Omphalocele-exstrophy-imperforate anus-spinal defect syndrome in a monozygotic twin pregnancy: a case report and literature review.

BACKGROUND: The Omphalocele, Exstrophy, Imperforate Anus, and Spinal Defects Complex (OEIS complex) is a rare birth defect with a prevalence of 1 in 200,000-400000 pregnancies and 1 in 82,000-200000 live births. While cases in female monozygotic twins have been reported, the perinatal outcomes of the unaffected co-twin remain insufficiently addressed. CASE PRESENTATION: We report a spontaneously conceived monochorionic monoamniotic twin pregnancy where Twin B was diagnosed with OEIS. Prenatal ultrasound at 12 + 4 weeks identified an abdominal cystic mass in Twin B, which enlarged progressivel

Spectrum of potentially lethal cardiac conditions presenting with fetal sinus bradycardia: a report on 34 cases.

OBJECTIVE: Fetal sinus bradycardia (FSB) is usually a benign antenatal finding but it can be a marker of an underlying serious condition that manifests in later life. The aim of this study was to report the antenatal course and postnatal outcome of FSB. METHOD: This study was a retrospective review of all cases of persistent FSB, defined as a fetal heart rate (FHR) < 3rd percentile for gestational age (GA) with 1:1 atrioventricular conduction on at least two consecutive fetal echocardiograms, diagnosed between January 2014 and December 2024 at the Brompton Centre for Fetal Cardiology, London,

Enhancing fetal spina bifida repair: do paraspinal myofascial flaps and acellular dermal patches improve postnatal outcomes? A retrospective cohort review.

OBJECTIVE: The primary objective of this study was to assess whether incorporating paraspinal myofascial flaps and/or an acellular dermal matrix (ADM) patch during open fetal surgery for open spina bifida (OSB) impacts the need for CSF diversion by 1 year of age. A secondary objective was to determine whether these interventions affect pregnancy outcomes. METHODS: The authors retrospectively reviewed all fetuses undergoing open fetal OSB repair at their center from June 2017 through April 2024. Factors associated with CSF diversion surgery at 1 year of age were collected and included lesion t

Clinical and multi-omics characterisation of early neurodevelopmental disorders associated with critical congenital heart disease: the prospective cohort CATAMARAN neonatal study protocol.

INTRODUCTION: Critical congenital heart disease (CHD) is associated with neurodevelopmental disorders, recognised as the most common long-term morbidity in affected children. In critical CHD, that is, CHD requiring cardiac surgery within the first 3 months of life, 30%-50% of children have lower developmental scores. Therefore, early identification of at-risk infants is crucial, yet there is no scientifically evaluated care programme in France. This study aims to evaluate early neurodevelopmental status in infants with prenatally diagnosed critical CHD and to determine how intrinsic susceptibi

Hypertrophic Cardiomyopathy as a Key Feature of MRAS-Related Noonan Syndrome: New Case and Comprehensive Literature Review.

Noonan syndrome (NS) is a rare multisystemic condition among the RASopathy group, characterized by a broad phenotypic spectrum and genetic variability. It results from pathogenic variants in genes regulating the RAS/MAPK pathway, affecting cell proliferation and differentiation. While the PTPN11 gene accounts for approximately 50% of cases, other genes, including MRAS, have been implicated. NS presents with features such as facial dysmorphism, short stature, and cardiac anomalies. Hypertrophic cardiomyopathy (HCM) is a major contributor to mortality, with specific variants conferring higher ri

Patient perspectives on hemolytic disease of the fetus and newborn and fetal and neonatal alloimmune thrombocytopenia: common experiences and insights.

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN) and fetal and neonatal alloimmune thrombocytopenia (FNAIT) are two rare alloimmune diseases of pregnancies where maternal alloantibodies target fetal blood components, potentially leading to risks of serious health conditions and developmental delays for babies, and sometimes loss of the baby. HDFN and FNAIT often have similarly complex journeys, from diagnosis through to burdensome treatments such as intrauterine transfusions and intravenous immunoglobulin. OUR STORIES: We, the authors, are two mothers with lived experience of t

Prenatal diagnosis of glucose-6-phosphatase catalytic subunit 3 deficiency (Dursun syndrome) using whole-exome sequencing: A case report of severe fetal cardiomyopathy in a consanguineous family.

Glucose-6-phosphatase catalytic subunit 3 deficiency, also known as Dursun syndrome, is a rare autosomal recessive disorder characterized by severe congenital neutropenia and variable multisystem malformations, particularly affecting the cardiovascular system. Most reported cases have been identified postnatally, following infectious or hematologic complications. Prenatal identification remains exceptionally rare. We describe the case of a fetus from consanguineous parents with a history of multiple neonatal deaths. Serial prenatal imaging demonstrated progressive fetal growth restriction, car

Timing of birth to improve outcomes in chronic or gestational hypertension: the WILL RCT.

BACKGROUND: For women with chronic or gestational hypertension who remain well, early term birth (at 37-38 weeks' gestation) may reduce maternal complications, caesareans and stillbirths, but it may increase neonatal morbidity compared with expectant care. Expectant care may increase costs. There are no high-quality data to guide care, which currently involves maternal-fetal surveillance and intervention for maternal or fetal compromise, which may be rapid or unexpected. OBJECTIVE: To investigate optimal timing of birth for women with chronic or gestational hypertension who reach term and rem

Regional differences in fetal fat accretion in small-for-gestational-age fetuses assessed by quantitative magnetic resonance imaging.

BACKGROUND: Fetal fat accretion follows a spatiotemporal pattern. Fetuses who are small-for-gestational-age (SGA) demonstrate reduced fat accumulation, but whether specific body regions are disproportionately affected remains unclear. OBJECTIVE: To characterize regional fat differences between SGA and appropriate-for-gestational-age (AGA), assess the modifying effects of SGA-onset timing and cerebroplacental ratio (CPR), and evaluate associations with neonatal morbidity. MATERIALS AND METHODS: SGA pregnancies were prospectively recruited, and AGA controls retrospectively identified. SGA was

Trisomy 18 and the possibility of choice: The importance of Perinatal Hospice's support.

Trisomy 18 is a severe aneuploidy associated with multiple malformations and a poor prognosis. The diagnosis is typically made prenatally, leading to a high rate of pregnancy terminations. The aim of this study is to demonstrate that even though the prognosis is heterogeneous, prolonged survival is possible and these children are an enrichment for their families after all. A retrospective, descriptive and monocentric study was conducted on fetuses diagnosed with trisomy 18, evaluated between March 2017 and June 2023 at our Institution. The objective was to investigate the natural history of tr

Management of anti-M antibody during pregnancy: a case report.

BACKGROUND: Anti-M antibodies are relatively common and naturally occurring. When anti-M antibodies cross the placenta, they may cause hemolytic disease of the fetus and newborn (HDFN). Anti-M antibodies account for less than 15 cases of HDFN reported in the published English literature. HDFN can lead to foetal anaemia, hydrops fetalis, hypoxia, heart failure, and even death. OBJECTIVE: To review the general guidelines and propose a less intensive management approach of anti-M antibody during pregnancy through the context of a case report. METHODS: We report a 25-year-old healthy pregnant G3

Assessment of the maternal key micronutrient supply and its correlation with cord blood parameters in twin pregnancies.

BACKGROUND: Multiple pregnancy constitutes a large metabolic expense, so women with twin pregnancies and neonates born as twins might be at risk for micronutrient deficiencies. Therefore, the aim of the study was to assess the supplementation used and supply with key micronutrients: iron, vitamin B12, folic acid and vitamin D in women with twin pregnancies and the correlations with cord blood indicators. METHODS: Maternal and cord blood samples were collected from 51 patients with twin pregnancies and 102 newborns born from those pregnancies between October 2020 and September 2023. Ferritin,

Aplasia cutis congenita associated with fetus papyraceous.

Aplasia cutis congenita (ACC) represents a rare group of disorders that, due to their infrequent occurrence, can pose challenges in diagnosis. It commonly presents as an isolated defect on the scalp and affects the dermal and epidermal layers of the skin. While the prognosis for isolated ACC without underlying defects is generally benign, life-threatening complications like sagittal sinus bleeding or secondary infections can occur. Our case report highlights a case of type 5 aplasia cutis with associated fetus papyraceous in the surviving twin of a dichorial diamniotic pregnancy. This case repo

Clinical Practice Guideline: The Treatment of Cleft Lip and Palate Deformities.

BACKGROUND: Cleft lip and palate deformities (CLPD) constitute one of the more common types of congenital malformation. In Germany, the prevalence is more than 1 in 500 births. The treatment is complex; multistep interdisciplinary treatment is needed for full recovery. In this S3-level clinical practice guideline, we provide evidence- and consensus-based recommendations for the treatment of patients with CLPD. METHODS: A systematic literature search based on key questions was carried out. All recommendations and statements were formulated by specialists from multiple disciplines and were adop

Fetal bone loading is a critical determinant of fetal and young infant bone strength.

BACKGROUND: The Utah Paradigm is the contemporary model of bone physiology that allows for an understanding of risk factors for bone fragility and can be used in understanding infants who present with multiple unexplained fractures in contested cases of child abuse. METHOD: The Utah Paradigm was applied to the fetal period in over 500 cases of contested child abuse cases in infants with multiple unexplained fractures to determine if risk factors for bone fragility could be identified. FINDINGS: Multiple risk factors for fetal bone fragility were identified: inadequate provision by the mother

Association of birth outcomes with maternal and infant FADS1 rs174547 genotypes in Japanese participants.

N-3 long-chain polyunsaturated fatty acids (n-3LCPUFAs) are crucial for child growth and development particularly for fetal growth in utero and brain development and function. This study examined the relationship between birth outcomes and FADS1 rs174547 genotypes in Japanese mothers and infants. The study included 406 mothers and 373 infants, i.e., 373 infant-mother pairs, from a supplementary survey of the Japan Environment and Children's Study. Multiple regression analysis revealed that infants with the CC genotype had significantly smaller head circumference at birth compared to those with

Anesthetic Dilemma in a Full-term Parturient with Facial Trauma and Suspected Head Injury Presenting as Difficult Airway Posted for Emergency LSCS in a Limited Resource Setting: Spinal versus General Anesthesia.

Trauma frequently occurs during pregnancy and occurs 1 in 12 pregnant women affecting maternal mortality, morbidity, and also pregnancy outcome. Hence, it is essential to consider pregnancy in every female trauma patient of reproductive age. Motor vehicle accidents and domestic/intimate partner violence account for most cases of major maternal trauma. Most of the causes mentioned above can lead to facial injuries/head injuries making it challenging for the anesthesia provider. We report a parturient who presented to us with multiple facial injuries with a suspected head injury following a road
