Clinical impact of treprostinil in neonates with persistent pulmonary hypertension refractory to inhaled nitric oxide: A retrospective cohort study.

Persistent pulmonary hypertension of the newborn (PPHN) has limited options when unresponsive to inhaled nitric oxide (iNO). We evaluated the role of intravenous treprostinil and whether early response classification could inform management. We retrospectively analyzed 114 neonates who had received iNO therapy during the period from 2014 to June 2025. Based on oxygenation index change during the 1st 24 hours after iNO initiation, infants were classified as fast, slow, or non-responders. Treprostinil was used selectively in severe cases; among 32 non-responders, 16 received adjunctive intraveno

Decreased AMP-kinase function in pulmonary artery smooth muscle cells contributes to pulmonary artery remodeling in persistent pulmonary hypertension of the newborn.

Pulmonary vascular remodeling contributes to persistent pulmonary hypertension of the newborn (PPHN); the mechanisms remain unknown. 5'-AMP-activated protein kinase (AMPK) is a critical regulator of energy balance and metabolism. We investigated the hypothesis that decreased AMPK function in pulmonary artery smooth muscle cells (PASMCs) leads to impaired mitochondrial capacity to perform oxidative phosphorylation and altered notch ligand expression, which together promote vascular remodeling in PPHN. Studies were performed in fetal lambs with PPHN induced by prenatal ductus arteriosus constric

Evidence-Based Management of Persistent Pulmonary Hypertension.

Persistent pulmonary hypertension of the newborn is characterized by elevated pulmonary vascular resistance, impaired oxygenation, and delayed transition from fetal to adult circulation. Managing infants with pulmonary hypertension is particularly challenging because of the intricate and multifactorial nature of the underlying pathophysiology. A comprehensive and consistent approach to care for these critically ill infants is essential. Although numerous interventions have been extensively studied, a lack of consensus persists in clinical practice, and until recently no formal evidence-based g

Genetic Variants Associated with Persistent Pulmonary Hypertension of Newborn: A Systematic Review.

INTRODUCTION: Persistent pulmonary hypertension of newborn (PPHN) occurs due to the impairment in the expected fall in pulmonary vascular resistance during the fetal to neonatal circulatory transition, with a prevalence of 1.9 per 1,000 live births and a significant mortality rate of 4-33%. We aimed to systematically review the genetic variants associated with PPHN in term and late preterm infants without a known genetic syndrome. METHODS: In February 2025, the MEDLINE Ovid, Scopus, and Cochrane databases were searched for eligible studies without publication date restriction. Our review incl

Cooccurrence of myelinated retinal nerve fibers in pediatric unilateral persistent fetal vasculature: report of three cases.

We report 3 pediatric cases of persistent fetal vasculature (PFV) and myelinated retinal nerve fibers (MRNF) coexisting in the same eye. All cases presented with congenital cataracts, vitreous strands, and an unusual axial length (AL) profile-specifically, the affected eyes exhibited longer ALs than their fellow eyes, contrary to typical PFV patterns. Preoperative Color Doppler imaging consistently revealed a linear echogenic band extending from the optic disk to the posterior lens capsule. Intraoperative and postoperative fundus examinations confirmed the presence of MRNF in each case, with v

Plasma Asymmetric Dimethylarginine as a Biomarker of Disease Severity and Therapeutic Response in Neonatal Persistent Pulmonary Hypertension: An Observational Prospective Case-Control Study.

BACKGROUND: Persistent pulmonary hypertension of the newborn (PPHN) is a life-threatening condition characterized by elevated pulmonary vascular resistance and impaired oxygenation. Endothelial dysfunction plays a central role in its pathophysiology. Asymmetric dimethylarginine (ADMA), an endogenous inhibitor of nitric oxide synthase, has been implicated in pulmonary vascular disease; however, its clinical utility as a biomarker of disease severity and therapeutic response in neonatal PPHN remains insufficiently defined. METHODS: This prospective observational case-control study included neon

Small partial deletion of a highly GC-rich FOXF1 exon 1 in two deceased siblings with alveolar capillary dysplasia.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) is a rare lethal lung developmental disorder caused by haploinsufficiency of FOXF1. While larger-sized coding and noncoding copy-number variant (CNV) deletions involving the FOXF1 locus are detected in approximately half of histopathologically-diagnosed ACDMPV patients, small CNVs remain diagnostically challenging. Here, we revisited an unsolved case of familial ACDMPV with two affected siblings. Whole genome sequencing (WGS) of 30-year-old archival lung autopsy tissue analyzed using AI powered platform revealed a 151 b

[Use of inhaled nitric oxide during neonatal transport - a review of seven years of experience].

INTRODUCTION: The management of newborns with persistent pulmonary hypertension has critical importance. These infants require immediate on-site intensive care, which must be continued during transport until arrival at a tertiary regional center. Inhaled nitric oxide is an effective pulmonary vasodilator, and its use during transport may support the treatment of pulmonary hypertension and ensure adequate gas exchange until handover in the intensive care unit. Since March 2018, the Neonatal Transport Service of the Peter Cerny Foundation has been the only provider in Hungary to administer inhal

Advances in the Management of Persistent Pulmonary Hypertension of the Newborn.

This article details an integrated approach to optimize management strategies for hypoxemic respiratory failure and hemodynamic instability in persistent pulmonary hypertension of the newborn (PPHN). Even with adequate lung recruitment, inhaled nitric oxide, the only food and drug administration (FDA)-approved therapy for PPHN, fails in one-third of cases. Animal models of PPHN have enhanced understanding of PPHN pathophysiology, resulting in new targets for the development of alternative vasodilators, including phosphodiesterase inhibitors, prostaglandins, and endothelin antagonists. However,

Prenatal Presentation of Congenital Alveolar Capillary Dysplasia With Misalignment of Pulmonary Veins: A Case Initially Suspected as Posterior Urethral Valves.

Alveolar capillary dysplasia with misalignment of the pulmonary veins (ACDMPV) is a rare, typically lethal pulmonary disorder characterized by underdeveloped pulmonary vascularization. It usually presents within the first 48 h of life with progressive respiratory distress and therapy-resistant persistent pulmonary hypertension of the newborn (PPHN). ACDMPV is associated with genomic alterations at the FOXF1 locus, with most cases arising de novo. We present a case in which suspected urethral valves, managed prenatally with fetal vesicoamniotic shunting, were the first indication of ACDMPV. In

Management of Hypoxemic Respiratory Failure in the Newborn During Therapeutic Hypothermia.

Therapeutic hypothermia (TH) is the gold standard for the management of hypoxic ischemic encephalopathy (HIE) in newborns. Both hypoxia and TH may contribute to an increase in pulmonary vascular resistance (PVR), resulting in persistent pulmonary hypertension of the newborn. Infants with worsening hypoxemic respiratory failure in the setting of HIE and TH may require optimization of respiratory support to achieve adequate lung recruitment, surfactant, and inhaled nitric oxide to improve oxygenation and ventilation strategies to avoid acidosis and hypocapnia. Furthermore, management of systemic

Severe Suprasystemic Refractory Pulmonary Hypertension in a Neonate with Stüve-Wiedemann Syndrome Associated with Biallelic LIFR Variants: Molecular Insights and a Neonatal Case Report.

Stüve-Wiedemann syndrome (SWS) is an ultra-rare autosomal recessive skeletal dysplasia caused by loss-of-function variants in the leukemia inhibitory factor receptor (LIFR) gene. While characterized by bone deformities and dysautonomia, severe persistent pulmonary hypertension of the newborn (PPHN) significantly contributes to high early mortality. We report a neonate with genetically confirmed SWS who presented with severe, suprasystemic PPHN refractory to standard pulmonary vasodilators, including inhaled nitric oxide. This case provides a detailed longitudinal hemodynamic characterization o

Evaluation and Management of Genetic Respiratory Disorders Presenting as Hypoxemic Respiratory Failure in the Newborn Infant.

Hypoxemic respiratory failure is a common reason for admission to the neonatal intensive care unit for term and late preterm infants. Some infants have rare disorders due to genetic mechanisms including surfactant dysfunction disorders, alveolar capillary dysplasia with misalignment of the pulmonary veins, and other developmental lung disorders that result in more severe, persistent, and even fatal disease. In this article, we summarize clinical characteristics of the most common causes of neonatal respiratory failure and of rare genetic causes, to help clinicians differentiate between common
