Pyruvate Kinase Deficiency: Markedly Decreased Reticulocyte PK Activity and Limited Specificity of the PK/HK Ratio.

Diagnosis of pyruvate kinase deficiency (PKD) remains challenging in clinical practice. The pyruvate kinase (PK) to hexokinase (HK) activity ratio (PK/HK) was proposed to reduce the confounding effect of reticulocytosis on PK activity measurement. However, decreased PK activity and PK/HK ratios have also been observed in other anemias, raising doubts about their diagnostic value. We assessed the diagnostic accuracy of PK/HK ratio versus PK activity in differentiating PKD from other hereditary anemias. This study included 41 patients with molecularly confirmed PKD and 62 patients with other ane

Head and Neck Cancer in Fanconi Anemia: Clinical Challenges and Molecular Insights into a DNA Repair Disorder.

Fanconi anemia (FA) is a genetic disorder characterized by congenital anomalies, bone marrow failure, and cancer predisposition. Among other solid cancers, head and neck squamous cell carcinoma (FA HNSCC) is the most common cancer type in individuals with FA. The FA pathway is required for the complete repair of DNA interstrand crosslinks (ICLs), and unresolved ICLs result in cell cycle arrest, apoptosis, or complex chromosomal rearrangements due to chromosome breaks, ultimately leading to tumorigenesis. FA HNSCCs present earlier (median age of onset in the 30s) and exhibit a more aggressive c

Sickle cell disease: suspect, check, diagnose-practical tips for non-SCD experts to suspect and diagnose SCD in low-prevalence European settings.

Sickle cell disease (SCD) is the most common monogenic disorder, including a group of autosomal recessive hemoglobinopathies characterized by hemoglobin polymerization and sickling of red blood cells when low oxygen concentrations are present. SCD has a growing public health significance, affecting nearly 8 million people globally, with a high prevalence observed in Sub-Saharan Africa and Mediterranean countries. Improved understanding of SCD is essential, particularly given recent migratory flows that have contributed to an increase in the number of affected individuals in Europe and Italy. A

Overlooked parahiatal hernia: diagnostic challenges and implications of a case report.

BACKGROUND: Parahiatal hernias (PHHs) comprise of less than 1% of all diaphragmatic hernias and are frequently mistaken for paraesophageal hernias due to their close anatomical proximity to the esophageal hiatus. Unlike classic hiatal hernias PHH herniate laterally through a defect in the left crus separated from the esophageal hiatus. Because of their rarity and diagnostic ambiguity, PHHs are underrecognized and often incidentally found during imaging or endoscopy. The etiology can be congenital or acquired with known risk factors including trauma, prior esophageal and abdominal surgeries, or

[KLF1 (Krüppel-like factor 1) variants in the pathogenesis of hematological diseases].

KLF1 (Krüppel-like factor 1) is an erythroid transcription factor involved in various stages of red blood cell formation. KLF1 plays key functions at the megakaryocyte-erythrocyte progenitor stage, where it is involved in the development of the erythrocyte lineage. It participates in the global expression of erythrocyte genes, playing a direct role in globin switching. KLF1 also participates in the final maturation of erythrocytes, controlling exit from the cell cycle and the enucleation process. KLF1 has a domain structure: N-terminal transactivation domain and C-terminal DNA-binding domain.

Clinical Benefits and Safety of Multiple Micronutrient Supplementation During Preconception, Pregnancy, and Lactation: A Review.

OBJECTIVE: In this review we sought to determine the clinical benefits and safety of a multiple micronutrient supplement/supplementation (MMS) throughout preconception, pregnancy, and lactation in the mother and their child. BACKGROUND: No guidelines for pregnancy specifically recommend supplementation with micronutrients other than folic acid and iron or continuing the use of MMS beyond the first trimester. Yet micronutrients are essential during all stages of pregnancy for healthy fetal growth and development and maternal health, with an increased intake of many micronutrients recommended d

A Study on Feto-Maternal Outcome Among Elderly Pregnant Women During the Intrapartum and Postpartum Period in a Tertiary Care Center.

INTRODUCTION: The rising incidence of pregnancies among older women, typically defined as those aged 35 and above, has raised concerns regarding both maternal and fetal health outcomes. Advanced maternal age is linked to various pregnancy complications, such as a higher risk of chromosomal abnormalities, gestational hypertension, preterm birth, and other adverse outcomes. This prospective cohort study aimed to investigate maternal and fetal outcomes in this population of elderly pregnant women, shedding light on the risks and complications they face during pregnancy. The study also explored th

Using Late C-reactive Protein Measurement in Transient Tachypnea of the Newborn to Predict Bacteremia and Reduce Blood Sampling Frequency.

Background Frequent blood draws in neonates pose significant challenges, including procedural pain, risk of anemia, and increased healthcare burden. Hospitalized neonates with respiratory distress often undergo multiple blood tests to rule out bacterial infections, despite the low incidence of neonatal bacteremia. C-reactive protein (CRP) levels rise 10-12 hours post-infection, suggesting that early testing may not be optimal for predicting bacteremia. This study evaluates whether delaying the initial blood test and relying on a single blood draw can effectively predict neonatal bacteremia, ai

Targeted long-read sequencing facilitates effective carrier screening for complex monogenic diseases including spinal muscular atrophy, α-/β-thalassemia, 21-hydroxylase deficiency, and fragile-X syndrome.

BACKGROUND: Next-generation sequencing (NGS) has been applied for carrier screening, effectively reducing the incidence of severe diseases. However, some severe, high-prevalent and complex diseases, including spinal muscular atrophy (SMA), α-/β-thalassemia, 21-hydroxylase deficiency (21-OHD), and fragile-X syndrome (FXS), cannot be fully addressed by NGS, resulting in a high residual risk ratio. This study aims to evaluate the clinical utility of a long-read sequencing (LRS) panel for carrier screening of these five complex diseases. METHODS: A total of 2926 participants were retrospectively

Genotype-Phenotype Variability in Congenital Thrombotic Thrombocytopenic Purpura (TTP): Two Novel ADAMTS13 Variants.

Congenital thrombotic thrombocytopenic purpura (cTTP; Upshaw-Schulman syndrome) is an ultra-rare hereditary thrombotic microangiopathy caused by biallelic pathogenic variants in ADAMTS13. The condition is characterized by microangiopathic hemolytic anemia, thrombocytopenia, and ischemic organ injury. Although more than 450 variants have been described, genotype-phenotype correlations remain imperfect, and presentation is highly heterogeneous. We describe two children with cTTP, illustrating opposite ends of the clinical spectrum. Case 1: a 10-year-old girl with recurrent anemia and thrombocyto

Network Analysis of Legg-Calve-Perthes Disease and Its Comorbidities.

Background/Objectives: Legg-Calvé-Perthes disease (LCPD) is characterized by idiopathic avascular necrosis of the femoral head in children. There are several hypotheses regarding the cause of LCPD; however, the exact cause remains unclear. Studies on comorbidities can provide better insight into the disease. We aimed to perform a network analysis to identify the associations between LCPD and comorbidities. Methods: We analyzed patients aged ≤ 12 years with LCPD as defined by ICD-10 codes M91.1, M91.8, and M91.9 registered in the National Health Insurance Service cohort database from 2002 to 20

A frameshift mutation in the PRG4 gene causing camptodactyly-arthropathy-coxa vara-pericarditis syndrome: a case report.

Camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP) is an example of a rare non-inflammatory familial arthropathy inherited in an autosomal recessive manner. The proteoglycan 4 (PRG4) gene, located on chromosome 1q25-q31, is responsible for encoding a lubricating glycoprotein found in the synovial fluid and on the surface of articular cartilage. Pathogenic mutations in the PRG4 gene have been associated with CACP disease. The present study investigated the clinical and molecular findings of the patient with CACP. Whole-genome sequencing was performed in this patient to investigate

Congenital hepatic haemangioma: navigating the diagnostic dilemma of an antenatally detected abdominal mass.

Congenital hepatic haemangioma is a rare benign vascular tumour in neonates. This case describes a neonate with an antenatally detected abdominal mass, suspected to be a hepatic neoplasm. Postnatal imaging revealed a large hepatic lesion with diagnostic uncertainty between hepatic haemangioma, hepatoblastoma and mesenchymal hamartoma. The infant developed progressive anaemia requiring transfusions, and he underwent surgical excision of the mass. Histopathological examination confirmed hepatic haemangioma, showing thin-walled vascular channels with areas of haemorrhage and fibrosis. This case b

Inherited Predispositions to Myeloid Neoplasms: Pathogenesis and Clinical Implications.

Myeloid neoplasms with and without preexisting platelet disorders frequently develop in association with an underlying germline predisposition. Germline alterations affecting ANKRD26, CEBPA, DDX41, ETV6, and RUNX1 are associated with nonsyndromic predisposition to the development of myeloid neoplasms including acute myeloid leukemia and myelodysplastic syndrome. However, germline predisposition to myeloid neoplasms is also associated with a wide range of other syndromes, including SAMD9/9L associated predisposition, GATA2 deficiency, RASopathies, ribosomopathies, telomere biology disorders, Fa

Mutant Fam20c knock-in mice recapitulate both lethal and non-lethal human Raine Syndrome.

BACKGROUND: Inactivation or mutations of FAM20C causes human Raine Syndrome, which manifests as lethal osteosclerosis bone dysplasia or non-lethal hypophosphatemia rickets. However, it is only hypophosphatemia rickets that was reported in the mice with Fam20c deletion or mutations. To further investigate the local and global impacts of Fam20c mutation, we constructed a knock-in allele carrying Fam20c mutation (D446N) found in the non-lethal Raine Syndrome. The Fam20cD446N allele replaced the WT Fam20c by 3.6Kb Col1a1-Cre to get the conditional knock-in mice, and by Hprt-cre to get conventional

Potential Causal Relationship Between Chronic Obstructive Pulmonary Disease and Diabetes: A Bidirectional Two-Sample Mendelian Randomization Study.

PURPOSE: Diabetes, particularly type 2 diabetes (T2D), is a common comorbidity that occurs at a higher frequency in chronic obstructive pulmonary disease (COPD) patients compared to the general population. The COPD-diabetes association is documented epidemiologically and experimentally. Potential mechanisms, including systemic inflammation and metabolic dysregulation, are discussed as plausible pathways. However, their causal relationship still needs to be confirmed. METHODS: We conducted a comprehensive bidirectional two-sample Mendelian randomization (MR) analysis to evaluate the causal lin
