Diagnosis and treatment of deep-vein thrombosis and approach to venous thromboembolism in obstetrics and gynecology.

Deep vein thrombosis (DVT) is a common condition in which the approach to its diagnosis has evolved over the years. Currently, an algorithm strategy combining pre-test probability, D-Dimer testing and compression ultrasound imaging allows for safe and convenient investigation of suspected lower-extremity thrombosis. Patients with low pre-test probability and a negative D-Dimer test result can have proximal DVT excluded without the need for diagnostic imaging. The mainstay of treatment of DVT is anticoagulation therapy, whereas interventions such as thrombolysis and placement of inferior vena c

How I manage venous thromboembolism in pregnancy.

In pregnancy, the combination of altered coagulation factors, stasis and vascular damage heightens the thrombotic risk and venous thromboembolism (VTE) is a major cause of maternal morbidity and mortality. In women presenting with pregnancy-related VTE, the wellbeing of both mother and fetus must be considered, necessitating a multidisciplinary approach. The risk of VTE is present throughout the pregnancy and is maximal during the postpartum period. Diagnosis may be difficult because the symptoms might mimic those of pregnancy and prospectively validated diagnostic protocols are lacking. Compr

Genetic analysis of patients with deep vein thrombosis during pregnancy and postpartum.

Deep vein thrombosis (DVT) is a serious pregnancy-related complication. Recent studies indicate that the genetic background for DVT differs with ethnicity. In our study, we enrolled 18 consecutive Japanese patients who had developed DVT during pregnancy and postpartum. We performed a genetic analysis of three candidate genes for DVT, protein S, protein C and antithrombin, in these patients. We found that four patients had missense mutations in the protein S gene, including the K196E mutation in two patients, the L446P mutation in one patient, and the D79Y and T630I mutations in one patient, as

Strategies for diagnosis and prevention of venous thromboembolism during pregnancy.

Pregnancy and the postpartum period have an increased incidence of venous thromboembolism (VTE). The condition is unique during this period for several reasons. Primarily, because there is complexity in diagnosing this condition in view of altered physiology and preexisting edema in pregnancy and also because there are restrictions on the use of certain drugs and a need for vigilant monitoring of anticoagulant activities of drugs during the period. The problem is compounded and assumes the highest order of significance since two lives are involved and all the investigations and management done

Phlegmasia cerulea dolens in patient with systemic lupus erythematosus in the remote postpartum period.

Vascular manifestations are not rare in systemic lupus erythematosus and, in most cases, are associated with antiphospholipid antibodies. Phlegmasia cerulea dolens is an unusual and severe complication of deep venous thrombosis of the lower limbs, which has a high mortality rate. In the literature, only two cases of phlegmasia cerulea dolens associated with primary antiphospholipid syndrome have been reported, but none associated with systemic lupus erythematosus. We report one case of phlegmasia cerulea dolens with rapid evolution to death in a patient with systemic lupus erythematosus in the

[Methods of operative interventions in postthrombotic disease of the crus].

Analysed herein are the findings of examination and outcomes of surgical management of 43 patients presenting with crural postthrombotic disease. Of these, eighteen patients were diagnosed as having stage II chronic venous insufficiency (CVI) and the remaining 25 had stage III CVI. All the patients had previously experienced acute thrombosis of deep crural veins. Eight patients had developed thrombosis during or in the immediate terms after typhoid fever, seven women in the postpartum period, and a further four women after endured operations on the organs of the abdominal cavity or small pelvi

Recommendations for the diagnosis and treatment of deep venous thrombosis and pulmonary embolism in pregnancy and the postpartum period.

Venous thromboembolism (VTE) in pregnancy and the postpartum is an important cause of maternal morbidity and mortality; yet, there are few robust data from clinical trials to inform an approach to diagnosis and management. Failure to investigate symptoms suggestive of pulmonary embolism (PE) is a consistent finding in maternal death enquiries, and clinical symptoms should not be relied on to exclude or diagnose VTE. In this consensus statement, we present our recommendations for the diagnosis and management of acute deep venous thrombosis (DVT) and PE. All women with suspected DVT in pregnancy

How I treat pregnancy-related venous thromboembolism.

Venous thromboembolism (VTE) complicates ~ 1 to 2 of 1000 pregnancies, with pulmonary embolism being a leading cause of maternal mortality and deep vein thrombosis an important cause of maternal morbidity, also on the long term. However, a strong evidence base for the management of pregnancy-related VTE is missing. Management is not standardized between physicians, centers, and countries. The management of pregnancy-related VTE is based on extrapolation from the nonpregnant population, and clinical trial data for the optimal treatment are not available. Efficacy and safety of nadroparin and unfractionated heparin for the treatment of venous thromboembolism during pregnancy and puerperium.

INTRODUCTION: The optimal treatment of pregnancy associated VTE (venous thromboembolism) has not been established yet. OBJECTIVE: The assessment of the efficacy and safety of low molecular weight heparin (LMWH) nadroparin and unfractionated heparin (UFH) used for the treatment of pregnancy and puerperium related VTE. Primary study goals were to analyze the incidence of recurrent VTE (proximal extension or pulmonary thromboembolism), thrombocytopenia, major and minor hemorrhages and skin allergic reactions. The study also included the incidence of miscarriages, stillbirth and neonatal abnormal

[Antithrombotic therapy in pregnancy].

Despite the fact of low prevalence of maternal death, deep venous thrombosis remains one of the most serious complication in pregnancy and puerperium. Virchows triad--vascular stasis, hypercoagulability, and vascular trauma plays the main role in the pathogenesis of deep vein thrombosis in pregnancy. Low molecular weight heparins and unfractionated heparins are the best treatment option. The aim of the treatment is to be effective in extension of thrombus and prevention of the postthrombotic syndrome and pulmonary embolism. Management of pregnant women with increased risk of venous thromboembo

Upper extremity deep venous thrombosis in the population-based Malmö thrombophilia study (MATS). Epidemiology, risk factors, recurrence risk, and mortality.

BACKGROUND: Deep venous thrombosis (DVT) is much less common in the upper than in the lower extremity. Furthermore, there is limited information on risk factors for and the prognosis of upper extremity (UE)DVT in the general population. AIMS: To estimate incidence, risk factors, and prognosis in UEDVT. MATERIAL AND METHODS: Among a total of 1203 patients with venous thromboembolism (VTE) diagnosed during 1998-2006 in the prospective population-based Malmö thrombophilia study, 63 (5%, 33 men [52%, age 54+/-17years], and 30 women [48%, age 55+/-22years]) had UEDVT and were evaluated concerning

Deep vein thrombosis and pulmonary embolism in pregnancy: diagnosis, complications, and management.

Venous thromboembolism is a leading cause for maternal mortality. Because of the increase risk for thromboembolism in pregnancy and the postpartum period, the clinician must be on high alert for the signs and symptoms and use appropriate diagnostics in a timely manner so that prompt anticoagulation therapy can be initiated. A diagnostic and management approach for both deep vein thrombosis and pulmonary embolism and for prophylaxis against thromboembolism in the obstetric patient are crucial to decreasing morbidity, mortality, and long-term sequelae.

Letter to the editor. Unilateral thalamic infarction in vein of galen and straight sinus thrombosis.

Deep cerebral venous thrombosis is an uncommon event. It usually involves the halami and adjacent areas on both sides. Unilateral thalamic infarction is extremely rare subsequent to deep cerebral venous thrombosis. Imaging findings of a female in the postpartum period with unilateral thalamic infarction and basal ganglia edema are presented.

Deep vein and intracardiac thrombosis during the post-partum period in Behçet's disease.

A 22-year-old woman presented on the 10th post-partum day with deep vein thrombosis involving the right ilio-femoral and popliteal veins. This thrombosis was refractory to conventional anticoagulation and subsequently over a period of 6 weeks progressed to involve inferior vena cava and right ventricle. A diagnosis of Behçet's disease was made on the clinical grounds of fever, night sweats, and recurrent oral ulcers. In view of refractory thrombosis, anticoagulation with lepirudin was commenced followed by thrombolysis with streptokinase. After thrombolysis, anticoagulation was switched to fon

[Exclusion of deep-vein thrombosis and pulmonary embolism using clinical decision rules and D-dimer tests].

Clinical diagnosis of a venous thromboembolism (VTE) is often difficult because the symptoms of this disorder are diverse and unspecified. The combination of a low probability clinical decision rule and an unremarkable D-dimer test is a safe way to exclude the presence of a VTE. Clinical decision rules for the diagnosis of a deep-vein thrombosis are available for primary and secondary care and clinical decision rules for the diagnosis of a pulmonary embolism is available for secondary care. Various D-dimer tests are available that differ with regard to sensitivity, specificity and duration of
