Blood demand and utilization among pregnant and postpartum women with and without HIV infections at the university teaching Hospital, Zambia.

BACKGROUND: Complications from pregnancy, primarily post-partum hemorrhage, including infections like HIV, are the leading causes of maternal mortality in sub-Saharan Africa (SSA). Blood products are not always available for obstetric emergencies because of chronic shortages in SSA countries. Understanding the quantity of blood products used by pregnant and post-partum women with and without HIV infections could inform strategies for the appropriate use of blood in resource-limited settings. METHODS: A prospective study was conducted of medical charts of all pregnant or postpartum (42 days fo

Recurrence of HIV-associated cryptococcal meningitis in pregnancy and the post-partum period.

We report a case of recurrent cryptococcal meningitis in a pregnant woman living with vertically acquired HIV. Despite effective treatment of primary infection prior to pregnancy, she later experienced a microbiological relapse of cryptococcal meningitis during pregnancy, and an immune-reconstitution inflammatory syndrome in the post-partum period. This case emphasises the impact of pregnancy related immunological changes on the natural history of complex infections. This case illustrates the challenges of managing opportunistic infections throughout pregnancy and the influence of perinatal im

Virologic suppression among HIV-positive pregnant and lactating women receiving antiretroviral therapy in Africa: A systematic review and meta-analysis.

BACKGROUND: The elimination of mother-to-child transmission of human immunodeficiency virus (HIV) is a key global public health priority. In Africa, virologic failure among people living with HIV continues to pose a significant public health challenge, affecting both individual well-being and community health. Maintaining viral load suppression is crucial to prevent vertical transmission of HIV and to minimize maternal morbidity and mortality. To stop the vertical transmission of HIV and lower the risk of maternal morbidity and mortality, it is important to achieve viral load suppression. Alth

Hypertension in a Randomized Trial of Dolutegravir- Versus Efavirenz-Based Antiretroviral Therapy in Pregnant and Postpartum Women with HIV.

INTRODUCTION: We performed an analysis of incident hypertension in a randomized trial comparing dolutegravir (DTG) + emtricitabine (F)/tenofovir alafenamide (TAF) versus DTG + F/tenofovir disoproxil fumarate (TDF) versus efavirenz (EFV)/F/TDF in pregnant and postpartum women with HIV. METHODS: Women were randomized at 14-28 weeks gestational age (GA) to start DTG + F/TAF, DTG + F/TDF, or EFV/F/TDF and followed through 50 weeks postpartum. The composite incident hypertension outcome was defined as initiation of antihypertensive medication or ≥2 elevated blood pressures categorized as elevated

HIV acquisitions, safety, and pharmacokinetics of lenacapavir for HIV pre-exposure prophylaxis in pregnant and lactating women (PURPOSE 1): a substudy of a phase 3, randomised controlled trial.

BACKGROUND: Lenacapavir demonstrated high efficacy and safety as pre-exposure prophylaxis (PrEP) in cisgender women, but its use during pregnancy and lactation when women are disproportionately vulnerable to HIV acquisition has not previously been described. We aimed to evaluate HIV acquisition, safety, and pharmacokinetics of lenacapavir for PrEP in pregnant and lactating women. METHODS: The randomised, double-blind, multicentre, active-controlled, phase 3 PURPOSE 1 study compared twice-yearly subcutaneous lenacapavir with daily oral emtricitabine-tenofovir alafenamide or emtricitabine-tenof

Advancing the inclusion of pregnant and lactating populations in HIV PrEP research: ethical, regulatory, and surveillance recommendations from a multisectoral working group.

Despite a 60% decline in new HIV infections since 1995, significant challenges persist. In 2023, 1.3 million new HIV infections occurred, with 645,500 in sub-Saharan Africa, where cisgender adolescent girls and women represent over two-thirds of new cases. Approximately one-quarter of vertical HIV transmissions are linked to infections during pregnancy and lactation, yet data on PrEP use in these populations remains generally limited due to their exclusion from clinical trials, despite a recent movement toward inclusion, such as in the PURPOSE 1 trials evaluating the efficacy and safety of len

Prognostic value of late gadolinium enhancement in peripartum cardiomyopathy: a systematic review and meta-analysis.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a rare but life-threatening condition occurring during late pregnancy or the postpartum period. Although late gadolinium enhancement (LGE) has prognostic value in other cardiomyopathies, its role in PPCM remains unclear. This study aimed to evaluate the prognostic significance of LGE in PPCM. METHODS: Three electronic databases were systematically searched to identify studies assessing the association between LGE and clinical outcomes in PPCM. Outcomes included reduced left ventricular ejection fraction (LVEF), arrhythmia, heart failure decompen

Salivary cortisol, progesterone, and estradiol levels across the perinatal period and associations with depressive symptoms.

Depression during pregnancy and postpartum has impacts on both the birthing parent and offspring. This period is characterized by changes in which stress (e.g., cortisol) and reproductive hormone (e.g., progesterone and estradiol) levels gradually increase across pregnancy and decrease rapidly postpartum, but there are mixed associations of cortisol, progesterone, and estradiol levels with depressive symptoms. Prior studies typically have focused on single hormones across time or multiple hormones at a single time point. The present study aimed to expand the literature by examining patterns of

Biological Mother-To-Child Living Donor Liver Transplantation: Early Vs. Late Postpartum Donation.

INTRODUCTION: Biological parental donations provide the best option for many pediatric recipients, yielding unique immunological benefits that may enable minimization of immunosuppression in transplanted children. However, living related maternal donation in the postpartum period may introduce an increased risk of donor complications due to the physiological changes of pregnancy and childbirth, and the optimal timing of postpartum living donation is unknown. METHODS: Using US national registry data, we characterized donor and recipient outcomes for pediatric living donor liver transplants per

Genital tract infections, antiretroviral treatment and HIV-1 viral load in early postpartum women: : implications for sexual and breastfeeding HIV transmission.

BACKGROUND: Maintaining viral suppression in the postpartum period is critical for preventing HIV transmission to sexual partners and breastfed infants. Despite systemic viral suppression, genital HIV shedding may still occur in some women. We examined the association between bacterial vaginosis (BV) and sexually transmitted infections (STIs) and genital HIV shedding in postpartum women. METHODS: In this cross-sectional postpartum study, we hypothesised that BV and other genital tract infections would modulate HIV viral load (VL) in the genital tract. Postpartum women with HIV, aged < 25 years

BACKGROUND: In recent times due to the advancement of haemodialysis techniques, there is increase in the life span of the patients with end-stage renal disease, which tends to increase the risk of complications of bacteraemia due to skin penetration for blood access and leading to propensity for more skeletal and spinal complications. Hence, early diagnosis of spine infection and early initiation of appropriate therapy are important in ensuring successful treatment and preventing further morbidity. Global estimates of tuberculosis incidence during pregnancy and postpartum: a rapid review and modelling analysis.

BACKGROUND: Despite known maternal, perinatal, and infant health risks of tuberculosis during pregnancy, global estimates of incidence remain scarce. Existing estimates are outdated, and do not include the postpartum period, HIV co-infection, age, or specific changes in risk, limiting our understanding of the true scale of disease in this understudied population. METHODS: In this rapid review and modelling analysis, we estimated the global tuberculosis incidence in pregnant and postpartum women using a population-based modelling approach. We searched MEDLINE and EMBASE, with no date or langua

Tuberculosis prevention in children, adolescents, and pregnant and postpartum women in South Africa.

Tuberculosis (TB), particularly drug-resistant TB (DR-TB), remains a major public health concern in South Africa (SA), with children, adolescents, and pregnant and postpartum women (CAPPW) facing heightened risks because of biological and social vulnerabilities. This article highlights the importance of a multipronged prevention framework that combines infection control measures, psychosocial support, education, and nutritional supplementation, alongside pharmacological interventions such as Bacillus Calmette-Guérin (BCG) vaccination and tailored TB preventive therapy (TPT). Drawing on nationa

[Expression of Concern] Gambogic acid inhibits invasion of osteosarcoma via upregulation of TIMP‑1.

UNLABELLED: The sirolimus-eluting stent (CYPHER( trade mark )) is a metal stent coated with 140 micro g/cm(2) of sirolimus blended with synthetic polymers. After stent implantation, sirolimus is slowly released causing localized cytostatic inhibition of proliferation of vascular smooth muscle cells in the peri-stent arterial wall over a period of about 1 month. Only minimal amounts of sirolimus enter the bloodstream and these appear to be insufficient to be of clinical relevance. In clinical trials that evaluated single de novo lesions and in-stent restenosis in patients with coronary artery d

Psoas and Thigh Abscesses As Rare Complications of Cesarean Delivery: A Case Report.

Iliopsoas abscess is an uncommon but potentially severe condition in obstetric practice, posing significant diagnostic challenges due to its deep anatomical location and nonspecific clinical presentation. Its occurrence in the postpartum period, particularly following cesarean delivery, remains exceptionally rare. We report the case of a 32-year-old woman who developed an extensive left iliopsoas abscess with lumbar and thigh extension one week after cesarean section performed for prolonged rupture of membranes. She presented with high-grade fever, severe low back pain, and a tender lumbar swe

Gastrointestinal Basidiobolomycosis with Biliary Tract Involvement in the Early Postpartum Period: A Case Report and Literature Review.

Introduction: Gastrointestinal basidiobolomycosis is an uncommon invasive fungal infection caused by Basidiobolus ranarum, an environmental mould endemic to the arid south-west of Saudi Arabia and to a handful of other hot, dry regions. Its non-specific presentation routinely invites misdiagnosis as inflammatory bowel disease, tuberculosis or malignancy, and extension to the biliary tree is decidedly rare. Case Presentation: We report a 25-year-old Saudi woman who developed chronic watery diarrhoea and pronounced weight loss two months after her first delivery. Severe microcytic anaemia, leuko

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr
