Histoplasma capsulatum, a dimorphic fungus, is the causative agent of histoplasmosis, a respiratory disease with a wide clinical spectrum ranging from asymptomatic infection to severe pulmonary or disseminated disease. Although Ecuador is recognized as an endemic country, histoplasmosis remains underreportesd. This study presents the first investigation in Ecuador assessing occupational exposure to H. capsulatum among field biologists and environmental scientists, a group presumed to be at increased risk due to frequent contact with wildlife, caves, and rural environments. A total of 46 worker

I was consulted on a patient with disseminated histoplasmosis (affecting the lungs, liver, spleen, and bone marrow) when she was 20 weeks pregnant. In the hospital, she improved with amphotericin B but did not tolerate itraconazole. I scoured the literature to find the safest next steps for my patient and her unborn baby but was shocked at how little published information exists. We decided on amphotericin therapy via OPAT and switched to posaconazole after delivery. Her infant was asymptomatic but also treated with antifungals due to positive Histoplasma testing after birth. Our discussions,

Histoplasmosis is rarely diagnosed in pregnancy, though when disseminated it may infect the infant as well. Decisions regarding antifungal therapy in pregnant women can be difficult due to known toxicity risks and lack of rigorous study. Here we review the published literature regarding histoplasmosis in pregnancy as well as antifungal therapy in pregnancy and duration lactation. We also bring our clinical experience with a disseminated histoplasmosis case affecting both mother and infant, as well as the mother's perspective in her own words. "Sharing our story is important to me because I hop

BACKGROUND: Surveillance for serious fungal diseases (SFDs) relies on the capacity to recognize them and request and accurately perform diagnostic investigations. However, knowledge in resource-limited settings is suboptimal. This study evaluated the impact of a training intervention on the diagnosis and management of SFDs among clinicians and laboratory scientists in Nigerian tertiary hospitals.
METHODS: Clinicians in 6 hospitals were purposively selected and trained in 6 SFDs using standardized modules developed by international medical mycology experts. A quasi-experimental design assessed

Gastric involvement with Histoplasma capsulatum infection is rare but can mimic peptic ulcers or cancer, delaying diagnosis. A 71-year-old man presented with oral ulcer, abdominal pain, and weight loss. The patient had history of tonsillar histoplasmosis treated 8 years before the current presentation with confirmed resolution. The patient had no history of malignancy, autoimmune diseases, or recent travel. Initial laboratory evaluation revealed normocytic anemia. Imaging did not show acute abnormalities that would account for his symptoms. Esophagogastroduodenoscopy revealed multiple gastric

BACKGROUND: Histoplasmosis is a systemic fungal infection primarily involving the lungs. Dissemination to the vocal cords and bilateral adrenal glands is exceedingly rare. This case report illustrates the clinical and radiological manifestations of the disease at these atypical sites, providing crucial insights for clinical differential diagnosis.
CASE PRESENTATION: We present a 50-year-old male patient admitted with recurrent fever and weight loss, with a history of diabetes mellitus and bat exposure. Imaging studies revealed a nodule in the left thyroid lobe (ACR TI-RADS 5), bilateral adren

We report the case of a 35-year-old female with advanced HIV diagnosed with a mixed mycotic infection. Diagnosis was assisted by Karius test. She was admitted with postpartum psychosis and developed fever and acute hypoxic respiratory failure initially thought to be from a bacterial hospital-acquired pneumonia. Due to lack of sufficient improvement on appropriate antibiotics, a broad infectious workup was sent. A Karius test was performed early during the workup. It returned positive for Pneumocystis jirovecii, Coccidioides posadasii, and Histoplasma capsulatum. Eventually a standard work up r

BACKGROUND Histoplasma capsulatum is a thermally dimorphic fungus capable of causing progressive disseminated histoplasmosis, particularly in immunocompromised hosts. Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome resulting from uncontrolled immune activation. The coexistence of HLH and disseminated histoplasmosis is uncommon and poses significant diagnostic and therapeutic challenges. CASE REPORT A 72-year-old immunosuppressed woman with rheumatoid arthritis treated with methotrexate, hydroxychloroquine, and adalimumab presented with a 2-month

BACKGROUND: Endemic mycoses are caused by fungi inhabiting specific ecological niches. These infections remain underreported due to diagnostic challenges and limited awareness. This study aims to provide a comprehensive synthesis of endemic mycoses in India, focusing on their distribution, clinical presentation, and outcomes.
METHOD: A systematic literature search was conducted to identify Indian studies on proven endemic mycoses from 1960 to August 2025. Case reports, case series, and observational studies with laboratory-confirmed diagnosis of endemic mycoses were included, based on the Eur

BACKGROUND: Itraconazole (ICZ) has been approved by the FDA to treat many fungal infections including, blastomycosis, histoplasmosis, and aspergillosis. ICZ can be also used as prophylaxis in the population who are at high risk for developing systemic fungal infections, such as HIV patients, and chemotherapy patients.
AIM: However, since ICZ is a BCS Class II drug that has low solubility and high permeability, leads to low oral bioavailability. In addition, the absorption of ICZ from commercial oral dosage forms is highly affected by food intake and pH.
OBJECTIVES: The current study aimed to

PURPOSE: Invasive fungal and nontuberculous mycobacterial (NTM) infections are important complications following TNF-α inhibitor initiation, but the contribution of additional immunosuppressive medication exposure remains incompletely defined. We evaluated their incidence and spectrum and associations with time-varying immunosuppressive medication exposure.
METHODS: We conducted a retrospective cohort study using Merative MarketScan data (2018-2022) among adults initiating TNF-α inhibitors. Invasive fungal and NTM infections were identified using ICD-10-CM codes. Systemic glucocorticoid and n

IMPORTANCE: Laboratory testing is necessary to distinguish blastomycosis, coccidioidomycosis, or histoplasmosis from other causes of community-acquired pneumonia (CAP). Robust data about testing for and diagnosis of these fungal diseases among patients with CAP throughout the US are lacking.
OBJECTIVE: To examine proportions and characteristics of (1) adult outpatients with CAP who underwent diagnostic testing for blastomycosis, coccidioidomycosis, or histoplasmosis; and (2) tested patients who received diagnoses of these diseases.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort

INTRODUCTION: Uptake of penicillin prophylaxis to prevent recurrent rheumatic fever and its sequela rheumatic heart disease (RHD) is not optimal in the Northern Territory of Australia. The Full Moon Strategy (the Strategy) was introduced in the Central Australian region in June 2006 to improve the uptake of prophylactic penicillin: clients and healthcare workers were encouraged to use the full moon as a cue for the timing of the 4 weekly prophylactic penicillin injection.
OBJECTIVE: To determine the impact and effectiveness of the Strategy on knowledge and uptake of benzathine penicillin prop

OBJECTIVE: Diagnosing atypical pathogen-induced infectious diseases is a complex and challenging problem, given their unique characteristics. A-priori identification of features using modern AI approaches can facilitate the diagnostic process, potentially leading to improved outcomes, early diagnosis, and effective resource utilization. We aimed to develop an AI-driven clinical decision support system for identifying rare and atypical infections - a composite of six microbiologically and clinically distinct diseases (blastomycosis, cryptococcosis, histoplasmosis, mucormycosis, pneumocystosis,

Cryptococcosis is a life-threatening fungal infection with global public health impact, affecting both immunocompromised individuals and the general population. Accurate clinical management requires differentiation of Cryptococcus species and detection of subclinical infections. However, current diagnostic methods suffer from limited sensitivity and, notably, selectivity. Here, we report a highly sensitive and selective electrochemical immunosensor for rapid and simple serological diagnosis of cryptococcosis. The sensor employs novel multi-epitope chimeric proteins (A, B, C, and D) directly an

PURPOSE OF REVIEW: Current treatments for endemic mycoses are limited by toxicity, drug-drug interactions, the need for therapeutic drug monitoring, and suboptimal penetration to sequestered sites. At the same time, several new antifungal strategies and agents are nearing clinical availability and may change practice.
RECENT FINDINGS: Current literature highlights limitations of standard therapy for histoplasmosis, coccidioidomycosis, blastomycosis, paracoccidioidomycosis, talaromycosis, sporotrichosis, and emergomycosis, which often rely on amphotericin B-based induction regimens followed by

Fungal and atypical bacterial pneumonias are emerging infectious threats in companion animal medicine, fueled by ecological disruption, globalized pet travel, and immunomodulatory therapies being increasingly used. These infections challenge clinicians in ways that conventional bacterial pneumonia does not-presenting insidiously, extending beyond the lung, and not responding to empirical treatment. Delayed or missed diagnosis remains a costly, defining hallmark. Recognizing these conditions demands a shift in clinical thinking: one that weighs host susceptibility, exposure history, and geograp

Most histoplasmosis infections are asymptomatic or present as a self-limiting pulmonary illness; however, disseminated disease can occur in immunocompromised patients. Lower gastrointestinal (GI) bleeding (haematochezia) without other GI-related symptoms is an uncommon manifestation of disseminated histoplasmosis, posing a diagnostic challenge. We describe a renal transplant recipient in his 70s on chronic immunosuppressive therapy (tacrolimus and mycophenolate mofetil) who initially presented with progressive bilateral lower-extremity weakness and later developed haematochezia during hospitali

Gastrointestinal histoplasmosis is an under-recognized manifestation of disseminated Histoplasma capsulatum infection. Its ileocecal predilection closely mimics intestinal tuberculosis (TB) and Crohn's disease, posing serious diagnostic challenges, particularly in TB-endemic regions. A 26-year-old immunocompetent male presented with a 3-month history of diffuse abdominal pain, hematochezia, and 16 kg weight loss. He had received 60 days of empirical four-drug anti-tubercular therapy (ATT) without improvement. Examination revealed cachexia, pallor, digital clubbing, pedal oedema, a hard palate

Reactivation of latent fungal infections poses a substantial risk for solid organ transplantation (SOT) and hematopoietic cell transplantation (HCT) recipients. We describe receipt of pretransplant fungal infection testing in an exploratory analysis among patients in a large US commercial health insurance database. We identified patients who received an SOT or HCT during January 1, 2018-January 31, 2025, and evaluated testing practices for selected fungal (blastomycosis, coccidioidomycosis, cryptococcosis, histoplasmosis) and non-fungal infections (including hepatitis B virus [HBV], hepatitis
