The Use of Gadoxetic Acid-Enhanced MRI as a Diagnostic Tool in Cases of Biliary Atresia: A Two-Case Report.

Early diagnosis of biliary atresia (BA) remains challenging, although timely recognition is critical for achieving favorable surgical outcomes. We present two infants in whom gadoxetic acid-enhanced MRI demonstrated imaging findings that supported early suspicion of BA. Two neonates with persistent cholestasis underwent routine laboratory evaluation, targeted ultrasonography, and hepatocyte-specific magnetic resonance imaging using gadoxetic acid. This contrast agent is actively taken up by functioning hepatocytes and is normally excreted into the biliary system during the delayed hepatobiliar

Acute Bilateral Visual Loss as an Atypical Manifestation of Wernicke Encephalopathy Secondary to Hyperemesis Gravidarum: A Case Report and Narrative Literature Review.

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Familial Aplasia Cutis Congenita With Exposure of the Meningeal Membrane: A Case Report.

Aplasia cutis congenita (ACC) is a rare congenital malformation characterized by the localized absence of skin and occasionally subcutaneous and bone tissues. Its etiology is multifactorial, and management ranges from conservative wound care to surgical intervention. We report the case of a male newborn presenting with a 4-5 cm ulcerated lesion at the vertex of the scalp, with complete absence of bone tissue and dural exposure. Physical examination revealed syndromic features (facial asymmetry, hypertelorism, low-set ears, and cutaneous syndactyly) and an atrial septal defect. A strong autosom

Domestic abuse disclosure in perinatal healthcare settings - A qualitative study into the experiences of mothers who had their baby removed at birth.

BackgroundIn the UK, numbers of newborn babies entering state care through child protection processes have significantly increased in the last decade. Their mothers often have experienced cumulative and intersecting disadvantages, including childhood abuse and domestic abuse (DA). DA will often be a key factor in child protection decision-making, with a persistent narrative of mothers 'failing to protect' their children. Given the profound consequences of DA disclosures and the failure-to-protect discourse that might impact care proceeding outcomes for these mothers, it is important to explore

Congenital anomalies of the kidney and urinary tract in Nigeria: a thirteen-year single centre review.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) encompass a wide spectrum of structural anomalies of the kidney and urinary tract. Evidence from registries in high-income countries suggests that it is a major cause of morbidity in children, with up to 50%-60% of chronic kidney disease (CKD) resulting from CAKUT. The picture is not so clear in medium and low-income countries, mainly due to lack or nonexistence of reliable registries, constraints of skilled personnel, diagnostic capacities, accurate data gathering and poor health-seeking behaviour. METHODS: Information

Meeting the challenges to adolescent well-being: context, returns on investment, and financing.

Adolescents in many countries face serious challenges in realizing the level of well-being that is their human right. Many factors-deep-seated inequality within and between countries, rapid change in skill requirements for employment, climate change, and social media-are driving these deepening challenges. This paper develops three points about the need for an urgent response to this situation. First, there is now evidence that a powerful suite of cost-effective investments is available to improve adolescent well-being. To support this claim, we draw on recent modelling of investments for many

Hair cortisol as a biomarker of chronic stress in periparturient dairy cattle: associations with health status, lactation, and metabolic profiles.

Dairy cows experience long-term stress during the pre- and postpartum period, as during this period they face extreme challenges related to their health, productivity and overall welfare. Recently, hair cortisol has gained attention as one of the most promising and non-invasive biomarkers for evaluating stress. However, the exact mechanism of its deposition in the hair remains unknown. This review combines the utility of hair cortisol as a tool for evaluating chronic stress and its effect on health status, lactation, parity, body condition score (BCS), and metabolic parameters, specifically gl

In-hospital mortality and associated factors among term neonates with documented perinatal asphyxia in eastern Democratic Republic of the Congo: a retrospective cohort study.

BACKGROUND: Perinatal asphyxia remains an important contributor to neonatal mortality and neurodevelopmental impairment, particularly in resource-limited settings, yet contemporary evidence from eastern Democratic Republic of the Congo (DRC) is scarce. We assessed the hospital burden, clinical severity, and factors associated with in-hospital mortality among affected neonates admitted to a tertiary neonatal intensive care unit (NICU). METHODS: We conducted a retrospective cohort study at the Hôpital Provincial Général de Référence de Bukavu, eastern DRC, including neonates with documented per

Synthesizing Interest-holder Perspectives and Evidence-based Potentially Better Practices to Improve High-risk Infant Follow-through Care Quality.

INTRODUCTION: Despite specialized high-risk infant follow-up (HRIF) being the standard of care for preterm and medically complex infants after hospital discharge, program participation remains low, and equity gaps persist. Little work has examined HRIF's perceived value, or lack thereof, for families, staff, and other key interest-holders. Our objective was to examine interest-holders' perspectives of the value of the HRIF program using qualitative methods to inform continuous quality improvement (CQI) work. METHODS: Families (n = 6) and staff (n = 9) in a single, urban pediatric hospital HRI

Effects of Rhythm Control versus Rate Control on Atrial Fibrillation and Frailty Development in Older Patients: Protocol for the Cardioversion of Atrial Fibrillation and Frailty in the Elderly (CAFFE) Study, an Observational, Prospective Cohort Study.

BACKGROUND: Atrial fibrillation (AF), highly prevalent in older individuals, is associated with frailty and disability. AF therapy is based on rhythm control (RhythmC) to restore sinus rhythm or rate control (RateC) to lower the heart rate. Published guidelines do not indicate a preference for one over the other for older people. OBJECTIVE: This study aims to evaluate whether RhythmC is more effective than RateC in hindering the progression of frailty and disability. METHODS: The Cardioversion of Atrial Fibrillation and Frailty in the Elderly (CAFFE) study is a prospective, multicenter, obse

Skin Color Bias in Neonatal Transcutaneous Bilirubin Measurement: A Diffuse Reflectance Spectroscopy Approach With Melanin Correction.

Commercial jaundice meters rely on reflectance data from specific wavelengths for Transcutaneous bilirubin (TcB) measurements. However, skin color bias is introduced due to melanin contribution. In this study, we evaluated melanin concentration as an objective indicator of neonatal skin pigmentation and assessed the performance of melanin corrected TcB measurements (TcBcorr). A total of 333 neonates were categorized into four groups using the Neomar skin color scale. Melanin concentration estimated from neonatal reflectance spectra strongly correlated with Neomar classification (Spearman's r =

Factors associated with the development of neonatal acute respiratory distress syndrome in preterm infants: a retrospective cohort study.

UNLABELLED: Neonatal acute respiratory distress syndrome (NARDS) carries high morbidity and mortality. Preterm infants at risk need to be identified early so that treatment is not delayed. We aimed to identify factors associated with NARDS in preterm infants. Clinical data of 548 preterm infants admitted to the NICU between 2019 and 2023 were retrospectively analyzed. Preterm infants were categorized into the NARDS group (n = 275) and the control group (n = 273) according to the Montreux definition. Univariate analysis and multivariable logistic regression were used to identify factors indepen

A dual-teacher cross-modal knowledge distillation framework integrating visual and textual modalities for neonatal jaundice diagnosis.

BACKGROUND AND OBJECTIVE: Neonatal jaundice remains a major health challenge, particularly in resource-limited environments where reliable diagnosis is not readily accessible. While image-based and AI-driven approaches have demonstrated encouraging diagnostic performance, their limited generalizability across different datasets, populations, and clinical settings remains a major challenge. METHODS: This study proposes a Dual-Teacher Cross-Modal Knowledge Distillation (DT-CMKD) framework for efficient neonatal jaundice diagnosis. Complementary knowledge is distilled from two teachers: a vision

A rare conotruncal anomaly: absent pulmonary valve syndrome, interrupted aortic arch, and severe aortic stenosis.

The coexistence of absent pulmonary valve syndrome, Type B interrupted aortic arch, and severe aortic stenosis represents an exceptionally rare and complex congenital cardiac phenotype. The diagnosis was established in a newborn using multimodality imaging. Given the profound airway compression secondary to the massive pulmonary artery dilatation and the associated poor prognosis, palliative care was chosen.

Care of the Newborn and Cord During the Third Stage of Labour (Scientific Impact Paper No. 78).

The third stage of labour happens after the baby or babies are born and when the placenta (afterbirth) is delivered. This Scientific Impact Paper reviews new evidence related to clamping of the umbilical cord, and other interventions that impact the birthing woman and newborn during this part of labour. The paper summarises recent additional information on anatomy (structure of the circulation system) and physiology (how the system functions) of the newborn baby's circulation before the umbilical cord blood vessels close, whether naturally or after a clamp is placed on the umbilical cord. The

Assessment of Bone Bridging of a Novel Wet-Field Regenerative Bone Adhesive for Lumbar Spine Fusion in an Ovine Model at 12, 26, and 52 Weeks.

BACKGROUND: Current techniques for instrumented posterolateral lumbar fusion include the use of titanium rods and pedicle screws to provide mechanical fixation, while autograft and/or allograft bone is used to promote osseous union across the fusion site. Although bone graft substitutes may be used for this purpose, they can resorb before a solid fusion is established, leading to nonunion. Tetracalcium phosphate-phosphoserine (TTCP-PS) is a novel, bioresorbable, osteoconductive, wet-field mineral-organic bone adhesive and bone graft substitute that has been previously shown to promote bone hea
