Validation of the French version of the Sleep Inertia Questionnaire (SIQ) and its assessment in sleep disorders.

BACKGROUND: Sleep inertia is a transient state of impaired cognitive and motor performance following awakening, particularly, but not exclusively, observed in central disorders of hypersomnolence (CDH). The Sleep Inertia Questionnaire (SIQ), a 22-item self-report scale assessing behavioral, physical, emotional, and cognitive dimensions, was developed in 2015 in English. OBJECTIVE: To translate and validate a French version of the SIQ and to evaluate its psychometric properties in patients with CDH or obstructive sleep apnea (OSA). METHODS: The SIQ was translated using a forward-backward proc

Ventilatory burden and blood pressure response to CPAP: A secondary analysis of the APPLES trial.

BACKGROUND: Continuous Positive Airway Pressure (CPAP) is the standard treatment of obstructive sleep apnea (OSA), but has a heterogeneous blood pressure (BP) lowering effect. We hypothesized that Ventilatory Burden (VB), a metric better reflecting the entire burden of pathological airflow, may predict CPAP-induced BP reduction. METHODS: We conducted a secondary analysis of the Apnea Positive Pressure Long-term Efficacy Study (APPLES) cohort. Participants were stratified by baseline VB (>20% breaths with < 50% amplitude) and OSA severity (AHI ≥ 30). Linear regression models were used to asses

Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Obstructive sleep apnoea severity and insulin resistance in children with Prader-Willi Syndrome: the SPISE index as a candidate metabolic marker.

UNLABELLED: We compared two surrogate markers of insulin sensitivity - the single-point insulin sensitivity estimator (SPISE) and HOMA-IR - and evaluated their associations with OSA severity in children with genetically confirmed PWS and a symptomatic non-syndromic referral group (NSR) with available polysomnography (PSG) and fasting biochemical data. In this single-centre retrospective cross-sectional study, OSA severity was indexed by the obstructive apnoea-hypopnoea index (oAHI), central AHI (cAHI) and oxygen desaturation index (ODI), and caregiver-reported symptoms by the PSQ-SRBD scale. Plasma p-tau217 and neurodegeneration-related biomarkers predict incident cognitive impairment in obstructive sleep apnea: a prospective cohort study.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with cognitive decline, but biomarkers for identifying cognitively unimpaired patients at increased future risk remain limited. We evaluated the prognostic value of plasma biomarkers associated with neuroaxonal injury and Alzheimer's disease-related biology in OSA. METHODS: In this prospective cohort study, 286 cognitively unimpaired adults with polysomnography-confirmed OSA underwent baseline measurement of plasma NfL, GFAP, p-tau181, p-tau217, Aβ42, and Aβ40 and were followed for incident cognitive impairment. Cox regression, predictiv

Exploring the Link Between Inflammatory Gene Variants and Phenotypes of Periodontal Disease and Obstructive Sleep Apnea.

Periodontal disease (PD) and obstructive sleep apnea (OSA) are prevalent chronic inflammatory conditions affecting over 50% of Colombians and a substantial proportion of the global population. This study investigated genetic associations between interleukin (IL) gene variants (IL-1β, IL-10, IL-17) and the risk of PD, OSA, or their coexistence in 129 Colombian individuals, including cases (PD, OSA, PD + OSA) and healthy controls. Genotyping was performed using PCR amplification and Sanger sequencing. Key findings revealed that IL-1β variants (rs1143627, rs16944) significantly increased OSA risk

Mandibular Advancement Devices With Remote Monitoring Technologies: Expanding the Dentist's Role in the Management of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a prevalent disorder with significant systemic and quality-of-life consequences. Mandibular advancement devices (MADs) are an established treatment option, but conventional follow-up often relies on intermittent visits and self-reported adherence. Sensor-integrated MADs and remote monitoring may enable objective adherence assessment and personalized longitudinal care, although their additional clinical value remains uncertain. This narrative review critically appraises current evidence on sensor-integrated MADs with remote monitoring for OSA, focusing on treatm

The Neurochemical Basis of High-Altitude Sleep Disruption: Disentangling Respiratory and Sleep-Circuit Mechanisms Toward Targeted Pharmacotherapy.

High-altitude sleep disruption is a complex syndrome that many lowlanders experience when they reach plateaus above 2,500 meters, characterized by frequent awakenings, reduced slow-wave sleep, nocturnal hypoxemia, and central sleep apnea related to unstable ventilation. A prevailing assumption in high-altitude sleep medicine holds that stabilising ventilation, chiefly via acetazolamide, is sufficient to restore sleep quality. This assumption is challenged by a randomised controlled trial demonstrating that acetazolamide significantly reduced the apnoea-hypopnoea index (AHI) and improved oxygen

The sleep EEG spectral profile of narcolepsy with comorbid obstructive sleep apnea more closely resembles narcolepsy than obstructive sleep apnea.

BACKGROUND: Narcolepsy (NT) and obstructive sleep apnea (OSA) frequently coexist, yet the sleep electroencephalographic (EEG) spectral profile of narcolepsy with comorbid OSA (NT-OSA) remains unclear. We compared spectral power among NT-OSA, NT, and OSA across polysomnography (PSG) and Multiple Sleep Latency Test (MSLT). METHODS: We retrospectively analyzed 224 treatment-naïve adults with excessive daytime sleepiness (37 NT-OSA, 77 NT, and 110 OSA); 39 subjectively sleepy controls (SSC) were included in a secondary analysis. Relative EEG spectral power was calculated in central and occipital

Dense temporal sleep apnea profiling from single-channel PPG with downstream AHI estimation: a large-scale retrospective feasibility study.

Sleep apnea (SA) diagnosis remains challenging due to the cost and limited accessibility of polysomnography. Most automated screening approaches estimate the Apnea-Hypopnea Index (AHI) from oximetry-derived features and do not provide temporally resolved characterization of respiratory events. This study investigates the feasibility of dense temporal respiratory event profiling and clinically meaningful SA severity estimation from single-channel photoplethysmography (PPG).Approach: A two-stage framework was developed using single-channel PPG. Comprehensive analysis of the single-cell and transcriptomics uncovers the pathogenic network and potential biomarkers related to hypoxia, metabolic reprogramming and atherosclerosis in obstructive sleep apnoea.

This study aimed to identify key biomarkers for obstructive sleep apnoea (OSA) by analysing RNA-seq (GSE75097: 6 control and 42 OSA) and single-cell RNA-seq (GSE214865: 11 control and 11 OSA) datasets. Differential expression analysis revealed 575 DEGs between OSA and control groups. Using weighted gene co-expression network analysis (WGCNA) combined with machine learning approaches (Random Forest, SVM-RFE and LASSO), we identified four hub genes (C17ORF91, HBEGF, PGBD2 and ZNF616) as candidate biomarkers, showing exploratory discriminatory performance (AUC > 0.7). Gene set enrichment analysis

Unsupervised Clustering Analysis of Snoring Identifies Acoustic Biomarkers for the Severity of Obstructive Sleep Apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent public health concern linked to cardiovascular, metabolic, and neurocognitive consequences. Although polysomnography (PSG) is the diagnostic standard, the availability can be limited with long wait time. Snoring is a common symptom of OSA, but all individuals with snoring do not have OSA. In this study, we hypothesized the characteristics of snoring may be used to detect OSA. We applied an unsupervised clustering method to classify snoring patterns and examine their correlations with OSA parameters. METHODS: We enrolled 287 adults with

Vitamin C mitigates early auditory cortical hyperexcitability and antioxidant imbalance induced by acute intermittent hypoxia.

To test whether brief intermittent hypoxia (IH) induces early hyperexcitability and antioxidant imbalance in the primary auditory cortex (Au1) of rats, and whether high-dose vitamin C (VC) pretreatment mitigates these effects. Adult male Sprague-Dawley rats were assigned to four groups (n = 6/group): control, IH, IH plus intraperitoneal normal saline (IH+IPNS), and IH plus intraperitoneal VC (IH+IPVC). Rats underwent a single 3 h IH protocol; VC (500 mg/kg, intraperitoneal injection) or saline was administered 30 min before IH. Three hours after IH, in vivo Au1 multiunit recordings quantified
