Lung function and sleep-disordered breathing in preschoolers born very preterm.

OBJECTIVE: To determine the prevalence of abnormalities in small airway function and sleep-disordered breathing in preschoolers born very preterm and with very low birth weight, and their association with bronchopulmonary dysplasia and other neonatal risk factors. METHODS: Cross-sectional study of children aged 3 to 6 years born before 32 weeks of gestation with a birth weight of less than 1500 g. Respiratory oscillometry was performed, determining the resistance and reactance at 5 Hz (R5 and X5), the difference in resistance at 5 and 19 Hz (R5-19), area under the reactance curve (AX), and th

Large Airway and Lung Findings in Symptomatic Children and Young Adults With Down Syndrome.

BACKGROUND AND OBJECTIVES: Epidemiology studies report pulmonary disease, lung infection, and pneumonia as the largest causes of morbidity and mortality in individuals with Down syndrome (DS), but how significant airway and lung diagnoses contribute to morbidity remains unknown. Our goal was to evaluate the presence and presentation of structural lung abnormalities from chest computed tomography (CT) scans in this population. METHODS: Retrospective review of chest CT scans performed clinically between 2012 and 2023 merged with clinical data was conducted on individuals with DS (birth-22 years

Ketamine Procedural Sedation in 38,910 Children: Frequency and Predictors of Critical and High-Risk Events.

OBJECTIVES: Ketamine is a commonly administered sedative to facilitate painful procedures in children, and recommendations for its optimal practice require accurate estimates of the frequency and nature of adverse events. Our objective was to assess the frequency of critical and high-risk adverse events associated with ketamine as the primary sedative, and to identify clinical predictors of such adverse events. DESIGN: Retrospective analysis of a prospective registry of 38,910 children receiving ketamine for procedural sedation from July 1, 2004, to July 1, 2024. SETTING: Critical care units

The utility of caffeine citrate as a neuroprotectant in the early life of premature newborns: a literature review of the effects on neurodevelopmental outcomes.

The incidence of prematurity-related complications has decreased due to advancements in medical preventive and supportive measures, but the rate of neurodevelopmental impairment secondary to prematurity continues to increase. Due to the high metabolic demand of the central nervous system during early development and the age-related sensitivity of the cerebral white matter, perinatal intermittent hypoxia can lead to significant cerebral pathology persisting into adulthood. Caffeine citrate is one of the mainstay medical treatments for apnea of prematurity and is widely used in many neonatal int

Successful Respiratory Management Using Synchronized Nasal Intermittent Positive Pressure Ventilation for Abnormal Breath Patterns Associated With Joubert Syndrome.

Patients diagnosed with Joubert syndrome (JBS) frequently present with differing respiratory irregularities, including tachypnea followed by apnea, particularly during infancy. Consequently, it is essential to adopt respiratory support strategies tailored to atypical breathing patterns associated with high mortalities. Herein, we report a case of successful respiratory management of abnormal breathing patterns associated with JBS based on synchronized nasal intermittent positive pressure ventilation (SNIPPV) using the MEdiTRIG (Medin Medical Innovations GmbH, Puchheim, Germany) pressure trigge

Dexamethasone dosage and course effects on respiratory outcomes in preterm twins: retrospective cohort study.

This retrospective cohort study evaluated the effects of complete, incomplete, and multiple courses of antenatal corticosteroids (ACS) on respiratory outcomes in twin preterm infants born between 28 + 0 and 36 + 6 weeks of gestation. A total of 1,115 neonates were analyzed. Primary outcomes included respiratory distress syndrome (RDS) and other adverse respiratory conditions. Infants exposed to more than one course of dexamethasone had higher requirements for CPAP support (aRR 1.400, 95% CI 1.098-1.785; P = 0.007), increased lung atelectasis (aRR 2.456, 95% CI 1.052-5.730; P = 0.038), and elev

All-cause mortality and infection-related outcomes of hospital-initiated kangaroo care versus conventional neonatal care for low-birthweight infants: a systematic review and meta-analysis.

BACKGROUND: Kangaroo care has a well-established role in preterm infant stabilisation and in protecting low-birthweight newborns from mortality. Yet kangaroo care is far from fully embedded in conventional inpatient neonatal care practice. The evidence on infection outcomes of hospital-initiated kangaroo care is unclear. We aimed to evaluate the existing evidence to understand the role of hospital-initiated kangaroo care in preventing mortality, sepsis, and invasive infection in low-birthweight infants. METHODS: In this systematic review and meta-analysis, we searched Embase, MEDLINE, Cochran

Sustained versus standard inflations during neonatal resuscitation to prevent mortality and improve respiratory outcomes.

RATIONALE: At birth, infants' lungs are fluid-filled. For newborns to have a successful transition, this fluid must be replaced by air to enable gas exchange. Some infants are judged to have inadequate breathing at birth and are resuscitated with positive pressure ventilation (PPV). Giving a sustained lung inflation (SLI) at the start of PPV may help clear lung fluid and establish gas volume within the lungs. This is a review update; the last version was published in 2020. OBJECTIVES: To assess the benefits and harms of an initial SLI (> 1 second duration) versus standard inflations (≤ 1 seco

Continuous positive airway pressure versus methylxanthine for apnoea in preterm infants.

RATIONALE: Recurrent apnoea is common in preterm infants, particularly at very early gestational ages. These episodes of ineffective breathing can lead to hypoxaemia and bradycardia, sometimes severe enough to require resuscitation, including positive pressure ventilation. Various interventions have been used to manage apnoea of prematurity, including methylxanthines and continuous positive airway pressure (CPAP). However, CPAP and methylxanthines remain the most widely studied and utilised treatments due to their greater benefits and lesser harms compared to alternatives like CO₂ inhalation.

[Preexisting diabetes: Expert consensus from the College of French Gynecologists and Obstetricians and from the French Society of Diabetology].

In France, 0.2% of women who gave birth in 2021 had type 1 diabetes, and 0.3% had type 2 diabetes. Regarding preconception care, it is recommended that women with any type of diabetes achieve an HbA1c level of less than 6.5%. For women using continuous glucose monitoring (CGM), the recommended target range is 0.70-1.80g/L (3.9-10mmol/L), and it is recommended to achieve this range at least 70% of the time. The preconception assessment includes: 1) an HbA1c measurement, 2) an assessment of microangiopathic impact, 3) an assessment of macroangiopathic complications, 4) screening for associated c

Fifteen-minute consultation: A guide to pertussis.

Pertussis (whooping cough) is a bacterial infection caused by the organism Bordetella pertussis It can lead to severe illness and death, especially in neonates and young infants. Pre-vaccination, pertussis was a major contributor to childhood morbidity and mortality. Despite global vaccination programmes, it remains a considerable public health challenge. Infants at high risk of infection presenting with cough and/or apnoea with a peripheral hyperleucocytosis due to lymphocytosis should be investigated and managed with caution. It is important for clinicians to recognise the risk of severe dis

Prophylactic Oropharyngeal Surfactant for Preterm Newborns at Birth: A Randomized Clinical Trial.

IMPORTANCE: Preterm newborns at risk of respiratory distress syndrome are supported with continuous positive airway pressure (CPAP). Many newborns worsen despite CPAP and are intubated for surfactant administration, an effective therapy for treatment of respiratory distress syndrome. Endotracheal intubation is associated with adverse effects. Pharyngeal administration of surfactant to preterm animals and humans has been reported as an alternative. OBJECTIVE: To assess whether giving prophylactic oropharyngeal surfactant to preterm newborns at birth would reduce the rate of intubation for resp

A Quality Improvement Initiative to Increase Skin-to-Skin Care Duration in Preterm Neonates.

BACKGROUND: Skin-to-skin (STS) care effectively improves neonatal outcomes, particularly for preterm neonates. However, utilization of STS remains suboptimal for the most vulnerable preterm neonates in the first 4 weeks of life. This quality improvement (QI) project aimed to increase STS duration for neonates under 35 weeks gestation. METHODS: The QI initiative was conducted in a 35-bed level IV NICU within a teaching hospital in New York City from July 2021 to January 2023. Six months of baseline data and a staff survey determined interventions across "Plan, Do, Study, Act" cycles. Interim a
