[Dehydration in elderly: causes, prophylaxis and treatment].

Dehydration is common among older adults. A narrative literature review was conducted to determine the current state of knowledge regarding the symptoms, prevention, and early treatment of dehydration. Fifty-three full-text articles from the years 2015-2025 were selected for the final analysis. The most common cause of dehydration in older people is insufficient fluid intake. Dehydration can also be a symptom of diseases common in older adults, including urinary tract infections, pneumonia and other febrile illnesses, as well as diabetes. In addition to medical causes, dehydration can result f

What you saw was what you got? Correcting reported incidence data for testing intensity.

During the COVID-19 pandemic, non-pharmaceutical interventions were employed to limit the spread of the SARS-CoV-2 virus. In some countries, this included large-scale voluntary testing of non-symptomatic individuals to gauge the prevalence in the general population and to identify asymptomatic and pre-symptomatic infections in order to reduce disease spread. Differences between the intensity of testing means that comparing case-counts across borders may be deceptive. In this work, we use mathematical modeling to investigate the dynamics of both observed and unobserved infections as a function

ICU-acquired pneumonia beyond pathogen detection.

Intensive care unit-acquired pneumonia is often treated as a binary diagnosis, although clinical deterioration, imaging and microbial detection describe different aspects of disease. This Perspective proposes an explicit, graded and revisable assessment of whether a detected organism plausibly causes the current lung injury. The framework encompasses pneumonia arising during ventilation, pneumonia that precedes intubation and pneumonia in non-ventilated ICU patients. It places molecular results within clinical, anatomical, microbiological and host context and uses formal reassessment at 48-72

Mycobacterium tuberculosis complex Lineage 8 causes drug resistant pulmonary tuberculosis also in South Kivu, Eastern Democratic Republic of Congo.

BACKGROUND: Lineage 8 (L8) occupies a unique phylogenetic position as a sister clade to all other known Mycobacterium tuberculosis complex (MTBC) lineages. Although the initial identification of L8 strains was limited to Rwanda and Uganda, the complete geographical distribution of this lineage remains undetermined due to limited genomic surveillance across eastern and central Africa. METHODS: We performed whole genome sequencing (WGS) on MTBC isolates from rifampicin-resistant tuberculosis (TB) patients' sputum in Bukavu, South Kivu province, eastern Democratic Republic of Congo (DRC). Lineag

Malignant Large Bowel Obstruction Masquerading as Benign Inflammatory Disease in a Young Male: A Cautionary Tale of Diagnostic Delay.

We present the case of a 35-year-old male with an acute large bowel obstruction (LBO) secondary to a sigmoid mass, which was proximally diverted with a loop colostomy at an outside hospital. The mass could not be resected, and tissue could not be obtained for pathological diagnosis because of ongoing concerns about infection. Ultimately, the patient was found to have a frozen, hostile abdomen, and a palliative percutaneous endoscopic gastrostomy tube was placed for venting. We discuss surgical management options, pathology, and a brief review of the literature.

The dual nature of Fusarium oxysporum Fo47: from plant disease suppression to potential pathogenicity risks.

Fungal diseases threaten crop productivity, and the limitations of intensive fungicide use highlight the need for sustainable plant protection strategies. The non-pathogenic, endophytic strain Fusarium oxysporum Fo47 is one of the best-characterized models of disease-suppressive fungi. This review summarizes current knowledge on Fo47 as a beneficial endophyte and biological control agent, while also considering biosafety concerns related to the horizontal transfer of pathogenicity determinants. Fo47-mediated protection involves direct mechanisms, including competition, antibiosis and physical

Acute Post-traumatic Invasive Basidiobolomycosis With Concomitant Mucormycosis in the Southeastern United States.

Basidiobolus spp., belonging to the order Entomophthorales, cause a rare group of fungal infections known as basidiobolomycosis, which has been described as causing subcutaneous and gastrointestinal clinical manifestations in both immunocompetent and immunocompromised patients. Basidiobolomycosis is thought to be endemic to subtropical and tropical regions such as Asia, Africa, South America, and the southwestern United States. Herein, we describe a case of tissue-invasive Basidiobolus infection with concomitant mucormycosis in a middle-aged woman following a traumatic motor vehicle accident i

Periapical Granuloma: A Case Report.

A periapical granuloma is a fleshy mass of tissue invaded by varied numbers of chronic inflammatory cells, namely, epitheloid cells (modified tissue macrophages), mast cells, lymphocytes, and plasma cells, usually attached to the apex of a nonvital carious tooth as a sequelae of periapical infection from the diseased pulp. The growth of periapical granulomas develops in spurts amidst recurrent acute exacerbations. Periapical granulomas may form as the initial periapical pathosis or as a result of a quiescent periapical abscess. These lesions might not always be static. Radiographic changes are

Isolation and characterization of multidrug-resistant Proteus mirabilis from diarrheic sika deer fawns in Northeast China.

Diarrhea in sika deer fawns is a multifactorial disease that severely affects the development of deer farming in China. In the present study, we focused on Proteus mirabilis isolates recovered from diarrheic sika deer fawns in Heilongjiang, Jilin, and Liaoning provinces from 2022 to 2024. A total of 75 P. mirabilis isolates were identified using colony morphology and Gram staining together with 16S rRNA gene sequencing, type-strain-inclusive phylogenetic analysis, and atpD gene-targeted PCR as a supplementary molecular assay. Virulence genes and antimicrobial resistance genes were detected by

Melioidosis Presenting as a Solitary Liver Abscess With Culture-Negative Septic Arthritis: A Case Report.

Melioidosis is an infection caused by Burkholderia pseudomallei with varied clinical manifestations, particularly in patients with diabetes mellitus. Hepatic involvement usually occurs as part of multifocal disease, while a solitary liver abscess is uncommon. We report a 49-year-old man with poorly controlled type 2 diabetes mellitus who presented with fever and right upper quadrant pain. Ultrasonography showed a right lobe liver abscess, and empirical treatment with ceftriaxone and metronidazole was started. Despite treatment, he developed persistent fever, increasing leukocytosis, hypotensio

Comprehensive genetic screening of the South African Parkinson's disease study collection using the NeuroBooster array.

INTRODUCTION: Although there have been significant advances in the genetic etiology of Parkinson's disease (PD), most of this research has been conducted in European populations. This study aimed to characterize the genetic landscape of PD in South Africa, whose populations are renowned for their extensive genetic diversity and admixture. METHODS: We recruited 689 PD probands across South Africa. These participants were genotyped using the NeuroBooster array in collaboration with the Global Parkinson's Genetics Program (GP2). An in-house computational workflow was used to identify pathogenic

Immune mechanisms underlying endothelial and placental dysfunction in leptospirosis during pregnancy.

Leptospirosis is a zoonotic disease caused by pathogenic bacteria of the genus Leptospira and represents a major public health concern worldwide. Although its systemic manifestations are well documented, the consequences of leptospirosis for reproduction and pregnancy, particularly in humans, remain poorly understood. This narrative review summarizes the principal reproductive outcomes associated with leptospirosis and examines the mechanisms that may contribute to pregnancy complications. In livestock, leptospirosis is a well-established cause of reproductive failure, including abortion and i

Isolated Acute Tubulointerstitial Nephritis in a Myeloperoxidase-ANCA-Positive Patient Without Crescentic Glomerulonephritis.

Kidney involvement in ANCA-associated vasculitis is usually recognized by its hallmark lesion, necrotizing crescentic glomerulonephritis, and clinicians naturally anchor their diagnostic expectations to this pattern. Rarely, however, the injury appears largely confined to the tubulointerstitium, with glomeruli that show no necrotizing or crescentic lesions, a presentation that is easily mistaken for drug-induced or infection-related interstitial nephritis. We describe an elderly woman who developed dialysis-requiring severe kidney failure soon after a brief febrile illness and was found to hav

Immune-modulating copper-doped bioactive glass-based injectable hydrogel for periodontitis therapy.

BACKGROUND: Periodontitis, a chronic inflammatory disorder driven by microbial dysbiosis and aberrant host immune responses, poses a significant challenge to periodontal tissue regeneration owing to persistent infection and impaired osteogenesis. OBJECTIVE: Herein, we engineered an injectable hydrogel (denoted Cu@BG-HA) via borate ester crosslinking of copper-doped mesoporous bioactive glass nanoparticles (Cu@BG, 480 nm) with hyaluronic acid, aiming to concurrently address antibacterial, immunomodulatory, and osteogenic deficits in the periodontal microenvironment. METHODS: The Cu@BG-HA hydr

Social Determinants of Health and Behavioral Risk Factors in the Tuberculosis-Type 2 Diabetes Syndemic: A Narrative Review.

The co-occurrence of tuberculosis (TB) and type 2 diabetes mellitus (T2DM) represents a growing global syndemic that disproportionately affects populations with limited access to healthcare and preventive resources. T2DM is an established independent risk factor for TB, while social determinants of health (SDoH) further contribute to disease susceptibility, delayed diagnosis, and poor clinical outcomes. This narrative literature review examines the impact of SDoH and modifiable lifestyle factors on the TB-T2DM syndemic and highlights opportunities for intervention. A literature review was cond

Bacteroides fragilis Empyema Secondary to Ruptured Pyogenic Liver Abscess: A Rare Cause of Septic Shock.

Pyogenic liver abscess is an uncommon but potentially life-threatening intra-abdominal infection associated with significant morbidity and mortality. Although hepatic abscesses most commonly present with systemic infection or intra-abdominal complications, transdiaphragmatic spread into the pleural cavity resulting in empyema is rare. Bacteroides fragilis, an anaerobic gram-negative bacillus that forms part of the normal gastrointestinal flora, is a well-recognized pathogen in polymicrobial intra-abdominal infections but is infrequently reported as a primary pathogen in empyema originating fro

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba

Mandibular Chondrosarcoma Mimicking an Odontogenic Infection: A Case Report.

Chondrosarcoma is a malignant mesenchymal tumor exhibiting cartilaginous differentiation. It is a relatively common primary malignant bone tumor, but its occurrence in the jaws is rare, particularly in the mandible. The clinical presentation may be nonspecific and may mimic odontogenic or inflammatory conditions, potentially resulting in delayed diagnosis. We report the case of a 37-year-old woman with mandibular chondrosarcoma initially presenting with persistent pain in the region of the right mandibular first molar. The lesion was initially managed as a dental condition, and endodontic trea
