To assess the influence of gestational age (GA) and sleep states on respiratory pauses (RP), apnea index (AI) and periodic breathing (PB), we made polygraphic recordings on 40 normal newborns with GA of 31-41 weeks. Only 2 out of a total of 2,406 RP were obstructive. We never recorded greater than 15-second duration central RP. 10-15 s RP were rare, 2% of the total number observed in newborns with a GA of 31-38 weeks. Selective flattening of thoracic respiratory movements (while abdominal and nasal respirations persisted) were observed in all age-groups. In all GA groups, RP frequency, AI and

Respiratory pauses (3-10 sec in duration), apnea (less than 10 sex in duration), and periodic respiration observed in thoracic respirograms were measured in 226 polysomnograms obtained on 17 normal infants during the first year of life. All subjects had one or more respiratory pauses in a majority of recordings; 35% had respiratory pauses in all recordings; 75% of respiratory pauses were associated with body movement. There is marked intersubject and even intrasubject variability in respiratory pause rates. The range of mean respiratory pause rates among subjects was 2.0 - 14.4/h, and for sing

Respiratory control abnormalities may result in cor pulmonale. This report summarizes the clinical history, diagnostic evaluation, treatment, and outcome of 16 infants and children presenting with cor pulmonale subsequently found to be due to sleep-dependent hypoventilation. Eleven patients had cardiomegaly and electrocardiographic evidence of right ventricular hypertrophy (RVH) while 5 had only severe RVH or biventricular hypertrophy (BVH). Four infants with central hypoventilation syndrome (CHS)--absence of sleep-related ventilatory drive--had severe sleep-dependent asphyxia and resultant ac

The incidence of sleep-related breathing disorders (SRBDs) associated with hemoglobin desaturation was determined by nocturnal polygraphic evaluations in 26 healthy men, aged 55-70 years. Sixteen subjects (62%) had abnormal rates of at least 12 episodes per hour of sleep: 8 had occlusive, and 8 had central apnea or hypopnea. During waking ten of 16 SRBD subjects and only one subject without SRBDs exhibited either an elevated nasopharyngeal airway resistance (n = 4) or a reduced ventilatory response to hypercapnia (n = 4) and/or hypoxia (n = 3). However, these abnormalities were not related to

Thirteen patients with sleep apnea syndrome, nine with narcolepsy, and age-matched controls were studied to evaluate possible impairment of autonomic nervous control of cardiovascular and pulmonary function. The sleep apnea group had subnormal increases in heart rate and blood flow in the resting arm upon muscle contraction, although they were higher than seen in the narcolepsy group. Some sleep apnea patients had marked bradycardia in response to a dive reflex test. Other cardiovascular results did not differ from controls. Some sleep apnea patients had low ventilatory response to CO2. One ha

Physiologists as well as chest physicians are both interested in respiratory control and disturbances during sleep. Many studies have demonstrated episodic falls in oxygen saturation in lung disease during sleep related to episodic alveolar hypoventilation and/or apneas. A better understanding of these sleep-induced disorders has been possible since the use of electroencephalo-and oculogram (EEG-EOG) to separate the two stages of sleep i.e rapid eye movement sleep (REM sleep) and slow wave sleep (non-REM sleep), and of non invasive methods in assessment of respiratory parameters. Several mecha

The sleep apnea syndromes have been recognized clinically in the United States only within the past ten years. The true extent of the problem is not known, but it seems certain that these syndromes are much more common than was generally assumed five years ago. Every clinician should be aware of the signs and symptoms of sleep apnea because of the rapid and prompt response to therapeutic measures. Sleep apnea syndromes, whether obstructive or central, can result in systemic or pulmonary hypertension, arterial blood gas abnormalities, life-threatening cardiac arrhythmias, chronic respiratory fa

Sleep apnea occurring in association with neurologic lesions is usually of the central type. We have described a patient in whom obstructive sleep apnea followed a lateral medullary syndrome. We hypothesize that the upper airway obstruction during sleep was due to weakness of the palatal and pharyngeal muscles resulting from the neurologic lesion. The patient initially had reduced respiratory center motor output (mouth occlusion pressure) which increased significantly after tracheostomy. We conclude that obstructive as well as central apnea may follow neurologic lesions. Each patient should be

Insomnia, daytime sleepiness, and nocturnal wandering, so common in the elderly, are caused largely by two specific pathophysiologic processes. Sleep apnea is a condition where respiration pauses during sleep, leading to arousal. Sleep apnea is due either to obstruction in the throat or failure of the central respiratory center. Periodic movements in sleep are characterized by frequent ankle and leg flexions, leading to arousal. Sleep apnea and periodic movements in sleep require specific diagnoses and treatments. Each process occurs in 20%-30% of people over 65, and perhaps the majority of ol

1. The changes in heart rate surrounding a period of apnoeic asphyxia, produced by either central or obstructive sleep apnoea, are characteristically a progressive bradycardia during the apnoeic period followed by an abrupt reversal to tachycardia with resumption of lung inflation. 2. In a subject who had undergone bilateral pulmonary vagotomy with sparing of the cardiac branches on the left side, the bradycardia is still present during the apnoeic period; however, there is no abrupt response to lung inflation. 3. A subject who had undergone unilateral right-sided pulmonary vagotomy demonstrat

Eight adult patients with sleep apnea were studied to evaluate the ventilatory and cardiac effects of hyperoxia on an acute basis. Five patients then used low-flow nighttime oxygen for 30 to 90 days. The results of the acute study showed that for 30-min study periods, the total number of apneas and per cent apnea time (duration of apneas divided by sleep time) decreased significantly from the room air to the oxygen period (207 to 68, p less than 0.05; 41.1 +/- 18.3% SD to 20.5 +/- 14.4%, p less than 0.05, respectively). Also, the apnea-associated slowing in heart rate is blocked by the supplem

A total of 75 polysomnograms recorded during night sessions in 25 control subjects, 25 siblings of sudden infant death syndrome victims, and 25 near miss infants were studied in order to correlate the duration of central apnea to the decrease in transcutaneous oxygen pressure (tcPO2). A total of 1,650 central apneas were recorded. The three groups of infants presented the same tendency for a lower tcPO2 in indeterminate and active sleep as in quiet sleep (P greater than .10). The apneas were not preceded by a decrease in tcPO2, but were followed by a decrease in tcPO2, directly proportional to

Documented observations of a 5-week-old infant during a "near miss" for a Sudden Infant Death Syndrome (SIDS) episode by a physician were carried out during an in-hospital physiological recording of respiratory and cardiac activity. This "near miss" event occurred during quiet sleep and was characterized by a prolonged apneic attack with marked bradycardia, cyanosis and limpness which required immediate vigorous resuscitative efforts by a physician and trained nurse. Parental descriptions of similar events parallel these documented sudden unexpected changes in cardiorespiratory parameters. Obj

Impedance respiratory monitoring is the standard technique for detecting apnea in infants. A miniature microphone/amplifier system has been used to sense breath sounds over the trachea in order to detect both central and obstructive apnea episodes in infants and children. The tracheal microphone has several advantages over other devices that sense airflow (thermistors, CO2 sampling catheters) because it is not easily dislodged during restless sleep. This simple device may be used in monitoring of neonates and infants; it may eventually replace impedance monitoring.

To determine the characteristics of and mechanisms causing the bradycardia during sleep apnea (SA), both patients with SA and normals were studied. Evaluation of six consecutive SA patients demonstrated that bradycardia occurred during 95% of all apneas (central, obstructive, and mixed) and became marked with increased apnea length (P less than 0.01) and increased oxyhemoglobin desaturation (P less than 0.01). Heart rate slowed 9.5 beats per minute (bpm) during apneas of 10-19 s in duration, 11.4 bpm during 20-39s apneas, and 16.6 bpm during 40-59-s apneas. Sleep stage had no effect unexplaine

A 58-yr-old man with primary alveolar hypoventilation, central sleep apnea, and secondary polycythemia failed to improve when treated with respiratory stimulant medications, including oxtriphylline, acetazolamide, and medroxyprogesterone. In contrast, after institution of treatment with low-flow nocturnal oxygen, there was a marked decrease in the number and duration of sleep apneas, and an increase in the level of ventilation during sleep. These changes were sustained during 5 months of nocturnal oxygen therapy. The improvement produced by oxygen may have been due to the fact that the patient

Three- to 4-hour polygraphic sleep studies were carried out in 16 infants aged between 1 and 6 months during and after recovery from acute bronchiolitis. During bronchiolitis 35% of total sleep time was active sleep compared with 31% after recovery. Respiration rate was increased during bronchiolitis and was higher in active sleep and quiet sleep irrespective of the stage of the illness. Apnoeic pauses were invariably shorter than 15 seconds, the mean duration for active sleep and quiet sleep being similar during infection and after recovery. Apnoeic episodes were central in type and generally

In order to determine whether reversible alterations in ventilatory control are found in obstructive sleep apnea syndromes, 5 male patients 31 to 57 yr of age presenting with the syndrome were polygraphically monitored before and several times during the 6 months after tracheostomy. They also had a hypercapnic ventilatory response study before surgery and again 3 months after surgery during the daytime. A dramatic decrease in overall apnea index was seen immediately after surgery, but the number of central apneas and the central apnea index did not reach low values until several months after t

A 17-year-old man presented with daytime sleepiness, episodic attacks of sleep and probable cataplexy. His EEG showed rapid eye movements and central sawtooth waves at sleep onset, and supported the clinical impression of narcolepsy. He improved with methylphenidate but died suddenly, and had cardiomegaly, right ventricular enlargement, and pulmonary hypertension at autopsy. These findings suggested concomitant features of sleep apnea which were not evident by history or examination.Central apneas have been frequently described in the sleep of narcoleptic patients. Few patients have had indica
