A carbohydrate loading fasting protocol versus 'Sip til Send': a randomised trial of two different fasting protocols at elective caesarean delivery.

INTRODUCTION: Pulmonary aspiration under general anaesthesia is a rare but serious risk in obstetric populations, leading to conservative fasting protocols prior to elective caesarean delivery (CD). However, prolonged fasting may negatively impact maternal comfort and metabolic status. 'Sip til Send' (STS), a liberal fasting protocol permitting clear fluids until theatre admission, has been shown to improve the perioperative patient experience. Pre-operative carbohydrate loading (CHO), as part of Enhanced Recovery After Surgery (ERAS) pathways, may offer additional benefits, yet its role in ob

Intrathecal dexmedetomidine for cesarean delivery and postoperative outcomes: a single-center retrospective cohort study (2019-2020).

BACKGROUND: Intrathecal opioids are the standard adjuvant for spinal anesthesia in cesarean delivery, but investigating alternatives is warranted. A community hospital began adding intrathecal dexmedetomidine 10 µg to spinal anesthetics for cesarean deliveries due to a local shortage in opioids. This retrospective study evaluates its impact on postoperative pain, opioid use, and hemodynamic outcomes. METHODS: After IRB approval, cesarean delivery cases conducted between 2019 and 2020 were screened. Exclusion criteria included epidural labor analgesia, general anesthesia, intrathecal catheters

Performance of Shock Index as a predictor of severe postpartum hemorrhage following cesarean section under spinal anesthesia: a retrospective cohort study.

BACKGROUND: An early diagnosis of a postpartum hemorrhage (PPH) can be challenging. The Shock Index (SI), the ratio of heart rate to systolic blood pressure, appears to be a useful indicator for the early diagnosis of severe PPH after vaginal delivery. Our primary aim was to evaluate if the SI has a predictive value for severe PPH following cesarean section (C-section) under spinal anesthesia (SA) and its best predictive threshold. METHODS: Retrospective single-center trial including women scheduled for C-section under SA with prevention of maternal hypotension by phenylephrine, and complicat

Factors for successful vaginal delivery in twin pregnancies: a ten-year single-center retrospective study.

BACKGROUND: When certain conditions are met, a large randomized trial has suggested that for twin pregnancies, vaginal delivery does not increase adverse maternal or neonatal outcomes compared with cesarean section. However, the choice of the delivery mode is sometimes controversial, and policies can vary across different facilities. Identifying factors associated with successful vaginal delivery is important in reducing unnecessary cesarean sections. METHODS: We retrospectively analyzed 370 twin pregnancies delivered after 32 weeks of gestation at our hospital between April 2012 and March 20

The Impact of Maternal COVID-19 Severity on Obstetrical and Neonatal Outcomes: A Retrospective Study.

INTRODUCTION: Pregnancy induces physiological and immunological changes that increase susceptibility to severe outcomes from viral respiratory infections. Global meta-analyses have established that coronavirus disease 2019 (COVID-19) in pregnancy is associated with significantly elevated risks of adverse outcomes, including maternal ICU admission and preterm birth. However, region-specific data from Lebanon and the Middle East are scarce. This study aimed to evaluate the association between COVID-19 severity and obstetrical/neonatal outcomes in a Lebanese cohort. MATERIALS AND METHODS: A retr

Comparison of Blood Loss Between Volatile and Total Intravenous Anesthesia in Cesarean Delivery: A Single-Center Retrospective Observational Study.

BACKGROUND: Volatile anaesthetics have been suspected to increase intraoperative blood loss during caesarean section by causing uterine relaxation, but evidence from human studies remains inconclusive. METHODS: We conducted a retrospective observational study using a comprehensive perioperative dataset from the University of Tsukuba Hospital. We included 276 caesarean deliveries performed under general anaesthesia between April 2012 and March 2022, comparing patients who received volatile anaesthesia (n = 230) versus total intravenous anaesthesia (TIVA, n = 46). The primary outcome was intrao

Machine Learning Approach to Predict Emergency Cesarean Sections Among Nulliparous Women.

Introduction The obstetrical team's efforts are consistently focused on minimizing the number of cesarean sections, particularly in nulliparous women. One of the most crucial steps is to understand the risk factors that predispose the woman to a cesarean section. This study aimed to identify the predictors of emergency cesarean sections in nulliparous women using a machine learning approach. Methods A retrospective cohort study was carried out at a maternal tertiary center in Iran among nulliparous women with a single cephalic pregnancy, ≥37 weeks of gestation, and induced or spontaneous labor

Cell Salvage in the Management of Postpartum Hemorrhage: A Comprehensive Review of the Current Literature.

Postpartum hemorrhage (PPH) remains a leading contributor in maternal illness and death worldwide, and its incidence continues to rise. This has led to an increased reliance on allogeneic blood transfusions (ABT), which are limited in supply and carry significant health risks. Cell salvage (CS), which collects and transfuses a patient's own blood, has been proposed as a useful adjunct or alternative. This narrative review aims to evaluate and summarize the current literature on obstetric CS (OCS) in the management of PPH, with a focus on efficacy, safety, cost-effectiveness, and future researc

Uterine Tone Numeric Rating Score as an Early Indicator of Major Postpartum Hemorrhage during Cesarean Delivery: A Prospective Observational Study.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading preventable cause of maternal mortality. Most PPH cases are caused by uterine atony, which is inconsistently defined in clinical care. The electronic health record was used to prompt communication between the anesthesia and obstetric care teams about uterine tone using a validated 11-point numeric rating scale (NRS) at 0, 5, and 10 min after placental delivery for all cesarean deliveries at our institution. The primary hypothesis was that lower uterine tone NRS would be strongly associated with progression to major PPH. METHODS: This was

Effect of Epidural Analgesia on the Duration of Induced Labor in Primiparous and Multiparous Women: A Retrospective Study.

Context Epidural analgesia is commonly used during labor; however, its effect on the duration of labor, particularly in induced labor, remains unclear. Most previous studies have focused on spontaneous labor, with limited data on induced labor. Notably, very few studies have directly investigated how epidural analgesia affects labor duration and delivery outcomes in the setting of planned labor induction. Aims This study aimed to elucidate the effects of epidural analgesia on labor duration and delivery outcomes in primiparous and multiparous women undergoing labor induction. Settings and desi
