Clinical and Molecular Characterization of Children with Neonatal Diabetes: Experience from Bangladesh.

Neonatal diabetes mellitus (NDM) is a rare monogenic form of diabetes presenting before six months of age which can be permanent (PNDM) or transient (TNDM). In rare cases, NDM can be diagnosed between 6 and 12 months of age. Whilst TNDM is commonly associated with chromosome 6q24 methylation defects, the commonest causes of PNDM are KCNJ11 and ABCC8 gene mutations. This observational study's aim was to assess the clinical features and genetic aetiologies in patients with NDM diagnosed in Bangladesh. The clinical characteristics of 28 patients who presented with NDM from 2001 to 2018 in the in

Paradoxical Maturity-Onset Diabetes of the Young Arising From Loss-of-Function Mutations in ATP-Sensitive Potassium Channels.

UNLABELLED: Pancreatic β-cell ATP-sensitive K+ (KATP) channel closure underlies electrical excitability and insulin release, but loss or inhibition of KATP channels can lead to paradoxical crossover from hyperinsulinism plus hypoglycemia, to glucose intolerance or diabetes. We report genotype-phenotype information from a set of patients clinically diagnosed with maturity-onset diabetes of the young (MODY) and carrying coding variants in the KATP regulatory subunit gene ABCC8. In contrast to the naive prediction that diabetes should be associated with KATP gain-of-function (GOF; as in KATP-depe

Additive Effects of Dorzagliatin and Glucagon-Like Peptide 1 Receptor Agonism in a Novel Mouse Model of GCK-MODY and in Obese db/db Mice.

UNLABELLED: Glucokinase (GK) catalyzes the key regulatory step in glucose-stimulated insulin secretion (GSIS). Correspondingly, hetero- and homozygous mutations in human GCK cause maturity-onset diabetes of the young (GCK-MODY) and permanent neonatal diabetes mellitus, respectively. To explore the possible utility of GK activators (GKAs) and of glucagon-like peptide 1 (GLP-1) receptor agonists in these diseases, we have developed a novel hypomorphic Gck allele in mice encoding an aberrantly spliced mRNA. In islets from homozygous knock-in (GckKI/KI) mice, GK immunoreactivity was reduced by >85

Targeted analysis of whole exome sequencing in Thai patients with neonatal diabetes.

Neonatal diabetes mellitus (NDM) typically presents within the first 6 months of life and generally lacks islet autoantibodies. Genetic elucidation of NDM is a prime example of precision medicine in diabetes. However, no published genetic data exist for NDM in Thailand. We aimed to assess the genetic etiology of Thai NDM using whole exome sequencing (WES). We enrolled 14 Thai patients with NDM and measured GAD65, IA-2, and ZnT8 autoantibodies. We then performed WES and analyzed 43 NDM-related genes to identify causative variants. All subjects tested negative for the three islet autoantibodies.

Compound Heterozygosity of PTF1A Exonic and Enhancer Variants in a Japanese Boy With Pancreatic Hypoplasia.

A Japanese boy carried a paternally inherited single-nucleotide deletion in PTF1A exon 1 (c.775delC) and a maternally inherited nucleotide substitution in the distal enhancer region (g.23508356T>G). Both variants were hitherto unreported. The patient exhibited transient anemia in addition to typical clinical features of pancreatic hypoplasia, but no neurodevelopmental abnormalities. These results highlight the clinical importance and broad mutation spectrum of PTF1A variants as a cause of pancreatic hypoplasia. In addition, our data imply that phenotypes of PTF1A abnormalities are variable and

Anxiety, social responsiveness, and grit among patients with KCNJ11-related neonatal diabetes compared to unaffected siblings.

AIMS: Neonatal diabetes mellitus (NDM) occurs before 6-12 months of age and is commonly caused by activating mutations in KCNJ11 (KCNJ11-NDM) or ABCC8. Because of brain expression of these mutant ATP-dependent potassium channels, a spectrum of divergent neurodevelopmental difficulties have been described, including developmental delay, epilepsy, and neonatal diabetes (DEND). However, information on anxiety, social responsiveness, and grit is limited. METHODS: Individuals with KCNJ11-NDM (N = 12) and their unaffected siblings (N = 12) were recruited through the University of Chicago Monogenic

The Genetic Landscape and Precision Medicine in Neonatal Diabetes Mellitus: From Molecular Mechanisms to Clinical Management.

Neonatal Diabetes Mellitus (NDM) is a rare, heterogeneous monogenic disorder typically presenting within the first six months of life. Unlike type 1 or type 2 diabetes, NDM is caused by single-gene mutations that disrupt pancreatic β-cell function or development. With the advent of next-generation sequencing, the genetic spectrum of NDM has expanded significantly, necessitating a shift from symptomatic management to precision medicine. This narrative review summarizes the genetic basis and pathogenic mechanisms of NDM, categorizing them into three major pathways: (1) ATP-sensitive potassium (K

Neonatal Diabetes in Ireland over the Past 19 Years: Clinical Presentation, Management, Genetics, and Outcomes.

INTRODUCTION: Neonatal diabetes mellitus (NDM) is a rare monogenic form of diabetes presenting before 6 months of age. It may be permanent (PNDM) or transient (TNDM). Individuals with KATP channel variants may respond to oral sulphonylurea instead of insulin. The study aimed to review the presentation, genotype, phenotype, management, and outcomes of patients diagnosed with NDM in Ireland over 19 years. METHODS: Data on Irish NDM cases from 2006 to 2024 were collated through paediatric endocrinologists nationally and electronic databases. Analyses were performed using SPSS, with ethical appro

Implementation of a Hybrid Closed-Loop Automated Insulin Delivery System in a 29-Day Infant With Neonatal Diabetes.

Neonatal diabetes mellitus (NDM) often requires insulin treatment to maintain adequate glycemic control and support growth and development. Conventional insulin treatment regimens are particularly challenging in NDM due to young age, developmentally appropriate feeding variation, and heightened insulin sensitivity leading to very small insulin dosing. We present this case as-to our knowledge-the earliest implementation of a hybrid closed-loop (HCLS) automated insulin delivery system in an NDM patient. HCLS with U10 significantly improved glycemic control compared to conventional insulin delive

Five-year follow-up of incretin-based therapy in a patient with KCNJ11-related permanent neonatal diabetes: Achieving insulin discontinuation and sulfonylurea reduction.

Permanent neonatal diabetes mellitus (PNDM) is commonly caused by pathogenic variants in KCNJ11 that impair insulin secretion through ATP-sensitive potassium (KATP) channel dysfunction. Sulfonylureas (SUs) can stimulate insulin secretion and enable insulin discontinuation, but insulin withdrawal may remain difficult even under high-dose SU, especially when therapy is initiated later in life. We describe a woman with KCNJ11-related PNDM who achieved insulin discontinuation after adding sitagliptin, followed by further improvement with semaglutide during a five-year follow-up. HbA1c remained bet

Transient neonatal diabetes mellitus as an early diagnostic clue to HNF1B-related disease - two case reports and a literature review.

BACKGROUND: Pathogenic variants in the HNF1B gene cause a multi system disorder encompassing organ abnormalities-primarily affecting the kidneys and pancreas-as well as metabolic disturbances, collectively referred to as HNF1B-related disease. While maturity-onset diabetes of the young type 5 is a well-recognized manifestation, neonatal diabetes mellitus (NDM) associated with HNF1B is exceedingly rare, and has only been reported in patients harboring single nucleotide variants. CASE PRESENTATION: We describe two unrelated female children presenting with transient NDM caused by a complete HNF1

Insights from maturity-onset diabetes of the young into impaired insulin secretion in type 2 diabetes.

Monogenic diabetes arises from pathogenic variants in a single gene that are sufficient to cause disease predisposition. In general, a greater functional impact of genetic abnormalities is associated with an earlier age of onset. Accordingly, monogenic diabetes encompasses a wide clinical spectrum, including neonatal diabetes mellitus presenting within the first six months of life, and maturity-onset diabetes of the young (MODY) typically manifesting from childhood to early adulthood; both diabetic types exhibit impaired insulin secretory capacity. Although MODY is currently classified as diab

Monogenic Neonatal Diabetes: Clinical Presentations, Genetic Findings, and Response to Therapy in a Retrospective Case Series.

Introduction Monogenic neonatal diabetes mellitus (NDM) is a rare form of diabetes, presenting within the first six months of life and caused by pathogenic variants affecting pancreatic β-cell development or function. Because its initial presentation may overlap with type 1 diabetes, molecular diagnosis is crucial, as it directly influences prognosis and treatment - particularly the potential responsiveness to sulfonylureas in ATP-sensitive potassium (KATP)-channel-related NDM. This study reports a retrospective descriptive case series and aims to characterize the clinical and genetic features

Early-Remitting Neonatal Diabetes in a Growth-Restricted Preterm Infant Requiring Insulin: A Case Report.

Neonatal diabetes mellitus is a rare cause of persistent hyperglycemia in early infancy and is usually monogenic. We describe a male preterm neonate born at 34 weeks' gestation with intrauterine growth restriction who developed severe hyperglycemia from the first days of life. Plasma glucose reached a maximum of 570 mg/dL with persistent glycosuria, negative urine ketones, and a normal acid-base profile. C-peptide was profoundly suppressed (0.01 ng/mL), supporting marked endogenous insulin deficiency. Following optimization of glucose administration, intravenous short-acting regular human insu

An Interesting Case of Sulphonylurea-Responsive Permanent Neonatal Diabetes Mellitus With Potassium Channel Mutation.

We present a case report of an infant diagnosed with a pathogenic de novo variant in KCNJ11 (Potassium Inwardly Rectifying Channel Subfamily J Member 11), associated with both transient and permanent neonatal diabetes. This patient is one of the earliest diagnosed and genetically confirmed patients with neonatal diabetes. Early detection is key, and this case report emphasizes the need for early consideration of rapid whole-genome sequencing as an option for diagnosis to significantly improve patient outcomes.

A Novel Renal Manifestation in GLIS3-Related Disorder in a Pediatric Patient With Cystic Nephroma.

A 10-year-old Saudi girl with a homozygous GLIS3 gene deletion presented with the classic triad of neonatal diabetes mellitus, congenital hypothyroidism, and congenital glaucoma. During routine surveillance, an incidental complex left renal mass was identified. Comprehensive imaging and histopathological analysis following a core needle biopsy diagnosed a benign multicystic nephroma with features of a mixed epithelial and stromal tumor (MEST). This represents a novel renal manifestation, expanding the phenotypic spectrum of GLIS3-related disorders beyond simple cystic dysplasia. Management fav

Congenital Hyperinsulinism With Paternally Inherited ABCC8 Variants: A Single Center Experience Over a Decade in Singapore.

ABCC8 pathogenic variants have been found to cause neonatal diabetes mellitus (NDM), maturity-onset diabetes of the young (MODY) and congenital hyperinsulinism (CHI), depending on the nature of the mutation. Few studies have reported on the carrier frequency of CHI-related ABCC8 variants. In Singapore, the carrier rate is 1 in 754. Paternally inherited ABCC8 recessive variants have been classically associated with focal lesions causing CHI. In our case series of five patients with paternally inherited ABCC8 variants, one patient had a second maternally inherited recessive variant, making him a

Neonatal Diabetes Mellitus Due to Homozygous ZNF808 Mutation Associated With Skeletal Anomalies: A Novel Presentation of Pancreatic Agenesis-3.

Neonatal diabetes mellitus (NDM) is a rare genetic condition characterized by persistent hyperglycemia presenting within the first six months of life. It is most commonly caused by mutations affecting pancreatic development or β-cell function. ZNF808 is a recently identified gene that plays an essential role in human pancreatic development, with biallelic pathogenic variants causing autosomal recessive pancreatic agenesis. We report the case of a seven-month-old Saudi male infant with a history of intrauterine growth restriction who presented at 2.5 months of age with severe hyperglycemia and

Transient Neonatal Diabetes Mellitus Potentially Associated With a Novel Homozygous MS4A6A Gene Variant: A Case Report.

Neonatal diabetes mellitus (NDM) is a rare metabolic disorder characterised by hyperglycemia within the first six months of life. While commonly monogenic, the MS4A6A gene, known for immune modulation and calcium signalling, has not previously been linked to NDM. We report a case of transient NDM (TNDM) potentially associated with a novel homozygous variant in the MS4A6A gene. A male infant born at 27 weeks of gestation (720 g) developed severe hyperglycemia (>200 mg/dL) and polyuria on day 14 of life, following the resolution of suspected meningitis.
