Left Atrial Thrombus and Cardioembolic Stroke in Chagas Cardiomyopathy Presenting with Atrial Flutter: A Case Report.

Background: Chagas disease, caused by Trypanosoma cruzi, remains endemic throughout Latin America but is increasingly reported in urban areas due to migration and vector adaptation. The cardiac form is the most severe manifestation, associated with arrhythmia, mural thrombus formation, and a high risk of cardioembolic events. Stroke secondary to Chagas cardiomyopathy is uncommon and poses diagnostic and therapeutic challenges. Case Presentation: A 58-year-old woman with serologic evidence of T. cruzi infection presented with sudden-onset dyspnea, oppressive chest pain, and left-sided weakness.

Suspected Acute Stanford Type A Aortic Dissection: A Case Report.

BACKGROUND: Acute Stanford type A aortic dissection carries 1-2% hourly mortality if medically managed. Misdiagnosis as hypertensive emergency delays surgery. In African settings without immediate computed tomography angiography (CTA) access, bedside echocardiography guides triage. CASE PRESENTATION: A 58-year-old man with uncontrolled hypertension presented to the emergency department of the Federal Teaching Hospital, Owerri, Nigeria, with sudden tearing chest and back pain, diaphoresis, and dyspnea. Blood pressure was asymmetric at 210/115 mmHg in the right arm versus 180/100 mmHg in the le

Bilateral Avascular Necrosis of the Femoral Head in a Patient With Sickle Cell Trait: A Case Report.

Sickle cell trait (SCT), historically regarded as a benign carrier state, may nonetheless be associated with severe complications such as bilateral avascular necrosis (AVN) of the femoral head in select individuals. This risk is often underrecognized, particularly in regions with high SCT prevalence, leading to delayed diagnosis and management. We report the case of a 43-year-old Yemeni man who presented with progressive bilateral hip pain over eight months without traditional risk factors except smoking. His family history was notable for sickle cell disease and trait. Physical examination re

A Clinical Genetics-Driven Dual Diagnosis of Prader-Willi Syndrome Due to Mosaic Maternal UPD(15) and NOTCH3-Related CADASIL.

Maternal uniparental disomy of chromosome 15 [UPD(15)mat] and imprinting defects account for about 30% of cases of Prader-Willi syndrome (PWS). Mosaic UPD(15)mat is rare and may escape routine testing. We describe a 45-year-old male patient in whom persistent clinical suspicion of PWS was not genetically confirmed by repeated methylation-based analyses. Clinical manifestations included neonatal hypotonia with low birth weight, early hyperphagia, severe obesity, short stature, growth hormone deficiency, type 2 diabetes mellitus, dyslipidemia, and MASLD/MASH with compensated cirrhosis. He presen

Plasma Biomarkers, Brain Volume, and Cognitive Performance in Service Members and Veterans With mTBI: A LIMBIC-CENC Study.

IMPORTANCE: Repetitive mild traumatic brain injury (mTBI) may result in neurobiological changes that contribute to persistent symptoms of cognitive dysfunction. Elevations in plasma biomarkers (glial fibrillary acidic protein [GFAP], neurofilament light chain [NfL], total-tau [t-tau], and ubiquitin C-terminal hydrolase-L1 [UCH-L1]) have been reported acutely after mTBI, but few studies have assessed these biomarkers during the chronic phase of mTBI. OBJECTIVE: To examine whether the associations among plasma biomarkers, brain volume, and cognition varied by mTBI among US active-duty service m

Relapsing Seronegative Immune-Mediated Cerebellar Ataxia with Evolving Intrathecal Immune Activation and Rituximab Consolidation in a Toddler: A Case Report.

Seronegative immune-mediated cerebellar ataxia in young children is poorly defined and may be mistaken for self-limited post-infectious ataxia when magnetic resonance imaging and antibody panels are negative. A previously healthy 2-year-old girl developed acute cerebellar ataxia with gait instability, frequent falls, limb tremor, and choking while drinking. Brain/cerebellar magnetic resonance imaging was normal. At relapse, an equivocal cervical cord T2 signal was not confirmed on dedicated cervical imaging or accompanied by myelopathic signs. Extensive serum and cerebrospinal fluid antibody t

Fanconi Anemia in a Young Adult Male Presenting with Pancytopenia and Bilateral Ectopic Kidneys.

Fanconi anemia (FA) is a rare inherited disorder of bone marrow failure that presents with progressive pancytopenia, multiple congenital abnormalities, and an increased predisposition to various malignancies. We describe a 19-year-old male who reported generalized weakness, shortness of breath on exertion, and a dry cough persisting for 1 month. He had undergone three blood transfusions within the preceding 3 months. Clinical examination revealed marked pallor without evidence of jaundice, cyanosis, clubbing, or lymphadenopathy. Laboratory investigations showed pancytopenia, while bone marrow

Inborn errors of immunity in Low German Mennonite communities in Mexico: a case series and narrative literature review.

BACKGROUND: Inborn errors of immunity (IEI) comprise a genetically heterogeneous group of disorders with increased prevalence in consanguineous and endogamous populations due to founder effects. Low German Mennonite (LGM) communities in Mexico represent a high-risk population characterized by genetic isolation and an increased frequency of autosomal recessive conditions. Although IEI have been reported in LGM populations from Canada and Europe, systematic data from Mexican LGM communities remain limited. This study describes four novel clinical cases of IEI in LGM patients evaluated at two ter

Real-world clinical effectiveness of trimethoprim-sulfamethoxazole for primary prophylaxis of pneumocystis pneumonia in non-hodgkin lymphoma patients treated with rituximab.

There are no definitive clinical practice guidelines regarding the necessity and dosage of trimethoprim-sulfamethoxazole (TMP/SMX) prophylaxis for Pneumocystis jirovecii pneumonia (PJP) in individuals undergoing rituximab therapy. This retrospective study evaluated the effectiveness and safety of various TMP-SMX prophylactic dosing regimens over a 1-year period in 690 patients with non-Hodgkin lymphoma treated with rituximab at a university hospital in Thailand from 2013 to 2022. Out of these patients, 622 (90.1%) received TMP/SMX, with a mean duration of prophylaxis of 265.7 days (SD 85.66).

Sleep-Disordered Breathing in Heart Failure: Phenotypes, Mechanisms, and Precision Approach.

Sleep-disordered breathing (SDB), comprising obstructive sleep apnea (OSA) and central sleep apnea (CSA), is highly prevalent in heart failure (HF) and is associated with adverse remodeling, arrhythmias, and worse clinical outcomes. However, despite improvements in physiological markers, treatment of SDB has not consistently translated into reductions in mortality or HF hospitalization. In OSA, continuous positive airway pressure (CPAP) improves symptoms, oxygenation, and selected physiological parameters, but evidence in HF is derived largely from small, short-term trials and reductions in ha

When Pneumonia Is Not Pneumonia: T-cell Acute Lymphoblastic Leukemia Presenting With Mediastinal Mass, Airway Compression, and Pericardial Effusion in a 4-Year-Old.

Pediatric leukemia may initially present in a way that mimics common respiratory infections, particularly when fever, cough, chest pain, and radiographic findings are observed. This overlap may obscure the diagnosis of malignancy, especially when the initial imaging is interpreted as community-acquired pneumonia. T-cell acute lymphoblastic leukemia (T-ALL) is commonly linked with anterior mediastinal masses that may result in airway compression, atelectasis, pleural or pericardial effusions, and cardiopulmonary instability. We describe a previously healthy four-year-old boy who presented with

Transient Cardiac Dysfunction Due to New-Onset Mitral Chordal Rupture With Concomitant Congenital Absence of the Right Coronary Artery: A Case Report.

BACKGROUND Congenital absence of the right coronary artery (RCA) is a rare anomaly often identified incidentally. We report a case of transient left ventricular systolic dysfunction followed by recovery of ejection fraction and new-onset mitral chordal rupture, which became the primary driver of pronounced N-terminal pro-B-type natriuretic peptide (NT-proBNP) elevation. The coexistence of 2 distinct pathologies complicated causal attribution; we discuss proposed mechanisms while recognizing their limitations. CASE REPORT A 58-year-old woman presented with exertional chest pain. At initial admi

Intrauterine transfusion in monochorionic twins with parvovirus B19 infection: Evidence of intertwin blood exchange.

BACKGROUND: Parvovirus B19 infection can cause severe fetal anemia. Management in monochorionic twin pregnancies is challenging due to placental vascular connections. CASE: We report a case of severe fetal anemia in both fetuses of a monochorionic diamniotic twin pregnancy following maternal parvovirus B19 infection. Elevated middle cerebral artery peak systolic velocity (MCA-PSV) was first detected at 16 weeks in twin A and reached 2.32 multiples of the median (MoM) in twin A and 2.30 MoM in twin B at 21+6 weeks. These findings were associated with polyhydramnios in both fetuses and ascites

Current State of Moderate Congenital Haemophilia A and the Need for Preventive Treatment.

INTRODUCTION: The severity of haemophilia A is classified by the degree of factor VIII (FVIII) deficiency, rather than by clinical manifestations. However, FVIII activity alone does not necessarily accurately reflect clinical severity such as bleeding tendency, and patients with mild-to-moderate haemophilia A can experience significant disease burden. AIM: This narrative review explores the clinical relevance of non-severe haemophilia A (focusing on moderate disease), and examines the profile of patients with, and the evidence supporting the use of prophylaxis to manage, this disease form.
