BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

The International Neonatal Consortium Seizure Working Group of the Critical Path Institute provides an update to the original recommendations for design of clinical trials to treat neonatal seizures based on recent experiences from several trials and developments in the field. Although there aren't sufficient new data to inform definitions of optimal efficacy endpoints, the Working Group recommended inclusion of alternate measures of seizure burden reduction as secondary or exploratory endpoints, to elucidate clinically meaningful efficacy endpoints for future trials. It was recommended to inc

BACKGROUND: Given the incidence and impact of postpartum depression (PPD) on families in the NICU, we aimed to increase the percentage of birthing parents screened for PPD from 0 to 90% over 18-months and provide psychosocial support.
METHODS: Planned experimentation was used to design an effective screening framework in a level IV NICU. Interventions included process standardization, electronic health record (EHR) support, and data visualization for feedback. Data were displayed on a control chart and stratified for race and language to ensure equitable delivery.
RESULTS: Provider screening

OBJECTIVES: This scoping review evaluates the physiological, neurodevelopmental, and psychosocial effects of reading interventions on critically ill infants and young children.
INTRODUCTION: Critically ill children, including premature infants and those with critical CHDs, are at elevated risk for neurodevelopmental complications that may be exacerbated by noxious stimuli in hospital settings. Given the increased awareness of the significance of developmental care in the critical care environment and the well-established role of reading and literacy interventions in healthy children, a potent

OBJECTIVES: Very preterm infants may display poor postnatal growth despite receiving recommended enteral intakes. This has been linked to transient exocrine pancreatic insufficiency, indicated by low fecal pancreatic elastase-1 (FPE-1) concentrations. We aimed to determine whether growth velocity may be improved by exogenous digestive enzyme replacement.
METHODS: In this bicentric prospective observational study, changes in growth velocity were assessed in very preterm infants (<32 weeks gestational age, <1500 g birth weight) with low FPE-1 concentrations (<200 µg/g). Infants received 350 U l

Background/Objectives: Individualized ("target") fortification of human milk is promoted to improve growth and subsequent neurodevelopment in very low birth weight (VLBW) infants, because standard fortifiers may not provide sufficient protein given the considerable intra- and interindividual variability of human-milk composition. We compared modified-standard fortification (MSF), an optimized high-protein standard regimen, with high-protein target fortification (TFO). Methods: In a single-center, double-blind randomized trial, VLBW infants (24 + 0 - 31 + 6 weeks) receiving 100 mL/kg/d of stand

BACKGROUND AND AIM: Intimate partner violence (IPV) during pregnancy is a serious public health concern with potential harm to mothers and newborns. This systematic review and meta-analysis synthesised Australian studies examining the association between IPV or family violence during pregnancy and preterm birth (PTB), low birth weight (LBW), small for gestational age (SGA), stillbirth (SB), and miscarriage.
METHODS: English-language, peer-reviewed original studies conducted in Australia and reporting IPV during pregnancy as the exposure and at least one specified adverse birth outcome were so

CONTEXT: Preterm birth remains the leading cause of neonatal mortality worldwide and contributes substantially to neonatal deaths in India. Ascorbic acid (Asc.A) (Vitamin C) is an important antioxidant involved in immune function and protection against oxidative stress. Vitamin C concentration declines rapidly after birth in preterm neonates. Evidence supporting the role of Vitamin C supplementation in reducing respiratory morbidity (oxidative stress) in preterm neonates, in developing countries is limited.
AIMS: The study aimed to investigate the impact of Asc.A on reducing the oxidative str

AIM: The optimal age to initiate islet autoantibody (IAb) screening in first-degree relatives of individuals with type 1 diabetes (T1D) remains uncertain. The TRAKR cohort prospectively characterized IAb patterns in newborns of parents with T1D to assess the interpretability of early-life screening.
METHODS: We analyzed 148 children with available follow-up. Child blood samples were collected at birth and at 8, 18, 30, and 42 months, and parental samples during pregnancy. Insulin (IAA), GAD (GADA), IA-2 (IA-2A), and ZnT8 (ZnT8A) IAbs were measured using bridging ELISAs. Child-mother IAb speci

A diagnosis of necrotizing enterocolitis (NEC) often affects families for a lifetime. Given the profound burden of NEC, families are uniquely positioned to harness the power of their experience to advocate for and contribute to research. Clinicians and scientists can also collaborate with families affected by NEC to improve care as well as prevention and treatment strategies. Elevating the voices of patient-families affected by NEC can help advance the field by helping identify meaningful gaps in science, conveying the urgency of improving outcomes, and providing knowledge only known by famili

The practical considerations for using pasteurized donor human milk (PDHM) require a standardized yet individualized approach. Mother's own milk should be prioritized with PDHM serving as a bridge. Neonatal intensive care unit should focus on early feeding and a base diet of human milk MOM supplemented with PDHM. Human milk is critical to reducing necrotizing enterocolitis risk in very preterm infants. Given variability in PDHM composition, strategic fortification is necessary to optimize growth. Reliable milk handling, staff and family education, and shared decision-making with parents are es

Folate deficiency is associated with increased risk of conotruncal heart defects (CTHD), but its interaction with genetic factors remains unclear. Our objective was to investigate genome-wide associations between common genetic variants and birth before versus after universal folic acid fortification among children with CTHD and other heart defects. Sequencing data were available through the Pediatric Cardiac Genomics Consortium, aligned to the human reference genome (GRCh38/hg38), and jointly processed to ensure uniform variant detection and minimize batch effects. Analyses were restricted to

The generation of memory B cells following influenza virus vaccination and their rapid recruitment into the virus-specific immune response following infection is an essential component of vaccine-induced protection. We have previously reported the capacity of an influenza hemagglutinin (HA) expressing mRNA-LNP vaccine administered to newborn African green monkeys (AGM) to elicit both HA head- and stem-specific antibodies that exhibit a wide array of effector functions. Vaccine-induced responses culminated in enhanced protection compared to non-vaccinated newborn AGM. These results were notewor

BACKGROUND: Large-for-gestational-age (LGA) infants are a common complication of gestational diabetes mellitus (GDM), increasing the risk of obstetric trauma and long-term metabolic disorders. Pre-pregnancy body mass index (ppBMI) and gestational weight gain (GWG) are modifiable maternal factors, but their joint impact on LGA in sub-high-altitude, multiethnic populations remain unclear.
OBJECTIVE: To examine the independent and combined associations of ppBMI and GWG with LGA risk among women with GDM in a sub-high-altitude, multiethnic region of southwestern China.
METHODS: In this retrospec

OBJECTIVE: Endometriosis is a chronic gynecological condition that affects women in their reproductive age and could potentially lead to reproductive difficulties. Despite extensive research into the impact of endometriosis on fertility and reproductive health, its association with obstetric complications and short-term neonatal outcomes is still an area of limited understanding with conflicting evidence in published studies. We conducted this study to explore a potential effect of endometriosis on obstetric and neonatal outcomes.
METHODS: A retrospective population-based cohort study was con

OBJECTIVE: To examine, in children with acute gastroenteritis (AGE), the association between laboratory (and available clinical) findings and the real-world clinician decision to place a child under observation or to admit. The study is explicitly exploratory and association-based, not a prediction model.
METHODS: Single-centre retrospective analysis of 226 children aged 0-18 years (April 2024-March 2026). Disposition was classified hierarchically (outpatient discharge / observation only / hospitalization). Laboratory tests were obtained at clinician discretion at presentation, at intravenous

During 2021-2022, families with infants and young children were among the > 450,000 people who spent 2 weeks in Australia's institutional COVID-19 quarantine. Given pre-existing lack of emergency planning for children, there were concerns about whether parents would be adequately supported in this temporary accommodation. This paper reports on research considering the breastfeeding, breastmilk feeding and formula feeding of infants and young children in Australia's COVID-19 quarantine. Two-hundred and sixteen parents who quarantined with children aged 0-36 months completed an online survey wit
