UNLABELLED: Non-immune hydrops fetalis (NIHF) is a rare condition that can lead to serious cardiopulmonary compromise after birth, often resulting in the need for intensive cardiorespiratory support, and in selected cases including extracorporeal membrane oxygenation (ECMO). Outcome is complicated due to a high rate of preterm birth. Morbidity and mortality data regarding NIHF and outcome of ECMO treatment in this population are very sparse. This is the first update and analysis of the Extracorporeal Life Support Organization (ELSO) registry-based data of neonates with NIHF. Neonatal patients

BACKGROUND: Hemoglobin (Hb) Bart's hydrops fetalis is a major public health concern in Southeast Asia, particularly in Thailand. Current screening strategies target the two most common α0 -thalassemia deletions (--SEA and --THAI).
METHOD: In this study, we developed a single-tube multiplex real-time PCR assay for the simultaneous detection of four clinically relevant α0-thalassemia deletions (--SEA, --THAI, --CR, and --SA). The assay was validated using 538 clinical samples with diverse thalassemia genotypes and compared against conventional gap-PCR as the reference method. Analytical perform

False-negative results in non-invasive prenatal testing are rare but clinically significant, particularly in the presence of biological factors such as feto-placental mosaicism. This report describes a pregnancy affected by fetal trisomy 18 in which three consecutive non-invasive prenatal tests reported low risk despite abnormal first-trimester ultrasound findings (increased nuchal translucency, hydrops fetalis, and tricuspid regurgitation). Amniocentesis subsequently confirmed trisomy 18 in the fetus. Post-termination placental analysis revealed mosaicism, with high trisomy 18 levels on the f

Dnase2 deficiency is an ultra-rare type I interferonopathy resulting from impaired intracellular DNA degradation, leading to neonatal cytopenias and systemic inflammation. Whole-exome sequencing enables definitive diagnosis, and Janus kinase inhibitors have emerged as targeted immunomodulatory therapy. This report describes a preterm infant with an early and severe presentation of DNASE2 deficiency, manifesting antenatally with fetal hydrops, intrauterine growth restriction, and cardiomyopathy. Postnatally, the infant developed persistent pancytopenia, hepatosplenomegaly, cholestatic jaundice,

Nonimmune hydrops fetalis (NIHF) is characterized by abnormal fluid accumulation in ≥ 2 fetal compartments and may be genetic. The incremental diagnostic yield of prenatal exome sequencing (ES) for NIHF following a negative standard workup was previously explored on 22 cases. We report ES findings in the remaining cohort and investigate the incremental yield of genome sequencing (GS) in cases without a definitive diagnosis from exome. Enrollment criteria and exome methodology for the Hydrops-Yielding Diagnostic Results of Prenatal Sequencing (HYDROPS) study were previously reported. Exomes wit

OBJECTIVE: Congenital parvovirus B19 (PB19) infection can lead to severe fetal anemia and hydrops fetalis, necessitating in-utero transfusion (IUT) as a life-saving intervention. This study aimed to identify risk factors associated with unfavorable perinatal outcome following IUT in hydropic fetuses with PB19 infection, with the goal of optimizing transfusion strategies and improving fetal survival.
METHODS: A retrospective, multicenter, international cohort study was conducted across nine specialized fetal medicine centers in France, Belgium and the Czech Republic. This study included pregna

Non-immune hydrops fetalis (NIHF) is a phenotypically heterogeneous condition associated with substantial perinatal morbidity and mortality and its underlying etiology often remains unresolved despite comprehensive conventional investigations. We evaluated the etiological spectrum of NIHF and the additional diagnostic contribution of whole-exome sequencing (WES) in a prospective cohort from Western Rajasthan, India. Eighty consecutive fetuses with NIHF were enrolled between January 2020 and March 2026. Fetuses with immune hydrops or twin-to-twin transfusion syndrome were excluded. All cases un

Background/Objectives: Fetal hepatosplenomegaly is an uncommon but clinically significant prenatal finding associated with a wide range of heterogeneous conditions, including congenital infections, fetal anemia, genetic syndromes, metabolic disorders, and other, less common abnormalities. Because of its nonspecific presentation and diverse etiologies, it represents a diagnostic challenge that requires a structured, multidisciplinary approach. This review aims to present a practical, clinically oriented, stepwise approach to the prenatal evaluation of fetal hepatosplenomegaly, integrating curre

We present a case where the combination of severe hydrops fetalis, lymphangiectasia, and skin desquamation created a clinical picture that masqueraded as epidermolysis bullosa (EB). A neonate presented at birth with severe hydrops fetalis and extensive skin sloughing. Skin biopsy findings were consistent with EB; however, the clinical course and genetic testing were inconsistent with this diagnosis, and the postmortem examination revealed the underlying cause to be severe congenital diffuse lymphangiectasia. It is important for clinicians to consider edema bulla and tissue hypoxia as a cause o

Premature constriction of the ductus arteriosus accompanied by the dilation of the pulmonary trunk constitutes a rare congenital heart anomaly that may be underestimated and can result in fetal hydrops and ultimately death if not treated; therefore, it necessitates fetal post-mortem evaluation in all cases of legal abortion or fetal death.

OBJECTIVES: This work aims to characterize the clinical manifestations and diagnostic challenges associated with pregnancies affected by congenital myotonic dystrophy through a detailed case report of an individual seen in our center and a comprehensive case series overview.
CASE PRESENTATION: A 33-year-old woman presented at 33 weeks-gestation with symptomatic severe polyhydramnios (AFI 55.2) and an otherwise uncomplicated prenatal course with no anomalies on ultrasound. She underwent amnioreduction, which initially revealed normal genetic testing results (46,XX karyotype and normal microarr

OBJECTIVE: Parvovirus B19 infection during pregnancy can cause severe fetal anemia, hydrops fetalis, and fetal death. This study describes our experience managing ten cases of severe fetal anemia due to parvovirus B19 infection during the 2023-2024 national outbreak in Israel.
METHODS: This single-center descriptive cohort study included ten singleton pregnancies with fetal anemia (MCA-PSV >1.5 MoM) requiring intrauterine transfusion. Parvovirus B19 infection was confirmed by maternal IgM serology and/or amniotic fluid PCR. For pregnancies <20 weeks of gestation, we implemented a modified int

Primary fetal pleural effusion involves the accumulation of fluid in the fetal pleural space, posing considerable challenges for perinatal diagnosis and management. Primary effusions, emerging from the maldevelopment of lymphatic vessels, differ significantly from secondary effusions, which are associated with a spectrum of anomalies and conditions, affecting clinical approach and prognosis. The diagnosis of primary fetal pleural effusion is one of exclusion. The natural history of the condition ranges from spontaneous resolution in mild cases to progression to life-threatening complications.

INTRODUCTION: Bronchopulmonary sequestration (BPS), a rare congenital pulmonary malformation, is characterized by nonfunctional pulmonary parenchyma with aberrant systemic arterial supply (typically from the aorta or its major branches) and absence of communication with the tracheobronchial tree. Large BPS lesions pose significant fetal risks, including hydrops fetalis, mediastinal shift, and polyhydramnios, necessitating timely prenatal intervention to prevent fetal demise.

OBJECTIVE: We first report a case of familial hemophagocytic lymphohistiocytosis (FHL) diagnosed in utero due to third trimester hydrops fetalis.
CASE REPORT: A 28-year-old G1P0 woman had a normal nuchal translucency scan and 2 second trimester ultrasounds at 18 and 24 weeks. The third trimester ultrasound at 30 weeks showed mild polyhydramnios. The follow-up ultrasound at 32 weeks showed features of hydrops fetalis with pleural effusions, ascites, skin edema and polyhydramnios. Fetal blood sampling via cordocentesis showed a mild anemia (hemoglobin-100 g/L), thrombocytopenia (platelets-12×10
