Age-Adjusted Cerebellar Volumetry and Developmental Outcomes in Extremely Low Birth Weight Infants.

BACKGROUND: Infants with extremely low birth weight (ELBW) are at high risk for neurodevelopmental impairment, but the relationship between cerebellar structure and later developmental outcome remains incompletely defined. METHODS: In this retrospective single-center study, we examined total cerebellar volume on clinically acquired brain magnetic resonance imaging (MRI) in ELBW infants while accounting for postmenstrual age (PMA) at imaging. RESULTS: Of 65 infants screened, 42 were included. Mean gestational age at birth was 25.54 ± 1.68 weeks, mean birth weight was 733.2 ± 118.7 g, and mean

Iodine exposure and thyroid function after prophylactic intragastric iodinated contrast administration for meconium obstruction of prematurity in extremely low birth weight infants.

OBJECTIVES: Maternal hypertensive disorders of pregnancy (HDP) increase the risk of meconium obstruction of prematurity in small-for-gestational-age (SGA) extremely low birth weight (ELBW) infants. While prophylactic intragastric amidotrizoate (Gastrografin®) promotes meconium excretion, concerns persist regarding iodine load and thyroid dysfunction. METHODS: We retrospectively analyzed infants (<30 weeks, <700 g) born 2016-2024. The treated group (n=20) comprised SGA infants of mothers with HDP receiving prophylactic 5-fold diluted Gastrografin (iodine: 148-222 mg/kg) after stabilization. Co

Ultrasound-guided central venous access in the NICU: indications, insertion and management.

UNLABELLED: Ultrasound-guided cannulation of deep veins has advanced in little more than a decade from the operating room to the neonatal bedside and from a rescue maneuver to a planned procedure. Centrally inserted central catheters (CICCs) and femorally inserted central catheters (FICCs) are quite different from epicutaneo-cava catheters: they are 3-4 Fr power-injectable polyurethane catheters that are easy to tunnel, secured by subcutaneous anchorage, and suitable for blood transfusion, high-flow infusions, repeated blood sampling, and hemodynamic monitoring. This narrative review examines

Suspected Infective Endocarditis in a Premature Neonate After Patent Ductus Arteriosus Occluder Placement: A Case Report.

Patent ductus arteriosus (PDA) is the most common cardiac abnormality in premature neonates, and percutaneous, transcatheter occlusion devices are increasingly used for its nonsurgical closure. Even though PDA occluders are safe and effective, they are not without risks. Infective endocarditis (IE), a complication associated with percutaneous occlusion device placement in other congenital heart conditions, has not been described with a PDA occluder. We describe a case of suspected endocarditis in an extremely low birth weight (ELBW) neonate after PDA occluder placement.

Research progress on the use of beta-blockers in the progression prevention, and treatment of retinopathy of prematurity.

Retinopathy of prematurity (ROP) is a proliferative retinal vascular disorder unique to preterm infants with extremely low birth weight, ranking as the top cause of preventable childhood blindness. Retinal hypoxia, excessive secretion of vascular endothelial growth factor (VEGF) and dysregulation of insulin-like growth factor-1 (IGF-1) constitute the core pathogenic cascades of ROP. As competitive antagonists of β-adrenergic receptors (β-AR), beta-blockers suppress the HIF-1α/VEGF and PI3K/Akt/VEGF signaling axes to inhibit pathological retinal neovascularization, while exerting auxiliary anti

From invasive to non-invasive: point-of-care ultrasound parameters and a clinical decision algorithm for central venous pressure assessment in critically Ill neonates.

Point-of-care ultrasound (POCUS) is increasingly used in neonatal intensive care units (NICUs) for hemodynamic assessment, but standardized protocols for central venous pressure (CVP) estimation remain lacking. This narrative review synthesizes current evidence on POCUS parameters-primarily the inferior vena cava collapsibility index (IVC-CI), maximum IVC diameter, the inferior vena cava-to-aortic ratio (IVC/Ao), and internal jugular vein (IJV) indices-for non-invasive CVP assessment in critically ill neonates. Key advances include demonstrating that ultrasound-measured IVC diameter correlates

Heart rate variability after hospital discharge in perinatal risk populations: a systematic review and meta-analysis.

BACKGROUND: Perinatal risk factors such as preterm birth, intrauterine growth restriction, and low birth weight are linked to elevated cardiovascular and neurodevelopmental risk. Heart rate variability (HRV) is widely used to assess autonomic function in these populations, yet findings remain inconsistent. While autonomic differences during the NICU period are well established, less is known about HRV after hospital discharge across childhood and adulthood. METHODS: PubMed, EMBASE, Web of Science, Scopus, and APA PsycArticles were searched through April 2025, supplemented by ClinicalTrials.go

Institutional neonatal care as a manifestation of modern biosocial cooperation.

We don't see institutional neonatal care as a cold triumph of modern technocracy, but as the peak of an ancient human evolutionary strategy - biosocial cooperation and alloparenting. We trace care for vulnerable newborns from the evolutionary roots and selfless altruism seen in higher primates, through the deconstruction of myths about ancient eugenics with modern paleopathological findings in Greece, all the way to today's neonatal intensive care units. Using the example of kangaroo care and the neurobiological response of medical staff to newborn crying, we show that modern high-tech medicin

Delayed Thoracoscopic Repair Following Abdominal Esophageal Banding in a 539-g Infant With Triple Atresia: A Case Report.

Triple atresia, comprising esophageal atresia, duodenal atresia, and anorectal malformation, has no established management strategy in extremely low-birth-weight (ELBW) infants. We report successful staged treatment in a 539-g infant with severe respiratory failure. Transabdominal esophageal banding and gastrostomy on Day 0 controlled airflow through the tracheoesophageal fistula without thoracic intervention. Duodeno-duodenostomy and transverse colostomy were performed on Day 18 (680 g), permitting enteral feeding. After growth to 1924 g, thoracoscopic repair of Gross type C esophageal atresi

PDA Closure Rates and Safety after Repeated Courses of Cyclooxygenase Inhibitors in Preterm Infants: A Systematic Review and Meta-Analysis.

BACKGROUND: Cyclooxygenase (COX) inhibitors have been used to treat hemodynamically significant patent ductus arteriosus (PDA) in preterm infants. If the PDA remains hemodynamically significant after an initial course, additional courses of COX inhibitors or surgical intervention are considered. This systematic review evaluates PDA closure rates and safety after repeated courses of COX inhibitors in preterm infants. METHODS: Embase, Medline, and two other databases were searched through April 2026 (pre-registered in PROSPERO [CRD42023454003]). Studies reporting PDA closure rates after two or

Spirometry phenotypes at 8 years in children born extremely preterm or with extremely low birth weight.

INTRODUCTION: Preterm birth is associated with poor expiratory airflow, but spirometry phenotypes are not well-described. OBJECTIVES: To characterise abnormal spirometry phenotypes at age 8 years in children born either extremely preterm (EP; <28 weeks' gestation) or extremely low birth weight (ELBW; <1000 g birth weight), and to describe the early-life (perinatal and early growth) variables associated with each phenotype. METHODS: Participants comprised survivors born EP/ELBW in Victoria, Australia, in three eras (1991-1992, 1997 and 2005) and contemporaneous term-born controls with spirome

Stability of caffeine citrate intravenous dilutions in sodium chloride and glucose solutions.

OBJECTIVES: Caffeine citrate, a methylxanthine derivative, is commonly used in the management of apnoea of prematurity. In clinical settings, particularly in neonatal intensive care units, various dilutions of caffeine citrate in intravenous fluids are required to ensure precise and individualised dosing for extremely low birth weight infants. This study aimed to assess the stability of caffeine citrate solutions at concentrations of 1 mg/mL and 12.5 mg/mL in 0.9% w/v sodium chloride and 5% w/v glucose, stored at room temperature and under refrigeration up to 48 hours. METHODS: Caffeine citra

Sex differences in expiratory airflow among survivors born before 28 weeks' gestation or under 1000 g birthweight, and normal birthweight controls.

Expiratory airflow was measured by spirometry at ages 8 years, 18 years and 25 years in 297 survivors born extremely preterm (EP; <28 weeks' gestation) or extremely low birth weight (ELBW; <1000 g) and 260 normal birth weight (>2499 g) controls. At 8 years, expiratory airflows were similar between males and females in both EP/ELBW and control groups. Worsening expiratory airflows with increasing age were noted in the EP/ELBW group but not controls. Specifically, males born EP/ELBW had z-scores for forced expired volume in 1 s which were lower by a mean of -0.17 SD per decade (95% CI -0.27 to -

From the first months to later ages: Early spontaneous movements and developmental outcomes in extremely low birth weight infants.

BACKGROUND: Extremely low birth weight (ELBW; <1000 g) infants are at high-risk for developmental problems; however, no such long-term study has been conducted in this group before in our country. This study aimed to compare the early spontaneous movements and later developmental outcomes of ELBW infants with those of term born normal birth weight (NBW) infants, and to examine the association between the early spontaneous movements and later developmental outcomes at 9-to 12-months of corrected age and at 24-to 42-months in ELBW infants. METHODS: Early spontaneous movements of all infants wer

Optimal weight gain to reduce obesity risk in preterm infants in a National cohort study.

This study aims to identify the optimal postnatal weight gain through age 6 to prevent obesity in preterm infants. Nested case-control study using a nationwide South Korean cohort (2008–2015) included preterm infants with obesity (cases) and those with BMI between the 5th–85th percentiles (controls) at age 6. Weight-for-age and BMI z-scores were compared using Cohen’s d. Logistic regression identified obesity risk factors, and net benefit analysis determined optimal intervention points by birth weight. Among 41,286 preterm infants, 3349 (8.1%) developed obesity. Birth weight z-score difference
