We describe a rare case of Histoplasma capsulatum prosthetic aortic valve endocarditis in an immunocompetent patient with previous histoplasmosis. Diagnosis was established by direct microscopy, culture, and molecular identification from excised valve tissue. Antifungal susceptibility testing showed low MICs to amphotericin B and azoles, with limited echinocandin activity. Thus, after surgical valve replacement and liposomal amphotericin B therapy, the patient was discharged with a good outcome.

BACKGROUND: Pulmonary histoplasmosis is a fungal infection caused by Histoplasma capsulatum. Although endemic in certain regions, it is increasingly reported worldwide. Nodular pulmonary histoplasmosis (NPHP) represents a distinct form of the disease, characterized by the formation of granulomatous nodules within the lung parenchyma of the host. Clinically, patients may present with nonspecific symptoms or remain entirely asymptomatic, and the condition can mimic various other pulmonary diseases. High-resolution computed tomography (HRCT) plays a crucial role in identifying characteristic nodu

BACKGROUND: Histoplasmosis is an endemic mycosis that primarily affects the lungs, commonly found in the central and eastern United States, particularly the Ohio and Mississippi River valleys. While disseminated histoplasmosis (DH) is typically associated with immunocompromised individuals, rare cases have been reported in immunocompetent hosts following high-risk environmental exposures, particularly in high-risk occupational settings.
CASE PRESENTATION: We report a case of a 22-year-old immunocompetent male employee of a municipal water department in Texas who presented with fever, pleuriti

A previously asymptomatic male infant presented with persistent fever for 4 weeks with progressive abdominal distension due to hepatosplenomegaly. Investigations were suggestive of bicytopenia. His fever was non-responsive to intravenous antibiotics. Blood and urine cultures were sterile. Bone marrow showed normal haematopoiesis with evidence of yeast forms of Histoplasma capsulatum inside erythroblasts. Urine was positive for histoplasma antigen. The infant responded to intravenous liposomal amphotericin B. Fever resolved, anaemia and thrombocytopenia recovered. Hence, we report this case of

BACKGROUND: Data on the true burden of histoplasmosis in Uganda are limited, largely due to the scarcity of epidemiological studies. We aimed to describe the nationwide burden of histoplasmosis among persons with advanced HIV disease (AHD), highlighting the major challenges in diagnosis and treatment under routine HIV care.
METHODS: This was a prospective longitudinal study conducted at nine health facilities in Uganda. Persons with AHD (CD4 < 200 cells/mm3) were included and followed up for 2 years. Urine samples were sent to the Central Public Health Laboratories and tested using the Histop

OBJECTIVES: Though anti-IFN-γ autoantibodies are associated with adult-onset immunodeficiency among Asians, they are not routinely tested. Despite rising reports of disseminated opportunistic infections in apparently immunocompetent individuals, the clinical course and prevalence of anti-IFN-γ autoantibodies remain poorly characterized. We evaluated the association between severe opportunistic infections and plasma anti-IFN-γ antibodies and IL-12 levels.
METHODS: This cross-sectional study evaluated levels of anti-IFN-γ autoantibodies and IL-12 in HIV negative, apparently immunocompetent pati

We examine the impact of microbial cell-free DNA (mcfDNA) NGS for the diagnosis of Histoplasma capsulatuim in a quaternary referral center in Texas. Of eight included patients, five had a change in their antifungal coverage after mcfDNA NGS results with faster turnaround times than antigen testing and cultures.

BACKGROUND: Histoplasmosis is an emerging mycosis in India. Although clinical cases are increasingly recognized, the environmental niche of the Histoplasma (H.) capsulatum fungus remains poorly defined, with only a single soil isolation reported from India in 1975.
OBJECTIVE: To investigate the presence of H. capsulatum in soil samples from endemic regions of West Bengal using conventional culture and sensitive molecular methods.
METHOD: Forty-five soil samples were collected from Nadia and Bardhaman districts, targeting sites enriched with high nitrogen content, such as areas under the tree

UNLABELLED: During infection, Histoplasma capsulatum exists primarily as an intracellular pathogen of phagocytes, where it overcomes host defense mechanisms and replicates within the phagosome. This intracellular proliferation of Histoplasma yeasts requires the function of the fungal peroxisome for multiple, incompletely defined roles, with early pathogenesis functions dependent on the proper trafficking of peroxisomal matrix proteins carrying a type 1 peroxisome-targeting signal (PTS1). To better understand the potential roles of peroxisomes during Histoplasma infection, we identified the con

Histoplasma capsulatum is a pathogenic fungus known to exist across the world, especially in the Ohio and Mississippi River Valleys. While clinicians commonly rely on urine and serum Histoplasma antigen testing for rapid detection of disseminated Histoplasmosis, these diagnostic tests have limitations. We present a case of a patient who initially had negative urine and serum Histoplasma antigen tests and required additional testing with lymph node biopsy and immunodiffusion to establish a diagnosis of histoplasmosis. This case highlights the importance of employing multiple testing methods whe

The workshop on 'Reactive and therapy induced BM changes linked to systemic infectious and non-infectious disorders including MAS/HLH' of the 22nd meeting of the European Association for Haematopathology held in Dubrovnik, 2024, included 58 cases. These encompassed a broad range of infections, autoimmune disorders, malignancies and therapy-effects, or a combination of these factors, of which 28 had an associated Hemophagocytic Lymphohistiocytosis (HLH) / Macrophage Activation Syndrome (MAS). Histoplasmosis, the infection mostly associated with HLH, showed a wide variability of BM changes, with

Histoplasmosis is an endemic mycosis in the central and eastern USA, Central and South America, parts of Asia, and Africa. It commonly presents as a pulmonary disease, but can involve multiple organ systems in immunocompromised individuals. In patients with HIV and low CD4 counts, usually <150 cells/μL, it can present with gastrointestinal symptoms, lymphadenopathy, and rarely as meningitis, or with mucocutaneous and adrenal involvement. Diagnostic modalities include direct visualisation of the fungus in tissue specimens, Histoplasma antigen detection in blood or urine, antibody detection, det

Historically, infections are subjectively associated with fatigue, a common yet poorly delineated syndrome. The impact on patient quality of life is the ultimate justification to evaluate fatigue in patients with infections. While fungal infections are subjectively associated with fatigue, there are minimal quantifiable fungal research outcomes that evaluate histoplasmosis and fatigue.

BACKGROUND: Fine-needle aspiration cytology (FNAC) is a simple and reliable technique for diagnosing various inflammatory lesions. Some of these lesions are caused by fungal organisms, which are often missed clinically and thus remain untreated. Most of these infections require prompt diagnosis as they respond to early treatment, and delayed diagnosis may lead to unnecessary complications.
AIM: To study the clinico-cytomorphological spectrum of fungal infections in a tertiary care center.
MATERIALS AND METHODS: This is a retrospective study and includes a total of 25 cases of various fungal

Histoplasma capsulatum var. capsulatum is a thermally dimorphic fungus endemic in regions like the Midwestern USA, Africa, and Central/South America. It grows as a mold in the environment and as yeast in human tissues. The disease's severity and presentation depend mostly on the host's immunity. In immunocompromised individuals, it often causes progressive disseminated histoplasmosis (PDH), a severe systemic condition. PDH symptoms include pneumonia, enlarged spleen and liver, enteritis, pancytopenia, lymphadenopathy, skin and mucosal lesions, and joint pain, making the diagnosis a challenge.

INTRODUCTION: Progressive disseminated histoplasmosis is associated with high mortality in people with advanced HIV (Human Immunodeficiency Virus) disease or severe immunosuppression. The World Health Organization (WHO) recommends antigen testing for progressive disseminated histoplasmosis diagnosis.
OBJECTIVE: To assess the positivity of the Histoplasma antigen test in urine specimens referred to SYNLAB.
MATERIALS AND METHODS: A retrospective analysis of SYNLAB's database included specimens referred from February 2021 to August 2023. Specimens were tested using the Clarus Histoplasma GM Enz

Histoplasmosis is an endemic fungal infection caused by Histoplasma species. Disseminated forms, although rare, often involve the gastrointestinal tract and primarily affect immunocompromised patients. This article presents an unusual case of gastrointestinal histoplasmosis with jejunal perforation. A 57-year-old man presented with chronic abdominal pain, intermittent fever, cough, diaphoresis, nausea, and rapid weight loss. Findings on computed tomography scans showed thickening and malignant lesions in the intestine, further evaluated through laparotomy, in addition to pulmonary involvement.

Histoplasmosis is typically a self-limiting infection in immunocompetent individuals, but may present as a life-threatening disseminated disease in immunocompromised hosts. Gastrointestinal (GI) symptoms, radiological abnormalities and endoscopic findings are non-specific. We report a case of disseminated histoplasmosis in a kidney transplant recipient presenting with hemorrhagic shock due to severe colonic ulcerations. This case highlights the importance of maintaining a high index of suspicion for GI histoplasmosis in patients originating from endemic regions and underscores the need for car
