Is metformin therapy in conjunction with lifestyle modifications more effective than lifestyle modifications alone in lowering the risk of gestational diabetes mellitus in pregnant women with metabolic dysfunction-associated steatotic liver disease (MASLD)? A protocol for parallel-arm superiority randomised controlled trial in tertiary care centres, Puducherry, South India.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) and gestational diabetes mellitus (GDM) are prevalent metabolic disorders in pregnancy, posing significant risks to maternal and fetal health. This study evaluates the effectiveness of metformin, in combination with lifestyle modifications, compared with lifestyle modifications alone, in reducing the incidence of diabetes, pro-inflammatory liver markers, adverse maternal and neonatal outcomes and total gestational weight gain in pregnant women diagnosed with MASLD in the first trimester. METHODS: This parallel-arm, r

Sialic Acid Supplementation in Perinatal Mice Attenuates Gestational Diabetes Mellitus-Induced Liver Injury in Offspring at Early Adulthood.

BACKGROUND: Gestational diabetes mellitus (GDM) is prone to induce hepatic injury in offspring. Sialic acid (SA) exhibits hepatoprotective effects, whereas its potential against GDM-induced liver injury in offspring remains unclear. OBJECTIVES: The present study aimed to investigate the hepatoprotective effects of maternal SA supplementation in GDM-exposed offspring and to explore its underlying mechanisms. METHODS: C57BL/6J mice were divided into 3 groups: a control (NC-m) group, a gestational diabetes mellitus (GDM-m) group, and a gestational diabetes mellitus plus SA (GDM+SA-m) group. The

Bicuculline from Forsythia suspensa attenuates liver fibrosis via TLR4/MD2 inhibition and suppression of MyD88/NF-κB signaling.

Liver fibrosis, a cardinal feature of chronic liver injury, involves hepatic stellate cells (HSCs) activation and excessive extracellular matrix (ECM) deposition. Upon liver fibrosis, TLR4 is highly expressed on HSCs and Kupffer cells. TLR4 activation induces liver inflammation followed by exacerbated fibrosis. Inhibition of TLR4 signaling represents a validated anti-liver fibrosis therapeutic approach. In the realm of liver fibrosis treatment, traditional Chinese medicine (TCM) has demonstrated certain anti-fibrotic effects in clinical practice after long-term application. However, due to the

Metabolic Dysfunction-Associated Steatotic Liver Disease in Pregnancy: A Review.

IMPORTANCE: Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as nonalcoholic fatty liver disease, is characterized by fatty deposits in the liver, with histologic features like alcohol-induced liver injury, without associated alcohol misuse. This condition is associated with other metabolic comorbidities, particularly obesity and insulin resistance. MASLD is also associated with the development of pregnancy-related complications such as gestational diabetes mellitus and hypertensive disorders of pregnancy. MASLD is one of the most common liver diseases today, wi

Updates in Biliary Atresia: Aetiology, Diagnosis and Surgery.

Biliary atresia (BA) is an obliterative disease of the bile ducts affecting between 1 in 10,000-20,000 infants with a predominance in Asian countries. It is clinically heterogeneous with a number of distinct variants (e.g., isolated, Biliary Atresia Splenic Malformation syndrome, Cat-eye syndrome, cystic BA, and CMV-associated BA). Facts about its aetiology are hard to encounter but might include genetic, developmental, exposure to an environmental toxin, or perinatal virus infection. However, the cholestatic injury triggers an intrahepatic fibrotic process beginning at birth and culminating i

Gold Mesoporous Silica-Coated Nanoparticles for Quantifying and Qualifying Mesenchymal Stem Cell Distribution; a Proof-of-Concept Study in Large Animals.

Mesenchymal stem cells (MSCs) have demonstrated promising therapeutic potential across a wide range of diseases including (multi) organ injury in neonates. Despite the reported preclinical successes of MSC therapy, a major challenge in their clinical translation is a limited understanding of their biodistribution after administration. This knowledge gap needs to be addressed to allow clinical implementation. Accordingly, in this study, we propose that silica-coated gold nanoparticles (AuMS) are a promising tool for in vivo MSC tracing. This study explores the use of AuMS for both qualitative a

Long-Term Maternal Exposure to Triclosan Provokes Pulmonary Fibrosis in Mouse Offspring.

Triclosan (TCS) is a broad-spectrum antibacterial agent that is widely used as an additive in personal care products. Due to its extensive application, TCS has been frequently detected in human samples, posing potential health risks. To investigate the adverse effects on offspring following maternal exposure to TCS, 4-week-old female mice were exposed to TCS via daily gavage until the end of lactation. The results showed that the birth weight, 21-day survival rate, and skeletal length of offspring were all significantly reduced. Morphological examinations revealed distinct inflammatory infiltr

Effects of gestational diabetes mellitus on offspring: A literature review.

Gestational diabetes mellitus (GDM) is a health problem that is becoming increasingly serious worldwide. It has a high incidence and strongly affects both mothers and children. In recent years, more extensive and in-depth studies have been conducted. In the present study, we systematically reviewed the effects of GDM on offspring, including the short- and long-term outcomes. Related articles were indexed in PubMed, Web of Science, Science Direct, and the Cochran Library up to June 2024. Recommendations were extracted from the identified articles and collated as themes. Many studies, which are

β-hydroxybutyrate facilitates mitochondrial-derived vesicle biogenesis and improves mitochondrial functions.

Mitochondrial dynamics and metabolites reciprocally influence each other. Mitochondrial-derived vesicles (MDVs) transport damaged mitochondrial components to lysosomes or the extracellular space. While many metabolites are known to modulate mitochondrial dynamics, it is largely unclear whether they are involved in MDV generation. Here, we discovered that the major component of ketone body, β-hydroxybutyrate (BHB), improved mitochondrial functions by facilitating the biogenesis of MDVs. Mechanistically, BHB drove specific lysine β-hydroxybutyrylation (Kbhb) of sorting nexin-9 (SNX9), a key regu

Human Hearts Intrinsically Increase Cardiomyocyte Mitosis After Myocardial Infarction.

BACKGROUND: Myocardial infarction (MI) is a leading cause of death worldwide and can eliminate up to a third of the cardiomyocytes within the human heart. Although cardiomyocytes undergo mitosis during early development, most cardiomyocytes cease cell cycling soon after birth. In contrast, rodent MI models have shown that cardiomyocytes increase mitosis in response to ischemia; however, this has not been shown in humans. METHODS: Using a unique premortem post-MI human heart, immunostaining, bulk RNA sequencing, proteomics, metabolomics, single-nucleus RNA sequencing and a novel post-MI human

Magnetothermodynamic Therapy Enables Selective Elimination of Cancer Cells in Perivascular Hepatocellular Carcinoma.

Thermal ablation has emerged as a frontline clinical intervention for treating various malignant tumors, yet its therapeutic efficacy in perivascular hepatocellular carcinoma (HCC) is severely limited owing to the indiscriminate thermal injury to critical vascular structures. In this study, we demonstrate a magnetothermodynamic strategy wherein intracellular magnetic hyperthermia (IMH) thermally enhances tumor-specific chemodynamic reactivity, generating a synergistic cytotoxic effect that selectively kills cancer cells while sparing critical normal tissue. Leveraging ferrimagnetic vortex-doma

TCF3 activates super-enhancer-driven TRIB2 overexpression to suppress ferroptosis and promote hepatoblastoma proliferation.

BACKGROUND: Hepatoblastoma (HB) is the most common pediatric liver malignancy with an increasing incidence. However, the functional roles of 3D chromatin organization, epigenetic regulatory factors, and transcriptional reprogramming in HB pathogenesis remain poorly understood. METHODS: Integrated multi-omics analyses of HB and matched non-tumor tissues were performed, including Hi-C, H3K27ac CUT&Tag, ATAC-seq, and RNA-seq, to construct high-resolution 3D epigenomic maps and identify genes interacting with HB-specific super-enhancers (SEs). Functional assays of identified targets were conduct

Imaging Manifestations of Alcohol Use-associated Disorders and Diseases.

Alcohol use disorder (AUD) is a serious condition linked to multiple health issues such as nervous system damage, heart disease, alcoholic liver disease, development of cancers, fetal alcohol syndrome, mental health disorders, and injuries to self and others. AUD presents a growing problem worldwide with significant economic and health care costs. Despite its growing prevalence, AUD remains underdiagnosed, partly due to the underreporting of alcohol intake and the stigma associated with the diagnosis. While traditionally diagnosed through clinical evaluation and patient history, diagnosis of A

Adverse Metabolic Phenotypes in Parenterally Fed Neonatal Pigs Do Not Persist into Adolescence.

BACKGROUND: Nutrition during fetal and neonatal life is an important determinant for the risk of adult-onset diseases, especially type 2 diabetes and obesity. OBJECTIVES: We aimed to determine whether total parenteral nutrition (TPN) compared with enteral formula feeding [enteral nutrition (EN)] in term piglets during the first 2 wk after birth would increase the long-term (5-mo) development of metabolic syndrome phenotypes with adverse glucose homeostasis, fatty liver disease, and obesity. METHODS: Neonatal female pigs were administered TPN (n = 12) or fed enterally with a liquid enteral mi

Epigenetic programming mediates abnormal gut microbiota and disease susceptibility in offspring with prenatal dexamethasone exposure.

Prenatal dexamethasone exposure (PDE) can lead to increased susceptibility to various diseases in adult offspring, but its effect on gut microbiota composition and the relationship with disease susceptibility remains unclear. In this study, we find sex-differential changes in the gut microbiota of 6-month-old infants with prenatal dexamethasone therapy (PDT) that persisted in female infants up to 2.5 years of age with altered bile acid metabolism. PDE female offspring rats show abnormal colonization and composition of gut microbiota and increased susceptibility to cholestatic liver injury. The
