[Anti-amyloid antibodies against Alzheimer's disease : Lecanemab and donanemab in practice-Clinical importance, patient selection and safety management].

Lecanemab and donanemab are the first disease-modifying therapies for early Alzheimer's disease. These humanized IgG1 antibodies promote the clearance of aggregated amyloid-beta peptides and, in the pivotal CLARITY AD and TRAILBLAZER-ALZ 2 trials, significantly delayed cognitive and functional decline, with an approximately 30% relative risk reduction for the clinical stage progression; however, the clinical meaningfulness remains contested: critics point to the small between-group difference in relation to the total score. This is weighed against the first disease-modifying effect and the fir

[Stratified and graded diagnosis and treatment strategies for patients with primary aldosteronism].

Primary aldosteronism (PA) is the most common endocrine cause of secondary hypertension and carries a greater risk of cardiorenal complications than essential hypertension. Recent domestic and international guidelines and consensus statements have proposed screening all patients with hypertension for PA. To cope with the massive patient base resulting from universal screening, this article explores practical schemes for the stratified and graded diagnosis and treatment of PA. By employing "non-drug withdrawal" and "minimal-drug withdrawal" testing protocols, PA screening can be feasibly implem

PLCG1 gene mutation in a child with autoimmune hemolytic anemia: a case report.

Autoimmune hemolytic anemia (AIHA) is a highly heterogeneous disease characterized by immune-mediated destruction of red blood cells through cellular and humoral mechanisms, complement activation, cytokine release, and bone marrow compensation. Clinically, AIHA exhibits a broad spectrum of severity, ranging from mild or compensated anemia to life-threatening anemia. Traditionally, therapeutic approaches have relied on corticosteroids, immunosuppressive agents, and splenectomy as the mainstay treatment. Increasing evidence suggests that genetic factors, including variants in the PLCG1 gene-whic

Effects of 12-week 24-form Tai Chi on cognitive function, cerebral perfusion, and vascular function in postmenopausal women with mild cognitive impairment: an exploratory randomized controlled trial.

BACKGROUND: Postmenopausal women are at increased risk of mild cognitive impairment (MCI). Vascular and cerebrovascular dysfunction may contribute to this increased vulnerability. Tai Chi has been shown to benefit cognitive function, but its effects on cerebrovascular health in this high-risk population remain poorly defined. This study aimed to evaluate the impact of a 12-week Tai Chi intervention on cognitive performance, cerebral perfusion, and circulating vasomotor and Alzheimer's disease (AD)-related biomarkers in postmenopausal women with MCI. METHODS: This single-center, parallel-group

Associations of Mild or Trace Valvular Abnormalities With Cardiac Remodeling in Older Adults: The ARIC Study.

BACKGROUND: The impact of mild/trace valvular abnormalities on cardiac remodeling is not well understood, despite their association with poor cardiovascular outcomes. METHODS: Using data from 4684 participants in the ARIC (Atherosclerosis Risk in Communities) Study (2011-2013), aged 66 to 90 years, we examined cross-sectional associations of mild/trace valvular abnormalities (aortic stenosis, aortic regurgitation, and mitral regurgitation) with cardiac parameters using multivariable linear regression models. Cardiac parameters included cardiac structure (eg, left ventricular mass index), syst

Primary hyperparathyroidism in a 4-month-old German Shepherd mixed breed dog.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

Magnetized blood flow with Jeffrey ternary hybrid nanofluid in an inclined multi-constricted artery for therapeutic transport applications.

The transport of blood through narrowed arteries is strongly influenced by the presence of stenotic obstructions, which modify the flow structure and alter the associated heat and mass characteristics. These effects become increasingly complex when multiple constrictions are present within the same arterial segment. The present study develops a mathematical model to investigate the electroosmotic flow of non-Newtonian Ternary Hybrid Nanofluid (THNF) through inclined arteries with three serially developed geometries: triangular, W-shaped, and elliptical stenosis. Blood is modeled as a Jeffrey f

Severity-associated endothelial dysfunction in post-COVID-19 patients: flow-mediated dilation and vasoactive biomarkers.

COVID-19 is frequently associated with endothelial dysfunction, characterized by an imbalance between vasodilatory and vasoconstrictive mediators. This study evaluated endothelial function in the early post-COVID-19 period across groups of acute disease severity using flow-mediated dilation (FMD), circulating vasoactive biomarkers, and echocardiography. Serum endothelin-1 (ET-1), arginase-1 (Arg1), thromboxane A₂ (TxA₂)-related immunoreactivity, and prostacyclin (PGI₂)-related immunoreactivity were measured by enzyme-linked immunosorbent assay. An impaired FMD response was defined as < 10% bra

Rubella and congenital rubella syndrome in the GCC countries: A retrospective ecological analysis of elimination progress and scenario-based projections, 2020-2024.

INTRODUCTION: Rubella and congenital rubella syndrome (CRS) remain vaccine-preventable priorities. Although the Gulf Cooperation Council (GCC) countries maintain high routine rubella-containing vaccine (RCV) coverage and well-established immunisation programmes, the region is also characterised by large migrant populations, intense international mobility, and complex population immunity profiles that may sustain vulnerability to rubella transmission even in high-coverage settings. This study assessed progress toward rubella and CRS elimination in the GCC during 2020-2024 and examined the polic

Wesselsbron virus at the livestock-human interface: A practical guide to disease recognition and diagnosis.

Wesselsbron virus (WSLV) is a mosquito-borne Orthoflavivirus of the yellow fever virus (YFV) serocomplex, widely distributed across sub-Saharan Africa. It is neglected, and its true impact is likely underestimated. WSLV causes abortion, congenital malformations, and neonatal mortality in sheep, the primary recognized vertebrate host, but infections have also been documented in goats, cattle, camels, wildlife, and humans. The virus is maintained in complex cycles predominantly involving Aedes mosquitoes and incompletely defined domestic and wild vertebrate hosts. Direct infection via the placen

Combined non-contrast enhancement 3D bright-blood and 2D black-blood coronary magnetic resonance angiography for the assessment of coronary artery stenosis.

BACKGROUND: Non-contrast coronary magnetic resonance angiography (CMRA) offers a radiation- and contrast-free alternative for evaluating coronary artery disease, yet its clinical adoption has been constrained by suboptimal specificity, primarily due to flow-related artifacts and signal loss inherent to bright-blood imaging. Black-blood vessel wall imaging, by suppressing intraluminal blood signal, enables direct visualization of the coronary wall and may complement bright-blood CMRA to improve diagnostic accuracy. However, the additive value of a targeted sequential protocol combining these tw

Progressive Amyloid-Related Imaging Abnormalities After Discontinuation of Lecanemab in Early Alzheimer Disease in an APOE ε2/ε4 Carrier.

BACKGROUND: Amyloid-related imaging abnormalities (ARIA) are the most relevant adverse events associated with anti-amyloid therapies in Alzheimer's disease. METHODS: We report a case of progressive ARIA after discontinuation of lecanemab in a patient with early-onset Alzheimer's disease carrying the rare APOE ε2/ε4 genotype. RESULTS: After two infusions, mild ARIA-edema (ARIA-E) and ARIA-hemorrhage (ARIA-H) were detected on routine MRI, prompting treatment interruption. Despite partial radiologic regression, follow-up MRI 8 weeks after discontinuation demonstrated marked progression with mul
