Incidence, risk factors, and clinical outcomes of peripartum cardiomyopathy in Hong Kong.

INTRODUCTION: Peripartum cardiomyopathy (PPCM) is an uncommon but serious form of heart failure affecting women during late pregnancy or early postpartum. This territory-wide multicentre retrospective study aimed to evaluate the local incidence, risk factors, and clinical outcomes, including subsequent pregnancies, in Hong Kong. METHODS: Medical records were retrospectively reviewed for women who delivered at all public hospitals between 1 January 2013 and 31 December 2022 and met the 2010 European Society of Cardiology Working Group criteria for PPCM. Regression analysis was performed to inv

Severe Maternal Morbidity and near Miss-Events in Women with Heart Disease: Insights from a Cohort Study.

Background/Objectives: The maternal mortality ratio is one of the global health indicators, and cardiopathies are the leading indirect causes of maternal deaths. Proper management of pregnant women with heart disease is crucial, as the severity of these conditions can lead to complications during the perinatal period. This study aimed to evaluate the rate of severe maternal morbidity and associated factors in pregnant women with heart disease. Methods: A retrospective cohort study was conducted at a referral hospital in São Paulo from 2008 to 2017, including pregnant women with heart disease w

Global, regional, and national levels of maternal mortality, 1990-2021: a systematic analysis for the Global Burden of Disease Study 2021.

BACKGROUND: Despite global declines in the maternal mortality ratio (MMR), significant disparities persist across regions and socioeconomic strata. This study aims to examine spatiotemporal trends in MMR from 1990 to 2021, with a specific breakdown by age, socio-demographic index (SDI) and cause of death, to better clarify the underlying determinants of these patterns. METHODS: Utilizing data from the Global Burden of Disease (GBD) Study 2021, we examined MMR trends attributable to ten distinct causes across 204 countries and territories from 1990 to 2021. Analyses were stratified by the SDI,

Global maternal mortality associated with SARS-CoV-2 infection: a systematic review and meta-analysis.

BACKGROUND: Pregnant and recently pregnant women infected with SARS-CoV-2 are at increased risk of death and serious complications than those without the infection. The extent of variation in mortality rates in pregnant women with SARS-CoV-2 infection across regions, and the causes of death are not known. We systematically reviewed all available evidence on the variation in mortality rates in pregnant women with SARS-CoV-2 infection across geographical and country income groups, and the reported cause of death. METHODS: We searched major databases (December 2019-January 2023) including Medlin

Maternal mortality ratios in Southeast Asia (1990-2021): trends, disparities, and projections through 2050 based on the global burden of disease 2021, with reference to SDG 3.1.

BACKGROUND: Maternal mortality remains a critical barrier to achieving Sustainable Development Goal (SDG) 3.1 (target: Maternal mortality ratios (MMR) < 70 per 100,000 live births by 2030). Southeast Asia accounts for ~ 20% of global maternal deaths, with substantial disparities between and within countries. This study analyzed trends (1990–2021) and projected (to 2050) MMR in Southeast Asia using the Global Burden of Disease (GBD) 2021 database, addressing gaps in regional granularity. METHODS: Data GBD 2021, covering MMR and age-standardized MMR (ASMR) for 1990–2021, stratified by countr

[Amniotic fluid embolism].

Amniotic fluid embolism is one of the leading causes of direct maternal deaths worldwide. It cannot be predicted. Pregnant women who have undergone a cesarean section, a multiple pregnancy, a placental implantation disorder, polyhydramnion, the need for drug-induced labor and a maternal age ≥ 35 years have a significantly increased risk of developing a peripartum amniotic fluid embolism. The diagnostics of amniotic fluid embolism are purely clinical and include a triad of sudden onset of hypoxia and hypotension, followed by coagulopathy. A causal treatment is not yet known. Treatment must be i

The Perinatal Committee report: Review of the progress of obstetric healthcare in Japan.

BACKGROUND: Japan's maternal mortality rate and perinatal mortality rate have shown one of the world's most significant declines, positioning Japan at the global forefront of the lowest levels. The nearly logarithmic annual decline in maternal and perinatal mortality rates suggests that, in addition to advances in medicine and healthcare, various types of care for mothers and newborns have played a crucial role in this achievement. METHOD: From the period of World War II to the postwar era, up to around 1980, and then every decade thereafter, the events and movements surrounding perinatal hea

Pancreatic panniculitis associated with presumed advanced pancreatic tumor: A case report and literature review.

Pancreatic tumor-associated panniculitis is a rare paraneoplastic syndrome, primarily caused by systemic release of pancreatic enzymes (e.g., lipase) from malignancies like acinar cell carcinoma (ACC), leading to necrotizing inflammation in subcutaneous and other adipose tissues. This report details an 87-year-old female presenting with progressive skin lesions (erythematous rash → indurated swelling → ulceration with purulent drainage), lower limb pain, fever, and markedly elevated lipase (3686.2 U/L). Skin biopsy confirmed lobular panniculitis with characteristic "ghost cells". Imaging revea

[Excess of maternal deaths among immigrants, during the pandemic, in Brazil].

The aim of this study was to analyze the effect of the COVID-19 pandemic on maternal mortality among immigrants in Brazil. This was a descriptive study of maternal deaths that occurred between 2014 and 2021. The frequency of deaths and underlying causes of death were analyzed. The ratio between the average number of deaths before and during the pandemic were calculated. Of the 14,205 deaths, 136 were immigrants and 14,069 were Brazilian. The excess mortality was higher for immigrants (2.10; 1.40-3.04) than for Brazilian women (1.53; 1.40-1.59), with a significant increase for Venezuelans (7.46

The Spectrum of Placental Findings of First-Trimester Cytomegalovirus Infection Related to the Presence of Symptoms in the Newborns and Stillbirths.

Cytomegalovirus (CMV) is one of the most common congenital infections worldwide and a leading cause of prenatal neurological disorders, sensorineural hearing loss, and stillbirth. The placental factors involved in CMV transmission from mother to fetus remain poorly understood. We aimed to evaluate the histopathological placental findings associated with first-trimester CMV infection in relation to stillbirth and symptomatic presentation in newborns. This retrospective case-control study analyzed pregnancies with confirmed first-trimester CMV infection followed at 2 tertiary referral hospitals

Investing in midwifery for sustainable development goals in low- and middle-income countries: a cost-benefit analysis.

BACKGROUND: Maternal and neonatal mortality in low- and middle-income countries is frequently caused by inadequate management of obstetric and neonatal complications and a shortage of skilled health workers. The availability of these workers is essential for effective and high-quality healthcare. To meet the needs of sexual, reproductive, maternal, new-born, child, and adolescent health by 2030, more than one million health workers, including 900 000 midwives, are required globally. Despite this, uncertainty persists regarding the return on investment in the health workforce. METHODS: The obj

[Maternal mortality due to hypertensive disorders in France, 2016-2018].

Between 2016 and 2018, 13 maternal deaths were due to hypertensive disorders. During this period, the maternal mortality ratio was 0.6/100 000 live births. Hypertensive disorders were responsible for 4.8% of maternal deaths during the first year, 5.1% up to 42 days postpartum and for 13.5% of direct maternal mortality. Maternal deaths due to hypertensive disorders increased close to signification (p=0.09) compared to the last triennium (MMR=0.2/100.000). Classification of the hypertensive disorders was: 5 severe preeclampsia, 3 eclampsia, 4 HELLP syndromes et 1 undefined hypertension. In five

Venous thromboembolism in pregnant obese Individuals.

Venous thrombosis and thromboembolism (VTE) remain the leading cause of direct maternal deaths, occurring within 42 days of the end of pregnancy in the UK. Pregnancy is associated with an overall 10-fold higher incidence of VTE than in the non-pregnant state and has been reported to reach up to 30-fold higher in the puerperium. This increased risk is further exacerbated by maternal obesity in a relationship that appears to be proportional with increasing Body Mass Index (BMI). Maternal obesity is the most common health problem in women of reproductive age with clinically significant health ris
