AA in Hypertension and Preeclampsia.

Human essential hypertension is driven by a network of interacting nutritional, metabolic, inflammatory, and endothelial mechanisms in which AA and other PUFAs play a critical role. Beyond excess sodium intake, population data supports the role of inadequate calcium, potassium, and magnesium intake and low antioxidant vitamin status in shaping blood pressure regulation. These nutrients influence vascular smooth muscle tone and act as cofactors that support Δ6 and Δ5 desaturase activities, thereby governing tissue availability of AA, EPA, and DHA and downstream formation of vasodilator and anti

Neonatal Sequential Organ Failure Assessment Score (nSOFA) for Prediction of Mortality Among Preterm Neonates with Respiratory Distress Syndrome: A Prospective Observational Study.

Preterm birth is a major contributor to neonatal morbidity and mortality. This study evaluated the neonatal Sequential Organ Failure Assessment (nSOFA) score for predicting mortality in preterm neonates (< 34 weeks) with respiratory distress syndrome. Among 113 neonates, median (q1, q3) nSOFA scores at 0-6 h of life were higher in non-survivors than survivors [4 (4, 6) vs 0 (0, 2); P = 0.001], with an area under the curve (AUC) of 0.80 and a cutoff of 3. Predictive performance improved at 24 ± 3 h (AUC 0.93), with higher scores in non-survivors [8 (5, 11) vs 0 (0, 2); P < 0.001] and a cutoff o

Nasal high-frequency oscillatory ventilation as primary respiratory support in preterm infants with respiratory distress syndrome: current evidence and clinical implications.

Continuous positive airway pressure (CPAP) is the standard initial therapy for respiratory distress syndrome (RDS) and associated with failure rates of up to 50% in extremely preterm infants. Nasal intermittent positive pressure ventilation (NIPPV) has demonstrated consistent benefits over CPAP in reducing respiratory failure and intubation. Nasal high-frequency oscillatory ventilation (nHFOV) has also shown reduced treatment failure compared with CPAP, particularly in extremely preterm infants. However, direct head-to-head comparisons between nHFOV and NIPPV as primary support are lacking, an

Systemic outcomes following maxillomandibular advancement for obstructive sleep apnea: systematic review and meta-analysis.

Obstructive sleep apnea (OSA) is a common sleep disorder associated with an elevated risk of systemic comorbidities. Maxillomandibular advancement (MMA) surgery can reduce upper airway obstruction and improve sleep parameters, but its systemic health benefits remain less well characterized. This systematic review and meta-analysis was performed to synthesize evidence on the systemic health outcomes associated with MMA in patients with OSA. A comprehensive literature search was conducted in PubMed, Embase, Cochrane CENTRAL, Scopus, and Web of Science from inception through May 20, 2026. Inclusi

Persistent Sleepiness After Effective Positive Airway Pressure Therapy Deserves Independent Clinical Evaluation.

Positive airway pressure (PAP) therapy often improves excessive daytime sleepiness in obstructive sleep apnea (OSA), yet effective control of sleep-disordered breathing does not invariably restore normal daytime alertness. Persistent sleepiness after PAP therapy should therefore be regarded as a clinical observation requiring explanation rather than automatically attributed to treated OSA. The term "residual excessive daytime sleepiness" may become misleading when it is interpreted as an etiologic conclusion rather than a descriptive label. Evaluation should distinguish PAP adherence from trea

Modification of locomotion-respiration coupling in patients with obstructive sleep apnea syndrome.

INTRODUCTION: Locomotion is a complex motor task integrating sensory-motor and cognitive inputs through central pattern generator control. Locomotion-respiration coupling (LRC), defined as the synchronization between gait and breathing events, has been described during running and walking. Patients with obstructive sleep apnea syndrome (OSAS) exhibit altered gait patterns during wakefulness, which appear to be partially independent of nocturnal symptomatic treatment. While intermittent hypoxia may induce long-term ("fixed") central nervous system alterations, OSAS patients also display "revers

The role of polysomnography in Parkinson's disease: objective sleep assessment and clinical indications.

Sleep disorders (SDs) are highly prevalent in Parkinson's disease (PD), affecting up to 80% of patients during the disease and significantly contributing to impaired quality of life, cognitive decline, motor worsening, fall risk, and increased caregiver burden. Polysomnography (PSG) remains the gold standard for objective sleep assessment in PD and plays a central role in differential diagnosis and therapeutic planning. This review examines the spectrum of SDs identifiable by PSG, including rapid eye movement (REM) sleep behavior disorder (RBD), obstructive sleep apnea (OSA), periodic limb mov

Outcome of preterm newborns admitted with respiratory distress in relation to time of initiation of CPAP.

BackgroundRespiratory distress is a frequent complication in preterm infants, often requiring respiratory support. Respiratory distress syndrome (RDS), caused by surfactant deficiency in the immature lung, is the predominant etiology in this population. Continuous positive airway pressure (CPAP) is a common intervention, but the optimal timing of CPAP initiation remains less clearly defined in the late preterm population (33-36 weeks gestation). This study aimed to determine the outcomes of preterm newborns with respiratory distress in relation to the timing of CPAP initiation.MethodsThis pros

Pulmonary hypertension and acute hypoxic respiratory failure in preterm neonates.

Pulmonary hypertension (PH) in preterm infants is a complex and heterogeneous condition that significantly contributes to hypoxic respiratory failure (HRF), bronchopulmonary dysplasia (BPD), and mortality. Unlike term infants, PH in preterm neonates arises from multiple overlapping pathophysiological mechanisms and distinct hemodynamic phenotypes. These diverse phenotypes complicate diagnosis and limit the effectiveness of uniform treatment approaches. Despite increasing recognition of this heterogeneity, current literature and clinical guidelines provide limited clarity on the optimal diagnos

Electrical impedance tomography in neonatal respiratory diseases: a clinical review of applications and evidence.

Neonatal respiratory diseases remain a leading cause of morbidity and mortality in intensive care settings, with management complicated by the limitations of current monitoring tools. Chest radiography provides only static snapshots and cannot capture the dynamic evolution of lung aeration, while lung ultrasound, though portable, remains operator-dependent and incapable of quantifying regional ventilation. Electrical Impedance Tomography (EIT) has emerged as a noninvasive, radiation free imaging modality that addresses these gaps by enabling real time, continuous visualization of regional lung
