Post-cesarean Sepsis Secondary to Vernix Caseosa Peritonitis: A Diagnostic Challenge With Misleading Imaging.

Vernix caseosa peritonitis is an uncommon but important cause of postoperative intra-abdominal sepsis following cesarean delivery, resulting from a sustained foreign body inflammatory reaction to fetal vernix introduced into the maternal peritoneal cavity, typically manifesting within hours to days after surgery and often mimicking other acute surgical abdominal conditions with non-specific or misleading radiological findings. We report the case of a 43-year-old woman who presented five days after an uncomplicated cesarean section with severe abdominal pain, vomiting, and markedly elevated inf

Uterocutaneous fistula following cesarean section: a case report.

This case report details the clinical journey of a 37-year-old woman, gravida 6 para 6 + 1, who presented with persistent purulent discharge from her cesarean section scar following an elective cesarean delivery. Her obstetric history included two uncomplicated vaginal deliveries and four cesarean sections, with significant complications in the previous pregnancies, including gestational diabetes and placenta previa. Notably, ultrasound findings during her most recent pregnancy raised concerns for placenta accreta spectrum. Despite a vertical midline incision for cesarean delivery and intraope

Predictors of Postpartum Hemorrhage Following Repeat Cesarean Section: A Retrospective Cohort Study.

INTRODUCTION: Postpartum hemorrhage (PPH) is a major obstetric emergency that contributes substantially to maternal morbidity following cesarean delivery. The identification of high-risk patients undergoing repeat cesarean sections may help improve perioperative preparedness and maternal outcomes. The present study aimed to identify predictors of PPH following repeated cesarean sections. MATERIALS AND METHODS: A retrospective cohort study was conducted in the Department of Obstetrics and Gynecology on 200 women who underwent repeat cesarean section between January 2020 and December 2025. Demo

Surgical management of complex cesarean delivery.

Cesarean delivery is the most commonly performed surgery worldwide. Most obstetricians and gynecologists have become facile in performing cesarean delivery. However, the sequelae of multiple previous cesarean deliveries, such as postoperative uterine scar remodeling, and uterine dehiscence or rupture increase the complexity of cesarean delivery substantially. Adhesions, uterine dehiscence, uterine rupture, hysterotomy extensions, fibroids, and bleeding are common causes of increased surgical complexity, which varies depending on the degree of abnormal anatomy encountered and clinical acuity. Cervical Cerclage at 17 Gestational Weeks for Delayed-Interval Delivery at 15 and 27 Weeks of Gestation in a Dichorionic Twin Pregnancy: A Case Report.

Delayed-interval delivery refers to the delivery of the remaining fetus(es) at least 24 h after the delivery of the first fetus. There have been limited case reports of delayed-interval deliveries in twin pregnancies whose delivery of the first twin occurred in the early second trimester. We report a delayed-interval delivery of a dichorionic diamniotic pregnancy complicated by multiple fibroids. A 38-year-old primipara woman delivered the first twin at 15 weeks and 3 days of gestation. She did not have uterine contractions or any signs of infection, then McDonald cerclage was performed at 17

Mature cystic teratoma at a cesarean-scar uterine isthmus: a case report and literature review.

BACKGROUND: Mature cystic teratomas (MCTs) represent common ovarian germ cell neoplasms; primary uterine teratomas are exceedingly uncommon. We report an extremely rare MCT arising within a cesarean-section scar at the anterior uterine isthmus and summarize the key imaging clues and follow-up considerations. CASE PRESENTATION: A 41-year-old woman presented with persistent postmenstrual brown vaginal discharge for over six months. Transvaginal ultrasonography and pelvic magnetic resonance imaging (MRI) revealed a heterogeneous mass within the anterior uterine isthmus at the site of a prior cesa

Endometrium-free closure technique for hysterotomy incision at cesarean delivery.

Cesarean deliveries are associated with uterine scar defects that significantly impact women's health and future pregnancy outcomes. The methods used for hysterotomy closure following cesarean delivery have undergone significant evolution. Commonly used techniques, which are relatively rapid and maintain hemostasis, include the endometrium in a single-layer or "bulk" closure. Such endometrium-inclusive cesarean closure has been linked to an increased risk of scar defects and long-term complications. However, the available data show no difference in outcomes related to cesarean closure techniqu

Recurrent Hodgkin's Lymphoma Detected Using Abnormal NIPT in Pregnancy: A Case Report and Literature Review.

Background: Non-invasive prenatal testing (NIPT) examines cell-free DNA (cfDNA) in maternal serum, which includes both maternal DNA and apoptotic placental DNA. The presence of multiple aneuploidies or widespread abnormal patterns of gains and losses across chromosomes in a structurally normal fetus has been linked to maternal cancer. Case Presentation: The patient was a 22-year-old G1P0 with a history of classical Hodgkin's lymphoma in remission. Her NIPT collected at 14 weeks and 3 days was reported as a "no call". A second NIPT at a different laboratory showed multiple chromosomal aneuploid

The role of cesarean section surgical techniques in the prevention of isthmocele formation: retrospective cohort study.

KEY MESSAGE: Compared with conventional locked double-layer closure, an unlocked double-layer, endometrium-sparing closure was associated with lower isthmocele prevalence, greater residual myometrial thickness, and fewer postcesarean symptoms. If these findings are confirmed in prospective studies, adopting this closure approach could help to reduce morbidity related to the niche in routine cesarean practice. BACKGROUND: Isthmocele, a cesarean scar defect associated with abnormal bleeding, pelvic pain, and diminished fertility, is becoming more well known as a result of the increasing prevale

Preoperative risk score for invasive uterus-sparing hemostasis at cesarean delivery: a retrospective cohort study.

BACKGROUND: To develop a simple preoperative risk score identifying women requiring invasive hemostatic interventions during cesarean delivery. METHODS: This retrospective cohort included women undergoing cesarean delivery at a Hainan maternity hospital. The primary outcome was intraoperative use of intrauterine balloon tamponade and/or uterine artery ligation. Continuous variables were compared using Student's t test or Mann-Whitney U test, and categorical variables using chi-square or Fisher's exact test. Multivariable logistic regression identified independent risk factors, which were tran

Case report on resistant postpartum hemorrhage during cesarean section treated with a modified quadruple uterine belt suture technique for uterus preservation.

BACKGROUND: Severe postpartum hemorrhage during cesarean section is a critical obstetric emergency; delayed hemostasis may lead to hemorrhagic shock, hysterectomy, and even maternal mortality. Individualized uterine-preserving hemostatic strategies represent the central focus and greatest challenge in current clinical management. CASE PRESENTATION: This article reports a case of a patient at 40 + 2 weeks of gestation with concurrent gestational diabetes mellitus and prenatal fever. During cesarean section, the patient developed severe postpartum hemorrhage (with blood loss reaching 2,500 mL)

Bilateral anterior sacral meningoceles in pregnancy without sacral anomaly: a case report of a rare clinical entity.

This report describes a 32-year-old primigravida diagnosed with bilateral anterior sacral meningoceles without bony defect of the sacrum during pregnancy. The patient remained asymptomatic throughout the pregnancy, with regular monitoring via transvaginal ultrasound and MRI. An elective cesarean section was planned at 38 weeks. However, the patient presented in obstructed labor at 42 weeks and underwent an emergency cesarean section, resulting in the birth of a healthy infant. This case is unique as it involves bilateral anterior meningoceles without sacral anomalies. Anterior sacral meningoce

Successful pregnancy following extensive myomectomy for diffuse uterine leiomyomatosis.

BACKGROUND: Diffuse uterine leiomyomatosis (DUL) is a rare condition characterized by innumerable leiomyomas (fibroids) diffusely replacing the myometrium, often leading to severe infertility due to distortion of the uterine cavity. Although assisted reproductive technologies are commonly attempted, implantation failure is frequent and effective treatment strategies remain unclear. Extensive myomectomy, involving the removal of numerous fibroids with uterine reconstruction, has been proposed as a fertility-preserving option; however, it is associated with substantial surgical morbidity and pot
