Feasibility of transperineal ultrasound in the delivery room for the management of obstetric anal sphincter injuries: a randomized controlled trial.

INTRODUCTION: Obstetric anal sphincter injury (OASI) is one of the major complications of vaginal delivery. Several lesions remain clinically undiagnosed at delivery. The objective of this study was to evaluate the feasibility and impact of performing a transperineal ultrasound (TPUS) in the immediate postpartum period by a non-expert obstetrician. METHODS: We conducted a prospective bicentric randomized controlled trial comparing clinical examination alone (CE group) versus clinical examination associated with transperineal ultrasound (TPUS group), in primiparous women following a vaginal bi

Mapping historical diaries into knowledge graphs: a multi-granular approach.

PLAIN ENGLISH SUMMARY: There are new ways to engage people with science and research but many patient support groups and charitable organisations still hold traditional meetings to provide updates on their activities and to report new developments in their field of interest. These meetings often feature presentations given by medical doctors or, in the case of research-focussed organisations, by research scientists.Receiving feedback from people who are confused and sometimes upset by some types of information, and the way it is presented at meetings, made us think about better ways for resear

Subclassification of second-degree tears at delivery: creation and reported outcomes.

BACKGROUND: Perineal tears at delivery are common. The current WHO classification system compacts all the varying extents of second-degree tears into one code. Some tears lead to long-term injuries. The correct identification and classification of disease is necessary for correct clinical management as well as for research. Regulatory standards govern care practices. This article describes the process of creating and testing new subclassifications for second-degree tears at delivery. METHODS: The development and implementation of new subclassifications of second-degree perineal tears after de

Acute pain management after vaginal delivery with perineal tears or episiotomy.

BACKGROUND: A vaginal delivery may be associated with acute postpartum pain, particularly after perineal trauma. However, pain management in this setting remains poorly explored. OBJECTIVE: The aim of this systematic review was to evaluate the literature and to develop recommendations for pain management after a vaginal delivery with perineal trauma. EVIDENCE REVIEW: MEDLINE, Embase, and Cochrane databases were searched for randomized controlled trials (RCTs) and systematic reviews assessing pain after a vaginal delivery with perineal tears or episiotomy until March 2023. Cochrane Covidence

Continuous versus interrupted suturing for second-degree perineal tears and episiotomies: A meta-analysis of randomized trials.

INTRODUCTION: Perineal injuries during vaginal delivery, including second-degree lacerations and episiotomies, require effective suturing techniques. Despite being associated with decreased pain and improved short-term outcomes, continuous suturing has inconclusive results regarding long-term benefits. OBJECTIVE: To assess the comparative efficacy and safety of continuous versus interrupted suturing for the repair of second-degree perineal tears and episiotomies. METHODS: Data were analyzed using random-effects models to calculate pooled risk ratios (RR) with 95 % confidence intervals (CI).

Varying Incidence and Definitions of Infection and Wound Dehiscence After Second-Degree Tear or Episiotomy: A Systematic Review.

INTRODUCTION AND HYPOTHESIS: Obstetric tears are a common complication of vaginal delivery, with second-degree tears and episiotomies being the most prevalent types. Despite their prevalence, the incidence of healing complications are not clearly defined. This review aims to consolidate the current knowledge. METHODS: This systematic review examines the risk of wound dehiscence and infection following second-degree tears and episiotomies. Literature searches were conducted across three databases: PubMed, Embase, and Cochrane. Two authors independently and blindly selected studies deemed relev

Effects of manual perineal protection and pushing techniques used in the second stage of labor on perineal outcomes: a randomized controlled trial of combinations of strategies.

BACKGROUND: Manual perineal protection and pushing techniques can impact the incidence of perineal trauma. Limited data exist on the impact of combinations of management strategies employed during the second stage of labor on perineal outcomes. The objective of this study was to evaluate the combined impact of interventional and relatively more spontaneous techniques employed during the second stage of labor on perineal outcomes. MATERIALS AND METHODS: This randomized controlled study was carried out in the maternity clinic of a state hospital. Low-risk, term nulliparous women with vertex pre

Opioid consumption and pain scores following perineal lacerations: a retrospective cohort study (2019-2023).

BACKGROUND: Perineal lacerations frequently complicate vaginal deliveries and impact recovery. METHODS: We examined the relationship between laceration severity and opioid consumption with scheduled multimodal analgesia through a retrospective cohort study (2019-2023) of 14,477 vaginal deliveries. Propensity score matching was used to address baseline group imbalances, followed by logistic regression analysis. RESULTS: Distribution included: no lacerations (26.9%), first- or second-degree (69.0%), and third- or fourth-degree (4.0%). Patients with third- or fourth-degree lacerations more freq

Women's experience with a first- and second-degree perineal tear and episiotomy - a qualitative study.

BACKGROUND: Perineal tears are associated with both physical- and psychological complications, and studies have found that women’s individual experience of the pain from the tear does not necessarily reflect the extent and severity of the tear. Women seem to experience a lack of consistency and sufficient follow up in the post-partum period related to perineal trauma. The aim of this study was to explore women’s experiences with first- and second- degree perineal tears and episiotomy in the post-partum period in an outpatient perineal clinic at a tertiary hospital in Norway. Impact and safety of perineal massage in late pregnancy on delivery outcomes among primiparous women: a propensity score matching analysis.

BACKGROUND: Primiparas face a higher risk of perineal injury, which may adversely affect maternal and infant health. Perineal massage has been proposed as an intervention to improve birth outcomes, but evidence of its effectiveness remains inconsistent, and concerns about its safety persist. This study aimed to evaluate the effects and safety of late-pregnancy perineal massage on birth outcomes in primiparas using propensity score matching. METHODS: A retrospective cohort study was conducted involving 914 primigravid women who received routine perineal massage at a tertiary A-level maternity

Strategies for improving maternal care for ethnic minority women with obstetric anal sphincter injuries in the UK.

BACKGROUND: Women from minority ethnic groups in the UK have the highest prevalence of obstetric anal sphincter injuries (OASI), including third- and fourth-degree perineal tears sustained during childbirth. Incorporating the voices of mothers at higher risk of OASI is crucial in developing strategies to improve care and well-being. AIM: To identify strategies perceived as important by women with lived experience of OASI in Leicester, UK, to improve their care and well-being. METHODOLOGY: Women at high risk of and with lived experience of OASI in Leicester, UK, were invited, through our loca

Severity of perineal tears and deviations in perineal body anatomy: A three-dimensional ultrasound study.

INTRODUCTION: Perineal tears are classified into four degrees based on severity. Second-degree tears, which involve the muscles fusing into the perineal body, but not the anal sphincter, vary in extent and have recently been subcategorized. However, knowledge about their long-term anatomical impact on the perineal body remains limited, as postpartum evaluation has largely relied on inspection and palpation. This study aimed to examine the association between the severity of perineal tears not affecting the anal sphincter, diagnosed at delivery, and deviations in the muscles fusing into the per

Comparative study of postpartum sexual function: Second-degree tears versus episiotomy outcomes.

PURPOSE: The effects of episiotomy and second-degree tears on postpartum sexual function are key areas of enquiry in women's health research. Episiotomy and second-degree tears are common procedures and injuries that occur during childbirth. Understanding their impact on post-childbirth sexuality is crucial to women's overall well-being. This study aimed to examine the relationship between episiotomy, second-degree tears, and post childbirth sexuality. METHODS: A cross-sectional design was employed, including 83 women who gave birth to Cáceres in 2017. Participants were evaluated based on soc

Minimizing tearing during vaginal delivery with a perineal protection device: a randomized controlled trial.

BACKGROUND: There is a growing body of evidence indicating that second-degree tears cause both short- and long-term consequences. Very few preventative measures have been found to reduce the incidence of these tears. OBJECTIVE: This study aimed to investigate whether the use of a perineal protection device during vaginal birth reduces severe perineal tearing (grade ≥2 tear) in primiparous women compared with routine manual perineal support. STUDY DESIGN: A single-center randomized controlled trial was conducted in Sweden from 2019 to 2021. Primiparous women at term were randomly allocated to

Retrospective Case-Control Study of Extended Birth Perineal Tears and Risk Factors.

BACKGROUND: A perineal tear is a rupture of the skin or muscle between the vagina and anus (perineum). A third-degree tear is one type of extended perineal tear (EPT), and it involves the penetration of the anal sphincter muscle. Another type of EPT is a fourth-degree laceration, which penetrates deeper into the lining of the anus or rectum. The stretching of the perineum during childbirth may result in perineal trauma. Invasive surgical interventions are required for the treatment of EPTs. For this reason, the reduction of their incidence can be achieved by fully comprehending the risk aspect

[Obstetrical and neonatal prognosis of patients with a history of female genital mutilation].

OBJECTIVE: Female genital mutilation (FGM) covers all procedures involving partial or total removal of the external genitalia for non-therapeutic purposes. The period of pregnancy and childbirth is probably more at risk of complications for these women. The main aim of this study was to compare obstetrical, maternal and neonatal outcomes in patients with a history of female genital mutilation with patients without such a history. METHODS: All deliveries taking place between January 2005 and June 2022 at Besançon University Hospital in patients with a history of FGM were included. This group w

Occurrence and risk factors for second-degree perineal tears: A prospective cohort study using a detailed classification system.

BACKGROUND: The amount of tissue trauma within second-degree perineal tears varies widely. Therefore, subcategorization of second-degree tears and a better understanding of their occurrence and risk factors are needed. The aim of this study was to assess the occurrence of perineal tears when second-degree tears were subcategorized. Furthermore, we aimed to assess the association between variables related to perineal anatomy and other potential risk factors, with second-degree tear subcategories. METHODS: This prospective cohort study included 880 primiparous and multiparous women giving birth
