Perinatal Management of Esophageal Varices.

IMPORTANCE: Physiological changes in pregnancy pose unique challenges for cirrhosis and portal hypertension, particularly during the third trimester and intrapartum setting. Pregnancy may increase the risk of worsening of esophageal varices and life-threatening variceal hemorrhage. Therefore, it is important that clinicians recognize risk factors for appropriate referral and management for delivery planning to mitigate risks associated with acute variceal bleeding in pregnancy. OBJECTIVE: Review preconception counseling, antepartum surveillance, and intrapartum management of patients with eso

Time-surrogate variables enhance the association between cardiotocographic features and intrapartum hypoxic-ischemic encephalopathy.

Interruptions to the flow of oxygenated blood to the fetal brain during labor can lead to hypoxic-ischemic encephalopathy (HIE). Timely interventions for suspected hypoxemia are crucial to prevent neurological injury. Prior cardiotocography (CTG) based prediction systems have focused only on the end of labor, when preventive measures are unlikely to be effective. We previously demonstrated that accounting for proximity to birth improved the association between CTG features and outcome. Since proximity to birth cannot be known prospectively, other time-surrogate variables (TSVs) may be more cli

Fourier Evaluation of Tracings and Acidosis in Labor (FETAL) Framework: An In-Silico Evaluation of a Spectral-Divergence Method.

Cardiotocography (CTG) is central to intrapartum fetal surveillance, yet visual interpretation remains limited by poor reproducibility and low specificity for fetal acidemia. Spectral analysis may retain clinically relevant information that is lost when fetal heart-rate variability is reduced to a single amplitude measure or a broad frequency-band ratio, while also providing an interpretable input for clinical decision support. We therefore developed the Fourier Evaluation of Tracings and Acidosis in Labor (FETAL) framework and used Monte Carlo simulation to determine whether its proposed spec

Life-threatening fetal goiter: diagnosis and outcomes in four consecutive cases with critical appraisal-why this diagnosis is not obviously trivial.

BACKGROUND: The diagnosis of a fetal goiter constitutes a potentially life-threatening in utero condition for the fetus. Even with early diagnosis and close therapy with highly specialized interdisciplinary management, the prognosis remains poor. We discuss diagnostic and therapeutic challenges regarding the pre- and postnatal outcome based on four consecutive cases with a critical appraisal of latest heterogeneous recommendations in literature. METHODS: A retrospective review of four cases of fetal goiter is presented that were diagnosed at a tertiary university centre during targeted ultras

Adverse Pregnancy Outcomes and Prevalence Associated With Arboviral Infections (Zika, Dengue, and Chikungunya): An Umbrella Review of Systematic Reviews and Meta-Analyses.

Maternal infection with Zika virus (ZIKV), dengue virus (DENV), or chikungunya virus (CHIKV) may increase the risk of adverse pregnancy and foetal outcomes, but evidence credibility varies. The aim of this study is to evaluate and compare the strength of evidence from systematic reviews and meta-analyses on these arboviruses and pregnancy outcomes. PubMed, Web of Science, and Cochrane Library from inception to December 31, 2025 were searched. Two reviewers independently screened studies, extracted data, and assessed methodological quality using AMSTAR 2. Evidence credibility was graded as high

Epigenetic Suppression of RASAL1 by HDAC3 and Cofactor YY1 Promotes Fibroblast-Myofibroblast Transition and Renal Fibrosis.

AIM: Astronauts return to Earth with reduced red cell masses and hypovolaemia. Not surprisingly, when they stand, their heart rates may speed inordinately, their blood pressures may fall, and some may experience frank syncope. We studied autonomic function in six male astronauts (average +/- SEM age: 40 +/- 2 years) before, during, and after the 16-day Neurolab space shuttle mission. METHOD: We recorded electrocardiograms, finger photoplethysmographic arterial pressures, respiration, peroneal nerve muscle sympathetic activity, plasma noradrenaline and noradrenaline kinetics, and cardiac outpu

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Peripartum and Perinatal Outcomes in Pregnant Women with Elevated Preconceptional Body Mass Index in a Maternity Hospital (Care Level IV).

INTRODUCTION: Preconception obesity is a risk factor for pregnancy and delivery, which is why giving birth in a perinatal center (care levels I and II) is recommended. There are currently no studies which have investigated the birth outcomes of obese patients based on the care level of the maternity hospital. This study aims to assess the effect of a higher body mass index prior to conception on maternal and fetal outcomes in a maternity hospital (care level IV). PATIENTS AND METHODS: A total of 5616 pregnant women who gave birth between 2016 and 2023 were investigated in this retrospective c

Improving apgar scores and reducing perineal injuries through midwife-led quality improvements: an observational study in Uganda.

BACKGROUND: Globally, the quality of maternal and newborn care remains inadequate, as seen through indicators like perineal injuries and low Apgar scores. While midwifery practices have the potential to improve care quality and health outcomes, there is a lack of evidence on how midwife-led initiatives, particularly those aimed at improving the use of dynamic birth positions, intrapartum support, and perineal protection, affect these outcomes. OBJECTIVE: To explore how the use of dynamic birth positions, intrapartum support, and perineal protection impact the incidence of perineal injuries an

Association between diabetes mellitus and fetal and maternal outcomes during and after pregnancy-a nationwide study.

INTRODUCTION: Diabetes Mellitus (DM) affects both the mother and fetus during and after pregnancy. Multiple studies have shown the prognostic impact of DM on maternal and fetal outcomes, but studies at the national level are limited. Therefore, we aimed to conduct this nationwide study. MATERIALS AND METHODS: This retrospective study used a Nationwide Inpatient Sample 2020 (NIS) using International Classification of Diseases (ICD-10) codes for adult pregnant patients. STATA version MP14.2 was used mainly for analysis. We used Fischer's exact test to compare proportions, the student's t-test t

Vancomycin Concentrations in Umbilical Cord Blood After Intrapartum Exposure.

Little is known about the fetal-neonatal pharmacokinetics of maternally administered, weight-based vancomycin dosing for group B streptococcus (GBS) intrapartum antibiotic prophylaxis. Our objective was to quantify vancomycin concentrations in umbilical cord blood at birth after weight-based maternal intrapartum vancomycin administration and to assess cord blood vancomycin levels relative to the established GBS clinical minimum inhibitory concentration (MIC) breakpoint. Using a convenience sample of stored sera from our biorepository, we measured vancomycin levels in umbilical cord blood from

Global and regional causes of maternal deaths 2009-20: a WHO systematic analysis.

BACKGROUND: Maternal mortality is not on track to meet Sustainable Development Goal (SDG) target 3.1 of a global maternal mortality ratio below 70 per 100 000 livebirths by 2030. Updated evidence on causes of death is needed to accelerate progress. METHODS: We conducted a multi-strategy systematic review to identify causes of maternal deaths occurring in 2009-20. Data sources included civil registration and vital statistics systems data from the WHO Mortality Database, reports published by Member States, and national and subnational journal articles identified via bibliographic databases. Bifidobacteria support optimal infant vaccine responses.

Accumulating evidence indicates that antibiotic exposure may lead to impaired vaccine responses1-4; however, the mechanisms underlying this association remain poorly understood. Here we prospectively followed 191 healthy, vaginally born, term infants from birth to 15 months, using a systems vaccinology approach to assess the effects of antibiotic exposure on immune responses to vaccination. Exposure to direct neonatal but not intrapartum antibiotics was associated with significantly lower antibody titres against various polysaccharides in the 13-valent pneumococcal conjugate vaccine and the Ha
