EPH gestosis in the second trimester suggests the likelihood of a number of fetal anomalies; the status of the fetus and risk to mother are such that spontaneous delivery or hysterotomy is usually the outcome, often without a full prenatal investigation. The types of fetal disorders encountered in this clinical setting are listed. Effects of abnormal endocrine environment as well as primary fetal malformations are distinguishable in these fetuses.

An open-label study was undertaken to assess the clinical impact of multiple doses of intracervical prostaglandin E2 (0.5 mg) gel administered at 6-hour intervals. Fifty women with low Bishop scores requiring induction of labor were recruited. Two hundred eighty patients with vertex presentation who were in spontaneous labor and delivered neonates weighing 2 kg or less were examined for early ultrasound evidence of periventricular/intraventricular hemorrhage. This was correlated retrospectively with delivery route (cesarean section versus vaginal delivery) and delivery method (low forceps delivery versus normal spontaneous delivery) in the group born vaginally. No correlation was evident between early periventricular/intraventricular hemorrhage and delivery route or method.

Laufe forceps are divergent forceps designed to reduce compressive forces upon the fetal skull during delivery from the pelvic outlet. Here we have undertaken a retrospective matched analysis in which Laufe (N = 75) and Barnes (N = 75) forceps were used for occipitoanterior mid-cavity forceps delivery. A woman in labor and not wearing a seat belt sustained multiple fractures of the pelvis and femur while in an automobile accident. The fetus suffered fractures of the parietal and occipital bones. To minimize maternal pelvic and fetal injury, delivery by cesarean section was performed. The infant had generalized cerebral edema and intraventricular and subarachnoid hemorrhage, diagnosed shortly after birth. Motor abnormalities and residual injury to the periventricular white matter were evident at age 4 months. Use of seat belts may significantly diminish or prevent such injuries.

A 23 years old multiple pretransfused female with severe aplastic anemia (SAA) underwent bone marrow transplantation (BMT). Cyclophosphamide (200 mg/kg) was used for conditioning, donor buffy coat cells were given as rejection prophylaxis. 163 patients undergoing emergency cesarean section were prospectively and randomized evaluated to determine the effect of mezlocillin in reducing postoperative morbidity. We compared a single 5-gram preoperative dose and a perioperative 3-dose regimen, each of 2 g, with a placebo. During a 12-year period, when more than 106,000 women were delivered, 28 women with peripartum heart failure of obscure etiology that initially was diagnosed as peripartum cardiomyopathy were studied.
