IMPORTANCE: Bronchopulmonary dysplasia (BPD) is the most common in-hospital morbidity of prematurity, associated with significant long-term medical and neurodevelopmental sequelae and health resource utilization. The Neonatal Research Network (NRN) Hydrocortisone for BPD Trial evaluated the efficacy and safety of hydrocortisone to prevent BPD in high-risk very preterm infants; the impact of hydrocortisone on school-age outcomes in this trial cohort is previously unreported.
OBJECTIVE: To evaluate the impact of neonatal hydrocortisone treatment on early school-age functional motor, cognitive,

BACKGROUNDSuboptimal fetal growth (SFG), being born small for gestational age (SGA), and catch-up (CU) growth are, individually and together, linked to cardiometabolic risks. However, not all develop adverse outcomes. This study aimed to validate a transcriptomic signature to identify individuals at greatest cardiometabolic risk.METHODSUsing National Heart, Lung and Blood Institute (NHLBI) criteria to define cardiometabolic risk, healthy and prehypertensive 17-year-olds were identified in the Avon Longitudinal Study of Parents and Children (ALSPAC) (UK) childhood cohort. INTRODUCTION: Immediate postpartum long-acting reversible contraception (IPP-LARC) after delivery may decrease rapid repeat pregnancy (RRP). RRP is associated with preterm birth, small for gestational age, low birth weight, and infant/maternal mortality. In April 2017, the Ohio legislature passed SB 332, mandating immediate postpartum LARC coverage. Later that year, our hospital made the IPP-LARC Nexplanon (and not intrauterine devices [IUDs]) available to all delivering patients. This study aimed to see if SB 332 is associated with decreased RRP.
METHODS: Using electronic health record and s

BACKGROUND: Pre-pregnancy underweight (BMI <18.5 kg/m2) is notably prevalent among reproductive-aged Japanese women, affecting approximately 20%, compared to less than 10% in Western countries. However, its overall impact on maternal and infant outcomes in Japanese populations has not been systematically evaluated.
METHODS: We conducted a systematic review and meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. MEDLINE, EMBASE, CINAHL, PsycINFO, Cochrane Library, and Ichushi were searched from inception to February 2024. Cohort and case-

BACKGROUND: We aimed to evaluate the risk of perinatal complications in subsequent pregnancies after different types of myomectomy, viz. open, laparoscopic, or hysteroscopic. Moreover, we investigated whether the time interval from myomectomy to subsequent pregnancy (TIMP) is a risk factor for perinatal complications.
METHODS: This retrospective cohort study analyzed data from the vast Japanese health insurance JMDC database between January 2008 and July 2024. We identified primiparous women and excluded participants based on the following criteria: age < 20 years at delivery, diagnosis of ad

BACKGROUND: Routine measurement of gastric residual volumes involves regularly aspirating the entire stomach contents to assess the volume and colour of the aspirate to inform feeding. This is an established practice in many United Kingdom and Australian neonatal units for preterm infants receiving gastric tube feeds. The rationale is to assess feed tolerance and to predict and potentially prevent necrotising enterocolitis, a serious gut condition. Routine measurement of gastric residual volumes may also be associated with adverse outcomes and harm, including delayed achievement of full entera

TITLE: Outcomes from a procedural abortion training program for providers in underserved areas: findings from a four-year evaluation. BACKGROUND: In 2019, the Clinical Abortion Training Centers (CATC) program was initiated in collaboration with the National Abortion Federation to address gaps in abortion training for a diverse array of clinicians, not limited to ob-gyn specialists. The program aimed to increase the number of clinicians providing procedural abortion care, especially in underserved areas. Five abortion clinics, including independent and Planned Parenthood affiliates, trained phy

Background. Early life stress results in disrupted stress regulation, and less optimal neurodevelopment and oral feeding skills in very preterm infants. Despite these associations, there are few evidence-based interventions to help these fragile infants during critical stages of development. Purpose. This article describes an ongoing research protocol of a randomized controlled trial (RCT) to evaluate the multisensory early oral administration of human milk (M-MILK) as an early NICU intervention to improve stress regulation, support optimal neurodevelopment, and promote competent oral feeding

IMPORTANCE: Community water fluoridation (CWF) is a widely implemented public health intervention aimed at preventing dental caries. However, concerns have emerged about potential unintended birth outcomes of fluoride exposure, particularly birth weight, which is a widely accepted summary measure of infant health and has been found to be associated with later-life health and human capital.
OBJECTIVE: To evaluate the association between prenatal exposure to CWF and birth outcomes.

BACKGROUND: Maternal respiratory syncytial virus (RSV) vaccine, RSV prefusion F protein vaccine (RSVpreF (Abrysvo)), was found to be safe and efficacious in the MATISSE trial. However, post-hoc stratified analyses identified an excess of preterm births in the intervention arm in two upper-middle-income countries, most prominently in South Africa. This study weighs the potential benefits and risks in mortality associated with maternal RSV vaccination in South Africa, assuming the increased risk of preterm births observed in the trial was caused by vaccination.
METHODS AND FINDINGS: We compared

Caffeine, a habit-forming substance of no nutritional value, is consumed daily by most pregnant women. This focused narrative review examines evidence of association between maternal caffeine consumption and negative pregnancy outcomes, and assesses whether current advice guidelines are consistent with the available evidence. The majority finding from original empirical studies, systematic reviews, and meta-analyses is that maternal consumption of caffeine is reliably associated with serious negative pregnancy outcomes and negative outcomes in offspring. Evidence of harm is strong for miscarri

UNLABELLED: The overrepresentation of small-for-gestational-age (SGA) preterm infants among those with bronchopulmonary dysplasia (BPD) poses a challenge in quantifying the extent to which SGA independently affects respiratory outcomes. This two-center study aims to evaluate whether SGA status at birth is associated with reduced oxygen saturation (SpO2) to fraction inspired oxygen (FiO2) ratio (SFR) at 36 weeks (W) in preterm infants, independent of BPD. We retrospectively reviewed clinical data of preterm infants born between 24+0/7 and 30+6/7W of gestational age (GA). SGA was defined as a bi

OBJECTIVE: Previous studies have reported a higher rate of small for gestational age (SGA) newborns from fresh embryo transfers compared with frozen-thawed embryo transfers (FET), potentially due to the supraphysiologic serum estradiol levels observed during controlled ovarian stimulation. This study aimed to evaluate whether different regimens of exogenous estradiol administration influence live birth rates and neonatal outcomes in hormone replacement therapy (HRT)-FET cycles.
METHODS: We conducted a retrospective analysis of patients undergoing their first FET with HRT-based endometrial pre

BACKGROUND: Gestational weight gain (GWG) is a modifiable factor influencing maternal and neonatal health. Current GWG guidelines, largely derived from Western populations, may not align with the physiological and sociodemographic characteristics of Asian women. This study examined the associations between GWG and pregnancy outcomes across pre-pregnancy body mass index (BMI) categories in a nationally representative Taiwanese cohort.
METHODS: We analyzed 18,970 full-term singleton mother-infant pairs from the Taiwan Birth Cohort Study. GWG was calculated from self-reported pre-pregnancy and d
