Beyond the Usual Suspects: A Challenging Case of Disseminated Sporotrichosis Masquerading as Suppurative Lymphadenitis in a Dialysis-Dependent Chronic Kidney Disease Patient With Lupus.

Sporotrichosis, a subcutaneous mycosis, can manifest as severe disseminated disease in immunocompromised hosts. This report describes a challenging case of extracutaneous sporotrichosis in a 28-year-old female with systemic lupus erythematosus (SLE) on immunosuppressive therapy, who also faced significant socioeconomic constraints. The patient initially presented to the emergency department with a painful right inguinal mass. Physical examination revealed a hemorrhagic lesion, and aspiration of the material was performed for culture, which returned negative. She was discharged but returned 2 m

Farnesol attenuates inflammation and protects the liver of rats experimentally infected with Sporothrix brasiliensis.

Sporotrichosis, a neglected tropical disease, is an emerging implantation mycosis considered a global public health concern attributed to thermodimorphic fungus of the genus Sporothrix. An important step in controlling sporotrichosis is the implementation of suitable treatments. However, resistance to first-line antifungal therapies represents a growing challenge for sporotrichosis management, reinforcing the need for alternative and effective treatments, such as farnesol. Farnesol is a sesquiterpene alcohol considered a potential compound due to its antifungal activity in vitro and anti-infla

A Presentation of Necrotizing Disseminated Sporotrichosis.

We report a biopsy-proven case of disseminated sporotrichosis in a 42-year-old immunocompetent male, highlighting an uncommon presentation with rapid clinical deterioration. The patient initially presented with cutaneous lesions involving the face, thorax, and extremities that were suspected to represent a delayed hypersensitivity reaction to trimethoprim-sulfamethoxazole, and he was treated with systemic corticosteroids. He returned 10 days later with worsening necrotic lesions and constitutional symptoms. Otolaryngology and dermatology services obtained multiple skin biopsies, which demonstr

Farnesol modulates seric and splenic purinergic signaling during disseminated sporotrichosis by Sporothrix brasiliensis: improvement of immune response.

Sporotrichosis is an implantation mycosis induced by exposure to thermodimorphic fungi of genus Sporothrix, which affects both humans and animals, resulting in severe inflammatory responses. Purinergic signaling has been implicated in pathogenesis of fungal infections, recognized as a critical system for modulating immune responses and triggering effector mechanisms during sporotrichosis. Natural products have emerged as promising modulators of immune responses, particularly through their influence on purinergic signaling. Among these, farnesol, a sesquiterpene alcohol, demonstrated anti-infla

Disseminated sporotrichosis with orbital osteomyelitis in a child: a case report.

Sporotrichosis is an infection caused by fungi of the Sporothrix complex that can affect both humans and animals. In most cases, it is a benign infection limited to the skin and subcutaneous tissue, rarely disseminating to other organs with systemic involvement. Diagnosis is based on clinical data, epidemiological data, and laboratory tests, with culture being considered the gold standard. The disease generally has a good prognosis and is usually resolved with appropriate antifungal treatment. This case report describes an atypical presentation of sporotrichosis with suppurative nodules and or

Disseminated sporotrichosis masquerading as tuberculosis in a patient without overt immunodeficiency: challenges in tuberculosis-endemic area.

Sporotrichosis is a mycosis with multitudinous clinical manifestation based on varying factors. We report a 78-year-old man who came to an out-patient clinic for progressive ulcerative cutaneous lesions for 6 months with concurrent pulmonary symptoms and osteomyelitis. Initial investigations were negative for infectious organisms and therapeutic trial of standard anti-tuberculous regimen failed to improve clinical symptoms. Subsequent fungal culture from the ulcerative skin showed dark colonies of Sporothrix schenckii complex. A review of sporotrichosis cases mimicking tuberculosis is provided

Management of sporotrichosis in patients with inflammatory bowel disease using biological therapy (antitumor necrosis factor).

Antitumor necrosis factor (TNF)-alpha (TNFa) drugs are crucial for treating inflammatory bowel disease (IBD) but may increase opportunistic infection risk. Among such infections, sporotrichosis is a chronic granulomatous disease caused by saprophytic dimorphic fungi of the genus Sporothrix, which occurs worldwide. To date, there have been no reports of sporotrichosis in immunosuppressed IBD patients. The main objectives are to discuss clinical, diagnostic, and therapeutic aspects of sporotrichosis in IBD patients on anti-TNF therapy. We describe three patients with IBD on TNFa therapy who cont

Disseminated sporotrichosis caused by Sporothrix brasiliensis with testicular involvement.

Sporotrichosis is a growing public health concern especially due Sporothrix brasiliensis. We report a case of disseminated sporotrichosis involving the meninges, lungs, and testicles in a 35-year-old homeless man from Curitiba, Brazil. Fungal culture, histopathology, and molecular identification confirmed S. brasiliensis infection. Notably, testicular involvement-a rare complication of sporotrichosis-was initially misdiagnosed as a malignant tumor, leading to orchiectomy. A literature review identified only four previous cases of testicular sporotrichosis. This case underscores the potential f

Sporothrix brasiliensis Treatment Failure without Initial Elevated Itraconazole MICs in Felids at Border of Brazil.

Cat-transmitted sporotrichosis caused by Sporothrix brasiliensis is an emerging zoonosis in Latin America. Because treatment of feline sporotrichosis is often not effective, we sought to determine whether treatment failure results from S. brasiliensis strains that have existing elevated MICs for itraconazole, the primary treatment for this disease. During 2021-2023 at the triple border region of Brazil, Paraguay, and Argentina, 108 S. brasiliensis strains were isolated from felines before antifungal treatment. The main clinical manifestation was cutaneous disseminated sporotrichosis (61%), whi

Systemic Sporotrichosis With Testicular Involvement: Literature Review and Case Report.

INTRODUCTION: Sporotrichosis is an endemic fungal infection in Brazil, caused by a dimorphic fungus of the genus Sporothrix. Transmission occurs through traumatic inoculation from soil, contaminated plants, and zoonotic sources, mainly from cats, as well as through inhalation of conidia. It commonly presents as localized, lymphocutaneous, disseminated, or systemic forms. The testicles are among the organs that can be affected, often manifesting as a testicular mass. This work is aimed at analyzing a clinical case along with a bibliographic review on the testicular involvement of sporotrichosis

Dectin-1 and dectin-2 drive protection against Sporothrix brasiliensis in experimental sporotrichosis.

INTRODUCTION: The emerging fungal pathogen Sporothrix brasiliensis has been responsible for epidemic outbursts of sporotrichosis in Latin America, particularly Brazil, in recent years. The higher aggressiveness of the infection and its zoonotic nature are hallmarks of the pathogen, but the immunological markers of protection are not fully characterized. The C-type lectin receptors - dectin-1 and dectin-2 - drive key antifungal responses, and here we aimed to uncover their contribution against S. brasiliensis in a murine model of disseminated sporotrichosis. METHODS: Wild-type, Dectin-1 and/or

Extracutaneous Sporotrichosis With Dissemination to the Brain in a Patient Taking Biologic Medications: A Case Report.

Sporotrichosis is a fungal infection caused by Sporothrix species, primarily Sporothrix schenkii and Sporothrix brasiliensis. Disease manifestation varies from localized cutaneous form to disseminated cutaneous and disseminated extra-cutaneous forms. We present a case of sporotrichosis with widespread skin lesions and fungal dissemination to the brain in a patient taking biologic medications to treat psoriatic arthritis. Her diagnosis was made via positive skin tissue culture, and her symptoms responded well to amphotericin B. With this case report, we would like to highlight the risk of rare

Disseminated Cutaneous, Osteoarticular, and Tubulointerstitial Sporotrichosis in an Immunosenescent and Diabetic Host: A Case Report.

INTRODUCTION: Disseminated sporotrichosis is an uncommon presentation of this infectious disease, primarily observed in immunocompromised patients. Here, we present a case of disseminated sporotrichosis occurring in an immunosenescent patient. CASE PRESENTATION: An 81-year-old man presented to our clinic with skin ulcers in the scalp, back, right arm, and both legs, knee arthritis, and acute kidney injury. After skin biopsy and synovial fluid analysis, the diagnosis of disseminated sporotrichosis was made using fungal culture and in-house PCR. The only factor that favored immunocompromise was

Cutaneous disseminated sporotrichosis in an immunocompetent farmer.

Sporotrichosis is an implantation mycosis due to the genus Sporothrix. Cutaneous disseminated sporotrichosis is an unusual clinical form that often affects immunocompromised patients. We report a case of cutaneous disseminated sporotrichosis in an immunocompetent famer from a rural region of Madagascar, treated successfully with Itraconazole 200mg twice a day. This case highlights the role of multiple inoculation affecting different parts of the body as a risk factor of cutaneous disseminated sporotrichosis in an immunocompetent individually.

Humane Endpoint: Example from a Murine Model of Disseminated Sporotrichosis.

Vertebrate animal models are essential in research; however, efforts need to be made to decrease animal suffering as much as possible. It could be useful to determine humane endpoints that could serve as surrogates for a fatal outcome. We address this issue with respect to infectious diseases. We propose a humane endpoint for studies of Sporothrix brasiliensis infection. BALB/c mice were inoculated subcutaneously in the footpad. To define a humane endpoint, we considered two groups: animals who died during the experiment, and those euthanized at the end of the experiment. The groups were compa
