High-flow Nasal Oxygenation in Infants Undergoing Tracheal Intubation: A Single-center Randomized Controlled Trial.

BACKGROUND: High-flow nasal oxygenation (HFNO) is increasingly used as an alternative for preoxygenation and apneic oxygenation during tracheal intubation. However, its efficacy in infants remains underexplored. This study evaluated whether HFNO improves intubation outcomes compared to those with face mask ventilation in infants. METHODS: This prospective, single-center randomized controlled trial included 132 infants undergoing elective surgery under tracheal intubation. The infants were divided into HFNO and control groups, with both groups receiving 100% oxygen. The HFNO group received HFN

Characterization of Safety Signals for Spinal Muscular Atrophy Treatments in Children Aged 0 to 3 Years Potential Safety Patterns for Further Investigation.

BACKGROUND: Spinal muscular atrophy (SMA) has become a manageable chronic condition with the advent of three major disease-modifying therapies. However, real-world safety data in children aged 0-3 years remain limited. This study aimed to characterize safety signals using long-term pharmacovigilance data. METHODS: Adverse drug event reports related to SMA therapies submitted to the FDA Adverse Event Reporting System were extracted from the first quarter of 2015 to the third quarter of 2025. Disproportionality analyses were conducted to detect signals. RESULTS: A total of 1,155; 408; and 957

First neonatal case of primary ciliary dyskinesia caused by a pathogenic DRC1 variant presenting with recurrent atelectasis and airway hypersecretion: a case report and literature review.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a relatively rare hereditary disorder. Pathogenic variants in DRC1 are prevalent in East Asian populations and are frequently associated with situs solitus, which often obscures clinical suspicion. More than 80% of affected neonates with PCD present with respiratory distress; however, definitive diagnosis often requires specialized investigations, and many cases remain undiagnosed. Because PCD can progress to chronic obstructive lung disease and bronchiectasis, early diagnosis and appropriate management are essential. We report the first neonatal

Primary Ciliary Dyskinesia With Situs Inversus in Twin Premature Infants: Two Case Reports.

AIMS/BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare autosomal recessive disorder characterised by structural or functional abnormalities of the cilia, which prevent proper oscillation. Patients typically present with symptoms such as chronic respiratory infections, bronchiectasis, and, in approximately half of the cases, inversion of internal organs. Due to its diverse clinical manifestations and lack of specificity, PCD is often misdiagnosed or underdiagnosed, and diagnosis in the neonatal period remains notably challenging. Therefore, this study aimed to analyse the clinical characte

Systematic evaluation of extracardiac findings on cardiac CT angiography in children with CHD: a large single-centre experience.

INTRODUCTION: Cardiac CT angiography has seen a steady increase in use for the evaluation of CHD in children. While its primary role is to assess cardiovascular structures, CT angiography also captures extracardiac regions, often revealing previously unsuspected abnormalities. This study sought to determine the prevalence, classification, and clinical relevance of extracardiac abnormalities detected in paediatric patients undergoing CT angiography for CHD evaluation. MATERIALS AND METHODS: A retrospective review was conducted on CT angiography examinations from 1336 patients, aged from newbor

Early risk stratification for ICU admission in hospitalized children with pertussis: development and internal validation of an admission-time prediction model.

BACKGROUND: Severe pertussis in children can deteriorate rapidly, and early identification of patients likely to require intensive care remains challenging. We aimed to develop and internally validate an admission-time prediction model for ICU admission among hospitalized children with PCR-confirmed pertussis. METHODS: We conducted a single-center retrospective prediction-model study based on a hospitalized cohort at a provincial pediatric infectious disease center. Consecutive hospitalized children with PCR-confirmed pertussis between 6 June 2020 and 24 June 2025 were screened. Two children

High-frequency oscillatory ventilation with sigh breath increases pneumothorax in neonates born at 22-25 gestational weeks.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) is widely used in neonates with respiratory distress syndrome (RDS) to optimize lung recruitment while minimizing ventilator-induced lung injury. Sigh breaths have been incorporated into HFOV to improve alveolar recruitment in cases of atelectasis. However, the safety of this approach, particularly in extremely preterm neonates, remains unclear. This study aimed to evaluate whether the use of sigh breaths during HFOV increased the risk of developing tension pneumothorax within the first 96 h after birth in neonates born at 22-25 weeks o

Challenges in postoperative management of a patient with primary ciliary dyskinesia and Joubert syndrome and related disorders with congenital heart disease.

We report the case of a male infant born at 37 weeks and 4 days of gestation with a birth weight of 3396 g. He presented with hypotonia, congenital heart disease, heterotaxy and polydactyly. Based on physical examination and brain MRI, which revealed the characteristic molar tooth sign, he was diagnosed with Joubert syndrome and related disorders (JSRD). He experienced repeated episodes of atelectasis, making extubation challenging. Electron microscopy revealed ciliary structural abnormalities, leading to the diagnosis of primary ciliary dyskinesia as well as JSRD. Despite multiple medical int

Effect of high positive end-expiratory pressure on perioperative atelectasis in neonates and small infants (0-6 months) with healthy lungs: A randomized controlled trial.

INTRODUCTION: High positive end-expiratory pressure (PEEP) is commonly used to improve pulmonary ventilation in infants with injured lungs. However, its intraoperative application in infants with healthy lungs remains controversial. This study aimed to evaluate the effect of high PEEP on perioperative pulmonary outcomes (PPCs) in this population. METHODS: Neonates and small infants aged 0-6 months undergoing surgery were randomly assigned to receive mechanical ventilation with a conventional PEEP of 5 cmH2O or a high PEEP of 8 cmH2O. Lung ultrasound (LUS) score was measured at four time point

Development and validation of a pre-trained language model for neonatal morbidities: a retrospective, multicentre, prognostic study.

BACKGROUND: Early identification and monitoring of neonatal morbidities are critical for timely interventions that can prevent complications, optimise resource use, and support families. Although traditional tools based on tabular data and biomarkers are beneficial, they are restricted in assessing the risk of morbidities in newborns. In this study, we developed NeonatalBERT, a pre-trained large language model (LLM) that estimates the risk of neonatal morbidities from clinical notes. METHODS: This prognostic study investigated retrospective primary and external cohorts from two different quat

Dexamethasone dosage and course effects on respiratory outcomes in preterm twins: retrospective cohort study.

This retrospective cohort study evaluated the effects of complete, incomplete, and multiple courses of antenatal corticosteroids (ACS) on respiratory outcomes in twin preterm infants born between 28 + 0 and 36 + 6 weeks of gestation. A total of 1,115 neonates were analyzed. Primary outcomes included respiratory distress syndrome (RDS) and other adverse respiratory conditions. Infants exposed to more than one course of dexamethasone had higher requirements for CPAP support (aRR 1.400, 95% CI 1.098-1.785; P = 0.007), increased lung atelectasis (aRR 2.456, 95% CI 1.052-5.730; P = 0.038), and elev

Neonatal diagnosis of primary ciliary dyskinesia in a high consanguinity population: a single tertiary center experience.

Though PCD usually presents after birth in term neonates, diagnosing PCD during the neonatal and infancy stages is uncommon, particularly in children who do not exhibit laterality defects. We report our recent experience with the diagnosis of PCD in the neonatal and early infantile period in a highly consanguine population. This was achieved by implementing a novel genetic-based diagnostic approach based on direct testing for recognized regional genetic variants. We conducted a retrospective analysis of children diagnosed with PCD at Soroka University Medical Center during the neonatal or earl

Recognizing clinical features of primary ciliary dyskinesia in the perinatal period.

Primary ciliary dyskinesia (PCD) is a rare, motile ciliopathy inherited through mostly autosomal recessive variants that results in chronic ear, sinus, and respiratory disease. Despite neonatal respiratory distress being a common presenting symptom in term infants with PCD, the diagnosis is often delayed due to non-familiarity of neonatal caregivers with phenotypic and diagnostic features of this disease. Organ laterality defects, prenatal cerebral ventriculomegaly, and a family history of suppurative respiratory disease may occur in PCD and should prompt neonatal testing for this condition. I

Lung ultrasound-guided best positive end-expiratory pressure in neonatal anesthesia: a proposed randomized, controlled study.

BACKGROUND: Atelectasis is a common complication in neonatal anesthesia. Lung ultrasound (LUS) can be used intraoperatively to evaluate and recognize atelectatic lung areas. Hypotheses for the study are: (1) The use of LUS to guide choice of best positive end-expiratory pressure (PEEP) can lead to reduction of FiO2 to achieve same saturations of oxygen (SpO2). (2) In a less de-recruited lung, there will be less postoperative pulmonary complications. (3) Static respiratory system compliance could be different. (4) Hemodynamic parameters and amount of fluids infused or need for vasopressors intr

First reports of primary ciliary dyskinesia caused by a shared DNAH11 allele in Canadian Inuit.

BACKGROUND: Primary ciliary dyskinesia (PCD) is typically an autosomal recessive disease characterized by recurrent infections of the lower respiratory tract, frequent and severe otitis media, chronic rhinosinusitis, neonatal respiratory distress, and organ laterality defects. While severe lower respiratory tract infections and bronchiectasis are common in Inuit, PCD has not been recognized in this population. METHODS: We report a case series of seven Inuit patients with PCD identified by genetic testing in three Canadian PCD centers. RESULTS: Patients ranged from 4 to 59 years of age (at ti
