Placental abnormalities of malperfusion, inflammation, and meconium are associated with neonatal pulmonary vascular disease.

OBJECTIVES: The primary objective was to elucidate whether placental histopathology representative of intrauterine insults during pulmonary vascular development are associated with a diagnosis of persistent pulmonary hypertension of the newborn (PPHN). The secondary objective was to compare placental histopathologic lesions across PPHN etiologies. STUDY DESIGN: We conducted a case-control study of mother-infant dyads ≥35 weeks gestation who delivered at a tertiary care center between 2020 and 2025. Cases were infants diagnosed with PPHN and treated with inhaled nitric oxide; unaffected contro

Management of Hypoxemic Respiratory Failure in the Newborn During Therapeutic Hypothermia.

Therapeutic hypothermia (TH) is the gold standard for the management of hypoxic ischemic encephalopathy (HIE) in newborns. Both hypoxia and TH may contribute to an increase in pulmonary vascular resistance (PVR), resulting in persistent pulmonary hypertension of the newborn. Infants with worsening hypoxemic respiratory failure in the setting of HIE and TH may require optimization of respiratory support to achieve adequate lung recruitment, surfactant, and inhaled nitric oxide to improve oxygenation and ventilation strategies to avoid acidosis and hypocapnia. Furthermore, management of systemic

Severe Suprasystemic Refractory Pulmonary Hypertension in a Neonate with Stüve-Wiedemann Syndrome Associated with Biallelic LIFR Variants: Molecular Insights and a Neonatal Case Report.

Stüve-Wiedemann syndrome (SWS) is an ultra-rare autosomal recessive skeletal dysplasia caused by loss-of-function variants in the leukemia inhibitory factor receptor (LIFR) gene. While characterized by bone deformities and dysautonomia, severe persistent pulmonary hypertension of the newborn (PPHN) significantly contributes to high early mortality. We report a neonate with genetically confirmed SWS who presented with severe, suprasystemic PPHN refractory to standard pulmonary vasodilators, including inhaled nitric oxide. This case provides a detailed longitudinal hemodynamic characterization o

Evaluation and Management of Genetic Respiratory Disorders Presenting as Hypoxemic Respiratory Failure in the Newborn Infant.

Hypoxemic respiratory failure is a common reason for admission to the neonatal intensive care unit for term and late preterm infants. Some infants have rare disorders due to genetic mechanisms including surfactant dysfunction disorders, alveolar capillary dysplasia with misalignment of the pulmonary veins, and other developmental lung disorders that result in more severe, persistent, and even fatal disease. In this article, we summarize clinical characteristics of the most common causes of neonatal respiratory failure and of rare genetic causes, to help clinicians differentiate between common

Etiologies of Pulmonary Arterial Hypertension in Early Infants: A 4-year Experience from a Tertiary Care Center in Southern India.

INTRODUCTION: Pulmonary arterial hypertension (PAH) is relatively uncommon yet life-threatening morbidity in neonates and infants. South Asian countries including India may record a relatively higher incidence of pediatric PAH. The etiology of pediatric PAH is heterogeneous, and little evidence is available on the clinical spectrum of pediatric PAH from low- and middle-income countries. This descriptive study aims to describe the incidence and underlying causes of pediatric PAH at a tertiary care referral center in coastal Karnataka, India. MATERIALS AND METHODS: This is a retrospective study

Neonatal cyanosis: exploring the complexity of coexisting congenital methemoglobinaemia and pulmonary hypertension.

Neonatal cyanosis is commonplace, usually from respiratory or cardiac causes. Consideration of alternative or combined aetiologies was required when a 3-hour-old term male neonate with central cyanosis, low oxygen saturation (SpO2), and persistent pulmonary hypertension of the newborn (PPHN) confirmed on echocardiography, failed to respond to PPHN interventions. The high oxygen saturation gap and methemoglobin (MetHb) level of 16.2% shifted the working diagnosis to methemoglobinaemia. Possible triggering drugs were stopped and methylene blue was given without effect, prompting testing for haem

Persistent pulmonary hypertension, neonatal stroke, coagulopathy and multi-organ failure due to severe enterovirus sepsis: two case reports.

BACKGROUND: Disseminated neonatal enteroviral infection is a rare and life-threatening condition with significant multi-organ involvement. It often presents with nonspecific symptoms that may be mistakenly attributed to bacterial infections. Prompt recognition and treatment are crucial to prevent rapid disease progression. We report two cases of disseminated neonatal enterovirus, including a novel instance of persistent pulmonary hypertension (PPHN) and neonatal stroke. CASE PRESENTATION: A baby boy was delivered vaginally at 35 + 4 weeks, complicated by meconium-stained liquor. He developed r

Alveolar Capillary Dysplasia With Misaligned Pulmonary Veins (ACDMPV): Description of Two Cases and Literature Review.

INTRODUCTION: Alveolar capillary dysplasia with misaligned pulmonary veins (ACDMPV) is one of the major causes of neonatal interstitial lung disease, causing persistent pulmonary hypertension (PH) of the newborn, although symptoms can present later in infancy. Two patients were recently diagnosed with ACD at our Institution and prompted us to perform a structured review of the condition. METHODS: We considered published articles from 2018 to 2025. The final population analyzed was composed of 165 patients, including our cases. From each publication, we extrapolated: clinical presentation, rad

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Adenylyl Cyclase Isoform 6 in the Pulmonary Artery Is Inhibited by Hypoxia via Cysteine Nitrosylation.

Persistent pulmonary hypertension of the newborn (PPHN) is a hypoxic disorder of pulmonary vascular relaxation, mediated in part by adenylyl cyclase (AC). Neonatal pulmonary arteries (PAs) express mainly the AC6 isoform, followed by AC3, AC7, and AC9. AC6 expression is upregulated in hypoxia. We reported AC enzyme inhibition due to S-nitrosylation in PPHN PAs and in PA myocytes exposed to hypoxia. We hypothesized that hypoxia promotes cysteine thiol nitrosylation of AC6, impairing cAMP production. HEK293T cells stably expressing AC isoforms (AC3, 5, 6, 7, 9) or cysteine-to-alanine mutants AC6_

The effect of inhaled nitric oxide treatment on biomarkers of oxidative/nitrosative damage to proteins and DNA/RNA.

Inhaled nitric oxide (iNO) is a selective pulmonary vasodilator that is used as a treatment for persistent pulmonary hypertension in neonates (PPHN) with hypoxic respiratory failure. The generation of reactive oxygen and nitrogen species might induce oxidative/nitrosative damage to multiple organs. There is an increasing scientific and clinical interest in the determination of specific biomarkers to measure the degree of oxidative/nitrosative stress in non-invasively collected biofluids. A method for the simultaneous detection of a panel of oxidative and nitrosative stress-related biomarkers f
