Congenital Heart Block.

Congenital heart block (CHB) is a rare fetal arrhythmia resulting from disruptions in the heart's electrical conduction system, often linked to maternal autoantibodies or congenital heart defects. This article examines the clinical presentation, pathophysiology, and diagnostic strategies of CHB, emphasizing advances in fetal echocardiography, maternal hydroxychloroquine prophylaxis, and multimodal therapies for refractory cases. Management options, including corticosteroids, intravenous immunoglobulin, and pacemaker implantation, are discussed alongside emerging technologies and pacing systems

Long-Term Outcomes After Interventions for Infants with Congenital Heart Defects.

Congenital heart defects (CHD) affect approximately 1 of every 100 live births in the United States. Advances in surgical and transcatheter interventions since the 1970s have markedly improved survival, with over 85% of affected infants now reaching adulthood. Clinical registries have enabled crucial insights into peri-operative and long-term outcomes. While survival has improved, patients with history of CHD interventions often face comorbidities, such as heart failure, arrhythmias, and neurodevelopmental challenges. Lifelong surveillance is essential, as most interventions do not constitute

Risk Factors and Causes of Readmission to the Pediatric Intensive Care Unit After Cardiac Surgery.

IntroductionUnplanned readmissions to the pediatric intensive care unit (PICU) following cardiac surgery remain poorly understood. Such readmissions are associated with increased morbidity and present significant challenges to healthcare. This study aimed to identify characteristics associated with unplanned PICU readmission during the same hospitalization following cardiac surgery.MethodWe conducted a retrospective analysis using prospectively collected data from a tertiary care children's hospital from January 2015 to January 2021. All pediatric patients who underwent cardiac surgery and wer

Rare case of respiratory syncytial virus induced myocarditis in a neonate: A case report.

BACKGROUND: Respiratory syncytial virus (RSV) is a leading cause of lower respiratory tract infections in neonates. While typically associated with bronchiolitis and pneumonia, RSV can rarely cause extrapulmonary complications such as myocarditis, which may present with life-threatening symptoms if not promptly recognized. CASE SUMMARY: We describe the case of a 26-day-old male neonate who presented with respiratory distress, poor feeding, and irritability. Initial evaluation revealed an RSV infection confirmed via nasopharyngeal swab. As the clinical course progressed, the infant developed c

Swim Exercise Mitigates BCAA-Induced Atrial Remodeling and AF Susceptibility via Inhibition of Bax-Mediated Mitochondrial Apoptosis.

Atrial fibrillation (AF), the most prevalent sustained cardiac arrhythmia, has been shown in our prior work to be mitigated by swim exercise. While accumulating evidence links elevated circulating branched-chain amino acids (BCAA) to cardiovascular disease, their specific involvement in AF pathogenesis and the anti-arrhythmic benefits conferred by exercise remain less understood. In this study, we investigated the effects of BCAA supplementation on AF susceptibility and the potential cardioprotective benefits of swim exercise. C57BL/6J mice were supplemented with 2% BCAA via drinking water, ei

Approach to prolonged QT interval in paediatric and neonatal patients.

Congenital Long QT Syndrome (LQTS) is a potentially life-threatening cardiac channelopathy and an important cause of sudden cardiac death in young individuals. It is characterised by delayed myocardial repolarization, seen as a prolonged QT interval on a standard 12-lead electrocardiogram. This prolongation increases the risk of ventricular arrhythmias, particularly Torsades de Pointes, which may result in syncope, cardiac arrest, or sudden death, often secondary to physical or emotional stress. In infants and children, the upper limit of normal for a heart rate-corrected QT (QTc) interval is

Thyrotropin Directly Affects Cardiac Electrophysiology and Is Associated With AF Prevalence.

BACKGROUND: Although hyperthyroidism is known to increase the risk of atrial fibrillation (AF), subclinical hypothyroidism (SH) is an often-underreported condition characterized by elevated thyroid-stimulating hormone (TSH) levels and normal free triiodothyronine/free thyroxine (fT3/fT4) levels. This study aimed to clarify the association between SH and AF and to identify potential direct electrophysiological effects of TSH. METHODS: We retrospectively included 2311 patients diagnosed with SH between 2007 and 2020 who had an ECG within 7 days of diagnosis. Logistic regression analysis identif

Risk factors of early mortality within 72 hours of birth in neonates born at 22 23 weeks' gestation.

BACKGROUND: The survival rates of neonates are significantly influenced by gestational age, with further differences observed internationally. Survival rates for live births at 22 and 23 weeks of gestation range from 3.7% to 56.7% and from 20.0% to 79.3%, respectively. Despite advancements in neonatal care, the mortality rate in these preterm infants remains high, and the factors influencing mortality remain unclear. In this study, we conducted a comparative analysis of the risk of death within 72 h of birth in neonates born at 22 and 23 weeks' gestation. METHODS: This single-center, retrospe

Cardiac rhabdomyomas in tuberous sclerosis complex: clinical manifestations and genotype correlations.

BACKGROUND: Tuberous sclerosis complex (TSC) is an autosomal dominant genetic disorder characterized by hamartoma formation in multiple organs, including the heart. Cardiac rhabdomyomas (CRs) are the most common cardiac tumors in infancy and occur in up to 80% of individuals with TSC. While CRs often regress spontaneously, their clinical impact varies, particularly concerning arrhythmias and obstructive complications. This study aimed to assess the incidence, clinical manifestations, and regression rates of CRs in infants and children with TSC, with a focus on genotype-phenotype correlations.

Ivabradine in the treatment of congenital junctional ectopic tachycardia: a case report and review of the literature.

Congenital junctional ectopic tachycardia is a rare arrhythmia that poses significant management challenges. This report presents a case of neonatal-onset congenital junctional ectopic tachycardia treated with cedilanid, amiodarone, and propafenone but persisted in episodes. Sinus rhythm was restored following the initiation of ivabradine therapy. The review of the literature indicates that ivabradine demonstrates efficacy in the treatment of paediatric junctional ectopic tachycardia, particularly in refractory cases, without significant side effects. These findings suggest that ivabradine has

Hydroxychloroquine After Complete Heart Block Diagnosis and Relationship to Patient Outcomes: A Descriptive Cohort Study.

OBJECTIVE: Congenital complete heart block (CHB) carries significant morbidity and mortality. Maternal Hydroxychloroquine may prevent CHB in subsequent pregnancies. No studies have explored the benefit of Hydroxychloroquine initiated after the diagnosis of CHB. We hypothesized fetuses exposed to Hydroxychloroquine, even after CHB diagnosis, will have improved outcomes using time to pacemaker placement as the primary outcome. METHODS: A single-center retrospective study was conducted on patients with CHB secondary to maternal anti-SSA antibodies approximately 2005-2024. RESULTS: All patients

Ivabradine Use for Arrhythmias in Children.

BACKGROUND: Ivabradine is a hyperpolarization-activated cyclic nucleotide-gated channel blocker that has an off-label indication for arrhythmias in pediatric patients. OBJECTIVES: The primary objective was identification of the line of therapy and type of arrhythmia for which ivabradine was initiated. The secondary objective was to evaluate the initial and peak ivabradine doses, tolerance in partially fed patients (i.e., patients not receiving full feeds at the time of ivabradine initiation), number of patients continued home on ivabradine, concomitant drug interactions with a class C, D, or

ECG artefact or life-threatening arrhythmia? A neonatal presentation of Long QT syndrome type 3 with a de novo SCN5A mutation.

Electrocardiogram interpretation is critical in neonatal care, as artefacts may mimic life-threatening arrhythmias. We report the case of a preterm newborn girl, delivered at 31 weeks of gestation due to acute fetal distress, who developed tachycardia shortly after birth. The initial ECG showed chaotic electrical activity, initially raising suspicion of artifact; however, a subsequent tracing confirmed a life-threatening arrhythmia with wide QRS complexes, prolonged QTc, and torsades de pointes. As the patient remained haemodynamically stable, intravenous lidocaine and magnesium sulphate were

Fetal cardiac rhabdomyomas susceptible to prenatal treatment with mTOR inhibitors: literature review and proposal of a prenatal management algorithm.

Certain types of fetal cardiac rhabdomyomas can lead to severe complications, including intrauterine death, yet no specific criteria have been established for the prenatal use of pharmacological therapies to mitigate the impact of rhabdomyomas. We conducted a narrative review of case reports and case series published between January 1, 2000, and February 28, 2025, identified through PubMed, Scopus, Web of Science, and Google Scholar, describing the prenatal use of mammalian target of rapamycin inhibitors in this context. Thirteen studies reporting on 15 fetuses were included. Five fetuses (33.

Arrhythmias in Pediatric Age: A Narrative Review.

Cardiac arrhythmias in the pediatric population represent both diagnostic and therapeutic challenges due to their heterogeneous etiologies, age-dependent clinical presentations, and the variety of management strategies required. Although many electrophysiological mechanisms are shared with adults, several arrhythmias are unique to infancy and adolescence or are associated with congenital heart disease and inherited channelopathies. Clinical presentation varies considerably with age and may be unclear or nonspecific, especially in neonates and infants. In some cases, arrhythmias are benign and

Assessing Oxygenation Instability in Premature Infants <1,500 Grams: Nursing Charts versus SpO2 Histograms-Is Two Better Than One?

OBJECTIVE: Despite availability of advanced monitoring tools, most neonatologists still primarily rely on nursing documentation of desaturation and bradycardia events to assess the respiratory status of very low birth weight (VLBW) premature infants. We aimed to compare oxygenation instability as recorded in nursing charts versus SpO2 histograms in VLBW infants during their first weeks of life. STUDY DESIGN: An observational study including VLBW premature infants who required respiratory support on day 1 of life. We recorded the daily number of desaturation events <90% from nursing charts and

Tachyarrhythmia-induced cardiomyopathy in a neonate.

Tachycardia-induced cardiomyopathy (TIC) can result in both systolic and/or diastolic ventricular dysfunction as a result of the prolonged fast heart rate which is reversible on controlling the fast heart rate or arrhythmia. The exact heart rate that can lead to this is not clear; however, a heart rate >180, in general, needs attention in neonates and infants. TIC is a well-established cause of left ventricular dysfunction which usually happens due to an increased atrial or ventricular rate. The incidence of TIC is very low, although the exact incidence is unclear. It should be considered in a

Refractory electrical cardiac storm during twin pregnancy delivery: A multidisciplinary clinical challenge.

Electrical storm (ES) is a rare and life-threatening cardiac emergency characterized by recurrent ventricular arrhythmias, posing unique challenges when it occurs during pregnancy. We report the case of a 28-year-old woman presenting with refractory ES in the late stages of a dichorionic-diamniotic twin pregnancy. Initial management included emergent cesarean delivery under general anesthesia due to maternal hemodynamic instability and fetal bradycardia. Despite pharmacological stabilization and multiple synchronized cardioversions, the arrhythmia persisted, necessitating catheter ablation to
