Paediatric Cardiac Tumours: A National Population Study.

Paediatric cardiac tumours are rare. The most common tumour is a rhabdomyoma, a benign tumour of the myocardium associated with Tuberous Sclerosis Complex (TSC), a rare genetic condition caused by constitutional pathogenic variants in either the TSC1 or TSC2 genes. Although benign, complications related to obstructed flow through the heart or intractable arrhythmias occur. A 23-year retrospective study of patients referred to the National Scottish Paediatric Cardiology service with evidence of a cardiac tumour. 51 patients identified; 12 prenatally, 8 live born. Of the 47 patients born alive,

[Rhythm therapy for congenital heart defects with implantable electronic devices].

Patients with congenital heart defects have an increased indication rate for pacemaker systems and implantable cardioverter defibrillators (ICD), which however are not designed for children. Therefore, the combination of selected materials is essential for long-term and safe therapy. Epimyocardial systems are the best option for neonates and infants, as well as patients with Fontan circulation who do not have systemic venous access to the heart. For most other patients, transvenous systems are advantageous because the leads and batteries last longer and are easier to adjust as the patient grow

The maternal exposure of digoxin and flecainide in relation to the safety and effectiveness in the treatment of non-hydropic fetal tachycardia.

BACKGROUND: Sustained fetal tachycardia requires transplacental anti-arrhythmic therapy. Little is known about the dose-concentration-effect correlation and safety in the mother, fetus, and newborn. OBJECTIVE: This study evaluates the relationship between maternal dose of digoxin and flecainide therapy for fetal tachycardia, maternal and umbilical cord concentrations, and side effects. METHODS: This was a retrospective case series. We included 28 pregnant women initially treated with digoxin monotherapy for fetal tachycardia between June 2007 and January 2023. The main end point was the corr

Holter Monitor Rhythm Parameters in Healthy Infants, Children, and Adolescents: Defining Reference Limits With Meta-Analysis.

BACKGROUND: Ambulatory electrocardiography has been in clinical use for 5 decades. However, reference limits for rhythm parameters in healthy infants, children, and adolescents have not been adequately defined. We sought to determine these reference ranges using meta-analysis of existing published studies of 24-hour Holter monitoring in healthy pediatric populations. METHODS: Multiple literature databases were searched from 1969 to May 2024 for relevant studies. Data extraction and analysis were completed according to the Meta-Analysis of Observational Studies in Epidemiology guidelines. The

Prone Positioning Was Associated With Less Hypoxemic Events and Improved Feeding Tolerance in Preterm Infants.

AIM: Cardiorespiratory events such as apnea, bradycardia and hypoxemia are common in preterm infants and may contribute to an impaired neurodevelopmental outcome. We hypothesised a significant reduction in the incidence of hypoxemia, bradycardia and gastric residuals in the prone position. METHODS: In this monocentric, prospective, randomised, two-arm crossover trial, at a tertiary university hospital, 48 preterm infants with a gestational age of < 32 weeks and a postmenstrual age of < 34 weeks were randomised to either a prone-supine or a supine-prone sequence of cardiorespiratory monitoring

Demographic and Clinical Characteristics of Children with Primary Cardiac Tumors Hospitalized from 1997 to 2019 in the United States.

Cardiac tumors in children, although rare, can lead to obstruction of valves or cardiac conduction problems. Large multi-site or national data on pediatric cardiac tumors is lacking. The objective of this study was to determine the prevalence of primary cardiac tumors in hospitalized children from 1997 to 2019 in the US and analyze the demographic and clinical characteristics of children with cardiac tumors. We conducted a cross-sectional analysis using the Healthcare Cost and Utilization Project's Kid's Inpatient Database from 1997 to 2019. Children with cardiac tumors were identified using I

Congenital Complete Heart Block Requiring Temporary Pacemaker Placement at Birth: A Case Report.

Congenital heart block (CHB) is a rare congenital cardiac disease where disruption of the atrioventricular (AV) node results in cardiac conduction abnormalities. The majority of CHB cases are associated with neonatal lupus antibodies that are associated with autoimmune destruction and fibrosis of the AV node, most commonly resulting in a congenital complete heart block (CCHB). We report the case of a 23-year-old mother who was found to have a fetus with significant bradycardia at a 21-week anatomy ultrasound. Subsequent fetal echocardiograms demonstrated a third-degree heart block, and laborat

The Removal of Intravascular Foreign Bodies by Intervention in Pediatrics.

INTRODUCTION: The use of intravascular catheters has become increasingly widespread in children, due to their use in diagnostic procedures (such as coronary, intracardiac, cerebral, and renal angiography, as well as pressure monitoring) and for therapeutic purposes (including angioplasties, valvuloplasties, congenital defect closure, chemotherapy, among other uses). However, they are not free from complications, which may include catheter fracture and migration within the cardiovascular system, potentially leading to vascular or cavity perforation, arrhythmias, and even death. OBJECTIVE: To d

Peripartum antihypertensive use and the risk of neonatal hypoglycemia and postpartum uterine atony: a retrospective cohort study.

OBJECTIVES: Determine whether peripartum antihypertensive use increases secondary uterotonic use, neonatal hypoglycemia, bradycardia, or NICU admission. Hypertensive disorders of pregnancy are common and increasing. Treatment includes beta- and calcium channel- blocking medications. Outpatient beta-blockers were associated with increases in neonatal bradycardia, hypoglycemia, and NICU admissions. Calcium channel blockers are weak tocolytics and may precipitate uterine atony and hemorrhage. STUDY DESIGN: Retrospective, single-center study at a tertiary academic hospital. Maternal cardiac arrhythmia in congenital heart disease: identifying high-risk pregnancy indicators.

BACKGROUND: The onset of arrhythmia during pregnancy has been noted as a significant issue in clinical management of adult congenital heart disease. The authors sought to examine the actual incidence of arrhythmias in CHD patients during pregnancy, as well as to distinguish prognostic predictors of arrhythmia in CHD pregnant patients, specifically focusing on the predictive value of echocardiographic parameters. METHODS: This retrospective study involved 244 pregnant patients with CHD in a tertiary hospital. Baseline characteristics and clinical presentation were systemically compared between

Novel Role of Human Epicardial Adipocyte-Derived SPARCL1 in Postoperative Atrial Fibrillation Following Cardiovascular Surgery.

BACKGROUND: Postoperative atrial fibrillation (POAF) is associated with short- and long-term morbidity and mortality. However, the ability of the epicardial adipocyte to regulate POAF remains poorly understood. OBJECTIVES: The authors investigated the possible role of human epicardial adipocyte-derived adipokines in the regulation of the myocardial redox state and POAF. METHODS: The authors prospectively studied 274 patients who underwent scheduled open-heart surgery. Finally, 149 patients without known atrial fibrillation were enrolled and divided into POAF and non-POAF groups. Epicardial a

Proteomics reveals crosstalk between calcium disturbance and multiple stresses facilitating lethal ventricular arrhythmias-sudden cardiac death under the co-morbidity of acute myocardial ischemia and type II diabetes mellitus.

AIMS: Diabetes mellitus (DM) often co-exists with myocardial ischemia (MI). Whether DM increases sudden cardiac death (SCD) susceptibility in early MI remains unclear, and the underlying mechanism needs to be further explored. MATERIALS AND METHODS: We induced type II DM in mice to produce a co-morbidity model of DM and MI. Proteomic analysis was conducted to identify proteomic features associated with DM-SCD and MI. Additionally, a neonatal rat ventricular myocyte (NRVM) model was established under conditions of hypoxia and high glucose (HG) to elucidate the interplay between oxidative stres

Ivabradine Use in Premature Infants With Accelerated Idioventricular Rhythm.

INTRODUCTION: Ivabradine has been recently introduced to treat focal atrial tachycardia and junctional ectopic tachycardia in children. Its use for treating ventricular tachyarrhythmias, specifically accelerated idioventricular rhythm (AIVR), remains limited. METHODS AND RESULTS: We report two cases of premature infants with AIVR successfully treated with ivabradine as monotherapy after other antiarrhythmic agents had failed. The first infant had a structurally normal heart and the second infant a truncus arteriosus. In both patients, sinus rhythm was restored with ivabradine. Side effects on

Safety and Feasibility of Implanting Dual-Chamber Permanent Pacemakers in Patients Under 3 kg.

Purpose: Permanent pacing in the young pediatric population can be challenging, especially in small neonates. We describe our institutional experience with the surgical placement of dual-chamber permanent pacemaker (PPM) systems in neonates weighing <3 kg at the time of implantation. Methods: The Cincinnati Children's Hospital database was used to identify patients who had an epicardial dual-chamber PPM and weighed <3 kg at implantation from 2000 to 2022. Descriptive analyses focused on surgical techniques, postimplant complications, and survival. Results: Twenty patients underwent dual-chambe

Sudden Unexpected Infant and Perinatal Death: Pathological Findings of the Cardiac Conduction System.

Objective: Sudden infant death syndrome (SIDS), sudden neonatal unexpected death (SNUD), and sudden intrauterine unexpected death (SIUD) are major unsolved, shocking forms of death that occur frequently and without warning. The body of literature on the anatomo-pathological substrates in the cardiac conduction system of SIDS-SIUD and their possible relationship with risk factors and triggers is fragmentary and scarce. The work aims is to analyze the cardiac conduction system findings collected at the national referral center for SIDS-SIUD. Methods: A total of 123 autopsied cases of SIDS (59.35

Pregnancy and breastfeeding in the mother with cardiomyopathy.

Cardiomyopathies are diseases of the heart muscle, characterised by structural and functional abnormalities of the myocardium that are not caused by ischemia, valvular problems or congenital heart disease. They are responsible for one-third of pregnancy-related cardiovascular deaths. A woman may enter pregnancy with a pre-existing diagnosis, or the cardiomyopathy may emerge or develop de novo during pregnancy. The most common cardiomyopathies encountered in pregnancy are dilated cardiomyopathy, hypertrophic cardiomyopathy and peripartum cardiomyopathy. All cardiomyopathies can be complicated b
