A strain-specific metabolic role for the UDP-glucose 4-epimerase Uge3 in Aspergillus fumigatus virulence.

UNLABELLED: Expression of a fungal-specific sub-telomeric gene, hrmA , in Aspergillus fumigatus is important for a colony biofilm morphology termed H-MORPH, increased hypoxic fitness, and virulence in a murine model of invasive pulmonary aspergillosis (IPA). How expression of hrmA contributes to virulence and worse disease progression is ill-defined. Increased hrmA expression results in reduced attachment of the extracellular matrix (ECM) to the fungal cell wall resulting in decreased strain adherence. Fungal strains that are less adherent in vitro are typically less virulent as the ECM hetero

Diagnostic criteria for invasive pulmonary aspergillosis in COPD patients.

Over 400 million people have chronic obstructive pulmonary disease (COPD), with exacerbations representing a major health burden. Although the overall incidence of invasive pulmonary aspergillosis (IPA) in patients with COPD with hospitalised exacerbation is only 1 to 4%, certain factors substantially increase this frequency. Risk factors compromising defences against Aspergillus spp in COPD patients include systemic or high-dose inhaled corticosteroids, comorbidities including bronchiectasis, diabetes, and cardiovascular disease, and prolonged courses of antibiotics. An international group of

Management Challenges and Outcomes of Bronchiolitis Obliterans Syndrome After Hematopoietic Stem Cell Transplantation: First Case Series Report From Vietnam.

Bronchiolitis obliterans syndrome (BOS) is a serious pulmonary manifestation of chronic graft-versus-host disease (cGvHD) after allogeneic hematopoietic stem cell transplantation (HSCT). Data from low- and middle-income countries remain scarce. We conducted a case series of 11 patients diagnosed with BOS between November 2023 and June 2025 at the National Lung Hospital, Hanoi, Vietnam. Clinical characteristics, treatments, pulmonary function, and outcomes over 12 months were analyzed. The series was predominantly male (81.8%), with a median age of 34 years (range, 12-57). The median time from

Incidence, mortality and risk factors of invasive pulmonary aspergillosis in critically ill patients during the COVID-19 era: a global systematic review and meta-analysis.

BACKGROUND: Invasive pulmonary aspergillosis (IPA) represents a growing threat to critically ill patients, with mortality rates exceeding 50%. The burden of IPA has been exacerbated during the COVID-19 era due to prolonged ICU stays and widespread use of immunosuppressive therapies. This systematic review and meta-analysis aimed to synthesize global evidence on the incidence, mortality, and risk factors of IPA in ICU patients. METHODS: Observational studies reporting IPA incidence, mortality, or risk factors in ICU settings from their inception between November 1, 2019 to October 11, 2024 for

A meta-analysis-informed diagnostic stratification tool for invasive pulmonary aspergillosis in severe fever with thrombocytopenia syndrome: systematic review, meta-analysis, and single-center cohort-based assessment.

BACKGROUND: This study aimed to develop and assess a meta-analysis-informed diagnostic stratification tool for invasive pulmonary aspergillosis (IPA) in patients with severe fever with thrombocytopenia syndrome (SFTS). METHODS: A systematic review and meta-analysis were conducted to identify bedside clinical factors associated with IPA/SFTS-associated pulmonary aspergillosis (SAPA) in patients with SFTS. Because individual participant data were unavailable, pooled study-level odds ratios were translated into a pragmatic three-variable diagnostic stratification score rather than a conventional

The impact of different therapies on invasive pulmonary aspergillosis in patients with severe fever and thrombocytopenia syndrome: a systematic review and meta-analysis.

BACKGROUND: In recent years, a growing number of studies have reported that patients with severe fever with thrombocytopenia syndrome (SFTS)-associated invasive pulmonary aspergillosis (SAPA) patients exhibit a relatively high incidence and mortality rate. However, the conclusions vary considerably and SFTS patients frequently receive immunosuppressive therapies such as corticosteroid therapy, and the influence on the incidence of SAPA remains uncertain. Therefore, this study intends to conduct a comprehensive systematic review and meta-analysis to consolidate existing evidence. METHODS: Effe

Management of invasive fungal infections in the intensive care unit.

INTRODUCTION: Invasive fungal infections (IFI) are becoming more and more frequent in the intensive care unit (ICU), affecting not only immunocompromised patients. Invasive pulmonary aspergillosis, invasive candidiasis, and mucormycosis are associated with substantial mortality. Management of these patients remains challenging because the histopathological confirmation is often unfeasible. Consequently, clinicians must rely on a probabilistic approach integrating host factors, clinical and radiological features, and mycological evidence. AREAS COVERED: This narrative review summarizes current

Aspergillus fumigatus as a causative agent of a number of dangerous allergic diseases.

Aspergillus fumigatus is a clinically important airborne fungal pathogen capable of causing a broad spectrum of respiratory disorders. Persistent exposure or airway colonization can provoke severe allergic diseases, particularly in patients with asthma or cystic fibrosis, whereas impaired immunity or pre-existing structural lung damage may allow the development of chronic or life-threatening invasive pulmonary aspergillosis. Accordingly, the aim of this review was to synthesize current evidence on the role of A. fumigatus in the development of allergic and non-allergic diseases, with particula

A clinically actionable nomogram for predicting invasive pulmonary aspergillosis: A nested case-control study.

Invasive pulmonary aspergillosis (IPA) is a life-threatening infection with high mortality, often misdiagnosed due to non-specific symptoms. The lack of effective early prediction models delays treatment and worsens outcomes. To identify independent IPA risk factors and develop a clinically actionable nomogram for early prediction, we conducted a nested case-control study within a 10-year cohort of 27 100 pulmonary infection patients. The cohort was split into training (70%) and testing (30%) sets. In the training set, 1002 IPA cases (proven or probable, as defined by European Organization for

Inhaled and oral corticosteroid use and the risk of pulmonary aspergillosis: a Danish population-based case-control study.

BACKGROUND: Pulmonary aspergillosis is a rare, high mortality lung infection caused by the Aspergillus sp. Chronic lung diseases and immunosuppression are known risk factors, but data on the impact of corticosteroids and their route of administration are limited. METHODS: To evaluate the association between corticosteroid use and pulmonary aspergillosis, we conducted a retrospective case-control study in all Danish citizens who redeemed at least one prescription for inhaled medicine between 1994 and 2025. We identified 1351 patients with aspergillosis other than allergic bronchopulmonary aspe

Beyond detection: quantitative interpretation of Aspergillus-positive bronchoalveolar lavage fluid metagenomic next-generation sequencing for diagnostic stratification and prediction of respiratory deterioration.

BACKGROUND: The increasing use of bronchoalveolar lavage fluid (BALF) metagenomic next-generation sequencing (mNGS) has substantially improved the detection of Aspergillus species in patients with suspected pulmonary infections. However, positive mNGS results frequently present a clinical dilemma because Aspergillus may represent invasive pulmonary aspergillosis (IPA), airway colonization, or transient fungal detection. The clinical value of quantitative fungal burden assessment remains insufficiently defined, particularly regarding risk stratification among untreated patients. METHODS: We co

Efficacy of voriconazole combined with caspofungin in treating Chronic Obstructive Pulmonary Disease patients with invasive pulmonary aspergillosis: impacts on clinical outcomes, inflammatory cytokines, and immune markers.

OBJECTIVE: To compare the efficacy and safety of voriconazole plus caspofungin versus voriconazole alone in COPD patients with invasive pulmonary aspergillosis (IPA) and to determine whether the combination accelerates symptom resolution, reduces fungal and inflammatory load, and improves lung function and quality of life. METHODOLOGY: We retrospectively analyzed 152 COPD-IPA patients treated between April 2022 and April 2024, divided into an observation group (voriconazole + caspofungin, n=76) and control group (voriconazole alone, n=76). Outcomes included clinical efficacy, symptom resoluti

Discordant CYP2C19 genotype-phenotype relationship in voriconazole metabolism: impact of inflammation on drug exposure in a Chinese patient.

Voriconazole is commonly used for the treatment of invasive aspergillosis. Its metabolism is influenced by pharmacogenetic polymorphisms of CYP2C19 as well as by inflammatory states. We report a case of a 49-year-old Chinese woman with essential thrombocythaemia who was treated with voriconazole for possible invasive pulmonary aspergillosis. Despite possessing a CYP2C19 *1/*1 normal metaboliser genotype, she required unusually high doses (14.5 mg/kg/day), nearly twice the standard dose, to achieve therapeutic voriconazole concentrations without apparent adverse effects under therapeutic drug m

Severe fever with thrombocytopenia syndrome complicated by invasive pulmonary aspergillosis and septic shock: a case report highlighting the role of mNGS.

BACKGROUND: Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne viral hemorrhagic fever associated with high mortality, and no specific antiviral therapy is currently available. Patients with SFTS often develop immune dysfunction, rendering them susceptible to secondary opportunistic infections, particularly invasive pulmonary aspergillosis (IPA). Early diagnosis of this co-infection is critical but remains challenging due to nonspecific clinical manifestations and radiological findings.
