A unique case of triplet pregnancy in a woman with uterus didelphys is described, with fetuses A and B in the right uterine horn and fetus C in the left horn. Missed abortion of fetus A was diagnosed in the 22nd week of gestation by real-time ultrasonography, but only in the 27th week of gestation did the right uterine horn contract, resulting in expulsion of fetus A. Fetus B was delivered by cesarean section 2 hours later. The epidemiological, teratological and genetic data on 134 index patients with omphalocele (79 isolated and 55 multiple ones) and on 134 matched controls born in Hungary 1970-1976 were studied medical records and by retrospective interview. One hundred and forty-five elective repeat cesarean sections performed in one large medical community during a two-year period were studied. The results of a preliminary clinical appraisal of a newly designed vacuum extractor - the McGoldrick rotational vacuum extracter is presented. This instrument is designed to combine the rotation advantages of the various rotating obstetric forceps with the multiple advantages of the conventional vacuum extractor in the management of persistent occipito posterior and occipito transverse positions. The design features include an elliptical shaped cup attached to a series of bars and universal joints which allow both rotational and traction forces to be applied to the foetal scalp.

Forty-six neonates with intracranial hemorrhage were classified into three groups on the basis of the major computerized tomography (CT) scan findings: Group I consisted of 24 cases of subarachnoid hemorrhage, Group II 20 cases of intracerebral and/or intraventricular hemorrhage, and Group III two cases of subdural hemorrhage. Since the intent of this chapter is to discuss the operative intervention in a normal labor and delivery, there has been no discussion of such operative obstetrical procedures as midforceps delivery, breech presentation and delivery, management of multiple pregnancy, or cesarean section. The incidence of and clinical associations with eosinophilia in a cross-section of hospitalized newborns had not been studied previously. The medical records of 200 such infants less than 29 days of age were reviewed. Total eosinophil counts were calculated and associations with a variety of clinical factors were examined. The following perinatal intensive care management protocol is suggested to minimize morbidity and mortality in multiple pregnancy. 1. Early diagnosis is essential, with ultrasound examination regarding as invaluable for all pregnant patients. If ultrasound examination has not been done, multiple pregnancy should be suspected in all patients who have a family history of dizygotic births, are large for dates or anemic, or have a low-grade preeclampsia. 2. Maternal care should be provided at a tertiary care (level III) perinatatl center, which is more fully equipped to manage multiple gestation.
