Child Welfare Involvement and Health Outcomes in Infants With Prenatal Substance Exposure.

IMPORTANCE: The child welfare system (CWS) is often contacted when a child is born with prenatal substance exposure, but there is limited evidence on the beneficial and iatrogenic effects of CWS involvement. OBJECTIVE: To examine whether involvement with the CWS at birth is associated with injuries and moderate- to high-complexity emergency department (ED) use in the first year of life among infants identified with prenatal substance exposure. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study using probabilistically linked administrative data of Pennsylvania statewide child welfa

A mixed methods analysis of health literacy among adults with brachial plexus birth injury.

ObjectiveThis study aimed to characterize health literacy among adults with brachial plexus birth injury (BPBI) and identify challenges and opportunities to improve BPBI care for affected adults.MethodsA prior multiphase, mixed methods outcomes study of adults with BPBI identified health literacy as salient factor needing further analysis. The current convergent mixed methods secondary analysis thus combined relevant quantitative and qualitative data from this prior study specifically to analyze health literacy in this population. Quantitative data included five scales of the Health Literacy Q

Parental traumatic brain injuries and their association with major depressive disorder in offspring: A study based on national registers in Finland.

BACKGROUND: Traumatic brain injuries (TBIs) can cause long-term brain dysfunction. Clinical studies suggest that parental TBI increases the risk of depressive symptoms in offspring. We aimed to investigate: 1) the association between parental TBI of different severities and later diagnosed offspring major depressive disorder (MDD) and 2) the association between the first parental TBI after offspring birth and subsequent offspring MDD, depending on child's age at injury. METHODS: This nested case-control study identified 37,682 cases aged ≤25 years, 148795 matched controls, and their parents f

Risk factors associated with deep perineal wound dehiscence after second-degree perineal tears or episiotomy.

INTRODUCTION: Second-degree perineal tears and episiotomies, involving the bulbocavernosus and/or superficial transverse perineal muscles, are common after vaginal birth and may lead to wound dehiscence, infection, and long-term pelvic floor dysfunction. Despite this, risk factors for deep wound dehiscence involving perineal musculature remain insufficiently studied. This study aimed to identify risk factors for wound dehiscence in second-degree tears and episiotomies to inform potential use of antibiotic prophylaxis. MATERIAL AND METHODS: A single-center case-control study (1:2 ratio) was co

The estimated potential birth loss from female accidental mortality in Iran, 2011-2023.

BACKGROUND: Road-traffic injuries remain an important cause of premature death among women of reproductive age in Iran. We quantified the number of potential births not realised that are attributable to female deaths from road-related accidents, to inform injury-prevention and fertility policy. METHODS: We conducted a retrospective, population-based counterfactual analysis for 2011-2023. Annual female deaths from road-traffic injuries by age (Global Burden of Disease 2023) were regarded "aged forward" to identify women who would have been 15-49 years in a given year had they survived. Maternal Satisfaction and Childbirth Experience Following Vaginal Delivery Versus Cesarean Section: A Systematic Review.

Childbirth is a crucial experience that impacts women's lives, and the choice between vaginal delivery and cesarean section (C-section) is a critical decision in obstetrics. Maternal satisfaction is influenced by multiple factors since the childbirth experience is a composite of physical, emotional, and social components. Understanding patient evaluation is crucial for providing patient-centered care and improving maternity and neonatal care services. This systematic review aimed to compare patient satisfaction between vaginal and cesarean delivery patients and identify influencing factors. We

Risk Factors and Prevalence of Obstetric Anal Sphincter Injuries in a Quaternary Care Center in Southern Thailand.

INTRODUCTION AND HYPOTHESIS: Perineal tears constitute significant complications of vaginal delivery, resulting in maternal morbidities. This study was aimed at determining the prevalence, risk factors, and morbidities associated with obstetric anal sphincter injuries (OASIs) in Southern Thailand. We hypothesized that specific maternal and obstetric factors increase the risk of OASIs. METHODS: In this retrospective study, data were collected from a database of pregnant women who underwent singleton vaginal deliveries between 2006 and 2022. The participants were divided into two groups: women

The Anesthesiologist's Role in Trauma-Informed Care: An Update.

Trauma-informed care continues to follow the 4R principles (realize the widespread nature of trauma, recognize what unresolved trauma looks like, respond by changing practices and protocols, and resist retraumatization) but has expanded to include 6 core guiding principles that include safety, trust, peer support, mutuality, empowerment, and the understanding of an individual's cultural context. Additionally, it is now recommended that trauma-informed practices be offered as universal precautions to all individuals, regardless of trauma history, focusing on trauma prevention. Obstetric anesthe
