycosis is a serious fungal infection that can produce a wide range of clinical manifestations, from mild, self-limiting pulmonary disease to severe, disseminated infection that can involve nearly any organ system, including the skin. Coccidioidomycosis may also affect patients at the psychiatric level. Its clinical relevance has increased markedly in recent years, as incidence continues to climb, geographic distribution widens, and emerging presentations challenge traditional assumptions regarding fungal latency and host immunity. A notably important and developing area of study is

[Subacute pulmonary coccidioidomycosis: A differential diagnosis of lung cancer].

INTRODUCTION: Coccidioidomycosis is a fungal infection endemic to the southwestern United States, particularly Arizona. Rare in Europe, it can lead to diagnostic delays insofar as its clinical and radiological features may mimic lung cancer, mycobacterial and/or bacterial diseases. OBSERVATION: We report a case of pulmonary and mediastinal coccidioidomycosis diagnosed in France following travel to an endemic area by a 68-year-old immunocompetent woman, a former smoker, who presented with a persistent cough and constitutional symptoms (fatigue, weight loss and anorexia). Chest computed tomogra

Myeloid-derived suppressor cell (MDSC)-like neutrophils induced by pulmonary infection with Coccidioides posadasii exacerbate disease by suppressing CD4+ T cell immunity.

UNLABELLED: Coccidioidomycosis is a pulmonary disease caused by the dimorphic fungus Coccidioides, which is endemic to the southwestern United States and South America. Coccidioides grows as septate hyphae in soil and forms spores that can become airborne during mechanical disruption of the soil. In the lungs, the inhaled spores develop into large multinucleated spherules, a unique morphology among medically important Coccidioides species, and reproduce by endosporulation. A hallmark of the host immune response toward endosporulating spherules is the robust recruitment and infiltration of Ly6G

Right lung cavitation: Don't forget coccidioidomycosis - A case report.

A 65-year-old female patient developed recurrent cough and sputum production without an apparent cause for 4 months prior. A chest CT scan performed 1 month earlier revealed a right lower lung opacity with cavitation. Despite treatment, she continued to experience paroxysmal coughing. Upon admission, multiple investigations were conducted, including chest CT, T-cell testing for tuberculosis infection, bronchoscopy, lung biopsy, and next-generation sequencing. The final diagnosis was pulmonary coccidioidomycosis presenting as right lower lung opacity with cavitation. Improvement was observed fo

When the Skin Tells Another Story: Incidental Epidermolysis Bullosa in an Adult Hospitalized for Chronic Respiratory Symptoms.

Epidermolysis bullosa (EB) comprises a heterogeneous group of inherited blistering disorders characterized by skin fragility and trauma-induced blister formation. Although EB is typically recognized in childhood, milder phenotypes may remain underdiagnosed in adults and may be identified incidentally during evaluation for unrelated medical conditions. A 47-year-old man was admitted for chronic respiratory symptoms and a history of prior granulomatous infections. During a routine physical examination, extensive chronic blistering skin lesions were identified incidentally. Inpatient evaluation r

In silico identification and validation of vaccine and drug targets in Coccidioides posadasii through integrated genomic, proteomic, and molecular modeling approaches.

Coccidioides posadasii is a pathogenic fungus endemic to the arid regions of the Americas, causing coccidioidomycosis, a debilitating respiratory infection. Coccidioides infection is caused by breathing arthroconidia generated by mycelia that grow in desert soil. Arthroconidia develop inside the lung into massive, multinucleated spherules that are pathognomonic. Despite its significant impact on public health, there remains a paucity of research investigating novel therapeutic approaches to combat this disease. Addressing this research gap, our study employs a multi-faceted approach integratin

Severe Pulmonary Coccidioidomycosis Treated With Systemic Corticosteroids in an Adolescent Child.

Coccidioidomycosis (CM) is an emerging public health challenge due to an increasing number of cases in the U.S. Diagnosis may be challenging because of its nonspecific presentation, which may lead to treatment delays. In addition, the optimal management of severe pulmonary CM remains unclear. We present the case of an 18-year-old immunocompromised renal transplant recipient with a history of systemic lupus erythematosus who developed acute respiratory distress syndrome (ARDS) secondary to severe pulmonary CM. Following initiation of systemic corticosteroids treatment with standard-of-care anti

Suspected Macrolide-Refractory Mycoplasma pneumoniae Pneumonia With Discordant Coccidioides Serology in an Immunocompromised Host: A Case Report.

Suspected macrolide-refractory Mycoplasma pneumoniae pneumonia can be a diagnostic challenge in immunocompromised hosts, particularly after allogeneic stem cell transplantation. Screening serologies may further complicate diagnosis when false-positive or discordant results divert therapy. We report a 63-year-old woman with B-cell acute lymphoblastic leukemia status postallogeneic stem cell transplantation and hypogammaglobulinemia who was transferred for persistent cough, exertional dyspnea, and right upper-lobe consolidation after limited improvement with azithromycin and broad-spectrum antib

Dual activation of NLRP3 and pyrin inflammasomes mediates host responses to the human fungal pathogen Coccidioides.

Coccidioides is a dimorphic fungal pathogen that grows as a mold in soil and produces infectious arthroconidia. Inhaled arthroconidia transition to spherules containing endospores in mammalian hosts. How distinct developmental forms of Coccidioides interact with immune cells remains poorly defined. Here, we show that arthroconidia activates TLR2 and induces NLRP3-pyrin inflammasomes in macrophages, while ferroptotic signaling promotes arthroconidia killing. Upon transition to spherules, only ruptured spherules-not intact ones-trigger IL-1β production through NLRP3-pyrin inflammasomes and GSDMD
