Epidemiological assessment on birth outcomes among Marshallese migrants: a retrospective cohort study in the largest Marshallese community in the USA.

BACKGROUND: Marshallese migrants have experienced unique historical exposures related to nuclear testing, with potential implications for reproductive health. This study examined maternal and infant outcomes and birth defect prevalence among Marshallese individuals in Arkansas compared with Asian and other Pacific Islanders. METHODS: A retrospective cohort study using Arkansas vital records and birth defects surveillance data (2001-2020) assessed prenatal care, birth outcomes and major congenital anomalies. Adjusted prevalence ratios (PRs) and 95% CI were estimated using Poisson regression.

EHA Guidelines on the management of children and adults with hemoglobin SC disease.

HbSC disease is the second most common type of sickle cell disease (SCD) in populations of African origin. Although it is often grouped together with other types of SCD, particularly sickle cell anemia (HbSS), it has unique pathophysiology and a different spectrum of laboratory and clinical features. Compared to HbSS, hemoglobin levels are higher, HbF is lower at less than 5%, and most vaso-occlusive and vasculopathic complications are less frequent, although retinopathy is more common. Despite these differences, there have been no specific guidelines on the management of HbSC disease. This cl

Endocrine Complications in Patients with Congenital Dyserythropoietic Anemia: A Single-Center Cohort Study.

Congenital dyserythropoietic anemia (CDA) is a rare hereditary disorder marked by ineffective erythropoiesis and iron overload. Data on endocrine involvement in CDA remain limited. This study aimed to evaluate endocrine complications in patients with CDA and to assess their association with clinical and laboratory parameters. We retrospectively analyzed data from 12 patients with CDA (mean age 19.8 years; range: 6-49 years). Two patients were transfusion-dependent, and 10 were non-transfusion-dependent. We collected demographic, clinical, and laboratory data, including ferritin levels, and com

Development and validation of standard-criteria and age-corrected nomograms for post-stroke cognitive impairment risk stratification.

OBJECTIVE: The investigation aimed to develop and validate two complementary prognostic nomograms for post-stroke cognitive impairment (PSCI) among acute ischemic stroke patients. METHODS: In this prospective cohort study, 336 patients were enrolled for model development and internal validation, with 48 patients for external validation. Cognitive performance was evaluated using the Montreal Cognitive Assessment (MoCA) at six months post-stroke. The standard-criteria model defined PSCI as MoCA <26, while the age-corrected model applied age-specific cutoffs (<26 for <60 years, <25 for 60-69, <2

Differential genetic analysis of ectrodactyly in a Fanconi anemia pedigree with FANCA mutations.

Fanconi anemia (FA; OMIM: 227650) is a rare genetic disorder characterized by bone marrow failure, congenital anomalies, and cancer predisposition. While FANCA mutations account for most FA cases, phenotypic overlap with other disorders complicates diagnosis. This study analyzes molecular diagnostic pathways for FANCA-related FA and establishes a hereditary differential diagnosis for ectrodactyly. A Chinese FA family clinical phenotype was collected. The proband and father underwent whole-genome sequencing. Copy number variations (CNVs) in FANCA were assessed by genomic qPCR. Functional charac

Adult Diagnosis of Solitary Kidney and Renal Dysplasia in a Male Born Prematurely as a Twin: A Case Report.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) are a common cause of chronic renal disease and usually manifest either antenatally or in the early years of life. Solitary Kidney with Renal Dysplasia presents early in life due to regular antenatal sonographic scans. The rarity of diagnosis being deferred until adulthood has important implications when there is prematurity and other associated congenital anomalies, such as Congenital Heart Disease. CASE PRESENTATION: We present a 23-year-old male experiencing sequential generalized edematous changes, decreased urine ou

Germline TP53 Mutations Causing Diamond-Blackfan Anemia: A French Report.

Diamond-Blackfan anemia is a rare congenital erythroblastopenia typically caused by mutations in ribosomal protein genes. Recently, gain-of-function mutations in TP53 have been identified as a novel cause of Diamond-Blackfan anemia. We report two French patients who both harbored a heterozygous TP53 deletion (NM_000546.5: c.1077delA; p.(Ser362AlafsTer8)). They exhibited normocytic anemia, transient neutropenia at presentation, and distinct neurological impairments. Neither patient responded to a corticosteroid trial. One patient underwent hematopoietic stem cell transplantation from a matched

Management of Iron Overload in Infants and Toddlers With Diamond-Blackfan Anemia Syndrome: A French-Italian Study.

Diamond-Blackfan Anemia Syndrome (DBAS) is a rare congenital anemia often requiring chronic red blood cell transfusions from infancy. Without appropriate chelation, iron overload develops early and may be severe; however, no data are available on chelation in patients under 3 years of age. To address this, we conducted a retrospective, multicenter study collecting data from the French and Italian DBAS national registries. A total of 167 transfused DBAS patients were screened. Of these, 64 (38%) initiated chelation before the age of three (median: 18 months). Indications for chelation were a se

Acquired Amegakaryocytic Thrombocytopenia With Evolution Toward Aplastic Anemia Treated With Thrombopoietin Receptor Agonists: A Case Report.

Amegakaryocytic thrombocytopenia is a rare hematological disorder that is characterized by absent or near-absent megakaryocytes without evidence of other hematological abnormalities. The disease can either be congenital or acquired. Given that less than a hundred cases of acquired amegakaryocytic thrombocytopenia (AAMT) have been reported in the literature, its pathophysiology and treatment remain poorly understood. Various treatments have been proposed for AAMT, including glucocorticoids and other immunosuppressive agents, such as cyclosporine, antithymocyte globulin, and rituximab. To date,

Generation and characterization of human iPSC line SANi013-A from a Diamond-Blackfan anemia syndrome (DBAS) patient carrying a heterozygous RPS26 c.95-98 duplication variant.

Diamond-Blackfan Anemia Syndrome (DBAS) is a rare inherited bone marrow failure syndrome diagnosed in early childhood, marked by hypoplastic anemia, congenital anomalies, and increased cancer risk. Most cases involve loss-of-function mutations in ribosomal protein genes, disrupting ribosome biogenesis. We generated iPSC line SANi013-A from a patient with a de novo heterozygous RPS26 c.95-98 duplication. Proerythroblasts derived from peripheral blood were reprogrammed using a non-integrating Sendai virus method. The iPSC line SANi013-A displayed a normal karyotype, expressed pluripotency marker

Maternal and Foetal Risk Factors for Stillbirth: A Retrospective Case-Control Study at Al-Buraimi Hospital, Oman.

Background Stillbirth is a significant public health issue. The maternal and foetal risk factors often contribute to adverse outcomes. In Oman, while substantial improvements have been made in maternal and neonatal care, stillbirth remains a preventable tragedy in many cases. However, there is a gap in localised data to understand the specific risk profile for stillbirth in Oman, particularly at Al-Buraimi Hospital. Objective The main objective of the study is to identify maternal and foetal risk factors associated with stillbirths at Al-Buraimi Hospital in Al-Buraimi, Oman, in 2024 and determ

Generation of a human induced pluripotent stem cell panel from a family with a heterozygous RPS19 mutation including healthy wild-type, silent carrier and Diamond-Blackfan anemia individuals.

Diamond-Blackfan anemia (DBA) is a rare inherited bone marrow failure syndrome characterized by defective erythropoiesis and increased risk of congenital malformations and cancer. Progress in understanding DBA pathophysiology has been hindered by the lack of physiologically relevant disease models, limited public funding, and low commercial interest. Here, we generated four induced pluripotent stem cell (iPSC) lines from peripheral blood mononuclear cells (PBMCs) obtained from members of a family carrying a heterozygous c.184C > T RPS19 mutation (p.Arg62Trp) exhibiting variable clinical manife

Clinical clues to recognizing hereditary dehydrated stomatocytosis (DHSt) in children.

Dehydrated hereditary stomatocytosis (DHSt) is a heterogeneous group of rare hemolytic disorders with autosomal dominant inheritance. A series of 20 cases demonstrates that early identification of DHSt can prevent unnecessary interventions and improve the management of anemia, iron overload, and other complications in pediatric and adult patients. The presence of elevated mean corpuscular hemoglobin concentration (MCHC) with resistant erythrocytes suggested a possible association with variants in PIEZO1. Patients with KCNN4 variants showed no clear signs of erythrocyte dehydration, but, as wit

Fetal Hepatosplenomegaly: Stepwise Diagnostic Framework, Diagnostic Approach to Fetal Hepatosplenomegaly.

Background/Objectives: Fetal hepatosplenomegaly is an uncommon but clinically significant prenatal finding associated with a wide range of heterogeneous conditions, including congenital infections, fetal anemia, genetic syndromes, metabolic disorders, and other, less common abnormalities. Because of its nonspecific presentation and diverse etiologies, it represents a diagnostic challenge that requires a structured, multidisciplinary approach. This review aims to present a practical, clinically oriented, stepwise approach to the prenatal evaluation of fetal hepatosplenomegaly, integrating curre

[Ultrasound diagnosis of fetal infections: current possibilities and limitations].

INTRODUCTION: Fetal infections, particularly those caused by TORCH agents (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes), may significantly affect fetal growth and development, resulting in a substantial risk of morbidity and mortality. Since infections are mostly preventable causes of congenital anomalies, early diagnosis relies on serological and molecular testing and fetal ultrasound. OBJECTIVE: The aim of this study was to assess the prevalence of fetal infections and their detectable ultrasound features. METHOD: This retrospective s

Congenital Dyserythropoietic Anemia Type I in Association with CDAN1 c.2605 G>A Homozygous Mutation.

Congenital dyserythropoietic anemia type 1 (CDA-1) is a rare autosomal recessive disorder. We report the case of a two-year-old female who presented at three days of age with neonatal jaundice and anemia. She subsequently developed persistent, moderate normochromic anemia with relative reticulocytopenia. Bone marrow aspirate demonstrated erythroid hyperplasia and mild dyserythropoiesis. Whole-exome sequencing (WES) identified a homozygous c.2605G>A (p.V869M) mutation in the CDAN1 gene, consistent with the diagnosis of CDA-1. Although this variant has been previously documented in a compound he

Pre-existing vertebrojugular fistula unmasked following an uncomplicated internal jugular vein cannulation and its management.

A vertebrojugular fistula (VJF) is a vascular malformation characterised by abnormal communication between the vertebral artery (VA) and the internal jugular vein (IJV). It can occur following neck trauma, iatrogenic injury or spontaneously in connective tissue disorders, and rarely, it can be congenital. A young adult man in his early 20s with aplastic anaemia was planned for allogeneic haematopoietic stem cell transplantation (Allo-HSCT) for which right IJV cannulation was planned. After a single attempt at straightforward IJV cannulation, high flow was observed through all lumens; an anomal

Severe Chronic Kidney Disease Presenting as Asymptomatic Normocytic Anemia in a Child.

Anemia is a common finding in pediatric practice and is most often attributed to nutritional deficiency; however, it may also represent the earliest manifestation of underlying systemic disease. We describe the case of a six-year-old girl who presented for a routine well-child visit and was found to have severe normocytic, normochromic anemia despite appearing clinically asymptomatic. Subsequent laboratory evaluation revealed profound renal dysfunction with metabolic acidosis and electrolyte abnormalities. Renal imaging demonstrated bilaterally small kidneys without hydronephrosis, consistent
