Association between early pregnancy bleeding and neonatal outcome: a retrospective cohort study from low resource setting.

INTRODUCTION: Early pregnancy bleeding (EPB) is considered as an important risk factor for pregnancy loss. Therefore, the rest of the pregnancy is managed cautiously in order to prevent adverse outcome in these patients. Sri Lanka, although a developing country, maternal and fetal health is maintained as a high standard despite limitations. This study is aimed at analysing association of EPB with neonatal outcomes in a low resource setting. RESULTS: A total of 238 women were included in this retrospective case-control study, of whom 81 (34%) experienced EPB and 157 (66%) served as matched con

Risk factors for intraventricular hemorrhage in very low birth weight infants: a systematic review and meta-analysis.

BACKGROUND: Intraventricular Hemorrhage (IVH) is one of the common and serious complications of Very Low Birth Weight Infant (VLBW) that may lead to long-term neurodevelopmental deficits. Although several studies have been conducted to explore its risk factors, the results have been inconsistent. The aim of this study was to identify the major risk factors for intraventricular Hemorrhage in VLBW by systematic evaluation and Meta-analysis of the available evidence. METHODS: PubMed, Web of Science, Embase, Cochrane Library were systematically searched, and observational studies (case-control an

Successful Vaginal Delivery in a Pregnant Woman With Bernard-Soulier Syndrome: A Case Report.

Bernard-Soulier syndrome (BSS) is a rare inherited platelet disorder that poses major risks during pregnancy because of potential bleeding complications. Managing such cases in low-resource settings is especially challenging. We report the case of a 23-year-old Afghan woman with BSS who achieved a successful full-term vaginal delivery at a tertiary center. Early referral, coordinated management by hematology and obstetric teams, prophylactic platelet transfusions, and prompt treatment of postpartum hemorrhage led to a favorable maternal and neonatal outcome. This experience shows that, even wh

Aplasia cutis congenita of the scalp with skull defect: an individual patient data systematic review of clinical features and treatment approaches.

BACKGROUND: Skull defects in aplasia cutis congenita of the scalp are rare but high-risk due to exposed underlying tissues. Because of its rarity, no clear consensus exists regarding optimal management. This study reviews published cases to evaluate clinical features, outcomes, and their relation to treatment strategies. METHODOLOGY: A systematic review was conducted per PRISMA guidelines, including case reports and series (1970-2024) from PubMed, Scopus, Embase, Web of Science, and PsychINFO. Data were pooled and cases grouped by conservative vs. surgical management for comparison. RESULTS:

The Role of State Perinatal Quality Collaboratives in Addressing Maternal Morbidity and Mortality.

State Perinatal Quality Collaboratives (PQCs) exist in all 50 states, the District of Columbia, and the Armed Forces. All PQCs work to reduce preventable maternal morbidity and mortality through statewide implementation of evidence-based practices. PQCs have been successful in improving outcomes through key strategies, including strong hospital engagement; use of quality-improvement (QI) methodology such as rapid-cycle data review, collaborative learning, and supplemental technical assistance; and strategic partnerships. Here we provide examples of ways four state PQCs have implemented these s

Cesarean scar pregnancy as a precursor lesion for placenta accreta spectrum: evidence and implications.

BACKGROUND: Cesarean scar pregnancy (CSP) is a form of abnormal implantation in which the gestational sac embeds within the myometrial defect of a previous cesarean incision. Its incidence is rising in parallel with increasing cesarean delivery rates. Emerging evidence indicates that CSP and placenta accreta spectrum (PAS) share common histopathological and sonographic characteristics, supporting the concept that CSP represents an early phenotype within the PAS. OBJECTIVE: To synthesize current evidence on the diagnosis, classification and management of CSP and to clarify the biological and c

Determinants of labor epidural analgesia uptake and associations with maternal-neonatal outcomes: a stratified cohort study with risk prediction modeling.

BACKGROUND: Labor pain is a major physiological and psychological stressor for women during childbirth. Epidural analgesia is widely recognized as the most effective form of pain relief in labor, yet its utilization varies greatly, and uncertainty remains regarding its determinants and potential impact on maternal and neonatal outcomes. This study aimed to identify factors influencing the use of epidural analgesia and to evaluate its associations with labor progression and postpartum hemorrhage. METHODS: This retrospective cohort included 1035 women who delivered at our hospital between 2020

[The Association between Prolonged Second Stage in Primipara Women at Term and Excessive Hemorrhage According to the Mode of Delivery].

BACKGROUND: When prolonged second stage occurs, decisions regarding continuation of vaginal delivery, potentially further prolonging the second stage, or performing instrumental vaginal delivery or cesarean delivery (CD) should occur. It is important to know the specific risks in each mode of delivery in this setting. We assumed that prolonged second stage may increase the risk of complications, if CD would be performed. OBJECTIVES: To investigate the association between the duration of the second stage of labor in primipara women and excessive hemorrhage in each mode of delivery. METHODS: R

Impact of active phase duration on neonatal and maternal adverse outcomes: A systematic analysis of parity stratification, body mass index interaction, and predictive modeling in a retrospective cohort.

BACKGROUND: A prolonged active phase of labor is associated with adverse maternal and neonatal outcomes, but its clinical significance in contemporary obstetric populations, including the unique Chinese demographic context, and its modification by parity and body mass index (BMI) remain incompletely defined. METHODS: A single-center retrospective cohort study included 2873 women with term (≥37 weeks), singleton, cephalic vaginal deliveries. Active phase duration (defined as time from 5 cm to 10 cm cervical dilation) was categorized into short, medium, and prolonged groups (33rd/66th percentil

Intrapartum Ultrasound in Vacuum Operative Delivery: A Comprehensive Review and Proposal of the Novel Ultrasound Flexion Point Method.

Operative vaginal delivery (OVD) via vacuum extraction is a fundamental component of modern obstetric management, yet it carries specific risks of failure and maternal-fetal complications, such as cup detachment, cephalohematoma, and intracranial hemorrhage. The success and safety of the procedure rely heavily on the correct application of the vacuum cup over the "flexion point" of the fetal head. Traditional identification of this landmark via digital examination is often hindered by caput succedaneum and cranial molding, leading to high rates of diagnostic error, particularly in dystocic lab

The association between mode of delivery and severe intraventricular hemorrhage in very preterm infants.

BACKGROUND: Infants born before 32 weeks of gestation are at high risk of developing intraventricular hemorrhage (IVH), which is associated with significant mortality and morbidity as well as long-term neurodevelopmental impairment. There is currently a lack of consensus regarding the impact of delivery mode on these infants. Therefore, the aim of this study was to determine the association between mode of delivery and the incidence of severe intraventricular hemorrhage (sIVH) in very to extremely preterm infants born before 32 weeks of gestation. METHODS: This retrospective cohort study was

Outcomes for Labor Induction Compared With Expectant Management Among Women Receiving Hospital-Based, Midwifery-Led Care.

INTRODUCTION: Use of labor induction has increased rapidly for medical and nonmedical reasons, raising questions about its impact on cesarean birth rates. With growing incidence of elective induction of labor (IOL) at 39 weeks, uncertainty remains about how care practices associated with IOL influence outcomes. Observational studies have reported varied cesarean birth rates following IOL. This issue is particularly relevant for midwives, who prioritize physiologic labor and recognize benefits of spontaneous labor in achieving vaginal birth. This study evaluated cesarean birth rates after labor
