Nasal high-frequency oscillatory ventilation versus nasal continuous positive airway pressure for respiratory distress syndrome in preterm infants: A systematic review and meta-analysis.

PURPOSE: This systematic review and meta-analysis aimed to evaluate whether nasal high-frequency oscillatory ventilation (NHFOV) reduces the intubation rate compared to nasal continuous positive airway pressure (NCPAP) as a primary respiratory support strategy in preterm infants with respiratory distress syndrome (RDS). METHODS: We systematically searched Medline, the Cochrane Library, EMBASE, the Chinese National Knowledge Infrastructure (CNKI), and the Wanfang Database from inception to December 2025 for randomized controlled trials (RCTs). We included RCTs comparing NHFOV with NCPAP as the

Maternal Ureaplasma Species Colonization and Neonatal Outcomes: A Large Cohort Study on Preterm Birth and Vertical Transmission.

INTRODUCTION: The impact of maternal Ureaplasma colonization on vertical transmission and its contribution to acute neonatal morbidity remains unclear. METHODS: In this retrospective cohort of 1,647 mother-neonate dyads from a Chinese tertiary center (2020-2025), maternal vaginal and neonatal respiratory Ureaplasma colonization was detected via quantitative real-time polymerase chain reaction. We analyzed associations between maternal colonization and perinatal outcomes and assessed determinants of vertical transmission and whether neonatal colonization independently predicted severe morbidit

Lung ultrasound predicts surfactant need in neonates on different respiratory support: an international diagnostic accuracy study.

BACKGROUND: An early lung ultrasound score (eaLUS) predicts surfactant administration in neonates of any gestational age with respiratory distress syndrome stabilized on CPAP. Whether this is also true for more unwell infants, non invasively and invasively ventilated, is not known. AIM: To assess whether an eaLUS may predict surfactant replacement in infants on different respiratory support modes (CPAP, nasal ventilation and mechanical ventilation). We also investigated eaLUS as a potential marker of unfavorable respiratory outcomes. METHODS: Prospective, international, multicenter diagnostic

Basic Ventilator Graphics in the NICU: A Practical Overview.

Mechanical ventilation remains a cornerstone of neonatal intensive care, particularly for premature infants with respiratory distress syndrome. While blood gas analysis and radiographs provide clinical information, ventilator pulmonary graphics offer continuous, noninvasive insights into respiratory mechanics. Modern ventilators generate real-time data that help clinicians assess lung compliance, airway resistance, and patient-ventilator synchrony. Pressure-volume and flow-volume loops provide visual cues for detecting changes in compliance, air leaks, secretions, overdistension, air trapping,

Efficacy evaluation of pulmonary surfactant guided by lung ultrasound score in neonatal respiratory distress syndrome: a prospective observational study in Ganzi Tibetan Autonomous Prefecture.

We aimed to evaluate the therapeutic efficacy of pulmonary surfactant (PS) in neonatal respiratory distress syndrome (NRDS) when guided by lung ultrasound (LUS) score. A prospective cohort of 121 surfactant-treated NRDS infants was included. After 1:1 propensity score matching, 99 matched pairs (NRDS vs. healthy controls) were established. Clinical baseline characteristics, chest X-ray (CXR) grading, LUS, arterial blood gas indices, and adverse events were collected. Comparisons were made between pretreatment (within 4 h of birth) and post-treatment (12 h after PS) findings in the NRDS group.

The effect of gestational diabetes on maternal and neonatal outcomes.

OBJECTIVE: Women with gestational diabetes mellitus (GDM) are at increased risk of low blood sugar levels immediately after childbirth, and their babies are also at risk of macrosomia. Using the obstetric registry system in our hospital, we retrospectively assess the commonly occurring adverse maternal and infant outcomes in Taiwanese women with GDM. MATERIALS AND METHODS: We performed a hospital-based, retrospective cohort study. Women with singleton term pregnancy (≥37 weeks) aged 18-55 years at conception with live birth were identified from our registry from 2009 to 2022. Maternal outcome

Elevated Driving Pressure in Pressure-Controlled Ventilation: An Independent Risk Factor for Adverse Outcomes in Mechanically Ventilated Neonates.

BACKGROUND: Driving pressure (DP) has recently gained attention as a meaningful variable in lung-protective ventilation, particularly in studies of adult acute respiratory distress syndrome. Its role in neonates, particularly regarding bronchopulmonary dysplasia (BPD), is poorly defined. METHODS: This single-center retrospective study included 145 neonates who received invasive conventional mechanical ventilation (CMV) for ≥ 72 h between January 1, 2020 and December 31, 2024. In this study, DP was obtained by subtracting the applied positive end-expiratory pressure (PEEP) from the peak inspir

Introduction of a video-based heterogeneity index for quantitative lung ultrasound assessment in neonates with respiratory distress - a proof-of-concept study.

BACKGROUND: Lung ultrasound (LUS) is an established bedside tool for assessing neonatal respiratory distress. However, conventional semi-quantitative LUS scoring relies on subjective interpretation and may miss subtle but clinically relevant differences in lung aeration. We hypothesized that a novel quantitative grayscale-based Heterogeneity Index (HI) derived from LUS video loops could objectively quantify lung aeration and be associated with respiratory severity and clinical outcomes. METHODS: In this prospective single-centre observational study, neonates with respiratory distress underwen

Neonatal hypoxic-ischemic encephalopathy online registry in Shenzhen: protocol for a multicentre, prospective, open, observational cohort study.

INTRODUCTION: Hypoxic-ischaemic encephalopathy (HIE), a leading cause of neurological disability in neonates, is managed with therapeutic hypothermia, yet systematic data on its aetiology and prognosis are lacking, particularly in China. Herein, our study addresses this gap by establishing a prospective registry in Shenzhen and aims to clinically characterise HIE, identify its risk factors and document its long-term outcomes. METHODS AND ANALYSIS: We will recruit 200 neonates with HIE from 10 hospitals in Shenzhen, China, between January 2025 and December 2028. In parallel, clinical data will

Time to initiate trophic feeding and predictors among preterm neonates admitted at General Hospitals in Tigray, 2025.

BACK GROUND: Trophic feeding generally refers to providing small quantities of enteral feeding soon after birth. A study in Ethiopia highlighted substantial delays in starting therapeutic feeding (TF) for newborns. While guidelines recommend initiating TF within 24 hours of birth, an alarming 80-90% of infants did not begin feeding within 48 hours. Only 20% started TF within the advised timeframe. Furthermore, 29% of these infants did not survive until discharge, and 86.2% experienced extrauterine growth restriction. As a result, this study aims to assess the time to initiation of TF and its p

Non-invasive high-frequency oscillation ventilation versus nasal CPAP as primary respiratory support in preterm neonates ≥ 30 weeks with RDS: a non-inferiority randomized controlled trial.

UNLABELLED: The purpose of this study is to evaluate if non-invasive high-frequency oscillation ventilation (NHFOV) is non-inferior to nasal continuous positive airway pressure (CPAP) as primary non-invasive respiratory support (NRS) in preterm neonates of ≥ 30 weeks' gestation with respiratory distress syndrome (RDS). In this open-label randomized controlled trial (RCT) with a non-inferiority design conducted in a lower middle-income country, 142 preterm neonates were randomized to receive NHFOV (n = 71) or CPAP (n = 71) at equivalent pressures after lung recruitment with nasal mask (NM) inte

[Diagnosis and management of respiratory disorders in premature infants].

Neonatal respiratory distress is one of the leading causes of hospitalization of preterm newborns in intensive care units. Regardless of the pathology, initial respiratory support is usually delivered nasally (non-invasive ventilation), with intubation now being much less common than in previous decades, even for the most extremely preterm babies. The most frequent respiratory pathology is respiratory distress syndrome (RDS) also termed hyaline membrane disease. It is prevented by antenatal corticosteroid therapy and very effectively treated by the administration of exogenous surfactant directl

Lung ultrasound for neonatal respiratory distress in a resource-limited county hospital: a pilot study.

BACKGROUND: While tertiary centers have widely adopted lung ultrasound (LUS) as a diagnostic alternative to chest X-ray (CXR), recent evidence emphasizes its growing prognostic utility. This pilot study investigated the feasibility and clinical utility of LUS aeration scores in triage decisions within a resource-limited county-level hospital. METHODS: This prospective pilot observational study enrolled neonates exhibiting signs of respiratory distress within 24 h of birth, employing a symptom-driven approach without predefined gestational age restrictions. Bedside LUS and CXR were performed wi

Effects of Sample Deposition Medium and Drying on Spectroscopic Quantification of Lipid Biomarkers in Respiratory Distress Syndrome.

Rapid point of care assessment of pulmonary surfactant composition by measuring the lecithin/sphingomyelin (L/S) ratio could improve management of patients with neonatal respiratory distress syndrome (nRDS). Attenuated total reflectance Fourier transform infrared spectroscopy (ATR-FTIR) offers a practical route to making such measurements, but the influence of the sample solvent prior to drying on measurement repeatability is poorly understood. We compare films dried from dichloromethane (DCM) and water (AQ) solvents (DCM-dry route vs. AQ-dry route) by ATR-FTIR and show that spectra from the A

Two-Year Outcomes of Less Invasive Surfactant Administration Among Preterm Neonates: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: In preterm neonates supported with continuous positive airway pressure (CPAP), early caffeine administration and less invasive surfactant administration (LISA) results in a lower frequency of endotracheal intubation. It is unknown whether this regimen improves outcomes at a corrected age of 2 years in this population. OBJECTIVE: To determine whether LISA improves neurodevelopmental impairment (NDI) and pulmonary outcomes at a corrected age of 2 years. Evaluation of comparative efficacy of healthcare delivery by mothers versus trained healthcare workers in adjunct to Vojta therapy on clinical outcomes in neonates with respiratory distress syndrome: a protocol for randomised controlled trial.

INTRODUCTION: A common and serious illness that frequently necessitates specific respiratory and developmental assistance is Neonatal Respiratory Distress Syndrome (NRDS), which affects premature infants. A neurophysiological physiotherapy technique called Vojta therapy has demonstrated potential in enhancing neuromotor development and breathing patterns. However, the availability of qualified healthcare professionals frequently limits its administration. This gap might be closed by involving mothers as qualified caretakers, particularly in environments with limited resources. METHODS AND ANA

The Impact of Offspring Sex on Pregnancy and Neonatal Outcomes in Individuals with Pregestational Diabetes.

Pregestational diabetes mellitus (PGDM) is increasing in prevalence among pregnant individuals and is associated with adverse outcomes. Prior work suggests that offspring sex influences placental responses and may impact risk for adverse outcomes. We sought to assess the impact of offspring sex on adverse pregnancy and neonatal outcomes in individuals with PGDM.We conducted a retrospective cohort study of 503 pregnant individuals with PGDM and known offspring sex with care at a major urban academic center between 1998 and 2016. We assessed two co-primary composite outcomes: (1) a composite adv
